Two ECU grad programs ranked

Two graduate programs at East Carolina University have been ranked among the best in the nation according to an annual list from U.S. News & World Report.

The rehabilitation counseling program and the Brody School of Medicine were included in the magazine's 2013 listing of the best professional and graduate schools released today.

The rehabilitation counseling program in the College of Allied Health Sciences is ranked the 13th.

"Our graduates are in high demand in substance abuse, mental health and other rehabilitation settings," said Dr. Lloyd Goodwin, professor and interim chair of the Department of Rehabilitation Studies at ECU.

"Student interns are almost always offered jobs at the end of their internship and often before completion of their internship. It is rare for a graduate to not be offered a position within one month of graduation."

The Brody School of Medicine tied for 31st overall among primary care schools this year. ECU also sent the seventh-highest percentage of its graduates, 49.6 percent, into primary care residencies between 2009 and 2011. U.S. News defines primary care as family medicine, pediatrics and internal medicine.

"We are glad to be included among some very fine and well-established schools from across the country," said Dr. Paul Cunningham, dean of the medical school. "We will continue to pursue our mission to serve the citizens of North Carolina with passion and purpose."

Heart-Pacing Devices Hold Promise for Heart Failure

(HealthDay News) -- New research provides more evidence that a new type of pacemaker/defibrillator technology may be a good option for patients with mild forms of heart failure.

An international team of researchers reports that the kind of heart "pacing" provided by new defibrillators and pacemakers can help these patients. But the research still needs to be confirmed before doctors begin embracing its findings, said a cardiologist familiar with the research.

The technology in question "is expensive, resource-intensive, and associated with increased risk of complications compared with less complex devices," explained Dr. Jeptha Curtis, an assistant professor of medicine at the Yale University School of Medicine.

Heart failure occurs when the heart fails to pump blood properly, often as a result of heart attacks, and affects an estimated 5.7 million Americans. "Patients often feel fatigue, as well as shortness of breath from fluid building up in the lungs and swelling of the legs from fluid building up there as well," said Dr. Kevin Heist, a cardiac electrophysiologist at Massachusetts General Hospital. Beaufort Memorial Hospital community seminar to focus on anti-aging debate

Think your family history of coronary disease predisposes you to a heart attack? Think again. Comprehensive lifestyle changes can dramatically alter how your genes affect you.

"People tend to be fatalistic, but that's just an excuse to continue a bad habit," said Dr. Philip Cusumano, a board-certified internist with Beaufort Memorial Lady's Island Internal Medicine. "Seventy percent of the patients who come into our office have health issues that are preventable."

As part of a new "Life, Lived Better" community series being hosted by Beaufort Memorial Hospital, Cusumano will present "New Findings in the Anti-Aging Debate" at 4:30 p.m. March 7 at Magnolia Hall in Sun City Hilton Head.

The seminar will explore major causes of aging and practical changes you can make to improve your health, wellness and longevity.

"We're sedentary, we overeat, we don't sleep enough and we're stressed," said Cusumano, formerly with the Cleveland Clinic. "It's a recipe for disaster."

Based on the best-selling book, "YOU -- Staying Young" by Drs. Michael Roizen and Mehmet Oz, Cusumano will cover six of the 14 biological processes that control your rate of aging.

"I'm going to talk about the most salient findings in the book and relay them to people in a way they can understand," Cusumano said. "I hope it will encourage them to read the book and change their lifestyles."

In most cases, the ill effects of bad habits can be reversed. Smokers, for instance, can decrease their risk of heart attack and stroke 24 hours after they quit. The risk for lung cancer returns to that of a nonsmoker 10 years after smoking cessation.

Men with early-stage prostate cancer can decrease the expression of prostate cancer-promoting genes with improved nutrition, stress management, walking and social support, according to a study by the National Academy of Science.

By controlling their blood pressure, lowering their cholesterol and giving up cigarettes, men and women who carry the gene for premature coronary artery disease can greatly reduce their risk of developing cardio-vascular problems.

"Heart attacks and strokes cause 700,000 deaths a year in the United States," Cusumano said. "That's more than 200 times the number of people that died in 9/11. The terrorist threat is nothing compared to what Americans inflict upon themselves."

One of the most effective ways to delay the aging process is to reduce your caloric intake by 15 percent. Scientists say it both cuts the risks for some diseases and slows metabolism and tissue aging.

"You can add 15 to 20 years to your life by making two behavioral changes -- reducing your stress and dramatically changing your diet," Cusumano said. "If people took care of themselves like they were supposed to, I'd be looking for another job."

Black raspberries prevent colorectal cancer

Black raspberries contain natural compounds that may significantly reduce the risk of colorectal cancer, according to a study conducted by researchers from Ohio State University and published in the journal Cancer Prevention.

Researchers genetically engineered mice to develop either intestinal tumors or a type of colon inflammation, colitis, known to increase the risk of colorectal cancer. They then fed all the mice a high-risk diet low in calcium and vitamin D and high in saturated fat for 12 weeks. Some of the mice were fed 10 percent of their daily calories from freeze-dried black raspberry powder.

Among the mice engineered to get colitis, black raspberry powder significantly reduced inflammation and cut both the number of new and total tumors by 50 percent. Among mice engineered to get intestinal tumors, black raspberry powder reduced the number of new tumors by 45 percent and the number of total tumors by 60 percent. The effect appeared to be produced, in part, by the suppression of a protein that binds to the artificially modified gene. Read more...

Anti-Aging Protein Extends Life Span in Mice, and Maybe Humans

Things are looking up for aging mice and, if this research pays off, for aging humans, too.

Researchers have found that a long-suspected anti-aging protein called sirtuin can make male mice live about 16 percent longer than average, the first such advance for mammals in a field that has thus far only offered the blessings of extended life span to yeast, nematodes and fruit flies.

The findings, by scientists at Bar-Ilan University in Israel, appear today (Feb. 22) online in the journal Nature.

Although the Israeli scientists cannot explain why female mice didn't also live longer like the males, an accompanying commentary describes the findings as bringing the field of anti-aging research to a new level of maturity.

Long road to SIRT6

Sirtuin was hot news in 1999 when researchers found that a certain sirtuin called Sir2 could extend life span in yeast by 30 percent. That was about five lifetimes ago for your typical mouse, though. Studies that followed on worms and flies have had mixed results, and some researchers have started to question whether sirtuins could control life span in more complicated life forms.

Mammals, including humans, have seven types of sirtuins, called SIRT1 to SIRT7. Scientists aren't sure what these proteins do, although there is some evidence suggesting that they might help prevent chronic diseases such as cancer and cardiovascular disease. For example, resveratrol, the nutrient found in grape skins (and red wine), has been shown to have a positive effect on heart health, and it may work by activating the SIRT1 gene to make more SIRT1 protein.

The Israeli researchers, led by Yariv Kanfi, focused on SIRT6. The team's previous work revealed that mice genetically bred to have lots of SIRT6 could get fat on rich diets yet show no signs of heart disease, fatty liver disease and other diseases associated with obesity. Previous research by other groups showed that mice genetically bred to have little SIRT6 don't live very long. And, conversely, rats living longer on a calorie-restricted diet have increased levels of SIRT6 in their blood.

So, this time the Israeli team simply decided to let the SIRT6 mice live a natural life. These male mice lived longer, about 16 percent longer on average, than regular mice kept in the same conditions. The female mice with the SIRT6 gene enhancement didn't live longer than regular mice. The researchers speculate that, considering how male mice have a higher rate of cancers compared with the females, the SIRT6 could be acting as a tumor suppressor and thus have a larger effect on male life span than female life span.

Got some time?

It could be that SIRT6 is to mice (and humans) as Sir2 is to yeast, a regulator of life span. Given the title of the Nature study, "The sirtuin SIRT6 regulates lifespan in male mice," that's what the researchers are hoping. [Scientific Tips for a Longer Life]

David Lombard and Richard Miller of the University of Michigan School of Medicine, in an accompanying News and Views article, remain positive yet generally skeptical. They note that measuring life spans in longer-lived and more complex animals can be tricky, because, for example, life spans can vary by many months for genetically similar mice in similar environments with similar feed. The current study used 245 mice, but ultimately may need thousands of mice to produce more reliable statistics.

Meanwhile, questions abound for sirtuin researchers: How is SIRT6 affecting life span at the cellular level? Why are the effects more pronounced in males than females? And how can one activate the SIRT6 gene to make more SIRT6 without the unpleasantness of a caloric-restriction diet?

This could take years to unravel. But better to be a human than a mouse.

Bipolar Drug May Spur Weight Gain, Thyroid Problems: Review

(HealthDay News) -- A new medical review finds that lithium, a common treatment for bipolar disorder, can lead to weight gain and causes high rates of abnormalities in the thyroid and parathyroid glands.

But the researchers found few signs of a link to skin problems or hair loss, and a suspected connection to birth defects hasn't been proven, according to the report published in the Jan. 20 online edition of The Lancet.

Overall, the findings reaffirm lithium's role as "a treatment of choice for bipolar disorder," two doctors wrote in an accompanying editorial.

While lithium is less popular than it was in the 1970s and '80s as a treatment for bipolar disorder, it's probably the most effective available mood stabilizer, said Dr. Bryan Bruno, acting chairman of the department of psychiatry at Lenox Hill Hospital in New York City, who was not involved with the review but is familiar with the findings.

"It remains very beneficial, and it's still a first-line agent for bipolar disorder," Bruno said.

But lithium has a variety of possible side effects, noted the authors of the review, led by Dr. John Geddes of the University of Oxford, Warneford Hospital in Oxford, England. Their analysis included 385 studies. Read more...

Plastic surgery reported up despite economy

Anti-aging procedures that don’t involve going under the knife drove the growth of cosmetic medicine in 2011, a new report from the American Society of Plastic Surgeons says.

The plastic surgery market saw  ”signicant” growth for the second year in a row, the group says, with cosmetic and reconstructive plastic surgery each up 5%.

“While the rate of economic recovery in the U.S. is still uncertain, 2011 proved to be a good year for plastic surgery,” ASPS President Dr. Malcolm Z. Roth said.

Some local plastic surgeons in Orange County, one of the cosmetic medicine capitals, see things somewhat differently. 

“Even though these statistics are derived from surveying our membership, I find them not totally credible,” said Dr. Edward Jonas Domanskis of Newport Beach. “In my experience, the total number of cosmetic surgery procedures, both surgical and non-surgical, has either decreased from the past year or stayed the same.”

Dr. John Di Saia of San Clemente and Orange took the report in stride.

“Things are looking up in the cosmetic surgery market but only slightly. 5% is not a major increase,” he said. “The larger procedures are still slow to return in large numbers. It is a sign of the economy.”

Another plastic surgeon said he thinks the statistics are spot on.

“The recent figures from the American Society of Plastic Surgeons on procedures performed in 2011 are consistent with the trends in my practice,” said Dr. Donald Altman of Irvine, adding, “… Despite the modest increase in procedures performed in 2011, consumers seeking cosmetic surgery continue to be very price sensitive.”

 The ASPS says 13.8 million cosmetic plastic surgery procedures — both surgical and minimally-invasive such as Botox or filler injections – were performed in the United States in 2011.

Also, 5.5 million reconstructive plastic surgery procedures were performed last year, up 5%, as well.

Cosmetic surgery went up 2%, with nearly 1.6 million procedures in 2011, the ASPS said.

The top five surgical procedures were:

Breast augmentation (307,000 procedures, up 4%) Nose reshaping (244,000 procedures, down 3%) Liposuction (205,000 procedures, up 1%) Eyelid surgery (196,000 procedures, down 6%) Facelift (119,000 procedures, up 5%)

Facelifts were listed among the top five cosmetic surgical procedures for the first time since 2004, replacing tummy tucks.

“We are seeing notable increases in surgical procedures, such as facelifts, that reflect the demands of an aging boomer population,” Roth said. ”However, the overall growth in cosmetic procedures is being primarily driven by a substantial rise in minimally-invasive procedures.”

Cosmetic minimally-invasive procedures increased 6%, with nearly 12.2 million procedures in 2011, according to the ASPS.

The top five minimally-invasive procedures were:

Botulinum toxin type A — mostly Botox (5.7 million procedures, up 5%) Soft tissue fillers (1.9 million procedures, up 7%) Chemical peel (1.1 million procedures, down 3%) Laser hair removal (1.1 million procedures, up 15%) Microdermabrasion (900,000 procedures, up 9%)

Soft tissue fillers like hyaluronic acid (such as Restylane and Juvederm Ultra), calcium hydroxylapatite (Radiesse), and fat injections had  some of the largest growth in the minimally-invasive market last year.

The ASPS said more than 1.3 million hylauronic acid procedures were performed in 2011, up 9%; 286,000 calcium hydroxylapatite procedures, up 36%; and 68,000 fat injections, up 19%.

Lip augmentation practically exploded – up 49%, with more than 25,000 procedures performed, according to the ASPS.

Procedures by gender will be released in upcoming weeks. But chin augmentation, a procedure the ASPS says is popular with men, nearly doubled in 2011, with 21,000 procedures performed, up 71%, according to the organization.

“I think these numbers reflect that things have pretty much stabilized. No great increases or decreases in the demand or utilization of plastic surgery. The fact that most procedures seem to be up in single digit percentage point gains reflect in my mind nothing more than statistical variation. I have a rather established practice and I appear frequently on television and despite that some months last year were slower than I would expect if things were really improving.”

Dr. Edward Jonas Domanskis of Newport Beach:

“Definitely, the surgical cosmetic procedures continue at least in my practice and Orange County, to be significantly impacted by the economy with less expendable and spendable income. I feel that this is also because consumers confidence levels are still low.”

Dr. Donald Altman of Irvine:

“Breast augmentation continues to be the most popular cosmetic surgical procedure followed by rhinoplasty. There has been a dramatic increase in breast reconstruction this past year. Patients seem to be better informed in their decisions regarding cosmetic surgery, probably reflecting their increased ability to access information online. Email and text messaging has improved my ability to communicate with patients exponentially.”
