INDIANAPOLIS (WISH) IU Health fears the Indianapolis 500, even with a reduced crowd, could lead to a spike in COVID-19 infections and calls on the race organizers to consider an alternative to a race with fans.

Indianapolis Motor Speedway on July 21 announced attendance for the 104th running of the race on Aug. 23 will be approximately 25% of capacity. Face coverings will be required for all attendees. Ticket sales ended Friday. IMS also presented its nearly 100-page plan with its guidelines and protocols to address the need to control the spread of the coronavirus.

IU Health, though, also notes the other places where gatherings could occur restaurants, bars, accommodations and more as being a concern beyond the track.

Based in Indianapolis, IU Health is the largest network of physicians in Indiana. Its a partner with Indiana University School of Medicine, one of the nations leading medical schools.

We appreciate Indianapolis Motor Speedways detailed plan to safely host the 104th Running of the Indianapolis 500. We have concerns about the risks of infection beyond the scope of the IMS plan, including social gatherings, travel, restaurants, bars, accommodations and other event-related activities. This could lead to a spike in COVID-19 infections as we continue to see cases and hospitalizations increase every day. Until we sustain better control of this virus and its spread, we strongly encourage IMS to consider an alternative to running the Indy 500 with fans in August.

Indianapolis Motor Speedway responded late Wednesday night in an email to WISH-TVs newsroom:

IU Healths viewpoint is inaccurate and premature.

IMS has put together a plan that will serve as a model for the careful and conscientious management of events during this pandemic. It is informed by expert advice and reflects our belief that with carefully considered precautions in place, its not necessary or appropriate to return to shutting down events that are important to our state and city.

Despite attempts by our organization to engage with IU Health leadership, we have not received a single suggestion from IU Health as to how our approach might be improved. In contrast, public health officials from our city and state have engaged with us for months. They have given us numerous suggestions that we have implemented, and they have approved our plans subject to continued review.

We remain open to meaningful input from IU Health that will improve our detailed operational plan as we move closer to Race Day.

Coronavirus restrictions in force for Marion County since July 23 include all bars closed, and restaurants with 50% indoor capacity and outdoor seating with social distancing. Social gatherings including weddings, banquets, parties and club meetings are limited to 50 people. All other forms of gatherings, meetings, or special or seasonal events must be limited to 250 people or fewer, unless the citys Public Health department approves a risk mitigation plan for a special or seasonal event that anticipates more than 250 attendees.

The health networks statement came on the same day Gov. Eric Holcomb extended the Stage 4.5 reopening plan through Aug. 27 because of high seven-day average positivity rates for the coronavirus. On July 22, Holcomb initiated a statewide mask mandate. Indianapolis has been under a mask mandate since July 9.

With updated information from the Indiana Department of Health on July 28, this timeline reflects updated tallies of deaths and positive tests prior to that date.

Coronavirus: Hydroxychloroquine use in NZ studies abandoned due to lack of evidence and Covid-19 cases – Stuff.co.nz

A study found no evidence of benefit from a malaria drug promoted as a treatment for coronavirus. Hydroxychloroquine did not lower the risk of dying or needing a breathing tube in a comparison that involved nearly 1400 consecutive patients in New York.

A New Zealand Covid-19 study looking at the efficacy of weekly prophylactic hydroxychloroquine regimes for high risk frontline healthcare workers has abandoned the use of the controversial drug.

Based on the lack of evidence around the effectiveness of the anti-malaria drug which was controversially promoted by US President Donald Trump and Brazilian President Jair Bolsonaro the study wont move onto recruitment and trials.

The drug, which has been suggested as a possible Covid-19 treatment, is yet to be used in trials in New Zealand. It was to be included in three studies that received funding from the Health Research Council (HRC) and Ministry of Health in April.

Given the rapidly emerging evidence on potential Covid-19 treatments from other international trials, all three trials have adapted significantly and have removed hydroxychloroquine as a candidate for treatment or prevention, the council spokesperson said.

The drug has been at the centre of many global studies. Researchers in the US found it to be ineffective against mild Covid-19 cases, and the World Health Organisation (WHO) withdrew it from its Solidarity Trial after finding it produced little or no reduction in mortality rates of hospitalised cases.

The front line healthcare workers study will be funded until its set-up phase. The remaining $300,000 in funding hasnt been reallocated.

Completing the set-up phase would enable the researchers to be in a position to reinitiate a healthcare worker prophylaxis study in [the] future, should there be a major second wave of Covid-19, or an influenza pandemic, the spokesperson said.

If this were to happen, the study would recommence with an alternative medicine, not hydroxychloroquine.

The small amount of Covid-19 cases in New Zealand has also impacted progress of two other HRC-funded studies REMAP-CAP COVID and ASCOT.

Low case numbers in New Zealand has meant recruitment to the two patient trials has not begun, the spokesperson said.

As such, hydroxychloroquine has not been, and will not be, used in New Zealand Covid-19 patients.

REMAP-CAP COVID is an adaptive study that focuses on critically ill Covid-19 patients requiring intensive care. It is open in New Zealand but hasnt begun recruitment.

ASCOT enrols hospitalised Covid-19 patients who dont need intensive care. It is yet to open and recruit participants. The use of lopinavir/ritonavir, another drug the WHO withdrew from its study, has also been removed.

Hydroxychloroquine wont be used in any New Zealand studies.

Instead of using hydroxychloroquine, researchers in New Zealand and Australia have pivoted the two existing studies to focus on convalescent plasma.

Convalescent plasma involves transfusing antibodies in the plasma of recovering patients into newly infected patients. This treatment method was previously used during the 2009 influenza pandemic, and SARS and MERS outbreaks.

Over 20,000 patients in the United States have safely received convalescent plasma for Covid-19. While conceptually attractive, we still need clinical trials to demonstrate efficacy for patients with Covid-19, said ASCOT principal investigator, associate professor Steven Tong from Australias Doherty Institute.

The REMAP-CAP COVID study is also evaluating other therapies such as immune-modulation treatments, anti-coagulation strategies, vitamin C, and ventilatory strategies.

"This means that patients within these trials are randomised to treatments that are more likely to be beneficial, while interventions determined to be inferior are discontinued, the HRC spokesperson said.

They explained that both the council and the Ministry of Health see the importance of these studies progressing, especially if community transmission becomes an issue again.

In the absence of a vaccine, the clinical trials provide access to the latest emerging evidence on prophylaxis and treatment.

There are 22 active Covid-19 cases in managed isolation and quarantine facilities. None are receiving hospital-level care.

No confirmed cases have been hospitalised in New Zealand for a considerable amount of time, and over 90 days have passed since there was last evidence of community transmission.

Kerry Washington routinely shares quick yoga sequences and even a breathwork meditation flow with her 5.9 million followers on Instagram. The Little Fires Everywhere actress also often gives a shoutout to her own teachers who conduct wellness classes on IG Live. Across almost every platform, from Instagram (which launched a tool called Guides in mid-May that directs users to well-being content) and Wanderlust TV to the Mind-Body app, yogis, gurus and healers are in high demand to provide virtual sessions in meditation, sound baths and breathwork, practices that date back thousands of years.

"There's more of an adoption of these practices," says filmmaker Liz Goldwyn, founder of digital platform The Sex Ed and a fan of sound baths, which are sessions in which ambient music is played to help reach a meditative state. With so many people spending so much time at home, she continues, "We need to sit in these uncomfortable moments and not push away feelings of anxiety."

Goldwyn, who is teaching herself to play Tibetan singing bowls, also sees these modalities as a way of fueling up for current challenges, including battling social injustice. "Self-care is an act of revolution. To go out and fight and see real change, it's important to have these tools in our arsenal."

Some practitioners are starting to offer in-person sessions, such as L.A.-based writer-director Sebastian Siegel (sebastiansiegel.com), whose two-hour Guided Lucid Dreaming events incorporate sound, breath practice and meditation. (Guests who've been known to attend include Mena Suvari, Mariel Hemingway and Frances Fisher, who are in Siegel's upcoming film Grace and Grit.) Attendance is by referral to these small-group meetings, held outdoors at a private home. Says Siegel, "Any time we are going on an adventure exploring internal terrain is time well spent."

"There were no drop-in meditation studios that were secular, modern and clean like an art gallery," says the former fashion editor. When California's lockdown went into effect this spring, Schwartz and her team quickly launched live-streamed classes. Apple recently helped redesign the Unplug Meditation app, which features one-minute to one-hour classes from a global roster of carefully vetted experts in practices including sound healing, sleep training and stress management. Merck to start pivotal study of controversial remdesivir alternative in September – BGR

As promising as vaccines may seem right now, they cant help patients who are already infected with the novel coronavirus. The world still lacks a COVID-19 cure that can speed up recovery, prevent complications, and reduce the risk of death. Remdesivir helps in some cases, but its expensive, scarce, and difficult to make. Dexamethasone is cheap, but isnt enough to save every patients life either. Blood thinners can reduce the risk of coagulation and eliminate some complications, but theyre not a COVID-19 cure by themselves.

Several other promising antibody therapies are in the works, and some of them are already being studied in human trials. Merck has an antiviral drug in testing, and the company is expected to start large pivotal studies in September for an oral medicine to rival remdesivir. However, theres some controversy around this potential coronavirus treatment.

We will be embarking, probably in September, on very large pivotal studies, and so those are going to be the important ones, Merck president Roger Perlmutter said during the companys Q2 earnings call, via CNBC. The drug is known as MK-4482, and its being developed with Ridgeback Biotherapeutics. The report notes that Merck doesnt want to dribble out data on the drugs performance in Phase 2 trials, according to Perlmutter. Thats certainly strange.

The good news about MK-4482 is that because its an oral drug given in capsules, it can be easily administered from the time that people have symptoms, Perlmutter said. Its unclear whether theres any bad news. The executive didnt say how effective the drug is, but revealed that the goal of future studies would be to prove that the drug can reduce the recovery time and prevent complications.

The company also has a couple of vaccines in the works that will hit clinical trials in the near future.

MK-4482 was the most advanced COVID-19 therapy from Merck, per a C&EN report in late May. But MK-4482 wasnt always known as, well, MK-4482. BioSpace says the antiviral candidate was known as EIDD-2801 before being renamed under Merck. CNBC makes no mention of the compounds controversial history.

Getting back to C&EN, the May report said that Ridgeback was testing EIDD-2801 at the time in Phase 1 trials. The origin of the drug is Emory University, however:

The most advanced of Mercks three new programs is EIDD-2801. The small-molecule drug candidate was developed before the pandemic by scientists at Emory University looking for molecules that could inhibit the replication of several viruses. In March, Emory scientists discovered that the compound could also inhibit the replication of SARS-CoV-2, the virus that causes COVID-19, in human cells grown in the lab.

Emory licensed it to Ridgeback, which then partnered with Merck.

EIDD-2801 targets a viral enzyme called RNA-dependent RNA polymerase that stops the virus from copying its RNA genome, effectively preventing it from multiplying inside a human cell after infecting it. If effective, that would be a huge step in treating COVID-19. Remdesivir goes after the same enzyme, but Gilead Sciences medicine needs to be administered intravenously. Mercks drug can be taken orally.

This brings us to the controversy around the drug as explained by C&EN (emphasis ours):

EIDD-2801 has been viewed as a potential competitor to remdesivir, although a contentious one, because similar compounds are mutagenic in animal studies, meaning they produce birth defects. Rick Bright, who was removed from his position as head of the US Biomedical Advanced Research and Development Authority (BARDA) in April, was reluctant to provide funding for the drug for this reason, according to an 89-page whistleblower complaint Bright filed after being fired. Mercks investment in EIDD-2801 can be seen as a vote of confidence in the compound.

Brights concerns began in November 2019, more than one month before China disclosed the first COVID-19 cases in Wuhan ScienceMag has that story:

Raymond Schinazi, an Emory University chemist who has extensively studied the active ingredient in EIDD-2801 but has no connection to DRIVE, notes that his former pharmaceutical company, Pharmasset, abandoned it in 2003 after discovering its mutagenic properties. Schinazi says the small chemical tweaks made to increase the ingredients bioavailability and transform it into EIDD-2801 are unlikely to change its mutagenicity. Thank goodness someone is raising the red flag, about EIDD-2801, Schinazi says. You dont develop a drug thats mutagenic. Period.

Before a drug is approved, regulators have to see that its effective and safe after passing Phase 3 testing. The same applies to MK-4482. Rick Bright may have had concerns about the drug, but that doesnt make it unsafe for use. That said, the lack of Phase 1 results from Merck and the name change do seem strange. All the other promising drugs and vaccines that are in testing produced results for the public early during their human trails.

Emory did produce results in early April for preclinical trials that showed when used as a prophylactic, EIDD-2801 can prevent severe lung injury in infected mice. That study is available at this link.

Compound in pickled capers could benefit the heart and brain – Medical News Today

A new study shows that a compound in pickled capers activates channels that are important for the activity of the heart and brain. The findings could potentially lead to the design of new drugs for epilepsy and arrhythmia.

People have practiced traditional or folk medicine which the Western world generally considers alternative medicine for centuries, and it remains the mainstay of healthcare in many countries.

Much traditional medicine relies on the use of plants, which have been the basis of medical treatments for thousands of years. Sometimes, researchers examine this traditional knowledge, interpret it in modern scientific terms, and then translate it into Western medicine.

Aspirin, for example, comes from willow plants and has played a role in medicine since ancient Egyptian times, long before people understood its mechanism of action.

A new study from the University of California, Irvine School of Medicine shows a similar story for capers, which people around the world consume and also use in traditional medicine.

The study, which appears in Communications Biology, finds that a compound in pickled capers activates potassium channels that regulate the activity of the heart and brain.

The researchers say that their findings could aid the design of new drugs for epilepsy and arrhythmia (abnormal heart rhythms).

The authors of the study note that humans have been eating capers for more than 10,000 years. Findings in soil deposits in Syria and late Stone Age cave dwellings in modern day Greece and Israel have shown this to be true.

The ancient Roman cookbook Apicius also mentions capers. People continue to use them in traditional medicine for their antihelminthic, anticancer, antidiabetic, and anti-inflammatory properties and their possible circulatory and gastrointestinal benefits.

The research team behind the current study has now shown how capers act on the body, which includes activating a potassium channel important for heart and brain activity.

The researchers started looking at capers based on the results from a screen of plant extracts. They screened a range of extracts for activity on a family of potassium channels called KCNQ channels.

The KCNQ family of potassium channels plays several important roles in the body, including regulating the heartbeat, the contraction of muscles, and the function of the gastrointestinal tract. Their dysfunction is associated with certain diseases, including diabetes, arrhythmia, and epilepsy.

The researchers used an extract from pickled capers, the most common form in the United States, and found that a 1% extract was able to activate the channels.

The pickling process causes the formation of quercetin, which is the compound that activates KCNQ channels.

Further experiments with quercetin found that it binds to a part of the channels responsible for sensing electrical activity. This binding causes the channels to open when they would otherwise be closed, which may underlie the medicinal properties of capers.

Although capers are the richest natural source of quercetin, the authors note that other plant-derived compounds, such as E-2-dodecenal (present in cilantro), are more potent activators of KCNQ channels.

However, the authors also found that these compounds can work together. So, while eating capers alone may not have a significant effect, eating them together with other specific foods, such as cilantro, could increase their benefits.

Quercetin is also very common in food, being present in apples, berries, celery, chili peppers, and onions, as well as green tea and red wine. The compound may, therefore, accumulate in the body over time to provide benefits. Research has also shown that adding quercetin to food is safe, although this practice does not currently exist in the U.S.

Outside of food, the researchers say that their findings could help medicinal chemists design new drugs.

Increasing the activity of KCNQ channels in different parts of the body is potentially highly beneficial. Synthetic drugs that do this have been used to treat epilepsy and show promise in preventing abnormal heart rhythms.

Researchers are also investigating capers for their potential benefits against cancer, diabetes, and various inflammatory, circulatory, and gastrointestinal diseases. Understanding how capers act on the body on a molecular level could, therefore, lead to a wide range of new treatments.

From iridology, a detailed analysis of eyes, to selling herbal tonics, Mrs Dunn has been working every day, except Sundays, for 55 years.

Originally from Melbourne, Mrs Dunn said she started learning about herbs from her grandfather and has been operating a herbal clinic since 1965.

In 1975, we came over to Perth and we started a clinic in Scarborough, she said.

We bought this (Wanneroo) property in 1983, and then we moved here.

The property was developed to grow herbs used for formulas but Mrs Dunn said it took some effort for the family to start the business.

This (property) was just horse paddocks, there was nothing here except three trees that were out here, she said.

When we started here, the soil was extremely low, you could pick it up and itll blow away.

Then gradually by using mulch and straw...we built up the soil.

Dunns Herbal clinic sells standard remedies in its shop but Mrs Dunn also conjures up formulas specific to a customer.

We have things where when the person comes for their iridology, its an individual mixture, Mrs Dunn said.

Its a formula only for them and every time they come, the formula changes slightly because of the seasons.

Mrs Dunn said most customers found her through word of mouth and many had stayed for decades.

Mrs Dunn was featured on the front page of sister paper the Joondalup Times in an article about her business in February 1995.

A lot of people have been with me for 20 years, she said.

Ive got one man who, for over 30 years, has been coming to me and hes in his 80s.

Mrs Dunn will hold a 10-week herbal course in September after having to postpone it due to the COVID-19 pandemic.

The dilemma Last week I discovered that my wife of 40 years has had breast cancer for two years. She claims she kept it a secret from me in order not to upset me or our adult children. A few days later, we discovered the cancer has progressed to her bones.

I have to come to terms with the fact that I will probably soon be a widower, but also with the emotions surrounding my exclusion from the illness. Were it not for the fact that these last two years have probably been our best ever, this would feel like the ultimate betrayal of trust. Im convinced that one of the reasons she hid this from me was because she treated it exclusively with alternative medicine and knew I would oppose this.

Many of my emotions are unacceptable in the context of illness and bereavement. Anger at her lack of trust in me isnt a good companion to sadness. My tears alternate with resentment and the mixture is difficult to reconcile. Whereas she wants to stay positive, I know that the prognosis is quite the opposite. Added to my disarray is the fact that she wants no one else to know about her situation. To avoid lying and faking, I have cut off activities with my closest friends, so I now have to deal with this alone. Any advice will be appreciated.

Mariella replies Im so sorry. This is a sad story for all concerned and particularly, of course, your wife. I totally understand your feelings of betrayal and bewilderment. Were the circumstances different or the prognosis less bleak I would definitely advise that you express your frustration in no uncertain terms. However, based on what youve told me, I dont think there is room for anyone elses feelings but hers at the moment.

Until youve had a brush with your own mortality its impossible to know how you will react. Pledging our troth, promising to care for each other in sickness and in health, are well meant in the moment and indicative of our best intentions. To become a couple is seen as a chance to become one, and described as such in most marriage services, but, as any divorcee will tell you, that state of elevated congress is rarely achieved and nigh on impossible to sustain. For most of us our lives will be lived out in tolerable companionship. Even in the closest seeming couples most of what we think and feel would come as a total surprise, shock or indeed betrayal to our dearly beloveds. I say all this to try to put your wifes behaviour into some context, because I am pretty sure that quite a lot of your time at the moment is spent scrambling around for an explanation.

Her choice to keep her illness from you is puzzling, but there are mitigating factors that we have to try to understand in order to see her side of the picture. While perhaps misplaced, the sheer bravery of enduring these last two years without burdening anyone with her diagnosis is impressive. Its not the right thing to do, but it takes an enormous amount of courage not to seek out the support of those closest to you when the bottom falls out of your world. While I entirely empathise with your feelings, I do have to highlight the price in loneliness and grief your wife must have paid for that decision.

There are no unacceptable emotions in the face of illness and bereavement, neither for you nor for her. I suspect that, despite the courage its taken to conceal what is going on, its fear that prompted her to stay silent. The most agonising part of this is that it really wont help either of you to rake over those resentments. We are all able to play an important role in the lives of those we love but, ultimately, even the most intimately entwined couples make their own choices. Your wife may have cause to regret the course shes embarked on in terms of treatment, but living with our decisions is part of adulthood, and even more so when it comes to life and death.

As youve said, the prognosis doesnt sound good, but shes also right in being allowed to choose her narrative, and her desire to stay positive has much to recommend it. That said, demanding total secrecy borders on the inexcusably selfish. Others need to be allowed to prepare themselves for the brutal battle ahead and the possibility of loss. The first to be considered alongside you, are your children. I lost my father overnight literally at the age of 15. It was world-shattering and Id swap anything for the opportunity to prepare and say goodbye rather than suffer that absolute, unfathomable disappearing act.

Your wife may not want attention and may believe that the best way is her way, but as a parent you cant deny your kids the right to walk with her as far as they can. Id also encourage her to get in touch with an organisation such as Macmillan, who can offer help and advice. What your wife is going through needs to be recognised and reconciled, not only for those who will live on but also for her sake, to take as much weight as you can off her shoulders as she walks a road none of us will escape.

The Indian wellness industry growing at 12 per cent per annum is expected to get a further boost in the coming years with the Governments increased focus on health and fitness.

Since 2014, the Central Government has been laying emphasis on alternative systems of medicine and focussing on a holistic approach to meet emerging health challenges. The Government set up the Ministry of AYUSH (Ayurveda, Yoga, Unani, Siddha, and Homoeopathy) to promote the country's ancient and traditional healing and wellness methods. Some of the techniques like Panchakarma of Ayurveda has become world-renowned for its benefits in preventive healthcare and its treatment of many chronic lifestyle disorders. Due to these initiatives, India has become the second-largest exporter of AYUSH and herbal products in the world today.

The Indian wellness industry, estimated to be around 49,000 crore, has the presence of several large players. However, the sector is dominated by micro, small, and medium enterprises (MSMEs) capturing around 80 per cent of the market share.

The sector is, however, marred by several challenges today. As compared to the global wellness industry, which is estimated to be around $4.2 trillion, the Indian wellness sector commands a minuscule fraction and has a long way to go. One of the biggest impediments to unlocking the true potential of the segment is the lack of adequate infrastructure and an acute shortage of skilled human resources.

It is expected that by 2030, the country would require 2.07 million more doctors to take care of its increasing population. Additionally, as compared to the global average GDP spend of 6 per cent, India spends just around 1.3 per cent of its GDP on healthcare. The Government has set a target of increasing the spending to 3 per cent of its GDP by 2022. However, if the Government aims to achieve its sustainable development goals, then funding for the sector needs to go up by 20-25 per cent for the next five years.

The Centre, since the last few years, has made notable changes to revamp the wellness sector. It has begun transforming the existing 1.5 lakh sub-health centres and primary health centres across the country, into health and wellness centres to provide comprehensive primary health care and establish a holistic wellness model.

A budget of around 3,400 crore has also been earmarked to be spent over the period of the next five years, to help set up and strengthen Ayush Wellness Centers under the National Ayush Mission. There will be several benefits to these wellness centres. For instance, there would be a reduction of the burden on the secondary and tertiary healthcare facilities, especially given the rising stress on this health infrastructure amidst the unfolding crisis. The out-of-pocket expenditure would also fall due to the self-care model.

As mandated by the Governments policy think tank, NITI Aayog, these initiatives would also help in the integration of AYUSH along with the Sustainable Development Goal of the Central Government that is, good health and well-being for all its citizens.

India should focus on creating additional awareness on health and wellness in association with the Ministry of AYUSH, especially during challenging times like the coronavirus pandemic. Also, emphasis on initiatives like Make in India and Start-up India is going to give a further boost to the sector in the coming few years.

Now researchers led by James Bibb, Ph.D., professor of surgery at the University of Alabama at Birmingham, suggest using a broader lens of post-translational modification analysis to identify new biomarkers of cancer drivers that may allow a much more precise prediction of patient responses to treatments. In a study published in Proceedings of the National Academy of Sciences, they demonstrate this diagnostic alternative for neuroendocrine neoplasms driven by an aberrantly activated protein kinase called Cdk5.

Our findings show that precision medicine is a realizable goal for the treatment of rare and recalcitrant cancers, said Angela Carter, Ph.D., principle author of the paper and a postdoctoral fellow in the Bibb lab at UAB.

The same approach, Bibb said, could be adapted to identify markers of other tumor drivers. As the number of cancer types in which Cdk5 is implicated is rising, and as drugs with the potential to selectively target Cdk5 are moved toward the clinic, the translational potential of this diagnostic-therapeutic coupled system will be broad.

UAB researchers are now in collaboration with the National Cancer Institute to adapt the biomarkers the team discovered into a clinically applicable diagnostic assay, and move new drugs targeting these pathways to clinical trials.

Cdk5 is a cyclin-dependent kinase that phosphorylates target proteins as part of signaling sequences in cells. Cdk5 activity is mostly restricted to neuronal cells, where it is important for central nervous system development and cognitive processes such as learning and memory.

In the PNAS paper, Carter, Bibb and colleagues first examined multiple samples of neuroendocrine tumor and cancer types and found that Cdk5 and its activators p35 and p25 characterized a large portion of all neuroendocrine neoplasms. Also, a Cdk5 inhibitor blocked growth of the neoplasms, showing that Cdk5 activity is a major contributor to the growth of these cancer cells.

To find biomarkers, the researchers first used a unique mouse model of medullary thyroid carcinoma, or MTC, whose growth of the tumor can be turned on or off. They then looked for global differences in phosphorylated peptide fragments between arrested and growing MTC. The 50 most highly upregulated phosphorylation sites in growing MTC were examined further, using short interfering peptides that would block phosphorylation. Fifteen interfering peptides were found to inhibit growth of neuroendocrine cancer cells but not normal fibroblasts, which identified the downstream targets of Cdk5 needed for cancer cell growth. The 15 targets of the Cdk5 signaling cascade were shown to come from proteins associated with common cancer mechanisms.

Phosphorylation state-specific antibodies were generated to detect six of those sites and two other sites previously identified as targets of Cdk5. Six of the eight sites showed dose-dependent reduction by a Cdk5 inhibitor in a human neuroendocrine cancer cell line, meaning decreased phosphorylation of the proteins the peptides came from. Our overall data demonstrated that phosphorylation of these six proteins is critical in driving these cancers and is dependent upon Cdk5 activity. It also suggested that these phosphorylation sites could serve as biomarkers for many types of Cdk5-driven neuroendocrine tumors, Bibb said.

Three showed significant elevation in a cohort of human MTC patient tumors and a positive correlation with Cdk5 expression; another was increased in just a small portion of tumor samples but had a positive correlation with Cdk5 expression. Nearly three-fourths of patient tumors tested showed elevated Cdk5 levels; but only one-fifth had elevation of all four biomarkers of Cdk5 pathway activity, emphasizing that presence of a protein does not correlate completely with function of that protein.

The researchers then showed that Cdk5 inhibition blocked the growth of tumors in distinct MTC animal models. They then tested patient-derived xenograft models for presence of the putative Cdk5 biomarkers, and showed that growth of two biomarker-positive cancers was reduced by treatment with the Cdk5 inhibitor, while tumor growth of the two biomarker-negative cancers was not. These results strongly supported the ability of the biomarkers to predict responsiveness to anti-Cdk5 therapy.

However, tumor regression was not seen, and higher levels of the Cdk5 inhibitor were quite toxic. So the researchers tested a biomimetic nanoparticle-based drug delivery system called leukosomes, or LKs, which were generated from a combination of synthetic phospholipids and leukocyte membrane extracts. LKs have the ability to target the inflamed environment of a tumor, and their leukocyte membrane proteins camouflage them from removal by the immune system.

The LKs showed increased localization at MTC tumors in mice, where they could deliver a lower dose of the Cdk5 inhibitor that had the same effectiveness as a higher, toxic dose of free Cdk5 inhibitor.

The current study reveals that Cdk5 is likely a contributor to at least a portion of all neuroendocrine tumor types, Bibb said. This study also identifies a set of phosphorylation-based biomarkers, which indicate that not only are Cdk5 pathway components present, but Cdk5 is actively modulating the signaling network and regulating cancer physiology.

To be useful clinically, antibodies to detect these biomarkers will need to be developed into an easy-to-use assay that allows reliable quantitation of biomarker levels in patient tumors, preferably in relatively small samples such as core biopsies.
