Effects of COVID-19 on the Healthcare Delivery System – The Good, The Bad, and the Ugly: SF STAT! – South Florida Hospital News

A year ago, many healthcare transformation thought leaders were predicting that the greatest disruptors in our industry would be healthcare company megamergers or blue chip tech companies entering the healthcare delivery marketlike CVSs acquisition of Aetna, Apples foray into personal health records and interoperability, or Amazons empowerment of Alexa and acquisition of PillPack. Probably none of them would have named the novel coronavirus as the biggest prospective change agent in the industry. But here we aresmack in the middle of a COVID-19 pandemicand many of the medium- to long-term effects of the virus have become apparent to those of us in the field. Heres a glimpse at some of the trends and impacts on the healthcare industry from COVID-19: the good, the bad, and the ugly.

The Good:

Telehealth: From Wall Street to Main Street, one of the most significant effects of the pandemic has been the explosive adoption and acceptance of telehealth as a means of delivering healthcare services. Healthcare delivery was already moving out of the four walls of the hospital into outpatient sites of care and even into retail settings. But COVID has brought providers to the desktops and mobile devices of mainstream America. Some estimates indicate that, as of April 2020, telehealth (defined as synchronous video conferencing, store and forward of digital medical data, and remote patient monitoring) has grown by 80% year over year1, spurred on by the relaxation of telehealth guidelines and reimbursement restrictions previously adopted by public and private insurers. Sure, there are still challenges to addresslike continuation of these looser reimbursement policies, interstate licensure and credentialing issues, broadband access in rural areas, etc.but the horse is out of the barn and theres no turning back now. Telehealth has the ability to positively impact healthcare in many ways, including creating greater access to primary care, abating the overcrowding in the ED, and enabling a better quality of life for chronic disease patients. Most systems have already invested heavily in telehealth; those that havent need to catch up quickly.

Self-care and family care: Despite some mixed and often diffuse messaging from public health authorities early in the pandemic and the occasional deliberate flaunting of social distancing guidelines by segments of the population, by and large, most Americans are embracing more personal responsibility in the care of themselves and their loved ones. Mainstream America is now adopting many of healthcares sacred principles like frequent handwashing, cleaning and sterilizing ones immediate environment, and adopting practical infection control practices (think masks, gloves, etc.). In addition, baby boomers and Gen X-ers are becoming steadily aware of the hidden hazards of congregate living for their parents, and they are increasingly choosing noninstitutional settings for their aging or disabled family members. Even though these family members may be homebound, they often lead a more dignified, more comfortable, and happier life than their counterparts in more institutional settings. Innovative hospital systems like Johns Hopkins Hospital in Baltimore or Adventist Health in California have created Hospital-at-Home programs (virtual hospitals) which feature video visits and remote patient monitoring by hospitalists to enable patients to remain in the comfort of their own homes during their hospitalization and convalescence.2

Occupational Health and Safety: Employers are becoming increasingly aware of their new responsibilities to provide a COVID-free work environment (and their potential liabilities if they do not). Federal and state courts are already seeing a dramatic rise in the number and types of COVID -related lawsuits, including cases related to workplace safety, workers compensation, and employees in high risk settings.3 Many employers have responded by dramatically reducing business meetings and travel and by converting as much interaction as possible to virtual settings. For those employers who need on-site employees, there is a renewed interest in employee health and safety, to wit, a dramatic rise in plastic protective shields for frontline personnel, the provision of PPE for essential workers, more robust disinfectant processes for common areas and equipment, and the reconfiguration of offices spaces to allow for more social distancing. The pandemic provides the opening for health plans, healthcare providers, and occupational health and safety professionals to play a larger role in executive conversations, facility design, and employment practices.4

Artificial Intelligence: Before the pandemic, we had already seen the promise of AI in healthcare applications like using supercomputers to interpret radiology scans or identify skin lesions5, employing machine learning to aggregate and analyze huge amounts of diagnostic data to aid in accurate cancer diagnosis6, and applying real-time data analytics to speed up the healthcare revenue cycle7. But the pandemic has hastened the adoption of AI areas not heretofore imagined. For example, health departments are using AI to analyze large amounts of public health data to identify and predict new virus hot spots and to develop early warning systems, clinicians are using chest imaging data along with AI-enabled EHR data to improve risk stratification and to categorize the type of care COVID-19 patients receive8, and pharmaceutical manufacturers are using AI to accelerate vaccine research. Look for AI to be adapted to many more clinical and operational aspects of healthcare delivery in the future.

The Bad:

Effect on Medical Practices and Hospital Volume: The traditional bastions of healthcare deliverythe doctors office and the hospitalhave been hit particularly hard during the pandemic. One MGMA study showed that 97% of all medical practices have endured a financial hit, some with revenue declines of 60% or more.9 Many physician practices have had to furlough or eliminate staff and cut their own salaries; many will not survive at all. This will further exacerbate the already growing physician shortage and further limit access to those needing care. Hospitals have had a double whammy: eliminating or reducing profitable elective surgeries while simultaneously treating resource-intensive (but comparatively low-reimbursing) COVID patients. And despite the CARES Act and other federal assistance, many hospitals, particularly safety net institutions, are facing huge cash shortfalls and will be severely limiting capital expenditures for growth initiatives for the foreseeable future. Smaller, independent, and rural hospitals are particularly at risk for their survival. M & A activity, which was already strong prepandemic, is likely to increase as stronger organizations and institutions absorb the more financially-strapped providers. Government officials and private insurers should closely monitor the situation and be prepared to provide relief in the form of advance payments, expedited reimbursement, and grant funding when warranted.

Delays in Care: Some studies indicate that near of all Americans have delayed seeking care because of loss of income and the fear of infection from COVID.10 The negative consequences to the patient for not obtaining timely care can result in needless suffering and death. There are already signs that cancer rates are rising rapidly and caregivers are seeing more deaths from heart disease than expected.11 In addition to patient effects, the impact to healthcare organizations will also be significant. Time and again, research shows that delays in seeking care result in poorer clinical outcomes and more expensive hospitalization costs. Despite losing their profitable selective surgeries and service lines, healthcare organizations need to prepare now for the demand that has been suppressed by the pandemic. When bed capacity allows, healthcare organizations need to encourage and reassure those who have delayed care; to do so, they need to beef up their infection control processes and convince their patients that they are ready for the new normal of treating patients in an institutional setting. As they do so, recognize that this pent-up demand will probably result in more acute care episodes, longer lengths of stay, and higher costs.

Uptick in mental health and substance abuse conditions: The pandemic has not only created a greater need for behavioral health interventions; it has simultaneously upended the already patchy and fragile continuum of care for mental health and substance abuse patients. Even as 40% of Americans have reported that stress related to the coronavirus has negatively affected their mental health,12 staffing crises and shortages of protective supplies have caused a reduction in mobile crisis teams, residential programs, and behavioral health call centers. Also, during the pandemic, there has been a surge in alcohol sales and a dramatic spike in opioid deaths. Social distancing, for all the good that it has done in society at large, has imperiled social connection and support for addicts and has jeopardized their recovery. Some relatively novel interventions have cropped up, including community-led and family-led interventions, more robust remote therapy, and technology-enabled collaboration platforms for primary care and behavioral health providers.13 We as a healthcare system need to continue to find new ways to address the depression, isolation, and drug use exacerbated by the pandemic and to create treatment modalities which are safe and effective in dealing with behavioral health conditions.

The Ugly:

Disparities in Care: COVID-19 has laid bare the ignominious underbelly of our healthcare delivery system; that is, that vulnerable populations are more at risk for contracting the virus and less likely to recover. COVID-related morbidity and mortality for Black, Latino, and native Americans are three to four times higher than for white Americans.14 Although genetic or biological factors may have some small bearing on these statistics, the more likely explanation lies in the longstanding cultural, social, and environmental conditions associated with these ethnic and demographic groups. Societal factors contributing to these disparities in disease incidence and hospitalization rates include housing conditions, income, education, and wealth gaps, access to healthcare providers, and inherent societal discrimination.15 Healthcare executives alone cant be expected to fundamentally undo the years of socioeconomic injustice. They can, however, be vocal proponents of health equity and should be proactive in identifying and rooting out any vestige of inequitable care in their organizations. They can also use their position in the community to reach out to underserved populations, to promote positive policy changes, to address social determinants of care, and to espouse socially-conscious healthcare delivery.

Public Health and Health System Preparedness: Finally, COVID-19 has also exposed the potholes and gaps in our public health system. Clearly, there were miscommunications in our early minimization of the potential magnitude and dangers of the virus and missteps in our uncoordinated response efforts. Confusion between state and federal responsibilities, lack of communication among large federal agencies (such as the CDC, the FDA, and FEMA), state public health departments, hospitals, private laboratories, and equipment suppliers led to a suboptimal rollout of diagnostic testing and a lag in ensuring that hospitals had adequate personal protective equipment and ventilators. Ultimately, this delay and confusion resulted in lost time and lives. But the problem goes deeper than interagency or state/federal miscommunication. Simple complacency or a false sense of security led to an erosion of our public health infrastructure. As far back as 2000, the Institute of Medicine issued a report entitled Public Health Systems and Emerging Infections: Assessing the Capabilities of the Public and Private Sectors,16 warning that diminished funding levels and lackluster support for the American public health system, particularly at the state and local levels, would lead to diminished capacity to predict, detect, and respond to an emerging infectious disease. Despite this warning and others like it, the last decade has seen a further reduction in the spending on the public health infrastructure and a decline in the public health workforce by more than 15%.17 Even in the wake of SARS, Zika, Ebola, H1N1, and now COVID-19, the CDC has seen relatively flat funding for the past 10 years.18 As citizens, we should demand accountable leadership; and as healthcare leaders, we should rally for appropriate reinvestment in our federal, state, and local disease surveillance and response systems.

So there you have it. COVID-19 is a devastating and deadly disease, and it has tragically impacted us (as of late July) with over 4 million cases and over 146,000 deaths19and thats just in the United States. It continues to dominate our news cycle, our economic recovery, and, indeed, our very lives and livelihood. But it has also impacted us in ways we never imagined. We have learned that the pandemic has implications not just in healthcare, but in social justice, economic well-being, international cooperation, and world order. As much as we might want to put the pandemic in our rear-view mirror, lets not let this experience be for naught. Lets capitalize on our successes, learn from our failures, and candidly look at the opportunities to improve our preparedness and response efforts for the next inevitable disruptor in healthcare.

Performance in the Consumer Credit Market Holds Steady as Number of Borrowers in Financial Hardship Status Stabilizes – Stockhouse

CHICAGO, July 23, 2020 (GLOBE NEWSWIRE) -- A new TransUnion (NYSE: TRU) consumer credit snapshot found the percentage of accounts in financial hardship” started to level off for credit products such as auto loans, credit cards, mortgages and personal loans during the month of June 2020. Some of this leveling off was due, in part, to accounts coming out of financial hardship status in June.

Accounts in financial hardship defined by factors such as a deferred payment, forbearance program, frozen account or frozen past due payment have largely kept delinquency numbers in check as consumers continue to navigate the ongoing impacts of COVID-19. TransUnion’s financial hardship data includes all accommodations on file at month’s end, and includes any accounts that were in accommodation prior to the COVID-19 pandemic.

Accommodation programs have provided consumers with much needed payment flexibility as external triggers such as rising unemployment and a decrease in government relief funds have started to shape the future outlook of the consumer wallet.

In the early months of the pandemic, unemployment benefits and relief from the CARES Act gave consumers a bit of a cushion, leaving the consumer fairly well-positioned from a cash flow perspective,” said Matt Komos, vice president of research and consulting at TransUnion. Lenders have been working with consumers during this time of uncertainty by extending financial hardship offerings that help them understand and manage their financial situation. These accommodations have been working as intended and have helped thwart a material breakdown in delinquency performance in the near-term.”

Since the pandemic began in March 2020, delinquency performance has held steady, with credit products across auto, credit card, mortgage and personal loans all showing a recent month-over-month improvement in performance from May to June 2020.

Credit cards saw the greatest decline in delinquency over this period with borrowers 90+ days past due (DPD) decreasing from 1.76% to 1.48% month-over-month. This decrease also held true for accounts in 30+ DPD delinquency status an early indication that may signal consumer distress by decreasing from 3.06% to 2.66% from May to June (compared to 3.49% at 30+ DPD in June 2019).

Consumer balances for credit card also showed a 7.41% year-over-year decline from June 2019 to June 2020 as well as a monthly balance decrease of $43 since May. These decreases may signify that consumers are continuing to manage debt prudently and are paying down their existing card balances. At the same time, overall consumer credit lines have declined from $24,641 in June 2019 to $23,724 in June 2020, which is also down from $23,800 in May 2020.

These are signs of a credit market that continues to function despite the spike in consumer unemployment,” said Paul Siegfried, senior vice president and credit card business leader at TransUnion. When there is uncertainty in the market, consumer credit performance is highly scrutinized and new accounts generally will not receive the same type of credit limit as they might have prior to a crisis. However, the longer individuals who are not in an accommodation program perform well, the more likely additional credit will be extended.”

Over the course of the pandemic a substantial segment of consumers have continued to make payments, but are also proactively engaging with their lenders to discuss payment options. TransUnion’s ongoing Financial Hardship Survey indicated that of consumers with a current financial accommodation on a loan, 32% are in favor of repayment plans that will allow for paying down debt gradually while continuing regular payments. A smaller percentage (18%) preferred paying off all postponed payments with a lump sum and 21% indicated they would like financial accommodations to be extended further.

By many accounts, we are still in the early phase of the pandemic, and there is some uncertainty still around the nature of the economic recovery we may experience. It will likely be months before the financial impacts of COVID-19 begin to materialize from a credit performance standpoint, and some of this will be dependent on any additional government actions. During this period of time, lenders will need deeper consumer insights to better calibrate risk across their portfolios and make more informed decisions,” concluded Komos.

The fall of the girlboss is actually a good thing – Business Insider

June 2020 was the month the girlboss facade collapsed.

It began as a slow crumbling. In December, Steph Korey stepped down as CEO of cult-favorite luggage brand Away (she would go on to step back in a month later and resign a second time in July). In February, Tyler Hanley resigned as CEO from the millennial activewear company she founded, Outdoor Voices.

Come June, girlbosses began to fall like dominoes amid allegations of toxic work cultures that perpetuated racism:

In the midst of it all, Sophia Amoruso the pioneer of the term girlboss resigned on June 22 from the #GirlBoss media platform she had created years earlier. But while the girlboss has met a swift and public downfall, that's actually a good thing and it's not the end of the female leader. Instead, it's the beginning of a new space for more inclusive leaders to shine without being defined by gender.

So, what exactly happened to the girlboss? To fully understand her rise and fall, you have to go back to 2014.

The female founder trying to crack the glass ceiling isn't a new concept, but 2014 was the year she got a new name.

That year, Sophia Amoruso wrote the now-famous memoir #GirlBoss, laying the foundation for the #GirlBoss media platform she launched three years later. At the time, 30-year-old Amoruso was CEO of online fashion retailer Nasty Gal (Nasty Gal would go on to file for bankruptcy in 2016).

Suddenly, women everywhere were using the term as it came to encompass more than the definition of a female CEO and instead embodied the overall attitude of women particularly young ones being able to do anything. The girlboss Instagram account has 1.6 million followers, and to date, the term has been hashtagged more than 20.2 million times. Celebrities from Miranda Kerr to Gwyneth Paltrow have self-identified as a girlboss.

Other monikers like #bossbabe, SHE-E-O, and the boss bitch soon started cropping up, all reincarnations of the same ideal: the young, ambitious woman who can have it all while lifting up other women on her path to success.

The girlboss had hustle. She was an entrepreneur or a leader in her industry. In many iterations, she was a millennial. And she often had that effortless, "it"-like quality of knowing what she wants and of relying on being cool without seeming to care that she's cool.

This very aesthetic also became the heart of the brand she created and led.

Leandra Medine started Man Repeller in 2010 as a personal fashion blog with cheeky takes on how women can dress for themselves, not for men. The "I don't care what other people think of me" attitude struck a chord with cool girls, transforming the brand into the lifestyle site it is today.

Man Repeller features articles on Reformation clothes, which was dubbed the label for "cool girls" everywhere and seen on celebrities such as Karlie Kloss and Kendall Jenner. The secret to Reformation founder Yael Aflalo's success, wrote Emilia Petrarca for W Magazine, is that she "has taken her personal Los Angeles cool-girl style ... and translated it for the masses."

Similarly, Jen Gotch, who spent time "rubbing elbows with Instagram cool girls," per Buzzfeed's Stephanie McNeal, founded Band.O and served as its "muse." She built the brand on an upbeat mission that fused self-care with female empowerment, a combination she has personal experience with due to a history of anxiety and bipolar 2 disorder.

Meanwhile, Christene Barberich's Refinery29 played to the cool girl's ambitions and political savviness with series like Money Diaries and 2020 election. And that reader is the same girl likely to be a member of The Wing, co-founded by Audrey Gelman, a cool girl whose wedding was featured on Vogue.com and who made an appearance on the hit HBO show "Girls."

These platforms created a needed space for the stylish and career-oriented woman, becoming successful empires along the way. The Wing opened in 2016 with a 13,000-person wait list and a 20,000-follower Instagram presence. Today, the company is estimated to be worth $200 million. Within three years of its founding, Reformation had become an e-commerce business; in 2019, it projected yearly sales to exceed $150 million. And in 2019, Vice acquired Refinery29 in a $400 million deal.

But in her quest to tout gender inclusivity, the girlboss fell short in another critical form of inclusivity: diversity. As Leigh Stein for Medium wrote in a takedown of the girlboss, "Racial inequity was never really on her radar. That was someone else's problem to solve."

By ingraining her personal identity into the brand's identity, the girlboss left no room for the cool girl to be anything but white and wealthy. Her downfall comes as corporate America faces a reckoning

Since the peak of the Black Lives Matter protests in June, employees of color have revealed the very white core of these companies.

Two former employees told CNN Business' Kerry Flynn that Barberich would reject photos of Black or plus-sized models for Refinery29, deeming them "off-brand." Barberich acknowledged to CNN the brand's shortcomings came at "the detriment of Black women and women of color in particular."

Medine took to Instagram to address accusations of favoritism and a lack of diversity at Man Repeller:

Gotch, too, took to Instagram (her account has since been deleted) to respond to employee allegations of racism at Ban.Do after a Buzzfeed investigation revealed a racist, "mean girls" environment at the company. "I am guilty and not only am I guilty, I have been so ignorant and so insulated by the ease and comfort of my white privilege..." she wrote.

A New York Times expose from March on The Wing revealed low pay, poor treatment, and racism at the company. When these criticisms resurfaced in June, Gelman resigned, saying in an email to staff that stepping down was "the best way to bring The Wing along into a long overdue era of change."

It's a similar story at Reformation, where a racist culture pushed out Black employees, former employees told Business Insider's Bethany Biron. In an Instagram statement, Aflalo apologized for failing the Black community:

In what Fortune's Emma Hinchliffe dubbed a reckoning, a total of eight female founders had stepped down from the companies they were leading by the midway point of 2020.

Not every young female founder has stepped down. Emily Weiss remains with Glossier; Danielle Weisberg and Carly Zakin remain with The Skimm. Not every woman at the helm of a millennial-loved company is white: Consider Payal Kadakia of ClassPass or Arum Kang, co-founder and CEO of dating app Coffee Meets Bagel.

Not every company with a bad boss is a start-up helmed by a woman or a millennial. Recent investigations into companies like Pinterest, CrossFit, and Bon Appetit each of which is (or until recently was) helmed by male leadership have uncovered toxic workplaces.

And not every female helming a company stepped down for the same reason. Amoruso took to Instagram to announce that her departure from the #Girlboss platform was tied to the company's decimated revenue during the pandemic (past reports have indicated that she oversaw a culture rife with poor leadership and high turnover at Nasty Gal, allegations she did not respond to at the time).

But as the reign of some of the most prominent girlbosses has come to an abrupt end, what has opened up is space for a new type of leader. Refinery 29 and Ban.Do are both currently seeking successors to Barberich and Gotch, respectively, while Reformation has replaced Aflalo with the brand's current president. Gelman was replaced by an "office of the CEO" helmed by three executives. And Medine has stepped into an intern role at Man Repeller, which is hiring a diversity and inclusion specialist.

What's more, as the girlboss is meeting her downfall, so is the "girlboss" title and that's a good thing. Such titles might be intended to empower women, but they only reinforce the idea that women aren't equal to their male counterparts.

"While 'girlboss' immediately draws attention to the feminine, it also infantilizes the role of a female as a boss," Magdalena Zawisza, a Reader in Consumer and Gender Psychology at the UK's Anglia Ruskin University, told BBC in January. "Have we ever heard about 'boy bosses?'"

This infantilization makes disavowing the girlboss title a step toward gender equality. The girlboss downfall is not the end of the female leader, but the beginning of a new space for more inclusive leaders to shine without being identified by gender.

It's time the world sees that a bad boss is a bad boss and a good boss is a good boss, regardless of age or gender.

Perseverance Rover Launches on July 30, Here’s How to Watch – autoevolution

Just a little over a week separates us from the moment when the Perseverance rover is scheduled to take off on its mission to Mars. The American space agency is targeting a 7:50 a.m. EDT Thursday, July 30 launch from Cape Canaveral Air Force Station in Florida, but that may change depending on a variety of factors.

To get the hype up, NASA launched in the previous years a naming competition for the rover (Perseverance won) asked people to digitally write their names on chips that would travel to the Red Planet (about 11 million names were entered) and even had a live feed of the rovers bay up and running the whole time the machine was being assembled.

It only makes sense for the launch to be a very in-your-face event. NASA will kick off pre-launch festivities on Monday, July 27, with a pre-launch news conference and science briefing, On Tuesday, some more briefings will follow, this time related to the sample return part of the mission, and another news conference is scheduled for Wednesday, July 29.

On the day of the launch, NASA will air live the start of the mission on the NASA Television Youtube channel (video attached below) and the agencys website. By 11:30 a.m. EDT, the launch should all be done with, and a post-launch briefing is scheduled.

If you still have mixed feelings about Perseverance, you should know this: the rover is the single most important machine humans have sent to another planet. Not only is it tasked with all the chores described above, but it is also the first piece of hardware to be sent to Mars as part of the countrys Moon to Mars exploration approach that will culminate, some hope by the end of the current decade, with the first humans setting foot on another planet.

Triplet Therapeutics nabs Scholar Rock R&D exec as new CSO – FierceBiotech

As Scholar Rock shakes up its C-suite, their loss has been startup Triplet Therapeutics gain as it nabs chief scientific officer Alan Buckler, Ph.D.

Scholar Rock, which is testing drugs in spinal muscular atrophy and its cancer immunotherapy, loses its CSO Buckler who had helped lead these programs, and comes after Scholar also saw its founding CEO Nagesh Mahanthappa, Ph.D., who becomes its scientific adviser.

Buckler now moves over to Triplet, picking up the CSO role there, and comes off a distinguished career that has seen him serve as vice president, cell and protein sciences at Biogen, and a nine-year stint as leading drug discovery programs at Novartis Institutes of Biomedical Research.

His new home launched late last year with $59 million and a mission to develop drugs for repeat expansion disorders such as Huntington disease and fragile X syndrome.

The company is taking aim at disorders caused by too many repeats of certain DNA sequences. These include triplet repeat disorders, or repeat expansion disorders (RED), so called because they result from repeats of three nucleotide bases, or letters.

The biotech says its able to think more holistically about this group of diseases, which number about 40, because of a new understanding of how they develop.

Thanks to new genetic datamost of which werent even available when Triplet was founded in 2018the company identified one central pathway that drives repeat expansion disorders: the DNA damage response (DDR) pathway.

One kink can make the DNA more likely to develop more kinks, growing longer and longer, like a snowball rolling down a hill. Eventually, it reaches a point where it becomes toxic to the cell, he said.

Current treatments for Huntington disease (HD) and other repeat expansion disorders focus on improving symptoms and do not change the course of the disease. Development efforts by companies like Roche and Wave Life Sciences target RNA, which stops toxic proteins from being made, but they do not address the underlying mutation.

What the biotech aims to is to work on drugs against very specific members of the DDR pathway to stop that expansion from taking place.

The company is working on antisense oligonucleotides and small interfering RNA medicines, with its leading clinical candidate, TTX-3360, focused on HD, with the biotech also eyeing up the med for spinocerebellar ataxias, fragile X syndrome, and familial amyotrophic lateral sclerosis (ALS).

Targeting the DDR pathway means Triplet will develop drugs not on a disease-by-disease basis, but on a tissue-by-tissue basis. That means it could potentially target several diseases with one drug.

Meet Dr Sarah Gilbert, one of the scientists leading the race to find a coronavirus vaccine – The Indian Express

Earlier this week, there was positive news on the Oxford vaccine candidate, one of a clutch of frontrunner candidates to protect the world from the SARS-CoV-2 virus that had, until Friday (July 24) morning, infected nearly 15.5 million people and killed over 630,000 worldwide.

Leading its development is a British scientist who plays the oboe, cycles to work, and is the mother of triplets. Dr Sarah Gilbert was famous in the scientific community as a brilliant vaccinologist; with the success in early trials, she and ChAdOx1 nCoV-19, the vaccine candidate her team is working on has been spotlighted as never before.

Gilbert and co-authors published the results of the early trials in the medical journal, The Lancet, on July 20, titled Safety and immunogenicity of the ChAdOx1 nCoV-19 vaccine against SARS-CoV-2: a preliminary report of a phase 1/2, single-blind, randomised controlled trial. They wrote: ChAdOx1 nCoV-19 showed an acceptable safety profile, and homologous boosting increased antibody responses. Which means that the vaccine candidate had induced an immune response (which is what vaccines are supposed to do), and was safe for people.

What is Dr Gilberts work in the field of vaccine development?

Dr Gilbert is one of the leading vaccinologists in the world. She is professor of vaccinology at Oxford Universitys Jenner Institute, a prestigious vaccine research centre, and one of the two founders of its spin-out company, Vaccitech, which develops immunotherapy products to treat and prevent infectious disease and cancer.

For more than 15 years, Dr Gilbert has been making and testing vaccines that trigger T cells a type of white blood cells to respond to antigens from malaria, influenza, and tuberculosis, among others.

Her work also includes developing vaccines for influenza and emerging diseases such as Lassa, Nipah, CCHF, and Middle East Respiratory Syndrome (MERS). MERS, which appeared in 2014, too, is caused by a coronavirus. A vaccine against MERS has been tested in clinical trials in the UK, and is now in trials in Saudi Arabia, where the virus is endemic, says Gilberts page on the Jenner Institute website.

The vaccine for MERS involved using the adenovirus (which causes common colds) from a chimpanzee embedded with the genetic material of the MERS virus. For the Covid-19 vaccine, the Oxford scientists used the adenovirus of a chimpanzee embedded with the genetic material from the spike protein, which the coronavirus uses to pierce the cell. In tests till now, bodies of participants have responded as if they were infected with the coronavirus.

What is known about the acclaimed scientists life away from her work?

Dr Gilbert has not given many interviews since the publication of the results of the early phase trials. Earlier profiles of the scientist in the UK media have said that as a child in Northamptonshires Kettering High School for Girls, she was quiet, polite, and studious, getting a lot of As in studies.

Gilbert belongs to a family of musicians, and her mother, Hazel, was part of the towns operatic society. By the age of 17, however, Gilbert was sure that she wanted to be a medical researcher. After obtaining a degree in biology at the University of East Anglia and a PhD in biochemistry from the University of Hull, Dr Gilbert worked at a number of biotechnology companies, among them Delta, where she learnt about making medicines.

Profiles on Prof Gilbert have noted that she became a vaccine specialist rather by accident. In 1994, when she entered Oxford University to join Professor Adrian Hills lab in a senior postdoc position, it was to work on human genetics. That highlighted the role of a particular type of immune response in protection against malaria, and so the next thing to move on to was to make a vaccine that would work through that type of immune response and thats how I got into vaccines, she told UKs The Telegraph newspaper in an interview.

By 2007, Dr Gilbert, who had become a reader at Oxford University three years previously, had won a project grant from the Wellcome Trust, and had begun work on an influenza vaccine. She has developed two vaccines for the disease so far, and has said that her ultimate aim is to be able to develop her team of scientists to be the leaders in vaccine research in the world.

Many people are fascinated with Dr Gilberts success in balancing the extra demands that women with a career in science face.

According to UNESCO, women make up less than 30 per cent of the worlds researchers. In science, technology, engineering and mathematics, women also publish less, are paid less for their research and do not progress as far as men in their careers.

On the website of the Nuffeld Department of Medicine (of which Jenner Institute, where she works, is a part), Dr Gilbert has been quoted as saying: Work life balance is very difficult, and impossible to manage unless you have good support. Because I had triplets in 1998, nursery fees would have cost more than my entire income as a post-doctoral scientist, so my partner has had to sacrifice his own career in order to look after our children.

She described how 18 weeks of paid maternity leave with three premature babies to care for and work to be completed, was tough: If there is a three-year grant and a woman wants a year-long maternity leave, it can disrupt the progress of the project. The situation becomes worse if more than one person is away simultaneously.

Dr Gilberts advice to women: One of the good things about being a scientist is that the hours are not fixed, so there is a fair amount of flexibility for working mothers. Having said that, there are also times when things (such as overseas conferences and important meetings) are fixed and you have to make sacrifices. It is exceptionally hard work. Its important to plan ahead, and make sure you have people who are willing to cover for you at home while you work. That might be your partner or relatives, or you may be able to buy in help.

Her own children seem to have survived unscathed, but none of them wants to be scientists, she said.

Dr Gilbert is cautiously hopeful. In an interview to the BBC, she said, Nobody can be absolutely sure that it is possible. Thats why we have to do trials. I think the prospects are very good but it is not completely certain.

47 years ago, a Dallas police officer played a game of Russian roulette with an innocent child and murdered him.

Santos Rodriguez. It's a name the City of Dallas should never forget.

Forty-seven years ago, a Dallas police officer played a game of Russian roulette with an innocent child and murdered him.

Nearly 50 years later, neither Santos' mother nor brother has recovered.

"He meant everything to me. He was my son. What they did to him was an injustice," Bessie Rodriguez said.

Bessie hates having to relive what happened to her son. The brutal murder of 12-year-old Santos Rodriguez in 1973, at the hands of a Dallas police Officer Darrell Cain sparked outrage and protests in the streets of downtown Dallas.

Similar cries for justice we are hearing on the same streets nearly 50 years later.

"My boys didn't have a chance," said Bessie.

Not a day goes by that she doesn't think about her son and the way he died.

"He had that gun right there near his temple so he kept telling him if he didn't tell the truth he was gonna shoot him," she said.

In the middle of the night on July 24, 1973, Santos and his brother were taken from their home, handcuffed, and put inside a patrol car.

Two Dallas police officers accused them of stealing $8 from a cigarette machine at a nearby gas station.

While trying to force a confession, Cain pointed his police pistol at Santos' head and pulled the trigger, murdering him.

Cain was found guilty of murder and served only 2 and a half years of his 5-year sentence.

"He just got away with it," said Bessie.

Bessie and her son David, who was 13 years old when he witnessed the brutal murder of his brother, feel like they have been treated more as a nuisance than as loved ones who are still suffering.

Hadi Jawad, Executive Director of the Dallas Peace and Justice Center says both mother and son need help.

"His brother David who was in the police car when Santos was killed is psychologically damaged. He has had no help over these years," said Jawad.

In fact, for decades Dallas didn't even address the family's pain until the mayor publically apologized in 2013.

"It took us 40 years to get an apology from the mayor of Dallas to his mother," said Jawad, who has worked with local leaders for years to try to help Santos' now 75-year-old mother.

Bessie says she and David feel forgotten.

"We just have struggled. He (David) needs help. He needs to see a doctor," she said.

Jawad says the City can only change its future by remembering the sins of its past and present.

"Dallas has got a very dark history and for many years it has been trying to run away from this dark history."

"It's a significant story and Dallas must learn to embrace the story of Santos Rodriguez," he said.

Last year, the community center at Pike Park in Uptown Dallas where Santos used to play was renamed after him.

The City of Dallas plans on unveiling this memorial statue in Pike Park next year along with renovating the building.

Current city leaders are trying to do what's right because it's never too late to right a wrong.

"He was special. While I can, I will still go to the cemetery take him flowers and pray for him. I have gone all these years," Bessie says.

She still visits her son's grave every month. She wants to believe what happened to Santos won't happen again, but she has lived long enough to know otherwise.

"Santos ain't the first and I don't think he'll be the last," she said.

Bessie says before long she'll be gone and doesn't want what happened to Santos to be forgotten.

"I don't want them to forget the injustice."

Cain died last year in Lubbock. Missing in his obituary, was any mention that he was ever a Dallas police officer.

WFAA reached out to his family and spoke with them about this story. They declined to do an on-camera interview.

What is hydrotherapy and how does it work? – The New Times

Although hydrotherapy is very effective in physiotherapy rehabilitation, Charles Munyambonera, a physical therapist in Kigali, says it is not used as much within.

He says that this form of treatment is very effective, especially for people with rheumatic ailments.

This is because when this form of treatment is used, such patients have chances to do their movement with less pain, and as they do so, their joints get the movements they might have lost before, he says.

The medic goes on to warn that this form of treatment should only be carried out by skilled personnel to avoid risks and repercussions.

He, however, encourages hospitals to include this form of treatment in the physiotherapy modalities.

At the moment, the only known health facility with skilled personnel that offers hydrotherapy treatment in the country is King Faisal Hospital.

Understanding hydrotherapy

Francis Habumugisha Goodrich, PhD Public health practitioner in Kigali, says hydrotherapy or water cure is a part of alternative medicine, occupational therapy, and physiotherapy that involves the use of water for pain relief and treatment.

He says that it involves the use of water to treat conditions, such as arthritis, rheumatic complaints and other ailments.

Although it bears a resemblance to swimming, he says it is different because it requires the individual to undertake various exercises in a warm pool of water.

The water temperature in the pool, he says, should approximately be 33 to 36C.

This water is much warmer than what you will encounter in a normal swimming pool. Hydrotherapy treatment is usually conducted in a hospitals physiotherapy department, he says.

Why its important

Munyambonera says hydrotherapy uses the principle known as Archimedes.

He explains that this is when you put something in the water; it tends to go vertically with animation of gravity.

The power to come up is equal to the water that is moving it. This scientific principle is the one helping in treating people, he says.

Munyambonera says when treating patients with limited range of motion, limited movements in the joints, they can easily do movements underwater than when they are not.

But again, he notes that when one wants to apply resistance, like going downwards from the upper part, it becomes hard.

Conditions where hydrotherapy is applicable

According to Munyambonera, hydrotherapy is good for people with pain, or those who feel pain when they are doing movements, and if they use water, the pain lessens.

It is also used with people who have rheumatic conditions like arthritis, polyarthritis, as this helps them do their movements with less pain.

This, he says, doesnt mean that the pain will go down to zero, but at least it wont be as intense.

Hydrotherapy is also used for people who had surgeries, such as post joint surgeries, as they tend to have pain after surgery.

For them to go back to the normal movements of their body and restore them, they start with less painful procedures that can be done underwater, he says.

Its also applicable in post hip surgeries this is where the joint of the hip has been replaced by a metal.

In such cases, he says they expect the patient to have limited movements in the hip.

Back surgeries and gout are also some of the conditions where hydrotherapy can be applicable.

Habumugisha says gouts complications can sometimes spread to other places, like the knees.

For such people, they hardly do their movements, have bent knees and because this condition is so painful, its not possible to bring back their range in the normal setting, therefore hydrotherapy becomes the only option, he says.

He adds that any movement they do actively or perceivably may lead to a lot of pain.

In such cases, he says what is normally done is to put them under hot water as the hot water tends to relax the muscles.

What you should know

Munyambonera says after the patients do active exercises themselves, the muscles and joints become relaxed.

Another important aspect, he says, is that patients, when under water, need to be helped to stretch and mobilise stiff joints.

He goes on to add that an expert can include resistance, like using heavy balls underwater.

The balls tend to help bring them back, push them into the water, and this becomes resistance, he says.

For instance, he mentions that in the case where a patient has a weak leg, they push them back downwards, where the water acts as resistance.

With pain, limited range of motion can be used in muscle weakness, and arthritis conditions.

Celestine Karangwa, a physiotherapist at TCM Technology Clinic in Remera, Kigali, says in medicine, a contraindication is a condition or factor that serves as a reason to withhold a certain medical treatment due to the harm that it would cause the patient.

Contraindication is the opposite of indication, which is a reason to use a certain treatment, he says.

Munyambonera says there are contradictions where hydrotherapy isnt inducted.

For instance, he says, if a person has a fresh and open wound, hydrotherapy is contraindicated because it may lead to infection.

Even though it has to be cleaned with disinfectants before and after use, even the patient might re-infect themselves from the skin to the wound, he cautions.

Also, when treating people with urinary incontinence, hydrotherapy is contraindicated as well.

He says that when they go in hydrotherapy, controlling the urine could be hard for them.

How long does treatment take?

Depending on the amount of pain, Habumugisha says with hydrotherapy, the time it takes for one to be completely healed varies.

For example, he points out that people with rheumatic pains cant be relieved in a day.

Therefore, he says, as they have more sessions, they get to feel better with time. He says that normally, one session can go for 30 minutes to an hour.

Munyambonera explains that this is because when people are doing exercises in hot water, generally, they get tired within a short time.

Some can do an hour or 40 minutes per session, so they always consider the patients condition.

Generally, he says, they consider 15 sessions as a starting point for many conditions, but those with paralysis might need many more sessions because of their condition.

In a comment that surprised exactly no one, Justice Leaguedirector Zack Snyder has said his upcoming Snyder Cut of the troubled superhero film will not feature any footage that was shot by Joss Whedon for the theatrical release.

Snyder was forced to cede control of Justice League after a family tragedy, with Whedon stepping into the directors chair to complete the comic book team-up, featuring Batman, Wonder Woman, Superman, Aquaman and The Flash.

I would set the movie on fire. Id destroy it before I used a single frame that I did not photograph, Snyder said during the first inaugural Justice Con.

After approaching near-mythical status, with #ReleasetheSnyderCut a regular call out among fans on social media, Snyder announced in May he will release his fabled version of the 2017 filmnext year exclusively on WarnerMedias HBO Max.

With a reported $300 million budget and a worldwide gross of $658 million, Justice League was a commercial disappointment. Critics were equally unimpressed, and it sits with a 40% rating on Rotten Tomatoes. A planned sequel was shelved and the tumultuous shoot led Ben Affleck to exit a proposed Batman spinoff. Elsewhere, Ezra Millers solo Flash movie has suffered a number of setbacks (its now due out in 2022), while Cyborg has been postponed indefinitely.

While a full trailer wont be revealed until next months DC FanDome event on Aug. 22, Snyder has been teasing his take on social media with new images of the films stars and clips, including one of Henry Cavills Superman in a black suit and another showing Gal Gadots Wonder Woman uncovering an etching inside a cave that reveals the super villain Darkseid.

Snyder hasnt revealed whether his revampedJustice League will be available to stream as a film or if it will be split up into a miniseries, but he has said it will four hours or possibly longer. It will be completed with new effects and a production budget rumoured to be in the $20 million price range.

But in an interview with the Hollywood Reporter, Snyder promised his cut will be an entirely new thing.

#ReleasetheSnyderCut is the most-tweeted hashtag about a movie that WB has ever made, but its a movie theyve never released, says Snyder. Its a weird stat but its cool.

Fans may be curious to know: How tall is Batman? Is he the shortest member of the Justice League? Does Joker look down at him? Is he 7ft tall?

How tall is Batman actually supposed to be? Fans might be interested in knowing the exact height the Dark Knight comes to (we won't look into his weight, that's rude). Is he the tallest member of the Justice League? Perhaps he's the shortest but the rest of the League members are too afraid to make fun of him for it (he's still Batman, after all). Which villains look up at him and which look down to him? Read on to learn just how tall (or short) that Batman truly is compared to other characters in the DC Universe.

The official sizes and body metrics for a multitude of DC characters can be found in theofficialDC Comics Encyclopediafrom DC Comics in partnership with DK. It makes sense that Batman and other DC Comics characters would have official heights and other body type metrics. The publisher would want to maintain a base level of consistency between series and different comic book artists as time goes on as they're being depicted. They wouldn't want one artist to draw Batman a few feet taller than a Gotham rogue, only to have him be shorter than that same rogue a few issues or series later when drawn by another. In order to maintain a basic continuity of appearance, there needs to be an agreed upon set of character dimensions.

According to the official encyclopedia, Batman is 6'2". For reference, that puts him as the same height as Will Smith, Ryan Reynolds, Chris Pratt, and Arnold Schwarzenegger. He's also the same height as Batman actor Adam West, and Lego Batman voice actor Will Arnett. As another fun fact, Michael Caine and Jeremy Irons, who both played Batman's butler Alfred in their respective films, are 6'2" as well.

In the DC Universe, Superman comes in at just an inch taller than the Dark Knight, while Wonder Woman is two inches shorter, putting Batman right in the middle of the DC's Trinity of Heroes. Furthermore, he is neither the tallest nor shortest member of the Justice League: Cyborg is the tallest at 6'6 and Flash and Wonder Woman tie for shortest at 6ft, while Green Lantern Hal Jordan is the same height as Batman. On the villain side, Penguin is obviously shorter than Batman at 5'2". Surprisingly, the Joker is taller than Batman being 6'5", while Darkseid is taller than all listed previously at a whopping 8'9". One could imagine that being super tall is a prerequisite for being a cosmic dictator obsessed with ruling over all by means of a vicious Parademon army.

While the information of knowing Batman's specific height is not super important in the grand scheme of things even in the realm of comics knowledge, it's hopefully still fun, interesting, and entertaining for Batman and DC fans to learn all the same. Fans of the Dark Knight can rest easy knowing that Batman can ride all of the rides at Disneyland, if he were so inclined.

We're back with the Overwatch League coaches' poll, and this week, Genji continued his rise in the meta.

Nowhere was that more apparent than with Matthew "Super" DeLisi taking the cyborg ninja on a stroll against the hapless Boston Uprising. Normally a main tank player, Super styled on the Uprising and demonstrated two things: the Shock still rule the roost, and we might never hear the end of Super recounting his Genji play. There was still room for at least one upset, of course, as the Chengdu Hunters used yet another unconventional composition to defeat NYXL. How do the teams all stack up in their first week back after the Summer Showdown?

The Overwatch League coaches' poll ranks the teams from best to worst, with votes compiled into aggregate rankings. The coaches' poll will come out weekly during the OWL season.

Comets play the Feud – THE MERCURY – The UTD Mercury

The UTD Athletics department introduced a virtual tournament for student athletes called Comets Feud over the summer break, inspired by the Family Feud-style game show format.

To play, each UTD sport contributes a team of five players who compete against other teams in brackets, progressing from quarterfinals to semifinals to championships. Assistant Athletics Director Dave Wester said the tournament is a way to give student athletes a reason to be competitive with each other in a friendly way, even from home.

We started it around a month ago, Wester said. We didnt know where it would go. It was supposed to be just a fun game but student athletes loved it so much that we decided to turn it into a tournament. Our coaches loved it and students seemed to enjoy the competitive side of it.

With the onslaught of the current pandemic in March, athletic seasons were cut short and student athletes were sent home on extended leave from their respective sports. Comets Feud was the brainchild of Associate Athletics Director Bruce Unrue, who created it with the intention of keeping the athletics community together in the midst of uncertain times, inspired by seeing a Facebook livestream of a church holding a similar contest.

They werent seeing their teammates and we werent seeing them so there was a big disconnect going on, Unrue said. I was trying to come up with something fun to do that could get the student athletes in front of each other again. Just to have them at least see each others faces and interact a little bit, because thats a big part of being part of a team: being around your teammates and not only competing on the field but having time to spend together.

Each episode of the tournament is recorded through Microsoft Teams and then uploaded to YouTube for the general public to view. Wester said it is a way to give the fans further insight and taste of what student athletes are up to. Initially, the game show was uploaded to YouTube and advertised through the Comet Sports Twitter account, but since posting on Instagram TV, viewership has greatly increased, with some episodes on YouTube reaching over 150 views, Unrue said.

The idea was first introduced to the Student Athletics Advisory Committee (SAAC) as a way to test the waters to see if there would be interest within the student body. SAAC President Christian Kondor said that the tournament was definitely something the committee wanted to do, fitting with their goal of fostering a sense of community within the athletics department and building camaraderie between the teams.

Kondor was also involved as a participant and said that the game show had an overwhelmingly positive response. Athletes were eager to play and it gained traction quickly.

The prompts and responses were absolutely hilarious, Kondor said. We were super competitive, because thats one of the things you can really count on with an athletics department event. If you make it competitive, were going to be invested and its all in good fun but people get really into it and this was no exception.

Much of the logistical preparation for the tournament was handled by the athletics directors, with Unrue preparing graphics and sound effects as well as editing for each episode, while Wester prepared prompts and a script for each taping.

We had a group meeting and I was asked if I wanted to host, Wester said. I said Sure, it sounds like a blast! I enjoy interacting with our student athletes and I feel like I have a good relationship with many of them. I also saw this as an opportunity to get to know more of the student athletes better, so I jumped at the opportunity.

Wester said they are still going through the learning phases of remote access to everything, but the sports teams have gotten stronger as a group just by seeing and talking with each other. He said its great to be able to touch base with students weekly and see how theyre doing given the circumstances.

Its like a wellness check, Wester said. We care about their wellbeing and during the summertime we dont always get to see them anyway, but this pandemic has actually prompted us to reach out to them more often to make sure everything is okay. Theres a flux right now about whats going to happen. Nobody knows and the more familiarity you have in your life, the easier it is to deal with it. If they are going through a tough time, they have something to rely on that will be there for them, and thats us.

Backyard Universe: Comet NEOWISE thrills some, a yawn to others – The Fayetteville Observer

Although pulling away and slowly fading, Comet NEOWISE is still in our night sky.

Comet NEOWISE is now fading as it moves away from both the sun and Earth.

The feedback Im getting from friends and online comments tells me that while being the brightest and easiest to see comet for northern hemisphere observers in 23 years, the comet eluded some folks entirely. Some who did spot it were clearly disappointed.

In 1997, Comet Hale-Bopp remained at naked eye brightness for more than a year, allowing most anyone anywhere to glance upward and see this large bright comet.

But Comet NEOWISE was more challenging to see by comparison.

I understand fully the range of impressions Comet NEOWISE generated. Over the past month, Ive viewed and photographed the comet from suburban locations, local darker sky locations and in beautifully clear skies over a mountain in western North Carolina. Ive also checked it out when it was barely above the horizon and again when it rode higher in the evening sky.

NEOWISE looked different from each location and each changing position in the sky.

How much difference did my observing location play in how impressive the comet appeared?

It made ALL the difference.

From brightly lit suburban locations I never saw Comet NEOWISE with my eyes alone, but using a pair of 7X50 binoculars it was obvious. I couldnt imagine anyone in the center of Fayetteville seeing the comet naked eye, and it would have been tough with binoculars.

The view became more rewarding from a darker site I often use northeast of Fayetteville. There, I could see the comet naked eye although it wasnt an obvious evening sky object. Binoculars, however, revealed a very "cometlike" fuzzy object with a tail.

My best view came under the best weather conditions from the darkest sky location.

On July 18, I observed and photographed NEOWISE from Grandfather Mountain in western North Carolina. After obtaining special permission to be on the mountain after dark, I wasnt able to work from the top of Grandfather in the visitors center parking lot. The large rock face to the north obscured the view of NEOWISE. I had to set up around the 4,433 foot elevation of the near 6,000 foot mountain where no obstructions blocked the part of the sky where the comet was.

From that location, under clear, haze-free skies, NEOWISE was at its best. A very comet like object was very evident to the naked eye, and a large twin-tailed comet appeared in binoculars. Photographs I made that night were far superior to any I made locally.

This greatly improved view wasnt surprising to me, but may not have been evident to the more casual observer. The clearest skies over the darkest of location are everything when looking for and seeing a comet well, even the brightest ones.

Folks who went to the trouble to scope out and travel to those locations had a memorable view, while folks who simply walked outside to an urban backyard probably wonder what all the fuss was about.

Our local weather and the time of year is also a problem when viewing NEOWISE. We need a night thats clear, following an afternoon when we have the deepest blue sky thats free of haze and clouds, like we have in October. Thats a tall order in summer in our area when late day thunderstorms are the rule.

NEOWISE still around

Although pulling away from us and slowly fading, Comet NEOWISE is still in our conveniently timed evening sky. To make things better, its getting higher in the western sky and is observable for a longer period of time each evening.

But the bright moon, which becomes full in early August, is also in our evening skies now, the light from which can tend to wash out the fainter comet. Starting around Aug. 5, we start to have some moon-free time during our evenings once again.

Finder charts to help you locate Comet NEOWISE in the evening sky and additional information about the comet can be found on Sky and Telescope magazines website at https://skyandtelescope.org/ and the site http://www.space.com.

Although the comet is fading, the unpredictable nature of comets could mean a sudden increase in brightness or a dramatic and rapid fading.

Its all the more reason to get ourselves into the best position possible to see this visitor from the outer solar system, one that wont return again for 6,800 years.
