I have written to state Sen. Saud Anwar and state Rep. Christopher Davis asking that they investigate the issues surrounding delivery charges being assessed by Eversource.

Since early last year delivery charges have surpassed the charge for power supply. With the recent heat wave supply charges dramatically climbed and tremendous increases in delivery charges are being assessed. This is a huge financial burden on many.

As a retired homeowner who has seen my part-time consulting work virtually dry up as a result of COVID-19, having a $413 utility bill arrive was shocking. Equally as disturbing was the fact that the supply charges of $162.40 was 39 percent of the bill with the remainder $250.75 for delivery. I will not shut off my air conditioning in 100 degree weather and sacrifice my health.

Somewhere along the line Eversource received approval for its increases. I cannot believe our elected officials recognized the significance of these increases and the impact on the financial well-being of state residents.

I am asking my elected officials to investigate this matter and if necessary, initiate action to remedy it.

Social media pages are loaded with complaints by aggrieved area residents. Legislators need to recognize this is a major problem that needs attention immediately.

Eversource should be ashamed as it calculate its profits. Our leaders in Hartford have allowed this and now they need to correct it.

I am writing about the Ellington controversy concerning the post of a meme on social media by First Selectwoman Lori Spielman.

The specifics of the adjudication of her situation do not prompt this letter. I am concerned about the ways in which the two sides have been treating each other and the ways in which various perspectives and opinions have been stated.

Talking about racism and white privilege are difficult emotionally charged topics. On social media and in public gatherings we are seeing a full range of emotional reactions including anger, defensiveness, name calling, hostility, bringing up irrelevant subjects, disrespecting others perspectives, mean-spirited language, talking over one another, threatening and attempts at intimidation.

I fear the polarization that is occurring because of the ways that people are reactive, not listening and being disrespectful.

All of us are Gods children. All of us are entitled to the opportunity to express our understandings. All of us should treat others as we would like others to treat us. All of us should try to listen with an intent to try to understand and speak in ways that create an atmosphere in which we can be understood.

Going forward I hope that we can practice disagreeing without becoming disagreeable.

As the senior pastor of the Ellington Congregational Church, I want to be part of efforts to heal, to bring our community together, to find common ground, to move toward greater understanding of our diversity and appreciation for one another.

Its been three months since COVID-19 invaded Connecticut. All federal, state, and local experts trying to treat its symptoms have failed to come up with a successful protocol. Closing and restricting access to beaches have only kept medicine away from the cure of the coronavirus.

I believe it is now high time these so-called experts changed their protocol to help these poor victims instead of adding to their suffering.

I am not here just to criticize; I have the solution and the protocol to rid our state of COVID-19. My qualifications are 30 years of alternative medicine experience.

A mother has revealed how she overcame an addiction to painkillers that spanned a quarter of a century.

Nicki Hari, 52, from Hertfordshire, was first prescribed paracetamol and ibuprofen for persistent knee pain when she was 18.

The mother-of-two later developed neuralgic pain, which she now knows to be a symptom of withdrawal, prompting her doctor to dole out the more intense analgesics codeine, co-codamol and tramadol.

Whenever Hari tried to come off the drugs she endured depression, night sweats and pain over her entire body.

During the height of her addiction, Hari took up to 20 painkillers a day, buying them online on top of her repeat prescription.

Having lost all zest for life, her friends staged an intervention and drove her to a rehabilitation centre under the pretence they were off for an evening out.

After being an inpatient for 28 days, Hari has never relapsed, but stressed she will always be in recovery.

Having come out the other side, Hari supports The National Institute for Health and Care Excellences (Nice) recent guidance not to take painkillers for chronic discomfort as they can do more harm than good.

Haris mother took the then-teenager to see a specialist after she endured persistent knee discomfort, which medics put down to growing pain.

Although the knee pain eased, Hari began experiencing short, severe bursts of discomfort along her nerves, which prompted doctors to prescribe stronger analgesics.

When a course of medication would come to an end, Hari would endure really awful symptoms.

Depression, anxiety, night sweats, pain all over, she said.

I distanced myself from my family. I didnt want to engage in anything.

Unsure what was causing her discomfort, Hari visited her GP.

They didnt know what it was, she said. I was pooh-poohed really; they sent me away.

At the time I was in a job I hated so I put the depression down to that.

With Hari then in her early thirties on a repeat prescription, it was not until she was called in for a review with a new GP that a medic suggested she may be addicted.

The new GP said, I think youve become dependent on these and should come off them.

I went into complete panic and changed surgeries so that wouldnt happen. I manipulated the system a bit.

As well as her repeat prescription, Hari began sourcing the painkillers herself, at a price of between 200 ($261.99) and 300 ($392.99) a month.

When my body built up a tolerance I started buying more over the counter, she said.

I found ways of topping up my own medication.

In the last 10 years of my using, I was Googling the painkillers and it flashed up buy your codeine here.

At the height of her addiction, Hari would take two analgesics whenever the tell-tale withdrawal symptoms kicked in.

At my absolute worst I was probably popping 15 to 20 times a day, she said. I had them in the car, different rooms in the house, at work.

Hari claims she managed to look after her children throughout the ordeal, but the addiction took its toll on her mental health.

[My children] went to school and were washed and dressed and fed, she said.

But I was too wiped out to make dinner so my husband at the time would have to, or come home early because I couldnt pull myself together.

I lost all zest for life. I didnt care about my life, my appearance. I didnt care if I woke up the next day.

I became very withdrawn, very angry.

In the summer of 2011, Haris loved ones intervened when she slept through her childrens sports day.

Two of my friends climbed on the wheelie ban and were banging on my window, and said youve got a problem, she said.

I said: there is no problem because Im taking what the doctor told me.

That October, a group of friends arrived at her house saying they were going out for the evening.

We all bundled into the car, but I didnt realise they were driving me to rehab to see a psychiatrist, said Hari.

[When we arrived] my parents and then husband were there. I hated them at the time.

Hari spent the next 28 days as an inpatient.

I was in denial for the first seven to 10 days, she said.

I was in there with people with real addictions - crack, heroin, alcohol and I didnt see myself as one of them.

I was a bit grandiose, thinking I was better than these people. [Thinking] I have a nice job, house, car. I wasnt in an alleyway jacking up.

Despite her reservations, Hari completed the rehabilitation stint and has never looked back.

Youre never cured, youre always in recovery, she said. You take it one day at a time.

In 2019, Hari developed gallstones, describing the pain as horrendous.

I was very mindful of what I was taking, she said. I knew I could get addicted again but I know where that would take me.

Her experience with painkiller addiction has led Hari to support Nices recent guidance not to take painkillers for chronic discomfort.

The official health watchdog has warned painkillers can do more harm than good due to their addiction risk, and instead encourages patients be prescribed exercise, physical therapy or acupuncture.

Speaking of the somewhat controversial suggestion of acupuncture which has mixed evidence supporting its effectiveness Hari said: Alternative medicine should be available but without a six month waiting list because that pushes people to the internet.

Hari who now works for the UK Addiction Treatment Group is not against painkillers, providing they are prescribed appropriately.

If someone comes in with acute pain, if they need strong painkillers, that person needs to be aware of the dangers and it should be managed.

Maybe just give three to seven days [worth] at a time.

Chronic pain, whether a recurrent headache or back discomfort,affected between 13% and 50% of Britons in 2019.

While pain is a spectrum, 10.4% to 14.3% suffered moderately or severely.

The NHS recommends a combination of exercise, physical therapy and painkillers to ease the debilitating symptom.

In more severe cases, amitriptyline or gabapentin may be prescribed for pain caused by nerve sensitivity or damage, like shingles or sciatica.

The strongest painkillers available are morphine or similar-acting drugs, like opioids.

Androgen deprivation therapy (ADT) is historically the standard of care for patients with oligorecurrent, hormone-sensitive prostate cancer a setting where a small number of metastases emerge at some point following local treatment of the primary tumor. However, many men opt to postpone ADT due to substantial side effects of the therapy, and there is an unmet need for alternative therapeutic approaches that preserve patients quality of life while deferring systemic treatment.

A new wave of imaging technologies now allows for the direct detection of oligometastases and removal through local ablative techniques such as stereotactic body radiotherapy (SBRT); some of these technologies are emerging as new metastasis-directed therapies (MDTs) for patients with prostate cancer.

Indeed, a small number of prospective studies investigating the use of MDT mostly SBRT in men with oligorecurrent disease have suggested that the treatment is safe, and in 1 study, up to 50% of patients did not require ADT or other forms of systemic treatment for 2 years following MDT.1 Such findings have led to the idea that MDT could serve as an intermediate treatment between surveillance and systemic ADT.

While clinical trials are still investigating the long-term survival benefits of conducting upfront MDT prior to standard-of-care systemic therapy, a recent study estimated the cost-effectiveness of such a treatment approach and compared it to 2 hypothetical systemic approaches ADT alone, or combining more advanced hormone therapies with ADT.2

Computational cost-effectiveness models suggested that at 10 years, upfront MDT followed by standard-of-care systemic therapy was similarly cost-effective as the other treatment approaches, suggesting that it could be a viable treatment strategy, particularly for patients who wish to defer systemic therapy. The findings were published in the International Journal of Radiation Oncology, Biology, Physics.

Sometimes we think using fancy radiation is just adding cost, but it might actually be cost-effective if we are delaying the onset of more expensive other drugs, said Amar U. Kishan, MD, an assistant professor and vice chair of clinical and translational research for the department of radiation oncology at the David Geffen School of Medicine at the University of California, Los Angeles, and a coauthor of the study.

In the study, Dr Kishan and his colleagues compared 3 hypothetical treatment strategies. In the first, patients with oligorecurrent, hormone-sensitive disease were to receive upfront SBRT, and upon progression, antihormonal drugs abiraterone acetate plus prednisone (AAP) together with ADT, followed by docetaxel chemotherapy plus ADT combination approaches that have been associated with survival benefits in patients with de novo diagnoses of metastatic hormone-sensitive prostate cancer.3,4 In the second treatment scenario, patients received upfront AAP with ADT, followed by salvage docetaxel plus ADT. The third strategy entailed upfront treatment with docetaxcel plus ADT followed by salvage abiraterone plus ADT.

Upfront Radiation May Be a Cost-Effective Strategy in Oligorecurrent Hormone-Sensitive Prostate Cancer - Cancer Therapy Advisor

Hamilton, ON, Aug. 05, 2020 (GLOBE NEWSWIRE) -- The Canadian Vaping Association (CVA) is troubled by the recent op-ed published in The Globe and Mail, When will Canada get on the same page about the risks of vaping. Throughout the op-ed, the authors Ward Flemons and Brandie Walker, lung specialists at the University of Calgarys Cumming School of Medicine, state vaping is ineffective for smoking cessation and a tool intended to lure youth to smoking. Flemons and Walker call on the federal government to follow the lead of Nova Scotia and British Columbia by implementing plain packaging and restricting nicotine concentrations.

The CVA is aligned with the recommendation to limit nicotine concentrations to 20 milligrams per millilitre, an action our organization has also recommended numerous times. This would be identical to the nicotine cap imposed many years back in the European Union, where the data continues to show minimal youth uptake. However, the authors of this op-ed are misinformed on several other points. The inaccuracies within many of their other statements are of great concern since their status as lung specialists can lead others to believe their false statements, which would be to detriment of public health.

While Flemons and Walker claim that vaping is ineffective as a smoking cessation tool, many studies have proven otherwise. National Health Services (NHS) conducted a controlled trial in which participants were randomly assigned to varying nicotine replacement therapy (NRT) products (patches, gums, etc.) or e-cigarettes. After one year, 18% of e-cigarette users had completely quit smoking compared to only 9.9% of NRT users. The trial concluded that vaping is nearly twice as effective as the leading NRT products and that smokers increase their chances of quitting smoking by 83% using e-cigarettes compared to NRTs. The findings in this NHS study were validated through the peer-review process and were subsequently published both in the New England Journal of Medicine and British Medical Journal.

The Rutgers School of Public Health and the Columbia University Mailman School of Public Health also conducted a study into vaping effectiveness which concluded that 50% of daily vapers are individuals that have successfully quit smoking completely. This study also clearly demonstrates the effectiveness of e-cigarettes for smoking cessation.

Darryl Tempest, Executive Director of the CVA stated, While the research clearly demonstrates that vaping is the most successful smoking cessation product available, the harm reduction component of vaping is alone a significant reason to encourage smokers to switch completely to vaping. The Royal College of Physicians was first to conclude six years ago that vaping is at least 95 percent less harmful than smoking. Since that time Public Health England has conducted annual independent studies to further validate these findings and to date all studies have shown the same positive results. Lung experts must be held accountable to presenting the facts, rather than their own biased opinion.

Inaccurate statements such as the ones made by Flemons and Walker led Public Health England to release a series of statements reassuring the public of the harm reduction potential of vapour products. There are over 6 million smokers in England and smoking is still the leading cause of premature death and disease particularly among the most disadvantaged in our society. Helping more smokers quit is vital if were to achieve the governments vision of a smokefree society by 2030, and vaping has a role to play. Vaping has helped many thousands of smokers quit to date. But many thousands more could benefit if they were not put off by the persistent, worsening and inaccurate beliefs the public hold about vaping. Smokers should be reassured by todays authoritative and detailed report which shows that the UKs e-cigarette regulations are effective and vaping remains a safer alternative to smoking. I urge smokers to have confidence in our regulatory system and not be put off by alarmist headlines about the risk of vaping which are not backed up by the evidence.

In the op-ed published in The Globe and Mail, the authors also challenge the federal government to implement plain packaging legislation. This statement alone makes it glaringly clear that they have no understanding of the current regulations in place at a federal level. The Vaping Products Labelling and Packaging Regulations, which came into effect nationwide on July 1, 2020 after months of stakeholder input, addressed the concern for plain packaging by requiring that more than two thirds of the label contain mandatory federal government statements, amongst various other labelling restrictions. Furthermore, federal regulations in place prior to this already do not allow for any characters, cartoons, or persons to be on the label and prohibit naming conventions and flavours that would entice youth use including those that relate to any and all confectionery products, desserts, energy drinks, soft drinks and cannabis. With these various regulations in place, in all but the name, the federal government has already mandated plain packaging.

Flemons and Walkers claim that the vape industry is using vaping as a tool to entice youth to use tobacco is also completely unsubstantiated by the data. Health Canadas recent study, Canadian Student Tobacco Alcohol and Drugs Survey, found that while 8.4% of youth used vapour products, youth smoking rates were continuing to decline. Additionally, 75% of youth indicated that they understand there are risks associated with vaping, which goes against the authors notion that vaping products were being poised as a risk-free product to lure youth to tobacco.

In Health Canadas recent study, 65% of youth indicated they acquired vapour products from social sources. Since most youth are not acquiring vapour products through legal channels, further restrictive legislation surrounding the sale of vaping products will not protect youth and will instead discourage adult smokers from switching to a significantly less harmful product. While the authors commend the action taken by the Government of Nova Scotia, they fail to recognise the unintended consequences of Nova Scotias legislation. The legislation imposed there has caused a spike in Nova Scotias smoking rates and a significant rise in unregulated and thus potentially dangerous products. Nova Scotia has thus made it easier for youth to acquire these unregulated and dangerous products, while disincentivizing law abiding adult smokers from converting to the most successful harm reduction product globally. Nova Scotia should not be commended for the regulations imposed but instead condemned by public health experts everywhere for the detriment this regulation has and will continue to cause to the public health of their citizens.

Given the significant damage smoking causes to the lungs, our organization would expect that those who specialize in lung health would embrace an alternative to combustible tobacco that greatly reduces harm. We encourage Flemons and Walker to review the research and regulations surrounding vaping and to cease their unsubstantiated claims. It is absolutely critical that health advocates in trusted positions provide accurate data on vaping as false statements can deter the millions of Canadian smokers from switching to an alternative proven time and again to be at least 95% less harmful.
