Harvard Medical School: Dasatinib and Quercetin Could Be Used to Rejuvenate Older Organs for Transplantation – Gilmore Health News

Organ transplantation is a crucial aspect of modern medicine and responsible for saving millions of lives. It depends on organ donors, whether alive or deceased. However, there is a strict criterion to follow when selecting organ donors. In simple terms, the donor must be healthy and young. This narrow organ pool combined with an increased need for organs has resulted in more recipients than donors, with a lot of recipients all waiting on the transplant list for an organ.

This massive gap between the two groups has caused researchers to look into an alternative to provide organs for these patients. Iske and his colleagues from Brigham and Womens Hospital, Harvard Medical School, Boston recently published a study they performed in the Nature Communications Journal. The aim of their study was to investigate a way to use organs from older deceased individuals.

Organs from older donors are usually not considered, because of the higher risk of adverse effects associated with them. Studies have found that older organs usually have a certain type of cells called senescent cells. These cells are no longer going through the cell cycle and evade the elimination process which is usually applied to older cells.

Senescent cells are associated with a higher rate of transplant rejection. Transplant rejection can result in decreased quality of life and sometimes, even death. This would explain the medical communitys hesitation in using older organs which have been shown to contain accumulations of senescent cells.

Researchers have realized the mechanism of this rejection. Senescent cells release free mitochondrial DNA, which in turn activates dendritic cells. These cells lead to augmented IL6 response along with costimulatory molecules CD40/CD80. The increased IL6 levels are related to inflammation and a massive whirlwind increase in cytokines. This increase in cytokines and increased inflammatory response is related to organ rejection.

Iske and colleagues from Harvard found that a specific class of drugs called senolytics may push these senescent cells back into the cell cycle. This is of importance because the cell cycle usually removes old, aged cells. The study performed aimed to apply this theoretical concept to animal models. The researchers wanted to determine if this concept can be practically applied.

The team of researchers with Iske used two senolytic drugs, Dasatinib and Quercetin. These drugs were given to mice for 3 days, at a dosage of 5mg/kg and 50mg/kg respectively. On the fourth day, researchers prepped the mice for cardiac transplants. They then assessed the mitochondrial DNA levels and inflammation levels usually associated with senescent cells.

Both the experimental and clinical studies performed by Iske and colleagues showed higher levels of mitochondrial DNA in organs from older donors. Clinically, the peripheral blood sample from the older, deceased donors showed a high amount of senescent cells. Experimentally, the results were similar.

The use of senolytics depended on blocking pro-survival pathways for senescent cells and it showed great results, according to the researchers. These drugs blocked those pathways by blocking IL6 and inhibiting the activation of dendritic cells, all the mechanisms associated with organ failure.

Senolytics slowed down the aging and deterioration of organ function normally associated with senescence. This normal functioning of organs, plus the decrease in free mitochondrial DNA all together showed positive results in the animal models used by the researchers.

It is important to note that the study performed by Iske only showed results in animal models, and they recommend a detailed clinical trial to be performed to ensure the results of their experimental study.

However, this is a giant step in the right direction. The study by these researchers may have just given the medical community a solution to their biggest problem.

Mercy Medicals new Center for Breast Health, Gynecologic Oncology is all about womens cancer care – masslive.com

SPRINGFIELD - Dr. Nada Kawar, who represented her native Jordan in the shot put both at the 1996 and 2000 Olympic games, had plans to be an orthopedic surgeon.

However, a rotation in obstetrics and gynecology in her third year of medical school at Washington University in St. Louis, Missouri, changed her focus to womens health. She went on to a residency in obstetrics and gynecology at University of California San Francisco Medical Center and a fellowship in gynecologic oncology and breast surgery at Brown University/Women and Infants Hospital of Rhode Island that taught her how entwined breast and gynecologic cancers can be.

Her extensive training as a gynecologic oncologist she is board certified in obstetrics and gynecology and gynecologic oncology, and certified in breast disease as well as breast surgery and in the fundamentals of laparoscopic surgery qualifies her to both diagnose as well as provide surgical and medical treatment for womens cancers, including cancers of the breast, uterus, ovaries, cervix, vagina and vulva.

Kawar, who was an undergraduate at the University of California in Los Angeles when studies were emerging on inherited risks for womens cancers and how they can overlap, now practices at Mercy Medical Centers recently opened Center for Breast Health and Gynecologic Oncology, formerly its Breast Health Center, under Dr. James Frank, and feels very lucky to have the one-stop location adjacent to the Sister Caritas Cancer Center for her patients who sometimes have overlapping cancers.

This vision of seeing everyone in the same building, sharing resources, imagining my career in a different way - I am very lucky to be here, said Kawar, noting that inherited mutations in certain genes can put a small percentage of the population at risk for breast cancer as well as ovarian cancer and cancer of the endometrium-uterus.

I have already seen lots of patients who have both cancers of the breast as well as gynecologic. Here, they can come to one space, see one doctor or other doctors and do not have to go to other locations.

She sees such an approach where a physician like herself provides comprehensive care for female cancers in one location as all about womens cancer care.

Kawar added patients at the center have access to a genetics counselor who can help determine cancer risk based on inherited factors and treatment options that range from surgery to reduce such risk to minimally invasive surgeries as well as radical procedures for those with malignancies.

She said that as medicine evolved over the last century that there was this sort of arbitrary separation of breast surgery into the field of general surgery where gynecologic oncology evolved within a subspecialty of gynecology and obstetrics.

There was this separation and finally care of breast cancers separate from gynecologic and pelvic cancers, Kawar said.

Breast cancers are mostly uniquely female, although some men get breast cancer, it is a very much a womans cancer.

The greater prevalence of breast cancer and the growing number of disease survivors, Kawar added, has helped generated much greater publicity and funding around it than cancers of the reproductive organs that are much less common though can be related.

Kawar said patients are most often referred to her by their gynecologist after an imaging study indicating a tumor or some other abnormality.

She said the most common gynecologic cancers she sees in patients are uterine cancer, which is often found early due to abnormal bleeding or pain and has a high five-year survival rate when detected in the localized state; ovarian cancer which she termed much more deadly as it is often detected when the disease is more advanced; and cervical cancer that is often caused by exposure to certain strains of the human papillomavirus and for which there is now a vaccine to help prevent.

Screening for cervical cancer can be done by the HPV or Pap test and the five-year survival rate is very high when detected at the localized stage.

Risk factors for uterine or endometrial cancer, Kawar said, include metabolic syndrome, whose conditions include high blood pressure, obesity and diabetes, but just how this syndrome contributes to the development of the cancer is unknown.

She said too much of the hormone estrogen without the presence of progesterone can also pose a risk.

Kawar said hereditary breast and ovarian cancer syndrome, often related to certain mutations in the BRCA1 or the BRCA2 gene, can elevate an individuals risk for both as well as other cancers.

She noted that there is no good screening test for ovarian cancer and one recommendation for women with a known inherited risk for the disease is risk-reducing surgery to remove the Fallopian tubes and ovaries after child bearing years.

She added, The symptoms of ovarian cancer are normally very vague - urinary symptoms like urgency or frequency, abdominal pain, feeling full quickly - these usually cause women to ultimately seek care and are often have their symptoms mistaken for stomach problems or indigestion and with a diagnosis of ovarian cancer quite late because of that.

Anyone with a strong family history of breast or ovarian cancers should seek genetic counseling and possibly genetic testing because that is one way to become aware of the risk for ovarian cancer, Kawar said.

Other risk factors include infertility - not being able to bear children - and age. It normally affects women age 60 and above.

Kawar said not all women referred to the center end up being treated for a malignant condition, but surgery is sometimes needed because of a concern for cancer.

When a woman is referred to me with a pelvic mass found on imaging, I will talk about the possibility of cancer with the goal of surgery to remove the mass and determine diagnosis and if it is cancer, to recommend the appropriate treatment which may include more extensive surgery or chemotherapy, Kawar said.

I often treat women for endometriosis which is a benign condition but can mimic cancer in the problems it causes and might require a large-scale surgical approach. I often see women for uterine bleeding that may end up being polyps and not necessarily cancer.

Kawar noted that this is Gynecological Cancer Awareness Month, and next month brings Breast Cancer Awareness Month.

September brings awareness of all the problems of the gynecologic cancers and the importance to survival of catching them early, Kawar said.

With a lot of emphasis on breast cancer in October.

The American Cancer Society estimates this year that 42,170 women will die of breast cancer, and some 33,602 will die of cancers of the genital system.

These include 13,940 of ovarian cancer, 12,590 of uterine cancer, 4,290 of cervical cancer, 1,350 of vulva cancer and 1,450 of vaginal and related cancers.

Radiology’s work-life balance wins over many med students, but not all are impressed – Health Imaging

Last years Association of American Medical Colleges report on residents found only 1 in 8 stuck with radiology from the beginning to the end of med school.

Using this as a launching point, Ali et al. analyzed national data from the AAMC, which included 1,965 students who pursued radiology. Individuals were divided into three groups: 281 students who chose radiology in the first and last years of school or the committed group; 625 who opted into imaging early, but later switched to another field (switched-away); and 1,059 who started in a specific specialty, but then moved over to radiology (switched-to).

Students cited work-life balance (64%) as one of the top three factors influencing their decision, only behind individual fit (87%) and content of the specialty (80%). Income expectations came in seventh, the authors noted, with 27% citing salary as a top consideration.

Radiology is traditionally considered as a life-style specialty and continues to rank in the top five for work-life balance and thus it is no surprise that it continues to influence students, the authors wrote.

This Is Why I’m Working as a Medical Assistant While I Advance My Education – Nurse.org

Taylor Brune @heartsinbloomhealth is many things. To start, perhaps above all else, she is passionate about healthcare. Brune, who recently survived COVID-19, suffers from chronic autoimmune deficiencies, which began from a bite from a tick. As a result, she has had to learn and research much in the medical world so that she is as well equipped as possible to survive her severe afflictions.

On top of all that, Brune is also a Medical Assistant. In this capacity, she works with healthcare providers as a liaison to patients, in administrative capacities and other roles to ensure the facility operates smoothly. Brune, never one to shy away from a challenge, is also a student continuing her education. She is using her experience as a Medical Assistant to help transition to, one day, becoming a doctor. That is her ultimate aim.

We caught up with Brune to ask her about this long professional journey, her fight with COVID-19, her passion for healthcare and much more.

When and why did you decide you wanted to be a Medical Assistant?

The last ten years, I have dealt with my own health issues and my own health journey of developing chronic disease and autoimmune disease after a bite from a tic. So, when I lost my health, I was engulfed in the medical world and I was a patient 24-7 and having to do research for myself and be my own health advocate. In the process of learning how to heal myself, I grew the passion of wanting to help heal others.

While I was going through treatments, I was like, this is my calling. This is where Im supposed to be. This is why Im having my health issues and going through this huge life transition and transformation. When that realization happened, I decided to learn about medicine and how to switch my degree over to pre-med and integrative health.

Going into medical assisting school, was the first step in my path. And Im going to be a doctor one day no matter how long it takes! No matter what challenges I face, I know that everything Im going through in my own health is helping me transform into the best person that I can be so that I can be the best doctor for patients. Since Ive had the perspective of being a patient for so many years, I know exactly what theyre thinking and feeling.

What was the process like for you to become a Medical Assistant?

First, I prayed a lot about it. I knew I wanted to switch my degree to pre-med but, I also wanted to work in the medical field a lot sooner. Becoming a doctor takes years in pre-med and medical school.

So, I figured the first step to completely immerse myself in the medical field as soon as possible was to complete a medical assistant program and to actually start working in the field that I love so much. Once I graduated from my medical assistant and phlebotomy program I immediately started working in the field. The experience Ive gained has just confirmed that this is where Im supposed to be and I love it!

How long did the process take, what type of schooling did you?

For medical assisting in California , I needed to go to a medical assisting school. I went to a trade school and enrolled into a medical assisting program. Medical assisting programs are more about gaining hands-on experience in an actual doctors office. This is how the program was set up,

The program style was really beneficial to help me to get on-the-job training and also land a great position right after graduation,

At the same time, though, Ive been going to Arizona State University Online to finish my Bachelors degree, which Ill be finishing in the fall. This fall, Ill have my Bachelors as well as my medical assisting diploma.

As a medical assistant its important to be certified. I took a national certification exam and every two years, I complete the required credits and retake the test to keep my certification up to date.

How did you land your first job as a Medical Assistant?

It was actually pretty easy for me because I seem to interview really well with medical places. My first job was working at a naturopathic office. Next, I went to primary care and oncology. So, thats where Ive been working the last year. Now, Im at Scripps Health Hospital in dermatology.

Theres a lot to know for the job and you have a lot of responsibilities - from first-aid to computer work to patient liaison. Do you like having all these aspects to your workday?

I love it! I have gained so much experience including,

Drawing blood is my absolute favorite because I really enjoy direct patient care. But, overall working with different modalities, systems and technologies has taught me so much about the medical field. Each private practice is completely different and the providers are unique in that they offer different specialties and treatments. Its been fun learning all these different skills.

Your long-term goal is to be a doctor. How did you choose becoming a medical assistant for that aim and how has it helped?

There are a few reasons why I chose medical assisting as my first step towards my goal of becoming a doctor,

What do you like least about being a Medical Assistant or the healthcare field, in general?

For my own personal experience, I have an immune compromised self. I just dont like that I pick up illnesses so easily. I even picked up COVID. Flu season is also difficult for me. Im thankful for the fact that everyone is now taking more precaution in the medical field. Ive observed that people are more mindful of sanitizing and patients are wearing masks. It gives me hope that at least this year I will have stronger defenses and not catch as many illnesses. But theres many great things about working as a medical assistant, too. Each person has their own experience.

As a chronic illness patient myself, the one negative that I dont like is that I feel some doctors dont have time to really hear their patients. When Im a doctor, that is something that I really want to change. However, Ive been really fortunate to work with providers who are integrative in their mindset and do give their patients time. Even now, working with providers who arent integrative, they still give patients time and hear them. Ive been really fortunate to work with good providers. But, from the perspective of a patient, I had to go through multiple providers to get to good ones - and, I didnt like that.

What advice do you have for other people looking to become a Medical Assistant today - first steps, things to keep in mind?

As you said, you recently recovered from COVID-19. What was that experience like for you personally and professionally?

In the beginning of the year, we actually started seeing patients have lung flues and pneumonias with weird symptoms and they were just really sick in January. I believe that COVID-19 was definitely here in the United States in California in January. But we didnt know what it was yet. But then when they started announcing it in February, we definitely got more traffic in the office and we were taking care of patients - even though it was primary and oncology, a lot of the patients in the beginning stages when they were starting to get sick would come to us. So, we were being exposed to COVID-19 and it spread through the staff.

The COVID-19 symptoms started out mild for me. I just figured it was the stress from working and being the only medical assistant since we were short-staffed during that time. Then once the rest of the clinic tested positive, I was like, Oh, no! I think I may have it!

I got tested. The week after I tested positive the illness became super severe for me. I have asthma and type-one diabetes - it went straight into my lungs. I decided to just face it and not be crippled by fear. I set my mind to believe that I would not die from it. Because Id learned with my chronic illness that you just have to stay positive no matter how grim it looks. And really just focus on what you can do to heal.

During the time when it was really severe, I went to the hospital and towards the end, I was able to go home and recover.

Now, its been three months since I've been cleared. I still have lung damage. My body is still healing. My body is still recovering from it in that aspect. But I was very fortunate because Ive had over ten years of experience being a chronic illness patient and working in the medical field. With that experience in mind, I knew how to take care of my immune system and listen to my body. When I got it, it was the beginning of the pandemic when doctors really didnt know anything about it. They didnt know what to tell me. They didnt know how to help me. So, for a lot of it I was on my own. I had to figure out how to survive COVID-19.

Luckily, my knowledge and prayer and by the grace of God and blessings, I was able to fight it, survive it. I was able to apply my knowledge to quickly do a regimen of trying to boost my immune system fast. I made a point to not stress, not think negatively, not fear, and not feed into any of that. I just tried to stay calm through the whole thing. Eventually, after 70 days, I was cleared.

What was the most difficult symptom of COVID-19 for you?

I think, honestly, the worst part was not being able to breathe. Even now, its still hard to breathe. It felt like I was suffocating 24-7, there was no relief. I was even on a breathing machine, not a ventilator, but a nebulizer breathing machine every four hours and taking medication just to be able to survive. I probably should have gone on a ventilator at that point because my oxygen stats were just in the 80s and so low and I was so sick. But I just didnt want to go on the ventilator. When I went into the hospital, they gave me fluids, took all the tests to make sure there was no other organ damage.

So, it was just kind of traumatic, honestly! But I learned a lot. Im grateful to be alive. Im grateful to be able to help anyone, you know? Or to face it without as much fear, even though it is really scary. To have hope and to try to stay calm.

You seem connected with people, both in-person and digitally. How has this helped, how has Instagram helped spread your story?

Its so amazing because I just started sharing my story of being a chronic illness patient and so many people could relate to that. Then when I started sharing my story and my experiences working in the medical field so many more people would message me and connect with me. Ive met so many amazing people, its amazing whats come out of Instagram. Whats come out of this community is just so many different opportunities and abilities to be able to hear other peoples stories, share my story, be able to learn from others, be able to help others. Its made me a better person and its also helped me be able to have a stronger voice.

When youre about to start your day, what is the final thing that passes through your mind before you open the doors to work?

Every morning, I just say a prayer, walking in, in my mind. And I dont know if this is going to sound corny but because this is my calling, I really, truly want to make the most of my job.

I pray right before I walk in every single morning that God uses me as a light of warm light and love to each person that I come across so that my day is very purposeful and meaningful and that Im able to uplift someone in some way. Or help someone in another way or just comfort a patient in a way that they need. I pray every day that my light shines through to the patients and staff around me.

Dr. Seymour Schwartz, Who Wrote the Book on Surgery, Dies at 92 – The New York Times

Sy is the aspirational mentor that we all want to emulate, said Dr. David Linehan, chairman of the surgery department at the University of Rochester Medical Center, who also holds the title Seymour I. Schwartz professor of surgery. Dr. Linehan praised Dr. Schwartz for his towering intellect, spirit of collegiality, indefatigable work ethic, unending curiosity and enduring relevance.

Dr. Craig R. Smith, the chairman of the surgery department at NewYork-Presbyterian Hospital/Columbia University Medical Center, recalled Dr. Schwartzs insatiable intellectual curiosity and called him a polymath of the first order who was much, much more than just another academic surgeon.

Seymour Ira Schwartz was born on Jan. 22, 1928, in the Bronx, to parents from Jewish immigrant families. His father, Dr. Samuel Schwartz, was a physician who taught anatomy at Polyclinic Hospital in Manhattan and whose family was from what is now Belarus. His mother was Martha (Yampolsky) Schwartz, who managed her husbands medical practice and whose parents were from Poland.

A gifted actor in student productions at DeWitt Clinton High School, Sy was encouraged by his father to pursue a theatrical career, but decided to become a surgeon instead. Accepted by Yale but unable to afford the tuition, he enrolled at the University of Wisconsin on a scholarship and completed his pre-med degree in two years.

He began medical school at Syracuse University, earned his degree from New York University and, in 1950, completed his internship at the University of Rochester School of Medicine and Dentistry.

In the 1960s, while serving on the University of Rochesters admissions committee, he was appalled to learn that a highly qualified candidate was being rejected because he was Jewish. Dr. Schwartz inquired how he, a Jew from New York, had been admitted and was told that school officials had mistaken him for Sidney Schwartz, a young tennis star from Brooklyn.

So, in August, when the world series moved from WSOP.com to a site called GGPoker, players who remained in the United States were out of luck, particularly because the most lucrative events were scheduled for then, including the Main Event, which, for a mere $5,000 entry fee, offered a chance at a $3.9 million first-place prize. (The in-person version of the tournament last year had more than double the prize pool and a top prize of $10 million, and the entry fee was $10,000.)

Given travel restrictions to and from the U.S.A., it would have been impossible to achieve international participation, even online through WSOP.com, without a licensed third-party to serve these customers in their home market, the W.S.O.P.s Stewart said.

It would also have been a far smaller event had it remained in the United States.

GGPoker, which launched in 2017, is based in Canada and Ireland with gambling licenses in the United Kingdom, Malta and Curacao. It paid W.S.O.P. a licensing fee to host the tournament. As of mid-August, over 170,000 people have played in the international events.

Konnikova is not one of them because she refused to get on a plane for fear of the coronavirus.

Negreanu, who is a Canadian citizen, could have gone to Canada, but he and his wife preferred to be on a beach so they chartered a private plane from Las Vegas to Cabo, because it seemed like the safest and nicest option in Mexico. We looked at the Covid numbers and Cabo was by far the least affected, he said.

Poker is one of the most popular games played in casinos today. There are huge international events like the European Poker Tour and the World Series of Poker that attract tens of thousands of entrants, and many more people who follow along or play at home.

In the first half of the 20th century, things were very different. Poker was seldom found in casinos, particularly those in Las Vegas. Instead, variations of blackjack were the most popular card games of choice, which is a variation of foundational games like twenty-one and pontoon.

So how did it become the hit that it is today? There are several factors.

Introduction of Poker to the United States

Poker is a game that has several possible origins, all of them located in Europe, and some with very similar names. These are: poque from France, poca from Ireland, brag from the UK, primero from either Spain or Italy, and brelan from France.

While poque and poca have names that are more closely related, brags rules resemble poker the most. They game has antes and lasts until only two players are left, and has a heavy focus on bluffing, a strategy widely used in games of poker.

One of these European games was exported to the United States in the first half of the 19th century. From here, it spread throughout the southern United States, leading to the development of new game variations, including stud poker, new hands, community cards and lowball.

Around the turn of the 20th century, the game of Texas holdem, the most popular variant of poker today, was first played.

Tournaments

From the early 20th century onward, Las Vegas became the global gaming capital, with casinos popping up right along the now-famous strip. As these developed into the integrated resorts that we know today, Texas holdem was introduced to the city.

The game was first played in the California Club in 1963, with other major casinos at the time, like the Golden Nugget, Dunes and Stardust, all offering the game a few years later.

The first World Series of Poker was held in 1970 at Binions Horseshoe, and quickly became one of the most important tournaments in the sport. It has been held every year since, with several other major tournaments replicating and adapting its format.

Poker Boom

These tournaments were one of the factors that fueled the poker boom in the 2000s. Chris Moneymaker won the 2003 Main Event of the World Series of Poker after qualifying online. This saw a huge surge in players enter the competition the following year, all hoping to replicate his success. This happened around the same time that TV stations began to televise the games with the cards face-up, which proved entertaining enough to pull in the viewers.

Online Casinos

The poker boom was also driven by the increased availability of poker and other casino games online. These websites, which began operating from the mid-1990s, allowed players access to these games from wherever they were in the world, reaching far more people than had previously been possible.

Despite pokers growth, variations of blackjack continue to be popular online casino games. Youll find that most operators have a variety of different online blackjack games available on their websites, with varying rules, limits, and quantity of decks. Ultimately, it hasnt been one factor thats made poker the casino favorite that it is today. Tournaments and TV shows help to create more interest in the game, while online casinos facilitate more players. In turn, some of these customers enter major tournaments, creating role models that will help to attract more players again.

The poker boom was an accelerated version of this, created partly by the excitement of Chris Moneymakers win and the increased accessibility of online poker. And although the boom has fizzled out, poker remains a popular option among players in online and land-based casinos.

BROOKINGS The Disabled American Veterans Brookings Chapter No. 22 and the 727 th Transportation U.S. Army National Guard Unit will be hosting the 16th Annual DAV/727th Charity Poker Run on Saturday, Sept. 5 at the Swiftel Center in Brookings.

Registration for the Poker Run starts at 10 a.m. The Big Run departs with a parade at noon. There is a $20 registration fee. The event is open to all vehicles and will be held rain or shine with a $500 best hand and many other prizes. All proceeds go to the DAV Transportation Network.

Why Investing in Tesla Is a Bit Like Texas Hold ‘Em Poker – RealMoney

One day's change in the action isn't a trend. There are no victory laps to take for bears one day from all-time highs. I suppose there are some small victories to be had, but the bulls still control the narrative.

The biggest change may be the fact we're finally seeing a little weakness in Apple (AAPL) , though not eye-popping. The eye-popping belongs to Tesla (TSLA) which printed an unbelievable $540 in the pre-market on Tuesday and a low of $405 today. That's a 25% reversal in a 30-hour time period. Of course, we can't forget Tesla posted a 12% gain on Monday, the first day shares traded post-split.

I tried to think about the best way to explain the recent Tesla move higher. It wasn't exactly a short-squeeze in the traditional sense, but the weekly options market definitely created a scenario in which out-of-the-money call buyers could have possibly forced market makers to continually buy the stock in order to maintain a delta neutral position. And when you think of the post-split price, suddenly those far out-of-the-money calls could be bought with a few hundred each rather than the thousands of dollars required pre-split. If operating against an already open long position, you can more than offset the time decay of the calls with gains in the underlying stock.

However, I think another scenario comes into play. We have a new, fearless breed of momentum investors in this market. This is the explanation I've struggled with explaining to other people, but I think I have it now. Although the Texas Hold 'Em poker fad has regressed, the game is still popular and I'm betting most people have at least played a few hands.

Now, any rational individual will learn the rules first, watch several hands being played, and generally be a bit more conservative at the beginning. Even pros will observe and attempt to push when they hold a strong advantage. And you can have tables running smoother with the most skilled players generally coming out ahead, but what happens when a wrench is thrown into the system? What happens when a player sits down and gets aggressive pre-flop hand after hand after hand? Those big bets will scare off timid players. Those big bets will scare off some speculative hands as well. Those big bets will eventually draw in big money.

And the big money can suddenly move to the hands with a bit more luck than skill, at least for a while. We all know luck eventually runs out. We know that the big pusher will overvalue hands. They may win some, but if you continually overvalue hands, eventually your luck will run out and you'll realize the true value of the hands.

I believe that is what happened with Tesla when it ran into the $500s. Some of the recent traders took a seat at the table and bet heavy pre-flop day in and day out. In the case of the market, we're talking pre-market and post-market. Now, combine this with some larger money focused on pushing action into weekly and short-term out-of-the-money calls, and you create a scenario where multiple players are forced to go all-in pre-flop along with the aggressive buying players.

At some point, luck and firepower, erode. Today, we saw some of that with Tesla as shares nearly touched $400. Since that time, they've bounced 8.5% from the lows as some players sitting short likely pulled their chips from the table. After being down some 15%, I'd admit I was happy to get back into the black and decided to walk away from my short. I threw on a butterfly iron-condor for entertainment purposes, but I'm essentially out of anything meaningful here. I'm not equipped for the players at the table now.

Studying an alternative steroid treatment to calm cytokine storms in COVID-19 – The Mix

In a new trial funded through UABs urgent COVID-19 research program, investigators are comparing the widely available steroid methylprednisolone with dexamethasone, which lowered risk of dying by one-third in a U.K. trial this summer.

Early results from the United Kingdom's RECOVERY trial this summer were a welcome piece of good news in the midst of the COVID-19 pandemic. Critically ill patients on ventilators who received dexamethasone, a corticosteroid drug, were at a one-third lower risk of dying than those who did not receive the drug. Patients receiving oxygen therapy, but not on ventilators, had a 20% lower risk of dying. Nature, the scientific journal, called it a"coronavirus breakthrough."

Better yet, as Nature noted, dexamethasone is both cheap and commonly used. The trouble is, "dexamethasone is already in short supply," said Randy Cron, M.D., Ph.D., professor of pediatrics and medicine at UAB. So Cron and Winn Chatham, M.D., professor of medicine at UAB, are conducting a pilot study of another corticosteroid, methylprednisolone. Outside of COVID-19, oncologists often use dexamethasone to treat hemophagocytic lymphohistiocytosis, a cytokine storm syndrome seen in patients with blood cancers such as leukemias and lymphomas. Rheumatologists such as Cron treat cytokine storm syndrome (where it is commonly called macrophage activation syndrome) in patients with lupus and rheumatoid arthritis using methylprednisolone.

"Methylprednisolone is more readily available than dexamethasone," Cron said. "The thought is if both are equally effective, then physicians can use what is available to them to treat COVID-19 cytokine storm syndrome." And, "by implication, other glucocorticoids will likely be equally effective at the glucocorticoid equivalent dosing."

UAB urgent research grants against COVID-19

This study is one of10 pilot projectsrecently funded by $402,000 in donations as part of the second round of UAB's urgent, high-impact COVID-19 grant initiative. This follows 14 projects funded in the first round of urgent, high-impact COVID-19 grants.

Cron is a leading expert on cytokine storm syndrome, a potentially life-threatening overreaction by inflammatory immune proteins known as cytokines. (In 2019, he published thefirst textbook on cytokine storm syndrome.) "Cytokines are there to fight off infections and ward off cancers," Cron said. "But when they are out of control, they can make you very ill."

Anyone battling a serious infection, regardless of cause, may experience a cytokine storm, Cron said. In addition to blood cancers and rheumatic diseases, cytokine storm syndrome is seen in herpes virus family member infections, and case reports have noted nearly 100 different infectious agents that can cause cytokine storms. In patients with COVID-19, clinicians increasingly see cytokine storm syndrome as a major factor in poor outcomes. Cytokine storm syndrome develops in a large percentage of COVID-19 patients who are ill enough to require hospitalization, Cron said.

Glucocorticoids, such as dexamethasone and methylprednisolone, are both "used to calm the cytokine storm," Cron said. "They have very broad-ranging effects on the immune system," including decreasing production of pro-inflammatory proteins and "decreasing function of multiple immune cell types."

In a pilot project that was one of10 recently fundedby UAB's urgent, high-impact COVID-19 grant initiative, Cron and Chatham, plan to enroll at least 30 patients hospitalized with COVID-19 pneumonia who have features of cytokine storm syndrome.

The World Health Organization initially recommended against use of glucocorticoids in treating COVID-19 because studies of their effectiveness when used against coronaviruses in SARS and MERS outbreaks had varying results, and glucocorticoids increase the risk of secondary infection in patients. "Nevertheless, out of desperation, many overwhelmed centers have resorted to glucocorticoids in treating COVID-19 cytokine storm syndrome," Cron and Chatham wrote in their project proposal, and early reports "have shown promising results." These drugs' effectiveness at calming cytokine storms means the benefits of short-term use outweigh concerns about secondary infections, Cron said. "They can be tapered over time to keep the cytokine storm syndrome under control while the patient improves clinically and by lab markers of disease," he said.

Patients in the study will be randomized to receive either 6mg daily of dexamethasone or 32mg daily of methylprednisolone. The goal is to reduce the need for invasive mechanical ventilation and ICU care. "We believe methylprednisolone will minimize ICU admissions and enhance survival," wrote Cron and Chatham in their project proposal.

Methylprednisolone and dexamethasone have similar side-effect profiles, "but are often used at different glucocorticoid equivalent dosing" methylprednisolone tends to be used at higher doses "and they have different pharmacokinetics, which has implications for tapering dosing when patients are recovering," Cron said.

Patients will receive the drugs at the doses listed above for seven days or until they show stable improvement in oxygen saturation and lower levels of markers of cytokine storm syndrome, including serum ferritin, D-dimer and C-reactive protein.

Genetic signatures for cytokine storm syndrome in COVID-19?

"Methylprednisolone is more readily available than dexamethasone. The thought is if both are equally effective, then physicians can use what is available to them to treat COVID-19 cytokine storm syndrome. And, by implication, other glucocorticoids will likely be equally effective at the glucocorticoid equivalent dosing."

The researchers also plan to explore genetic risk factors that may be shared by COVID-19 patients who develop cytokine storm syndrome. Cron and colleagues have identified genetic risk factors in other cytokine storm syndromes, including thosetriggered by H1N1 influenza. This earlier work suggests that as much as 15% of the general population carries genetic mutations putting them at higher risk for developing cytokine storm syndrome. Finding such risk factors in COVID-19 patients would allow clinicians to target treatments to patients with the highest potential for success on those therapies.

Cron and Chatham are co-principal investigators on the methylprednisolone study, which also includes Devin Absher, Ph.D., of the HudsonAlpha Institute of Biotechnology, where whole genome-sequencing of patients will be done.

Privacy-focused browsers return more health misinformation, researchers claim – NS Tech

A new study overseen by the Oxford Internet Institute (OII) claims that privacy-preserving browsers such as DuckDuckGo return more health misinformation such as anti-vaccine sites than Google.

The study, published in Frontiers in Medicine, analysed the first 30 webpages returned for the search term vaccines autism, to gauge the volume of anti-vax information that came up. It found that alternative search engines (Duckduckgo, Ecosia, Qwant, Swisscows, and Mojeek) and other commercial engines (Bing, Yahoo) returned between 10-53 per cent anti-vaccine pages, while Google returned 0 per cent.

Google has attracted growing criticism in recent years over the vast amount of personal data it hoovers up. This has led to privacy-focused rivals such as DuckDuckGo popping up, which have gained a vocal support base in a short period of time.

DuckDuckGo expressly avoids personalising users search results to negate the filter bubble effect. It collates search results from more than 400 sources, but none from Google. Its website indicates that traffic has continually risen from its inception in 2013 reaching more than 55 million searches in a day this month. However, Google still massively dominates accounting for more than 90 per cent of the search market.

The authors of the study, which was overseen by the OIIs Professor Luciano Floridi and included contributions from research students across Europe, argue that their findings indicate a trade-off between a browsers privacy and information quality. But its not clear whether different approaches to privacy actually explains the differences in the respective browsers search results.

Googles search results are determined by a number of different algorithms that take into account various factors including words of your query, relevance and usability of pages, expertise of sources and your location and settings, according to its site.

Google proactively rates and ranks pages based on quality. In a Google document that provides guidelines to its search quality evaluators, it says that pages that potentially spread hate, cause harm, or misinform or deceive users, should receive the Lowest rating. This can include content such as demonstrably inaccurate content or debunked or unsubstantiated conspiracy theories.

Googles algorithmic tweaks are notorious among websites for resulting in sudden dips in traffic. There have even been allegations that Google maintains a blacklist of both right-wing and left-wing sites, but this has been denied by the company.

The new research was carried out as part of student projects and wasnt explicitly funded, although the OII has received funding from Google for other work this year. In March 2019, Floridi was also appointed to Googles short-lived Advanced Technology External Advisory Council, having previously advised it on the right to be forgotten. In 2015, he was described as the companys in-house philosopher.

The report argues that the greater volume of anti-vax content turned up by other search engines is an indictment of their search algorithms, and that mechanisms should be developed to test search engines from the perspective of information quality [] before they can be deemed trustworthy providers of public health information.

However, this betrays a somewhat twisted understanding of search engines, given that they are not considered trusted providers of information neither legally nor in terms of user expectations. (The raft of illegal and otherwise vile or offensive content hosted on internet search engines, means that if they were liable for the content that appeared on them, theyd all be shut down tomorrow.) The purpose of search engines is to index content on the internet. They perform the role of displaying and, to different degrees, curating, the available content on the web.

The World Health Organisation lists vaccine hesitancy as one of the top 10 threats to global health in 2019. However, its not clear that attempting to expunge or hide anti-vax information from the web would affect these attitudes. Its extremely hard to separate out the effect of online misinformation on behaviour primarily because people come into contact with a range of external stimuli all of which have an incremental effects on their eventual beliefs and actions.

Covid-19 has provided a useful test case for examining this hypothesis, and so far evidence that online misinformation has a serious and widespread effect on health outcomes is not conclusive. For example, a recent study published in the American Journal of Tropical Medicine and Hygiene mentioned that online Covid-19 misinformation was linked to thousands of deaths, but the sources it referred to all concerned people in Iran drinking ethanol in the misguided belief that it would cure the virus.

Iran is very different to countries in the west in terms of both its internet ecosystem, which is subject to a high degree of censorship, and health education. A World Health Organisation study published in 2019 found that 46 per cent of Iranian adults had inadequate health literacy. A BBC article notes that the number of deaths in Iran attributed to Covid-19 misinformation is itself uncertain because alcohol is banned in the country and the bootleg moonshine is often contaminated.

Floridis study itself didnt investigate material harms that have resulted primarily because of online misinformation, but it does highlight a couple of examples. It mentions a case in China where a cancer patient died by following an alternative therapy that they discovered online. The report notes that in response, Chinese authorities issued new rules that require search engines to provide objective, fair, and authoritative results appearing somewhat strangely to endorse the countrys censorious approach to the web.

However, most of the scare-mongering about the impact on fake news or misinformation on health behaviours is based on anecdotal and one-off cases, as opposed to rigorous academic research. In fact, a working paper by Ciara M. Greene and Gillian Murphy of Irelands University College Dublin and University College Cork, found that casual exposure to a fake news story was unlikely to have significant effects on future behaviour, concluding that at present, there is no empirical data that supports the assumption that fake news has a grave impact on health.

Greene told the Nieman Lab that the authors suspect that real-world behavioural effects will mostly emerge in contexts where individuals seek out many stories all advocating the same position, and which are congenial to the individuals existing views; anti-vax or climate change denial networks would be a good example of this.

Indeed, studies indicate that people who hold anti-vaccination beliefs are very likely to be plugged into a conspiratorial worldview that dictates the populace is being lied to by mainstream politicians, institutions and media. A 2016 study (PDF) found that new adoptees are already predisposed to the beliefs through government distrust and general paranoia.

As such, theres not much good evidence that hiding or removing misinformation from the internet actually dampens beliefs. In some cases, removing or demoting this kind of content can even feed into this paranoid worldview, where the individual believes that companies such as Google are colluding with the government to restrict certain information not because its bogus, but because its dangerously true.

Studies have found that those with less education (as well as older and more conservative people) are more likely to share misinformation. But many factors can come into play. A 2015 study published in Plos One, found that educational level and thinking style did not predict vaccination rejection, but psychosocial factors such as preferring alternative medicine and the endorsement of spirituality as a source of knowledge did lead to vaccine-negative attitudes.

The OII study argues that this presents a double jeopardy, in that those who hold anti-authoritarian views and an interest in alternative medicine are more likely to use alternative search providers because of privacy concerns. The report argues that therefore, while the alternative providers might reach a proportionally smaller audience they are reaching an audience that is already more receptive to anti-vaccine information and therefore more vulnerable to its effects.

However, these are also the very people likely to be more predisposed to believe that demoting search results is a result of the information being true. An emphasis on education and digital literacy might equip people to better deal with misinformation, as opposed to demoting search results. Even better, an analysis of why people are prone to mistrust of governments and experts could inform a more successful means of intervention.
