President Donald Trump, known as a lover of fast food and processed meats, is one of the last people you'd expect to try a vegan diet, or one that eliminates animal products like meat, milk, and eggs.

But a 9-year-old activist known as Vegan Evan hopes to he does just that, and challenged Trump to go vegan for 30 days as part of a campaign to "Make America Healthy Again."

If Trump accepts and completes the challenge, a nonprofit called Million Dollar Vegan will donate a million dollars to a charity of Trump's choice. That money, according to a spokesperson for the nonprofit, comes from private donations from individuals and other activist groups.

So far, Trump has not responded to the challenge, but has until December 31 to do so, according to the organization.

Vegan Evan, who's from Melborne, Florida, wasn't always vegan. He told Insider he remembers deciding to make the change on the way to kindergarten one morning as his mom, already a vegetarian, announced she was going vegan. Evan, then 5, decided to join her.

Although Evan's mom cautioned him that being vegan would mean cutting foods that kids typically love, like pizza and cupcakes, his determination held firm.

"I told her, 'If the animals are being hurt and I don't need it, I don't want it,'" he said.

Since then,Evanhas been vegan, and he's been encouraging others to do the same for nearly as long through videos on his YouTube channel. He also travels the country with his family to promote a vegan diet.

Evan recently announced his challenge to the president in ads in the New York Times and elsewhere, as well as through a vigorous online campaign.

He hopes his message to Trump will raise awares of the benefits of being vegan, and encourage more people to try it for themselves.

"I think that veganism is a gift and it's great to give that gift to people so they can be healthier and feel better, not hurt animals, and not destroy the planet," he told Insider.

Evan said he doesn't feel like he's missing out on anything. Contrary to popular belief, "anything that anyone else could eat, we can eat a vegan version," he said.

"Now I eat vegan cupcakes, and vegan pizza is one of our favorite foods," he said. He's also a big fan of quesadillas with cashew cheese, rice, vegan chicken, and spinach.

He said the biggest challenge most people face when going vegan is that they just don't know what to eat instead of animal products. Once they do, "it's not really that hard," he said. "You just have to look around your grocery store a little bit."

Evan added that the growing trend of plant-based meat substitutes has made it even easier and more accesible to eat vegan even if, like Trump, you're a big fan of burgers.

"The Impossible burger is delicious, of course. It's great because we don't have to eat animals but can still have the same taste," Evan said.

Evan said he's wanted to challenge the president to go vegan for a while, and partnering with Million Dollar Vegan seemed like the perfect opportunity to raise some awareness. "He has a lot of reach to people; if they see him doing this, they might want to do it too," he said.

Evan added that he also hopes to run for president in 2048.

Until then, he plans to continue promoting veganism. "It's not only good for the planet and the animals," he said, "it's great for your health."

London has been named the world's most vegan-friendly city, according to an annual ranking from an online restaurant guide.

The U.K. capital came top for a second year, in the rankings put together by HappyCow, which claims to be the largest vegan and vegetarian restaurant guide in the world, looking at options in more than 180 countries.

HappyCow's top 10 rankings were determined by the number of vegan restaurants within a closely proximity and its overall impression of each city's "vegan-friendliness."

London currently has 152 vegan restaurants and it was the first city on HappyCow to have more than 100 completely vegan restaurants in 2017.

Specialist eateries in the capital include the world's first vegan "chicken" shop, The Temple of Seitan, as well as the U.K.'s first vegan cheese shop La Fauxmagerie.

"Veganism continues to become a mainstream lifestyle in London, and this city takes veganism to a new level," said Ken Spector, a spokesperson for HappyCow.

The number of vegans in the U.K. has quadrupled between 2014 and 2019, with around 600,000 Britons now on a plant-based diet, equivalent to more than 1% of the population, according to charity The Vegan Society.

New York came second on the list, with 111 restaurants in a five-mile radius of the city.

Bangkok, Thailand was the only city to be added to the rankings this year, coming in eighth. HappyCow counted 74 vegan restaurants in the city. Spector said that most restaurants have "solid vegan options" on the menu thanks to Bangkok's "predominantly Buddhist background."

HappyCow cited data from Google Trends, which showed interest in "veganism" increased sevenfold in the five years between 2014 and 2019.

Edward Bergen, global food and drink analyst at market research firm Mintel, spoke about the shift to veganism on CNBC's "Street Signs" last week.

He pointed out that around half (51%) of consumers in the U.K. said they were either meat-free or were actively reducing their meat consumption in 2019, rising from around 41% last year.

Bergen said health tended to be the main factor driving people to cut out meat or become "flexitarians," those reducing their meat intake in favor of a "part-time" vegan or vegetarian diet.

"Consumers are seeing availability of shelves and it's easier for them to move over," he added. "And also they get bored more so they're trying more foods altogether."

Environmental considerations and concerns over animal welfare were also motivating this shift by consumers, he said.

The U.K. overtook Germany in 2018 as the world leader for vegan food launches, according to Mintel.

The HappyCow top ten list is determined by three factors: the number of vegan restaurants in a 5-mile radius in the most vegan restaurant-dense area in each city, the number of vegan / vegetarian / veg-option restaurants in the same 5-mile radius, and the HappyCow teams impression of the overall vegan-friendliness of each city.

Topping HappyCows list for the second year in a row is London, which was the first city on HappyCow to surpass 100 vegan restaurants in 2017. London currently has 124 vegan restaurants in a five-mile radius within the city.

As a sign that veganism has shifted permanently into the mainstream, KFC Canadas recent test-run of plant-based popcorn chicken in Mississauga, Ont., seems as good as any. When meat alternatives are being sold by the bucket, its hard to argue they are still the domain of urban elites.

But the mass markets embrace of plant proteins (The Economist called 2019 The Year of the Vegan) also coincides with reports that developed countries are getting fatter not slimmer. Which raises an obvious question: Are vegan diets as virtuous as they seem?

According to a recent op-ed by Frdric Leroy and Martin Cohen, academics at Vrije Universiteit Brussel and the University of Hertfordshire respectively, the health benefits of a vegan diet depend on careful nutritional balance. For example, attention to Vitamin B12 consumption or the appropriate intake of long-chain fatty acids.

Maintaining that balance can be difficult, however. And when vegans fail, they write, the negative health impact can be significant.

Moreover, Leroy and Cohen point to a study of 218,000 subjects from over 50 countries which found that the consumption of meat and dairy can be associated with less rather than more chronic disease.

But other research shows clear benefits to a vegan lifestyle. A study published in The Journal of Nutrition, for instance, documented both lower levels of fat and higher levels of antioxidants among those with a plant-based diet. These are the compounds in foods which help delay cell damage attributed to the aging process.

A study presented this fall by the Washington-based group Physicians Committee for Responsible Medicine also showed the potential for veganism as a treatment for obesity. Participants who followed a 16-week vegan diet experienced significant changes to gut microbes linked to improved body weight, body composition and blood sugar control.

Pamela Fergusson, a registered dietitian and devoted vegan in Nelson, B.C., sees the positive impact in her practice. One of her clients, a teacher in his 30s, came to her two years ago obese and concerned by a family history of heart disease. Inspired by the 2011 documentary Forks Over Knives on Netflix, however, he was committed to trying a vegan diet. One year in, he had lost 27 pounds and his blood tests were normal.

A vegan diet also makes you feel more clear-headed and able to focus, Fergusson adds, partly because it doesnt take so much energy for your body to digest.

When it comes to weight loss, though, Dr. David Jenkins, director of the Clinical Nutrition and Risk Factor Modification Centre at St. Michaels Hospital in Toronto, says the fibre and volumetrics of vegan foods are the critical advantages.

We love chewing and all the sensations that go along with eating, so we want to eat a lot of food, he says. But when you compare a steak to a plate of vegetables, you can have the same volume of food but theres a vast difference in calories.

Jenkins, a vegan for almost 20 years, is an industry pioneer when it comes to research and clinical trials on diet and health. He developed the concept of the glycemic index to show how different carbohydrates affect blood sugar (or glucose) levels, and which foods are best for people with diabetes.

But GI also affects how quickly we get hungry again after eating. Whole, unrefined plant foods are helpful to weight loss in part because they have slow-release carbohydrates that leave us feeling fuller longer. Vegetables that retain water when cooked, such as dried beans and peas, tend to increase satiety as well.

Ive put people on plant-based diets who found they felt full and sufficiently hydrated, says Jenkins.

Liquids in and of themselves, mind you, are no substitute for solid food. Its that predilection to chew again, he says.

When youre having wine with your steak dinner, you tend to move on to the Crpes Suzettes and then the next course of foods that are excessively stimulating. Whereas lentil soup is extremely pleasant, youre not driven to eat more and more.

So what about those plant-based KFC popcorn bites?

Veganism has come a long way when it comes to broad appeal. It has its own movement and theres a lot of innovation in the space, including foods like vegan ice cream and plant-based burgers, says Fergusson.

Thats a boon to those who would never have considered a vegan diet in the past as long as plant food isnt shorthand for junk food.

Not everyone wants to eat lots of lentils and rice all the time, but as long as we choose whole foods more often and enjoy others in moderation it works, Fergusson says.

So to test on animals for something that is purely vanity; there are so many other solutions and products that we can choosefrom.

Hayes, who is a popular vegan makeup artist on social media, notes that the community around cruelty-free and vegan beauty has significantly grown in recentyears.

Affordable brands include e.l.f and Pacifica, while mid-range brands she loves include Kat Von D, Lime Crime, Milk and Nabla. Hayes adds that beauty stores Sephora and Ulta have started making it easier to search for vegan products, so if you type vegan into the search bar on those sites, you will find a collection of their vegan productlines.

The inclusive beauty trend comes for menopausal women – Glossy

Call it the anti- anti-aging movement. Beauty brands are carving out niche products and corresponding marketing campaigns for pre- and post-menopausal women.

In March 2019, direct-to-consumer hair-care brand Better Not Younger launched, aimed at selling shampoo, conditioner, scalp serum and ingestibles for women 45 years and older. On October 15, it expanded to Sephora.com. Elsewhere, P&Gs corporate venture capital arm partnered with M13, a venture firm, on the creation of a DTC menopausal skin-care and wellness brand, Kindra. Launched in November, it sells products like sleep and energy ingestibles. It plans to launch body and a hand-foot cream in 2020. Greek skin-care brand Korres is also getting in on the action, launching two menopausal products, a moisturizer and wrinkle treatment, in January 2020 through Sephora and Korres.com.

This is a huge, untapped white space for this underserved demographic, said Elena Baltzoglou, a marketing consultant at Korres. We want to bring skincare to a customer who feels neglected.

This year, the beauty industry has seen the development of hyper-niche categories such as skincare for Gen Z and sun care for women of color, and menopausal women represent a particularly large and lucrative category. According to the American Academy of Dermatology, women officially enter menopause one year after their last period, which typically occurs around 51 years old, and this lasts for about four years. During this phase, skin becomes dryer, looser and thinner, and hair becomes thinner. Because of the unique impact of menopause on the skin, beauty brands are approaching the category as more than trying to roll back the signs of aging to preserve youth. Korres plans to launch an ongoing social media campaign for its products in January beginning with education on its products and ingredient profiles, and in April, it will launch a positive aging campaign with real customers speaking out about their menopause experiences.

Better Not Youngers communication strategy also relies on acknowledging women over 45 years old and accurately reflect their lifestyles. The brand began producing video content in July as part of Googles accelerator program, including a 42-second video asking women what life after 45 looks like. The video has garnered over 600,000 views on YouTube. Sonsoles Gonzales, Better Not Younger founder and CEO, said she had only seen brands (like Prestige Brands-owned Summers Eve) attempting to cater to the age group by using old stereotypes like a grandmother, a woman gardening or a woman on a cruise in advertisements. Since launch, Better Not Younger has grown sales by 50% month-over-month, and Gonzalez expects its Sephora sales for November to exceed the monthly estimate by 20%, which equates to about $48,000 in sales.

Women my age are starting businesses, dating again and hiking mountains, she said. Its not just about looking great, but its also about showing them in situations where they can see themselves.

There is a good reason to pay attention to women above 45: They are willing to pay for beauty. According to the New England Journal of Medicine, 50 million women will reach age 51 in 2020. The AARP found in a national survey of 2,000 women that 70% of those ages 40 and older want to see more beauty and personal care products for perimenopausal and menopausal women. Women 50 and older spend $22 billion a year on beauty products, the survey found.

Gonzales said she sees two primary reasons for the carving out of the menopausal beauty category: societal acceptance and the rising cost of advertising toward millennial women. As a result of chasing millennials, older women were being neglected, she said. The inclusivity movement has allowed women to talk candidly about the aging experience.

A brand like mine would have been a complete flop 10 years ago, because [the world] wasnt ready, she said. But with topics like inclusivity and #MeToo, this conversation has become part of pop culture and relevant.

Korres Baltzoglou agreed: [Menopause] is the next thing, she said. [The industry] was talking about race inclusivity and then body inclusivity, and age is the next step.

Because of this growing acceptance, P&G is also stepping in, albeit with some caution. Kindra (previously called Pepper & Wits) is operated through M13, for now unlike P&Gs other brands, like Olay and Secret deodorant, which the company owns outright.

Kindra has invested in Facebook ads and will roll out Pinterest and Google ads to customers 45 years and older in 2020. P&G and Kindra would not disclose the brands marketing spend. If the business meets certain undisclosed profitability and revenue targets, P&G will buy back the brand, according to the two companies.

What we want to do is change the way women [experience] menopause, said HongJoo Sun, Kindra CEO. Menopause has been treated as something to be cured rather than a natural phase of a womans life.

Remember to apply a fragrance-free moisturizer after showering and washing your hands. Consider: Could dry skin be caused by more than a change in the weather?If a person has an internal condition like low thyroid (hypothyroidism), diabetes or poor circulation, dry skin is often more of a problem. Dry skin is a hallmark of dermatitis and ichythosis.Many patients who are on cholesterol-lowering agents ( statins) often are troubled by dry skin as well. Dry, itchy skin can also be seen in patients with vitamin deficiencies like low vitamin D.* An easy step-by-step routine for caring for dry skin:* Wash with warm water, not hot* Use a mild fragrance free cleanser* Pat dry the skin with a soft towel* Apply a fragrance free, light non-comedogenic moisturizer (lotion). If its in the morning, make sure the moisturizer contains a broad spectrum sunscreen. For your face, some people prefer a tinted moisturizer so they can skip the foundation and moisturizer. Repeat the process at night. On your face, its important to remove make-up before bedtime to avoid clogging your pores.If its nighttime, some people will opt for a heavier moisturizer (cream or ointment)* Stay hydrated! Drink 6-8 glasses of water to keep your skin happy and glowing* If your home is dry in winter, you can use a warm steam vaporizer in your bedroom at night to normalize the humidity.* Nutrition matters! If the itching is worse at night, an antihistamine like diphenhydramine can ease the irritation.If these patches still wont budge, see a dermatologist. I love the marathon because its an event where there are all different types of people aiming for the same goal, Vannya Istarinda tells Claudia Rindiantika. Age, your work, gender, race nothing matters because at the start line everyone is the same.

Vannya Istarindas big news is that she is engaged to Arya Bakrie and will get married next year. Things are getting serious now, the 24-year-old food and fashion entrepreneur says, with a smile, of the couples preparations for the big day. But its not that hectic yet. We have just started planning, so everything is still not too crazy. The wedding is not until next year, so we still have time.

Its a Thursday afternoon, the day of Prestiges It Girl photo shoot, when Vannya walks into the studio. Our first impression is how friendly she is. She beams as she is greeted and shakes hands with everyone in the room. While retouching her makeup and getting her hair done, she has a chit-chat with the team with an amiable voice and constant smile.Photography completed, Vannya sits down for her interview. After her engagement to Arya, she tells us, her most important news is that she has been working hard on building her business ventures this year: clothing line Dua Awan and healthy restaurant Greenery.

We are finalising the new website for Dua Awan thats something I am paying a lot of attention to right now, she says, because, as you know, everything is online these days and thats a big market to tap into. I know there are a lot of negatives about the impact of social media on society but used properly its great for communicating to customers and for business. I find it really useful, and I think its actually a large part of my business today, likewise Greenery. A lot of people sell things online now that they couldnt have sold before, utilising a social media platform or e-commerce platform that is robust and full-featured in a good way. Instagram is a massive platform for selling nowadays.Vannya studied Architecture at the University of Indonesia before taking a double degree and transferring to Curtin University in Perth, Australia. At that time, I really loved design, she says. My mom also took architecture, so I thought that was a really good thing to do.

She switched to fashion after school simply because I have always been interested in clothes and fashion. As an architect, I am a designer too, and moving from architecture to fashion was logical. Right now, its what I really want to do but hopefully, in the future I could continue designing architecturally.

Maintaining a healthy lifestyle is a big issue for Vannya. It was during my second year at university that I realised I needed to change, she recalls. I was living a really unhealthy lifestyle, and that lifestyle really impacted my daily life, confidence and well-being.

I realised then that health is such an important thing. Health is our wealth. Its something you can never take for granted. If you always have an unhealthy lifestyle, you will regret it one day when you get sick. So, investing in eating healthy food and a good exercise regime is very important.

Vannya admits that it was hard to forget old habits and switch to a better lifestyle. I had to push myself every week to work out because before that I hadnt been taking exercise. Then I started eating better more vegetables and fruit. Thats what has gotten me into the habit of having a healthy lifestyle, and Ive never left it since.

She turned her newfound fitness into becoming a sports influencer. She was a brand ambassador for Adidas for a few years. Sport is important as one way to maintain a healthy lifestyle, Vannya points out.

This is why I started running marathons. I love athletics and I think the marathon is an event where there are all different types of people aiming for the same goal. Age, your work, gender, race nothing matters because at the start line everyone is the same. Throughout the journey of a marathon, everyone is always supporting each other. It is like a massive movement of positive vibes and good energy.

Vannya recently ran in two of six major marathons in the world: Tokyo in 2015 and Chicago the following year. It started from being consistent every single weekend, to running on car-free days. I trained for a whole year and would run every Sunday. Each time I would run an increased distance, and I kept increasing my goal each week. I was initially in disbelief of the things I could achieve. It was only because I built a passion for running and kept training consistently that I could cross the finish line, Besides being a self-satisfying achievement, completing these races taught me a lot about time management, dedication and sacrifice from months of training to get to the finish line. I think running marathons teaches you to be persistent and patient. It also teaches you to believe in yourself that if you keep doing it, you can finish it. I hope I shall be able to run more of the world major marathons in the future.

Marathon training aside, Vannyas regular exercise routine is daunting. I do HIIT (high-intensity interval training) three times a week, resistance and strength training twice a week, and a little Pilates too, she informs us. I also do a lot of cardio training, because I love it! I am a morning person. Getting up early and working out gives me plenty of energy and makes me more productive throughout the day.

What motivates her to exercise? When you have a goal to finish a marathon or any other health objective, you will be motivated most of the time, Vannya declares. I love the feeling of running and I love working out, so it is never a chore or something I have to push myself to do daily, but something very enjoyable for me. On days when I feel lazy, I do fun exercises like zumba with my friends.

With excitement, Vannya adds: I love being healthy. And I really support people that love living a healthy lifestyle. Especially nowadays there are a lot of junk foods fried food, for example and processed products that are quite unhealthy. There are a lot of things out there that can distract you from living a healthy lifestyle. Even just being stressed out can be really bad for your health. So you need to take time for yourself and to de-stress. Thats so important in maintaining a healthy lifestyle.

The conversation witches back to food. I must be very careful about dairy products as I am gluten and lactose intolerant. Its important to learn more about your body and to avoid allergens and intolerants so that your body can function and repair at its best.

For breakfast, I would have coconut yoghurt, fruits and the occasional coffee. Lunch would be a chicken salad or grilled fish. I love sweets so I have a chocolate, sweet potato or banana chips for snacks. And when I go out for dinner, my choice would definitely be sushi!

Greenery, which Vannya launched with some business partners, is to be found in Jl. Senopati. It specialises in wholesome healthy bowls (that) pack a satisfying and nutritious punch with the inclusion of their home-made ingredients, such as peanuts and almond butter.

Mountain Trek Retreat Changes the Way Couples Think About Health – DatingNews

Two years ago, Alex Timmons and his wife, Hannah, were feeling overwhelmed with their lives. At the time, Alex was running a tech startup in the San Francisco Bay Area, but his work didnt feel rewarding, and was deteriorating his health.

My work wasnt helping people; it was just helping influencers get rich, and the stress I put on myself to succeed led to countless sleepless nights and even a trip to the hospital, he told us.

Hannah, who was working a high-end marketing job, was dealing with chronic anxiety and struggling to quiet her negative self-talk.

They decided to reset their lives by attending a weeklong retreat at the Mountain Trek Fitness Retreat and Health Spa in Ainsworth Hot Springs, British Columbia. Alex said he and Hannah thought that the mountain setting would help them unwind, while the healthy eating and exercise components would help them find the balance they sought.

They werent just pleased with the experience they were amazed by it. That retreat changed their lives profoundly.

Daily hikes help guests get active while connecting with nature.

Each day, four levels of hikes depart from the lodge. They are led by trained guides who know the area and who make the experience enjoyable. And guests are treated to views of some of the most scenic vistas in the world.

Hiking is such a critical part of the process because time in nature is proven to lower cortisol levels, which is the hormone that causes stress. Further, along with the healthy eating habits promoted at the retreat, guests see their muscle mass increase, and their weight decrease.

Were not a place for couples to come to sit by the lake and drink wine. Its for couples who understand a little hard work goes a long way. In addition to the daily hikes, sunrise yoga and evening fitness classes are on the schedule. Alex said.

In addition to adopting a fitness routine, Mountain Trek teaches guests how to change their eating habits. According to Alex, many people dont eat properly, consuming the wrong foods at the wrong times in the wrong proportions. Instead, they eat too much starchy, sugary, and processed foods, resulting in lower energy levels and expanding waistlines.

Our program follows a 12-hour cycle. We eat six meals from 6 a.m. until 6 p.m. They are lower-carb, higher-protein meals that sustain and balance energy. We teach our guests to eat for fuel, not for fun, Alex said.

The retreat also doesnt allow guests to drink alcohol or caffeine. That can be a significant change in many peoples lives.

People really do go through a metabolic reset while at the retreat, said Alex.

But that doesnt mean the retreat deprives guests of anything tasty. For example, Chef Simon Vine recently cooked a delicious meal of rutabaga spaghetti with tempeh-almond meatballs and a tomato-cashew sauce as a healthier substitute to the traditional pasta dish. The chef also prepares snacks that help kickstart the metabolism of guests.

Eating healthy for a week or two is one thing, but Mountain Trek also provides guests with the skills to be mindful about cooking and eating at home. That is one of the key reasons that many guests decide to come to Mountain Trek with their spouses or partners; they want to have someone who can help them stick to these healthy-eating guidelines.

We educate you on how to implement a healthy lifestyle. So, if you come with a spouse or partner, you get to be each others support system when you return home, Alex said.

Though fitness and healthy eating are important parts of the Mountain Trek experience, they arent the only components. The retreat also focuses on ensuring that guests find a balance in each of the retreats other key elements; stress management, sleep, and detoxification.

For example, if youre not eating well, but youre sleeping enough, you still may not feel great. If youre eating and exercising regularly, but you never take a break from alcohol or caffeine, you likewise may not reach your full potential.

When you balance the five elements, they compound upon each other, Alex said.

During the program, guests see significant changes in themselves over a short time. Many people are overwhelmed by the lifestyle changes the retreat asks them to make, but, if they make those sacrifices, theyre satisfied with the results.

The gut microbiome in disease and health – McGill Tribune

There are thousands of different bacterial species living inside our intestines. This environment, called the gut microbiome, provides the body with key vitamins and ensures a healthy immune system. The composition of the gut microbiome is key: Dysbiosis, a condition that occurs when bad bacteria take over the gut, is linked to a wide variety of diseases ranging from inflammatory bowel disease to autism spectrum disorder.

A group of researchers from McGill and the Universit de Montral found that dysbiosis is often correlated with fibromyalgia, a disease marked by chronic pain, constant fatigue, and gastrointestinal problems.

Their results were published in a paper in the journal Painin November. Dr. Amir Minerbi, a pain physician-scientist at the Rambam Health Campus in Haifa, Israel, was one of the papers lead researchers.

Fibromyalgia is very frustrating for patients, Minerbi said in an interview with The McGill Tribune. They seem okay on the outside, but inside, [] it feels like having the flu for a very long time [.] It is frustrating for physicians, because we are not good at diagnosing fibromyalgia. It takes four years to diagnose, and [] after diagnosis, even with the best treatment, [the patient] may still [experience] symptoms.

This frustration was the catalyst for investigating the relationship between the gut and fibromyalgia. Their hypothesis was justified, since the microbiome has been associated with other chronic pain disorders, such as chronic dysfunction pelvic pain and chronic fatigue syndrome.

In the study, researchers collected stool samples from people with and without fibromyalgia. Through sequencing bacterial DNA found in the stool samples, they found that people with fibromyalgia had more bacterial species associated with factors such as inflammation, inhibition of brain activity, and metabolism of organic acids within their microbiome. Furthermore, patients with more pronounced symptoms of fibromyalgia had higher amounts of bacteria associated with such factors.

These findings, however, are still preliminary and represent only a correlation rather than causation. There is still the possibility that fibromyalgia causes an altered microbiome, instead of the other way around. Nevertheless, this opens the door for better diagnosis and treatment of fibromyalgia through examining the health of patients microbiomes.

Promoting a healthy gut microbiome might entail incorporating a wide variety of plant-based and fermented foods with plenty of prebiotics into ones diet. Prebiotics are compounds that alter the gut microbiome to grow beneficial microbes. They are frequently absent from the current Western diet, which is characterized by large amounts of meat and processed foods.

At McGill, the Microbiome Project aims to promote good bacteria and avoid dysbiosis. Founded by Julian Russell, U2 Agricultural and Environmental Sciences, and John Weilenmann, M1 Agricultural and Environmental Sciences, the club has hosted numerous workshops on how to make all types of foodstuffs, such as hot sauces and natural wines, that are based on fermentation and therefore contain plenty of live bacteria called probiotics. The group hopes to steer clear of artificial chemicals commonly used in commercial products. They also aim to tackle other aspects of life that promote a healthy microbiome, such as exercising and sleeping enough.

We focus on various types of fermentation, as well as a lifestyle component, Russell said. A healthy body and a healthy gut go together, so we encourage people to take their exercise and sleep seriously [.] If youre not living a healthy lifestyle, [that is] going to negatively impact the good bacteria that live in you, [allowing] you to be susceptible to pathogens.

Another view: of population health management – Digital Health

In his latest column, Neil Paul, talks all things population health management, including why it can be a confusing and why is it important to local planning.

Partly in my PCN role and partly in my shadow ICP director role, Ive been spending a lot of time thinking about population health management, about big data, about risk profiling and about data quality in general.

Population health, to me, seems to be a confusing topic as it appears to mean different things to different people.

In its broadest sense it is about understanding that health is affected by many things, including the environment we live in, the choices we make, the choices that are made for us by politicians and officials and all the other socio-economic factors. This is very important and understanding the implications of policies and is vitally important if you are in a position to change or influence decision making.

Planning at a local level

There is also no doubt that planning in a broader sense is useful, interestingly I dont see much evidence of anyone doing it locally.

At a very high level, understanding how the population in an area is changing, what risk factors that population has and what problems they have or will get, should get health planners excited but they are much more focused on the now.

Most clinicians, in my experience, glaze over a bit at the mention of population health management.

Start collecting data

It seems clear to me we need to start upstream and start collecting more data. We need to take every opportunity to get basic observations as data including perhaps our other primacy care colleagues in this dentists/opticians etc.

Can we be clever and use tech without just targeting the worried well?

Going back to clinicians (whom I represent) they often want to know how to help. Just telling me Ethel is frail or is at high risk of admission is pointless as I usually know this. Yes, Id like more services to help her so mapping out numbers of people whats wrong with them and planning services to help is key however I really want to know who I can help perhaps as upstream as I can.

I dont what to know who is at most risk but whose risk I can most influence with perhaps the least intervention. Some say smoking cessation is the most cost-effective intervention, as even brief interventional advice can make a difference in year to peoples health. Yet do GPs do enough of it? Do we have enough smoking cessation services, alcohol services? Drug addiction services, weight managmenet services? Exercise on prescription, healthy lifestyle?

Do we know what medical interventions make a difference? Statins are meant to reduce your risk of a cardiovascular event by 20%. Do we think in terms of who on our list has the highest risk and have we reduced it? Have we treated to target? Have we maxed out the doses? Have we perhaps chased those that dont come and dont engage?

Should we be screening more? Should we be auditing more? Should we be harder on chasing people? Do we need to move away from just seeing people who think their sore throat needs antibiotics and give all that work to online doctors and concentrate on making people healthier?

Having the right tools

I think business intelligence (BI) and IT tools can help with a lot of this. We have some Quality and Outcomes Framework tools, but these arent perhaps enough to do this project management.

My clinical system allows me to bring up patients and see what problems they have; it doesnt easily allow me to bring up patients with groups of conditions or other characteristics and manage them as a group.

Locally my PCN is doing a heart failure project. The hospital started a heart failure clinic seeing people admitted with end stage heart failure and its been successful the readmission rates are down but to my mind its too late in the pathway. We are hoping to screen for risk factors but at moment we are making sure everyone thinks of it as a diagnosis in anyone presenting with compatible symptoms. Refers the patient for an echo and other tests, codes the ejection fraction and gets the diagnosis right, get the treatment right, record functioning in a standard way, audit our performance.

Heart failure isnt the only project Ive been working on. Ive been asking the BI people to play with what I call it the cohort problem. Effectively a multi-disciplinary team can manage 30-250 people in a year. (give or take find them, meet them, do things to them and see them again) What we need is groups where there is clear intervention that will make a difference that a team can deal with. The trick is finding these groups and putting the teams to work.

Delivering more outcomes

Collecting data and the presentation of it as useful information are key to new ways of working to delivering more outcomes with the same resources. I worry we arent spending enough time collecting good data and being agile in how we use and present it. I think as ICPs form they need to massively invest in getting people to record better data, think of innovative data sources and invest in BI teams.

The tormenter had infected her with the dreaded virus! Her dreams were shattered. After four years, she became critically sick. There were times when the sickness was in remission and her life seemed normal, when she would really forget the truth, or simply just refuse to believe it.

But time came when she slipped back into multiple infections. She had developed AIDS. Ndagire had to be admitted at Mulago Hospital for months. She painfully struggled with the sickness for three years until all she wanted was to die. She thought it was the only death that could set her free and take away her pain. But TASO always clung to her even in her desperate condition.

Today, over 30 years since she received her death sentence, life is going on well for Ndagire and she can afford to reminisce about her ordeal with nostalgia. She is an irrefutable example of health and vitality and she demonstrates no trace of her former condition. People who saw me where I was admitted for over a year can testify that a miracle truly happened.

I am completely free from AIDS and I have gone from frightened victim to an HIV dissident and a spokesperson for new views about HIV/ AIDS, Ndagire says. She attributes her good health and long life to positive living.

There are several things one can do to stay healthy and live longer and these are the things that longterm survivors like Maj. Rubaramira Ruranga have done, to stay healthy for as long as 20-30 years or more after getting infected with the HIV virus.

It is about not infecting others as well as not re-infecting yourself. It is also about having a positive attitude and a healthy lifestyle. Granted, finding out that you have HIV is not easy because it is scary and can torment you.

This is natural. The knowledge that you are HIV-positive is a transition. It is starting a new life where you must learn how to cope and focus on what is most important.

POSITIVE LIVING In the same way, when you are HIV-positive, you must learn how to cope with your new status and how to live with it. That is what is called Positive Living or Living Positively with HIV. It is a term used to describe steps taken by people living with HIV that enhance their lives and improve their health.

The importance of positive living is that it prolongs the productive lives of individuals infected with HIV, making it possible for them to contribute to the well-being of their families.
