A recent US study of people treated for cancer as children from the 1970s to 1999 showed that although survival rates have improved over the years, the quality of life for survivors is low. It also showed this was worse for those who were treated in the 1990s.

About 70% of childhood cancer survivors experience side effects from their treatment, including secondary cancers. And as survival rates improve, the worldwide population of childhood cancer survivors is growing.

Side effects cause stress for survivors and families and increase demand on health systems. But an emerging area of medicine, nanomedicine, offers hope for better children's cancer treatment that will have fewer side effects and improve quality of life for survivors.

What is nanomedicine?

Nanomedicine is the application of nanomaterials, or nanoparticles, to medicine. Nanoparticles are a form of transport for drugs and can go places drugs wouldn't be able to go on their own.

Nano means tiny. A nanometre (nm) is one-billionth of a metre. Nanoparticles used for drug delivery are usually in the 20 to 100 nanometre range, although this can vary depending on the design of the nanoparticle.

Nanoparticles can be engineered and designed to package and transport drugs directly to where they're needed. This targeted approach means the drugs cause most harm in the particular, and intended, area of the tumour they are delivered to. This minimises collateral damage to surrounding healthy tissues, and therefore the side effects.

The first cancer nanomedicine approved by the US Food and Drug Administration was Doxil. Since 1995, it has been used to treat adult cancers including ovarian cancer, multiple myeloma and Karposi's sarcoma (a rare cancer that often affects people with immune deficiency such as HIV and AIDS).

Currently, there is a stream of new nanomedicine treatments for adult cancers in clinical trials (trials in humans), or on the market. But only a limited number of these have been approved for children's cancers, although this is arguably where nanomedicine's strengths could have the most benefit.

How does nanomedicine work?

The nanoparticle drug-delivery systems can work in different ways. Along with carrying the drug for delivery, nanoparticles can be engineered to carry specific compounds that will let them bind, or attach, to molecules on tumour cells. Once attached, they can safety deliver the drug to the specific tumour site.

Nanoparticles can also help with drug solubility. For a drug to work, it must be able to enter the bloodstream, which means it needs to be soluble. For example, the cancer drug paclitaxel (Taxol) is insoluble so has to be dissolved in a delivery agent to get into the blood. But this agent can cause allergic reactions in patients.

To overcome these issues, chemists have developed a nanoparticle out of the naturally occurring protein albumin. It carries the paclitaxel and makes it soluble but without the allergic reactions.

Tumours commonly have disordered and leaky blood vessels sprouting through and off them. These vessels allow chemotherapy drugs to readily enter the tumour, but because chemotherapy molecules are so small, they also diffuse through the vessels and out of the tumour, attacking surrounding tissues. Nanoparticles are larger molecules that get trapped inside the tumour, where they do all the damage.

Once they have delivered their drug cargo to cells, nanoparticles can be designed to break down into harmless byproducts. This is particularly important for children who are still developing.

Types of nanoparticles

Nanoparticles vary in characteristics like shape and size. Researchers need to match the right nanoparticle to the drug it's to deliver and the particular tumour.

An array of nanoparticle structures are currently being engineered. One example of an interesting structure is the shape of a DNA origami. Because DNA is a biological material, nanoparticles engineered into DNA origami shapes won't be seen as foreign by the immune system. So these can transport a drug to diseased cells while evading the body's immune system, therefore lessening the side effects of drugs.

Another example of nanomedicine structures are polymeric nanocarriers. We have recently identified a gene that promotes the growth of tumours, cancer spread and resistance to chemotherapy in pancreatic cancers.

We used a nanomedicine called a polymeric nanocarrier and combined it with a drug that silences the cancer gene. We packaged this up to form a nanomedicine and delivered the drugs into the tumour.

These nanomedicines reduced the expression of the cancer gene, blocked tumour growth and reduced the spread of pancreatic cancer. But we also showed that polymeric nanocarriers can be combined in the lab with other gene-silencing drugs. This means the method can be used for a range of other gene-based cancers.

How can nanomedicines help treat kids' cancer?

In standard treatment for children's cancer, chemotherapy drugs are often prescribed at the maximum tolerable dose for a child's age or size, based on adult dosages. But children aren't small adults. The processes underlying children's growth and development might lead to a different effect and response to a chemotherapy drug not seen in adults.

Also, if a child becomes resistant to a drug and they're on the maximum tolerable dose, there's no scope to increase it without toxic side effects. By packaging up drugs and moving them through the body directly to diseased cells to reduce collateral damage, in theory, nanomedicine allows higher doses of drugs to be used.

Nanomedicine has great potential to safely treat children's cancer. However, it is currently stymied by too little research. About two-thirds of research attention in nanomedicine therapeutics, of more 250 nanomedicine products, is focused on cancer. Yet this isn't translating into new cancer treatments for children coming to market.

But we are making progress. Our work is exploring the design of nanoparticles to deliver gene-silencing drugs to treat the most common brain cancer in children medulloblastoma.

We're also working on nanomedicines for other significant childhood cancers. These include drug-refractory acute lymphoblastic leukaemia, the most common childhood cancer, and neuroblastoma, the cancer that claims more lives of those under five than any other.

The Fat Fad: Low-carb ketogenic diet catching on in Yakima – Yakima Herald-Republic

For dinner one night last week, Mandy Hale laid out ingredients for a pizza: bacon, sausage, onion, ricotta cheese, eggs, butter, almond flour. For dessert, she made a flourless strawberry shortcake.

This is the ketogenic diet, or keto for short the latest health craze to sweep the nation. And yes, it includes plenty of bacon.

From her office in Yakima, Hale has built an online community of people seeking advice and solidarity in following the high-fat food plan, where the majority of daily calories come from fat, and almost no carbohydrates.

In her own experience with the diet, Hale lost roughly 60 pounds in less than four months, lowered her triglycerides and got her hormones back in balance.

She has since leveraged her experience into a new health coaching business.

I just started having people contacting me going, What the heck are you doing? I want whatever youre having, Hale recalled.

But like any diet plan, keto is not a panacea for all ailments, physicians and licensed dietitians say. And its not something to enter into lightly.

Its not something that you can say, Im going to do this for a couple months and lose some pounds, Yakima dietitian Katie Thorner said.

It isnt something you can dabble in. Its something you actually need to know what youre doing to actually be effective in its application.

The ketogenic diet works by sending your body into ketosis, which causes it to burn fat for fuel.

Our body will first use glucose or sugar as energy, for our muscles, our brain. Thats No. 1, explained Rocio Petersen, a dietitian with Yakima Valley Farm Workers Clinic. Plan B, say, in starvation mode, (if youre) not getting enough food or fuels, our liver can use fat and some proteins, and will start using that as energy. Its essentially our backup.

Ketosis kicks in after two or three weeks, but Petersen cautioned that the switch is not pleasant at first: Thats starvation mode. You wont feel very good those first few days, if you are trying a ketogenic diet.

Keto proponents sometimes call this keto flu, as your body takes a week or so to adjust to no sugar.

The ketogenic diet requires some math: 60 to 80 percent of a persons daily calories are supposed to come from fat; 15 to 35 percent come from protein; and 5 percent or less from carbohydrates, including vegetables.

That 5 percent translates to only about 20 or 30 grams of carbs.

The average person consumes 30 to 75 grams of carbs in a single meal, Petersen said. To limit to just 30 grams, That would be, all day, you ate one-and-a-half apples, and thats all the carbohydrates you had, she said.

People who follow a ketogenic diet use a lot of coconut oil and butter in their cooking, along with olive oil and avocado oil, but not highly processed canola, vegetable or seed oils.

Keto recipes include significant amounts of avocado, eggs, almond or coconut flours, cauliflower (to replace carbs like potatoes or rice), cheese, beef, chicken and fish.

Fruit is minimal, though berries are typically an acceptable dessert. No added sugars; ketogenic cooks use low-carb sweeteners instead.

This diet doesnt have to be restrictive, Hale said. Through sharing her experience with others, shes decided to pursue a degree in nutrition and has set up an office from which she offers fat-fueled health coaching and meal plan help. This is the diet I tell people you can have bacon and your cake, too, just make your own cake with better ingredients, and sometimes it tastes better than the original.

Also, for people who have stayed in ketosis long enough to become fat-adapted which may take a couple months, Hale says indulging in a carb-heavy slice of cake every now and then wont send them back to square one.

(If, however, they go on a weeklong sugar binge and fall off the wagon, she also helps coach people into getting back on track.)

In the Yakima area, Hale has been talking with some local businesses about offering keto-friendly food.

Sundance Espresso in Selah, which occupies the former North Town building, has started offering cheese-and-salami snack packs as well as a salad with salami, bacon, egg and ranch dressing. Theyre also looking to serve coffee drinks with a dollop of coconut oil or Kerrygold butter melted in, for the added fat.

Owner Tim Lantrip is trying the diet himself.

A lot of customers were asking if we would be willing to provide keto-friendly foods, especially something thats like a grab-and-go type food, he said. One thing for me that makes sense with the keto diet, is it eliminates all the sugar. To me it seems like a lot of customers that are doing the keto diet have lost a lot of weight.

She started the keto diet at the start of 2017, and also started going to the gym to do high-intensity interval training. Shes lost weight and seen an improvement in muscle tone, but says the biggest difference is that she doesnt feel bloated anymore and her digestion has improved.

Diet science can be a maddening topic because it seems to flip-flop on an annual basis. A quick Google search for saturated fat will yield an endless list of contradictory results: American Heart Association recommends limiting saturated fat. Everyone was wrong: Saturated fat can be good for you. And so on.

Whether the keto diet is right or wrong depends on the individual and his or her goals, dietitians say.

Where the ketogenic diet is most proven is in targeted or therapeutic uses, Thorner said. Its good for people with seizure disorders, such as epilepsy, because ketones (the byproduct of ketosis) make for good brain fuel. It also may have some anti-aging properties or help with maintaining mental function.

Ketogenic diets are good for endurance athletes, though sprinters or athletes who engage in similar anaerobic exercise would need more carbohydrates, she said.

And ketogenic diets are very helpful to people with Type 2 diabetes who have not otherwise managed to get their blood sugar under control.

Eating keto also may help appetite, as eating fat and protein makes you feel more full.

While peoples main worry upon learning of a diet that sanctions bacon is usually heart health, emerging research is changing the way medical providers think about fat, cholesterol and heart disease.

Theres still little consensus among providers about whether saturated fats are OK or not, but the scales seem to be tipping away from blaming fat for all of societys ills. There also is a risk to eliminating virtually all carbohydrates from ones diet, providers say, as carbohydrates help the thyroid and adrenal glands run smoothly.

Youre going to be able to hold up to stress better if those organs feel supported and not in a starvation state, Thorner said.

For any diet to be effective and sustainable, it needs to be a long-term lifestyle change, rather than a temporary sprint.

If youre doing it for the weight loss perspective, its more of that fad diet, where you might lose it short term and if you go back to general, regular eating, most people will gain weight back, Petersen said.

Thorner said this is not a diet she would recommend to someone who has not yet stabilized his or her overall health.

The amount of work it takes to get to ketosis is usually pretty overwhelming, because its not natural to eat this way its a major change, she said. And there are lots of steps to get there, baby steps, that you can be eating better without actually being ketogenic.

Hale agreed that there are less-intensive options.

I highly recommend anybody who is considering a ketogenic diet to talk to somebody who is knowledgeable with it before jumping in, she said. That being said, anybody who decides to decrease their amount of carbs or sugars and increase their healthy fats is going to benefit.

Much of the initial weight loss with the keto diet (or any diet) likely comes from people just being mindful of what theyre eating for perhaps the first time, Petersen and Thorner said.

Anything where we can get away from the standard American diet theres middle ground here, and the pendulum doesnt have to swing one way or the other, Thorner said.

The bottom line is that to lose weight, you must consume fewer calories than you burn, says Dr. Tanny Davenport, who practices at Family Medicine of Yakima.

By restricting the types of food people can eat no carbs, for instance they are naturally going to lose weight, because they stop consuming those calories.

We know that when people lose weight, in general, they have better health outcomes. And if the tool they use to lose weight is a low-carb diet, which is usually higher in fat and protein, its hard to criticize that, Davenport said.

Theres data that show that low-carbohydrate diets, in the short-term, do better than low-fat diets, he said, but the data is hazy as to whether low-carbohydrate diets fare any better in the long run.

As for weight loss, he said, a reasonable goal for most people is to shed 5 to 7 percent of their total body weight. And a reasonable pace to do that is about 1 to 2 pounds a week, if you want to keep it off.

For the people who really succeed, its about watching what they eat and making lifestyle changes about their dietary habits, Davenport said. A change of 300 calories a day, one way or the other, can (cause a) swing from gaining a pound every couple weeks to losing a pound every couple weeks.

Living in a fast-paced world, Petersen says she wishes she could offer her patients a single easy fix to their weight and health problems.

But everythings individualized and everything takes time, she said. Lets say we got to a weight where were not happy. It didnt happen in a week; it happened over months and years. Its 100 percent normal to feel frustrated that maybe itll take weeks and months and years to get down from that.

In Hales case, eating keto is something she plans to continue for the foreseeable future. While the weight loss has slowed down not helped by the fact that shes currently writing a keto cookbook and testing recipes she still sees major benefits.

Since somatropin is a critical hormone for the health and well-being of all humans, not just athletes, the dosing of somatropin growth hormone must be fairly precise for the optimal functioning of the various physiological processes of growth and repair of body tissues including muscle and the metabolization of body fat. Not only is the dosing of somatotropin important, so is the time of day you take it. As with all things we employ to enhance performance, or quality of life, we continually strive for what is generally accepted as optimal. In the case of Somatropin, optimal is a fairly simple application.

Somatropin Dosing

As with most cases of off-label uses of compounds, it is the athletes looking to increase performance who end up being the de facto lab rats in the case, because the medical community at large tends to ignore non-medical uses of any compound, somatropin included. The bodybuilding community has taught us, over years and years of trail and error, that the maximum dose of somatropin should never exceed 8 IU (International Units) a day. That's not to say that some extreme practitioners haven't gone as high as 24 IU a day. The resulting bloat because of the water retention caused by such an extreme dose was about the only noticeable difference between such a high dose and what the athletic community would deem normal 4 to 8 IU a day. If nothing else, this goes to prove the rather innocuous nature of somatropin. You really can't overdose on it and experience any significant health risk not short-term, anyway. A long-term study on such unreasonable dosing would be as unethical, as it would be cost-prohibitive.However, suffice to say, based on what we do know today, somatropin, at any dose is a relatively safe drug. That's not say that there is such a thing as a safe drug, but as far as the multitudes of substances we routinely put into our bodies even items purchased over-the-counter comparatively speaking, somatropin is perhaps the least problematic of them all.

So what is the optimal dose of somatropin one should take to realize its maximum benefits, with the least water retention? Again, this was determined by trial and error among the athletic community. While the published studies on growth hormone rarely exceed 2 IU per day, athletes have been performing increasingly well on 4 to 8 IU per day, as depicted above. However, through the quasi-scientific, empirical, process of trial and error among athletes, we've learned that in order to reap the full benefit of somatropin, the timing and consistency is more important than the total amount taken. For example, through the cause and effect monitoring of athletes using somatropin, we know today that consuming a 100 IU box of somatropin over 20 days at 4 IU a day is more beneficial than taking the same 100 IU over a period of 10 days, at 8 IU a day. Additionally, we now know that somatropin taken in conjunction with testosterone is the most effective manner of use.

We know this because research in animals has demonstrated that somatropin, like testosterone, seems to divert the calories from food away from fat synthesis and toward protein synthesis, to the degree that somatropin is noted to be as powerful as testosterone in stimulating protein synthesis and nitrogen retention in muscle. We see this as proof in humans, based on the observation that those who are clinically GH and testosterone deficient, have reduced lean body mass and increased fat mass, particularly central abdominal fat. Conversely, the opposite is true. This gives us an empirical perspective as demonstrating that the higher the level of somatropin, concomitant with testosterone, the body synthesizes more muscle mass and less fat than with either compound alone. Male test subjects taking between 250 and 500 mg of testosterone a week, in conjunction with 4 to 8 IU of somatropin daily, noted extremely satisfactory increases in lean mass while shedding body fat, especially in the abdominal area.

Best Time To Administer Somatropin

Within 15 to 20 minutes after a subcutaneous (sub-Q) injection,somatropin blood concentrations immediately fall off. This is because the body does not use somatropin growth hormone in its original state. It must be converted in the liver to somatomedin-C, or IGF-1. IGF-1 is a 70-amino acid chain hormone that has been shown to be the most potent growth factor derived by GH release or administration. It is the hormone that turns on the processes of tissue growth and repair, especially in muscle. IGF-1 production is regulated by factors other than somatotropin growth hormone, most notably nutritional and thyroid function; however, when growth hormone is released by the body, or injected as somatropin, and it goes to the liver to stimulate a set of growth factors, IGF-1 has demonstrated to be the most potent and well-established of all.

The timing aspect of all of this revolves around insulin. This hormone works completely against somatropin HGH. While insulin can be highly anabolic, insulin, in the presence of somatropin growth hormone, renders both ineffective. They essentially cancel each other out. Regulating these hormones is a kind of natural balancing act the body goes through. Lower insulin levels can cause the body to produce more pituitary growth hormone. Therefore, somatropin is the most effective when insulin levels in the body are at their lowest.

Using blood sugar as an indicator, what is the best time to take somatropin?

There are actually two times of day when insulin is at its lowest, providing two effectively optimal windows of use. Immediately upon waking in the morning, insulin is at its lowest level. That gives us the first optimal window of use. It's important to take advantage of this window prior to eating, as any food ingested will immediately incite an insulin response. Bodybuilders and elite athletes also know that the next time insulin will be at a very low level is immediately following a strenuous workout session. This gives us two optimal windows of use, either immediately upon waking (before eating) or immediately post-training. It is not uncommon for many elite athletes to take advantage of both windows and administer somatropin twice daily once immediately upon waking and the other immediately post-training.

I am on the healthy living craze and I am loving every minute of it.

Living life the healthy way will prolong life and work wonders for your mental Health.

My personal healthy living journey has been long and tough and I am still working at it. I try to train at least three times a week mixing both cardio,muscle building and core exercises. I have been doing it now for just over two years and it has made huge difference both physically and mentally.

Physically Ive lost a bit of weight and toned up and mentally Ive never felt better. I can now wear clothes that I would never have dreamt I could wear before. Strangely I have become less clumsy and my reactions are a lot better which I didnt expect.

One of the good things about living healthy is when other people start to notice you have lost weight and tell you look great and you are doing well. I think we find it hard to say thank you to people for saying this. Maybe we find it hard because we dont see a difference but we should become comfortable with saying THANK YOU.

Luckily in Fermanagh and south Tyrone gyms are popping up everywhere as are classes in local halls and couch to 5k programmes in villages and towns across the counties. The same is happening with Slimming World and Weightwatcher groups. The 'in thing' to do now is to live the healthy lifestyle.

Dont get me wrong I still enjoy my treats but it is very important to find that balance. I have a treat day every week but I mix it with eating healthy during the week and training at least three times a week.

Living a healthy lifestyle is now literally at your fingertips. The emergence of Apps for your phone that can help you plan your session and Fitbit watches that measure everything from heart rate and sleeping pattern to counting how many steps you take in a day are all things that are helping people live healthy and more importantly making you more aware of how your body is working and coping with your current lifestyle.

The hardest thing to do is to find the will to keep going when times get tough, I sometimes buy myself gym gear or download some gym music so it makes me think now that I have bought I have to use it.

The healthy lifestyle is the in thing to do and I for one have proudly jumped on that bandwagon. Being healthy is a state of mind you adapt to and it does wonders for your mental health, but its not for everyone. If you are happy with way you look and feel then there is no need to change.

Throughout the years, Ive faced a variety of issues ranging from depression, body image problems, creating boundaries, and obsessive food behaviors.

Unfortunately, I let fitness become my ruler and I was its servant willing to do any and everything for results. This worshipping of fitness no matter what, may bring results for a short time period, but in the long run, isnt something that will last.

Below are 12 commandments that help me fit fitness into my ideal life while staying stress-free.

I tried a 28-day liquid diet where I had nothing but shakes and I lost 15 pounds at the end of it (mostly water). I also got sick, moody, had achy joints, and all my weight came back as soon as I started eating whole foods again.

Its not worth it to sacrifice your long term health just for short-term gratifications. A healthy approach to nutrition doesn't feel like a struggle.

Life is meant to have rich experiences. Fitness is a key component of being able to do those things, but it doesn't have to become your ruler. Lifestyle first, and then find the workout routine and nutritional method that suits your personality and preferences.

I started working out weighing 165 pounds and became an athletic and muscular 200 pounds. I should be happy, right?

Not exactly. The problem was my mindset never changed. I was exercising and putting on muscle out of hate and not feeling enough as a man, not for health or enjoyment.

You cant hate your way to losing weight and improving your self-esteem. If you dont address your inner world, those perceived deficiencies will still be there.

I know, maybe youre tired of seeing this on every health article, but its important. Weight loss, productivity at work, better (and healthier) relationships and your mood rely on you getting optimum amounts of sleep on a nightly basis.

Try meditating at night, cutting off electronics 90 minutes before bed, having an orgasm (Oxytocin is benefited), or reading a hardcover book to help fall asleep.

Maintaining a fitness regimen while juggling work and personal life can become overwhelming. Therefore, its great to find some accountability in the form of a gym partner or an online community.

Its important to get to the core root of why you want a particular goal.

Are you doing it for someone (kids, family, partner, etc)?

Whatever it is, search deeper than just relying on how your body looks. If youre searching for deeper meaning, start with asking yourself why three times about a desire for a particular goal.

What good are you to the world if youre burnt out, overweight, moody, lethargic, and sleep-deprived? Youre doing the world and those around you a disservice if youre letting this happen because the best version of you isnt showing up.

Set boundaries by scheduling time in the day for yourself. Block 30 minutes to exercise, have a quiet bath, or some time to meditate.

Paleo, whole 30, intermittent fasting, and the ketogenic diet work. If theres a caloric deficit, then youll lose weight. The key factor is making good food choices and enjoying your plan.

Its better to workout for 25 minutes a day than to overcommitment to 60 minutes and only workout one day. Set realistic goals and realize that repetition, time, and consistency are the true ingredients to long-term success.

Micronutrients help your metabolism, help against chronic illnesses, and helps your body function optimally.

All-over health

One aspect of eating healthy that is sometimes overlooked is oral health.

Studies show that people who dont eat a healthy, balanced diet that includes fruits and vegetables have a greater chance of severe gum disease, which is bad news for the 300,000 kids across Michigan who dont have access to fresh fruits and vegetables, said Ted ODell, manager of the American Heart Associations Michigan Healthy Food Access Campaign.

According to the U.S. Department of Agriculture, 1.8 million people across the state lack access to healthy food.

Wellness and economics are linked, said Michigan Governor Rick Snyder. For example, costs due to coronary heart disease, the most prevalent and preventable form of heart disease, exceed $5.9 billion per year in Michigan.

Treatment of heart disease accounts for 1 in every 6 health care dollars spent, he continued. While economic considerations are certainly important, the real value of a healthy lifestyle is quality of life.

Still enjoy your favorite foods

Eating healthy does not necessarily mean excluding favorite foods.

According to a study by the AND in 2011, 82 percent of adults in the U.S. stated they did not want to give up foods they like. This was a major reason for not wanting to eat healthy.

The overall diet is the important emphasis and not simply focusing on one type of food or meal.

All types of food in moderation combined with physical activity, can be enjoyed in an overall diet plan.

ThedaCare & Pleasant View partner to promote healthy lifestyle skills – Waushara Argus

During the first week in May, students at Pleasant View Elementary School in Wild Rose will see their school bustle with activities designed to help them build healthy lifestyle skills.

More than 300 ThedaCare employees will span out to five Northeast Wisconsin schools, including Pleasant View Elementary, for its third annual Good to Go initiative, engaging students through activities, advice, and encouragement to promote healthy habits and to share their learnings with their families.

This year, school leaders from Berlin, Clintonville, Kaukauna, Neenah and Wild Rose collaborated with committee members from ThedaCare to create programming that both engages children and leaves them with ideas about how integrate simple wellness behaviors into their future lives. Schools rotate each year, so ThedaCare employee volunteers will reach thousands of children over time. This years Good to Go initiative will be held May 1-5.

The overall goal is to spend some time with students during that one week to create habits that can last for a lifetime, said Heather Murphy, ThedaCare Cardiovascular Care manager and a Good to Go volunteer leader. The mission for our organization is to improve the health of our communities, and to work with the five schools in the communities we live and work in is really rewarding. Its a good way for us all to be able to live our mission.

Nanomedicine enables all-in-one cancer treatment – Medical Physics Web (subscription)

Cancer is a complex disease to treat, and yet the operating principle of many current treatments is to simply kill healthy cells a little slower than cancerous ones. In response, scientists at the University of Electronic Science and Technology of China have developed a sophisticated nanoparticle-based treatment. Their theranostic nanoparticles carry an anti-cancer drug cargo, and showcase multiple cutting-edge nanomedicine technologies to enhance the drug's efficacy, including selective drug delivery, photoactive agents, and even signal-jamming genetic material.

The researchers have designed each individual nanoparticle to be a toolbox for cancer therapy, able to passively and actively target tumours (Biomater. Sci. 5 1001). The nanoparticles can act as contrast agents for both magnetic resonance imaging and X-ray, they deliver a concentrated dose of anti-cancer drugs, and they also thwart the cancer's attempts at developing immunity to the drug. They even deliver a photosensitizer that can be used to specifically weaken cancerous tissue by photodynamic treatment.

Yiyao Liu and colleagues demonstrated the efficacy of their nanodevices in vitro and in vivo on a range of cell lines and on tumours in living mice. They found that their nanoparticle drug-delivery technique effectively stopped tumour growth, whereas tumours in mice treated with the drug alone grew at a rate half that of a control group that had not been treated.

The nanoparticles are complex, many layered spheres. Protected by a jacket of natural polymer is a nugget of silica, holey like a sponge and soaked in doxorubicin, a common anti-cancer drug, along with the photosensitizer. The polymer jacket is pH sensitive so that it falls off in the acidic microenvironment of the tumour, only then releasing the active cargo.

Doxorubicin has two flaws. Firstly, it works by slotting in-between DNA base pairs to stop the replication process needed for cells to divide. This kills cells that need to duplicate quickly, such as cancerous cells, but harms many healthy cell types too. Secondly, it triggers the body's natural defences, causing cells to over express p-glycoprotein, a microscale pump that removes toxic molecules like doxorubicin from cells, and making the drug less and less effective against cancer.

The scientists at The University of Electronic Science and Technology of China countered both of these flaws. Healthy cell exposure is reduced by the polymer jacket, which makes sure the drug is only released under the conditions expected in a tumour. The jacket itself is covered in signal-jamming RNA to inhibit the expression of the cellular pumps, keeping the doxorubicin trapped inside the cells to allow the drug to work for longer. This impressive display of multifunctional nanoparticle design and synthesis demonstrates the power of nanomedicine for producing synergistic effects, offering new solutions to previously unsurmountable problems.

Researchers develop new tumor-shrinking nanoparticle to fight cancer, prevent recurrence – Phys.Org

A Mayo Clinic research team has developed a new type of cancer-fighting nanoparticle aimed at shrinking breast cancer tumors, while also preventing recurrence of the disease. In the study, published today in Nature Nanotechnology, mice that received an injection with the nanoparticle showed a 70 to 80 percent reduction in tumor size. Most significantly, mice treated with these nanoparticles showed resistance to future tumor recurrence, even when exposed to cancer cells a month later.

The results show that the newly designed nanoparticle produced potent anti-tumor immune responses to HER2-positive breast cancers. Breast cancers with higher levels of HER2 protein are known to grow aggressively and spread more quickly than those without the mutation.

"In this proof-of-concept study, we were astounded to find that the animals treated with these nanoparticles showed a lasting anti-cancer effect," says Betty Y.S. Kim, M.D., Ph.D., principal investigator, and a neurosurgeon and neuroscientist who specializes in brain tumors at Mayo Clinic's Florida campus. "Unlike existing cancer immunotherapies that target only a portion of the immune system, our custom-designed nanomaterials actively engage the entire immune system to kill cancer cells, prompting the body to create its own memory system to minimize tumor recurrence. These nanomedicines can be expanded to target different types of cancer and other human diseases, including neurovascular and neurodegenerative disorders."

Dr. Kim's team developed the nanoparticle, which she has named "Multivalent Bi-specific Nano-Bioconjugate Engager," a patented technology with Mayo Clinic Ventures, a commercialization arm of Mayo Clinic. It's coated with antibodies that target the HER2 receptor, a common molecule found on 40 percent of breast cancers. It's also coated with molecules that engage two distinct facets of the body's immune system. The nanoparticle hones in on the tumor by recognizing HER2 and then helps the immune cells identify the tumor cells to attack them.

The molecules attached to the nanoparticle rev up the body's nonspecific, clean-up cells (known as macrophages and phagocytes) in the immune system that engulf and destroy any foreign material. The design of the nanoparticle prompts these cells to appear in abundance and clear up abnormal cancer cells. These clean-up cells then relay information about the cancer cells to highly specialized T-cells in the immune system that help eradicate remaining cancer cells, while maintaining a memory of these cells to prevent cancer recurrence. It's the establishment of disease-fighting memory in the cells that makes the nanoparticle similar to a cancer vaccine. Ultimately, the body's own cells become capable of recognizing and destroying recurrent tumors.

Since the late 1990s, the field of nanomedicine has focused on developing nanoparticles as simple drug delivery vehicles that can propel chemotherapy drugs to tumors. One pitfall is that the body tends to purge the particles before they reach their destination.

"Our study represents a novel concept of designing nanomedicine that can actively interact with the immune cells in our body and modulate their functions to treat human diseases," says Dr. Kim. "It builds on recent developments in cancer immunotherapy, which have been successful in treating some types of tumors; however, most immunotherapy developed so far does not harness the power of the entire immune system. We've developed a new platform that reaches tumor cells and also recruits abundant clean-up cells for a fully potent immune response."

Future studies in the lab will explore the ability of the nanoparticle to prevent long-term recurrence of tumors, including metastases at sites distant from the primary tumor. What's more, the nanoparticle is designed to be modular, meaning it can carry molecules to fight other types of disease. "This approach hopefully will open new doors in the design of new nanomedicine-based immunotherapies," she says.
