High Health-Care Prices: More Talk Than Action

People in the U.S. go to the doctor less frequently and have much shorter hospital stays than people in other countries thatspend far less per capita on health care. But health services are consistently more expensive here than in comparably wealthy countries. Consider:

*The average price of angioplasty in the U.S. was $27,907 in 2013. In Switzerland, it was $10,897; in the Netherlands, it was $5,295.

*For coronary bypass, the average price in this country was $75,345 in 2013. compared with $36,509 in Switzerland and $15,742 in the Netherlands.

*For MRIs, the average price here was $1,145, compared with $461 in the Netherlands and just $138 in Switzerland.

Price is the major factor that distinguishes the cost of our system from those in other developed nations. The sticker shock of some medical services and drugs is also the dimension of the health-cost problem most visible to the public. So its interesting that most efforts in this country to address health-care costs dont focus on price much at all. Instead, they focus on reforming the delivery of health care and provider reimbursement to reduce the volume of health care Americans use and to weed out unnecessary procedures and hospitals days. There is a simple equation: price times volume equals spending. A comprehensive strategy would focus on both sides of the equation: volume, including eliminating unnecessary care, and price.

To be sure, high medical prices aretalked about a lot. One reason there is more talk than action is the anti-government environment, which would inhibit regulatory action to constrain prices. Taking on price also means taking on health cares powerful industry interests. More effective competition between providers would help reduce prices, but the health-care industry appears to be consolidating more than competing, as is the health insurance industry. Greater price transparency would enable consumers facing higher and higher deductibles to make more price sensitive choices, but price information is very limited and price may not be uppermost in peoples minds when they are in the middle of a medical crisis and their doctor writes a referral to get a procedure or test at a particular place.

It would be a mistake to make price the only focus of a cost-reduction strategy. But its striking that while price is such an important reason our system appears to cost so much more than others, efforts to reduce the high prices of medical care are not a meaningful part of current cost-reduction efforts.

UTHealth research: Mental health care lacking in state and federal prisons

HOUSTON - (Jan. 12, 2015) - A significant portion of state and federal prisoners are not receiving treatment for mental health conditions, according to research by The University of Texas Health Science Center at Houston (UTHealth) School of Public Health. The study was published recently in the American Journal of Public Health.

Mental health disorders among prisoners have consistently exceeded rates of disorders in the general population. Twenty six percent of prisoners reported a mental health diagnosis in the study, compared to 18 percent in the general population in 2012, according to the National Institutes of Health.

State and federal courts mandate that inmates must have access to adequate health services in prison. However, that mandate usually covers only "severe or serious" mental illnesses, according to the paper.

"Individuals with untreated mental health conditions may be at higher risk for correctional rehabilitation treatment failure and future recidivism after release from prison," said Jennifer Reingle, Ph.D., principal investigator and assistant professor in the Division of Epidemiology, Human Genetics and Environmental Sciences at UTHealth School of Public Health Dallas Regional Campus.

Data was obtained from a 2004 national sample of state and federal prisoners, which included 14,499 participants in state prisons and 3,686 in federal prisons. Participants self-reported whether they had been taking medication for a mental health condition upon entering the facility and whether they were still taking medication while in prison, marking treatment continuity.

At the time of admission, 18 percent of each sample was taking medication for a mental health condition, but only 52 percent of that subset of the population in federal prisons and 42 percent in state prisons received medication during their sentence.

Depression was the most prevalent mental health condition, reported by 20 percent of inmates, followed by mania, anxiety and post-traumatic stress disorder.

"Screening tools are not consistent across prisons and inmates could be diagnosed with different conditions or not diagnosed at all when they get transferred to a new location," said Reingle. "A standardized mental health screening process could benefit the inmates and the prison system as a whole."

RECENT HEALTH CARE STRIKES

Arrowhead Regional Medical Center: Hundreds of nurses staged a three-day walkout at the Colton hospital and other sites in San Bernardino County Dec. 9-11. Negotiations have resumed.

Riverside Community Hospital: Nurses staged an informational protest in front of the facility in mid-December, saying they were working in unsafe conditions with a high patient-to-nurse ratio. Negotiations were expected to resume this month.

Dozens of protesters marched against Kaiser Permanente in Fontana on Monday, claiming the nonprofit was understaffing its mental health facilities.

The strikers were members of the National Union of Healthcare Workers, who say they have too little time for patients.

Kaiser keeps piling on more work to do in a standard clinical hour. Its now to the point where instead of giving a patient a clinical hour, you give them maybe 30 minutes, said psychiatric therapist Marty Needleman, a union member on the picket line.

He said the nations new health care law has caused an influx of patients, compounding an already bad situation.

The walkout affects about 2,600 workers statewide and 250-300 workers in Inland Southern California, according to Needleman. It began at 6 a.m. and will last until 6 a.m. Jan. 19. Negotiations are on hold this week, he said.

Needleman said that 70 people were walking the picket line this morning.

Kaiser officials deny the nonprofit is failing its mental health patients and says it has increased the number of therapists in the state by 25 percent, a claim the union disputes.

Fourth Freedom official released pending March trial

The fourth former Freedom Industries official indicted in connection with last Januarys chemical leak on the Elk River appeared in court Monday afternoon and was released on bond pending trial.

Charles E. Herzing, 63, of McMurray, Pennsylvania, was released on a $10,000 unsecured bond after he was arraigned by U.S. Magistrate Judge Dwane Tinsley in Charleston.

Herzing faces three misdemeanor counts of criminal Clean Water Act violations related to Freedoms Jan. 9, 2014, spill of MCHM and other chemicals that contaminated the drinking water supply for hundreds of thousands of people in Charleston and surrounding communities. If convicted, he would face a maximum statutory sentence of three years in prison.

Not guilty, Herzing said during Mondays hearing when Tinsley asked for his plea on the charges against him.

Herzing was one of Freedoms owners prior to December 2013, and was the company vice president and a board member, according to court records.

Two other former Freedom officials -- Dennis Farrell and William Tis -- are also free on $10,000 unsecured bond. They face the same charges as Herzing.

Former Freedom Industries President Gary Southern is free on a $100,000 unsecured bond, He faces similar Clean Water Act charges, but also was indicted on 10 felony counts based on the allegation that Southern lied about his role at Freedom to protect his personal wealth from civil litigation and Freedoms bankruptcy proceeding. If convicted on all counts, Southern, 53, would face a statutory maximum of 68 years in prison.

The four Freedom officials were indicted last month by a federal grand jury following a lengthy investigation by U.S. Attorney Booth Goodwins office.

Assistant U.S. Attorney Phil Wright had asked Tinsley last week to force Southern to post a secured bond to assure Southern doesnt flee prior to trial. Tinsley declined, ruling that Southern was not a flight risk.

During the time they were responsible corporate officers for Freedom, the officials allegedly approved funding only for projects that would result in increased business revenue for Freedom, or that were immediately necessary for required equipment maintenance, officials alleged. They allegedly failed to take action to fund other repair and upkeep projects for equipment and systems necessary for environmental compliance at Freedoms Elk River facility, including repairing a spill containment wall, addressing drainage problems in the containment area, and implementing proper water protection plans.

No individual desires to have his or her freedom clipped. Everyone desires that he or she should be allowed to do whatever pleases him or her. This is how the human race was made. On the contrary, freedom does not walk alone, it walks with responsibility. It can be compared to a child and its mother; separating them is an attempt to deny the child a motherly love and the mother, the exercising of a natural duty to show love, care and concern. The above analogy can be used to discuss media freedom and responsibility in Ghana. Media analysts have posited that the media is the fourth estate of the realm, the media is a watchdog of society; who watches the watchdog? In all of the above, the media seeks to create a bar of responsibility in all spheres of society, thereby seeking to create pretty nearly a utopia society. This is worth the plaudits of all and sundry. However, as the media seeks to create a pretty nearly utopia society, what are the guiding principles that inform how far, how wide, or how deep the journalist should go or what are the values that define our society in the course of exercising a professional right? As stated in the beginning paragraph, no individual wants to have his or her freedom clipped, it is of much concern to acknowledge that media is an aspect of society; therefore, the media itself must seek to consistently see how it is unfair to divorce freedom of the media from the responsibility of the media. Every society is guided by legal principles and moral values. It is not wrong to learn from others, but it is a sign of innovation and lack of self-esteem to copy others exactly without taking into consideration the legal, cultural and moral values that informs the ideas we tend to copy. Media freedom and responsibility is a universal phenomenon. We interact with other peoples of the world; the world is a global village and what relates to one group of people is probable to relate to another group of people. Be that as it may, as we live in the globalised world, we as professionals should identify how our intrinsic values that define us as Africans and Ghanaians relate with the norm and charters of the larger world when it comes to media freedom and responsibility. Media theorists have argued that, audiences are discerning; the audiences have the ability to determine what is false and true. Again, it is worthy to note that the power of the media to influence their audience is very high. In fact, people take whatever the media puts out as wholly true. Our values that are transmitted to us through African folktales never extols acts of telling lies about people or trying to making heavier the emotional burdens of our kith and kin. The African lives by the principle of the Bantu dictum, umuntu, ngumuntu nquabantu to wit, a human is human because of other humans. In Ghana, among the Ewes of the Volta Region, it is am3n) vi eny3 am3. Albeit, this principle has sometimes been used to further selfish interests, it still upholds the value that guides our positive attitudes and behaviours which we show towards our own families and friends in times of emotional distress. Chinua Achebe, noted at one point that, it is not enough for us that our art should merely report the nature of things; it should change it. As the media reports attacks on itself, the media should begin to take a deep look into its activities and pragmatically make efforts that will make responsibility a twin of the freedom that is sought. Responsibility should be consistently discussed; when the media seeks to be free from all the checks that sections of society desire to place on it, the media can make a case of checking its self. In addition, the emergence of social media poses a great burden of showing how responsibility is upheld by the media. Journalism in the social media is slightly different from what goes on in the traditional media. This call for discussion on how issues that are reported in social media can meet responsible standards without gagging the new media. The Ghana Journalists Association must begin to initiate this discussion of self-regulating its members to avoid the issue of the consistent attacks that are meted out to members of the media. Journalism is a dignified profession that must not be seen advocating being only free to operate; it must equally check itself and behave responsibly. This gives the impetus to the media to be professionally aggressive in the course of their duty to do follow-up on running stories, demand accountability from all those who have been given the privilege to serve the interest of society at one level or the other. Freedom of speech has taken center-stage in the wake of last weeks shooting at the headquarters of Paris based satirical newspaper, Charlie Hebdo. The attack is assumed to be a response to Hebdo printing caricaturized images of Mohammad, according to the New York Times.

The hashtag #JeSuisCharlie, a movement aimed to show solidarity for the victims of the shooting and their families, the weekly, and the Parisian community and its right to freedom of expression, continues to trend on Twitter. The Daily Mail reported the hashtag was tweeted over 250,000 times within four hours of the incident.

Few issues have so successfully united both the left and the right in American politics, with almost universal outrage over what most commentators believe to be an attack on the basic human right of freedom of speech.

But freedom of speech, and blasphemy in particular, isnt universally protected around the world, even among many liberal democracies. According to a 2011 study published by the Pew Research Center, 47 percent of the worlds countries have laws against blasphemy, apostasy and/or defamation of religion, including religious hate speech.

These laws are not limited to the countries one might typically expect. European countries such as Ireland, Germany and Denmark, among others, currently have laws against blasphemy and defamation of religion.

Eco-friendly system to power Cafe Kudumbasree units

The Kudumbasree mission is gearing up to open 20 branded cafe outlets utilising a bio-waste-based cooking system to promote alternative energy sources and drive down the cost of operation.

As many as 10 of the Cafe Kudumbasree units will be opened in urban centres in the pilot phase this year.

The project, scheduled to take off in December, had been held up by the delay in making land available for the units. We have cleared the initial hurdles. The land for the units has been identified and the equipment procured, an official said. The cafe units, operated by women entrepreneurs, will double up as catering training institutes under a project supported by the Centre.

The official said the steam cooking system would result in substantial savings on the cost front. A report by the Adebha Institute for Food Research and Hospitality Management, the official training agency for Kudumbasree, the patented system named Rapid Steamer could bring down the cooking cost to less than 50 paise per meal.

The system is in operation at the Kudumbasree Cafe on the premises of the Mental Hospital, Thrissur. The Government Vocational Higher Secondary School, Vattiyurkavu, also uses the Rapid Steamer to prepare the midday meal for 316 children at a cost of 43 paise per meal.

The Kudumbasree Mission has placed an order Rs.1.02 crore for 20 units of the steamer developed by a Thiruvananthapuram-based company.

Researchers Testing "Eco-Vapour" Toilet System in India

Washington: A new "eco-vapour" toilet system being tested in India uses a breathable fabric that traps human waste and allows only water vapour molecules to escape, rendering sewage less hospitable to bacteria and other disease-causing organisms.

A team of researchers led by Steven K Dentel, Professor of civil and environmental engineering at the University of Delaware in the US, has been working for several years on a breathable fabric that can be used to line pit toilets and other basic sanitary facilities in developing nations.

The fabric Professor Dentel and his team are developing is similar to that used in sports jackets and raincoats; it only allows tiny water vapour molecules through.

Professor Dentel said this could be a valuable way to filter out liquid water from human waste, letting the pure water escape while retaining everything else.

Funded by the Bill and Melinda Gates Foundation, researchers have been working on the project for some time now.

On December 28, a small group led by doctoral student Shray Saxena headed to India to begin the first field test of the new fabric.

"A lot of people in India right now don't have improved toilet systems. Even in cities like Kanpur (UP), which are really quite developed, people do not have these facilities available to them," Shrey said.

Families in two cities, Kanpur and Puri in Odisha, are trying out the new "eco-vapour" toilet system, with sewage collected in 55-gallon drums lined with the breathable fabric, allowing water vapour to evaporate.

The researchers are observing how the fabric performs under varying conditions of heat and humidity, which affect the rate at which water diffuses through the membrane.

If external humidity is high, the lined drums may fill up before enough of the water can evaporate.

Ive always associated human characteristics with cities I explore. Edinburgh feels like a moody yet brilliant gentleman. Belfast is a brawny rugby player you try to force into a business suit, and my city, Stockholm, feels androgynous. Very jean jacket, flipped collar and white pants. Stockholm wants you to find your own secret space and own it selfishly.

It is the most open society run by the most private people.

Buildings in Stockholm are all shades of ochre yellow and brown ochre buildings, pale lemon, dull pumpkin orange, mustard yellow, and blush red with the occasional grey futuristic-style buildings and 1960s block-style buildings with weird colour choices and white trims thrown in.

In my opinion, the best building in town is a toss-up between the gargantuan National Museum building which was built in 1866 and which is currently closed for renovations until 2017; and Stadshuset (City Hall) with its impressive series of archways and tower. The City Hall building was opened in 1923 and it hosts the Nobel Prize dinner and gala every year.
