ESSEX COUNTY, NJ John Mirrione knew it was a longshot that he'd win a seat in Congress.

After all, the political neophyte was running against incumbent U.S. Rep. Donald Payne Jr., a Democratic Party stalwart in the 10th District who ended up with 241,522 votes almost six times that of the Republican nominee, Jennifer Zinone, who had 40,298.

Payne also soundly defeated independent candidates Akil Khalfani and Khaliah Fitchette, who garnered 3,537 and 3,480 votes, respectively.

When the certified results were finally in, Mirrione, the Libertarian Party nominee, stood at 1,172 votes. And at first glance, it might seem like a discouraging result for the karate expert, small business owner and U.S. Air Force veteran. But each of those ballots represents something bigger than a single campaign, he says: empowerment.

"I feel that now more than ever people were more open to a third-party viewpoint," Mirrione recently told Patch after being asked to reflect on the 2020 election.

"With the growing disparity of the democratic and republication parties, people were asking way more questions with deep overall concern of our government and its policies," Mirrione said. "This all mixed in with a media war just added to the ongoing confusion."

There's an overwhelming temptation to give in to conformity when you turn in your ballot, he said. But having the guts to vote for the candidate you think is the best choice regardless of their odds can change the status quo.

"People feel that they are forced to vote for a party line and have no control of the candidate selection," Mirrione said. "By going the third-party route, people felt more empowered to have an opportunity having more of a voice that counts in government leadership and policy."

"As a first-time candidate for Congress as a Libertarian, I felt proud that the majority of those who voted for me were a vote I earned, instead of it being a vote for a party line," Mirrione said.

When he launched his campaign, Mirrione said that no matter how the grand finale turned out, simply making the ballot was a decisive victory because it gave him a platform to spread the word about his anti-bullying work, which will continue after the election.

Drawing on his own past of being picked on while growing up in Brooklyn and Long Island, Mirrione embarked on a 17-city crusade in 2010. Digging into his own pocket for food and gas, he visited schools and YMCAs across the nation, speaking to kids about "being self-empowered, believing in themselves and knowing that anything is possible."

Soon, Mirrione found himself chatting with the likes of Deepak Chopra about his anti-bullying efforts and regimen of positive thinking.

In 2015, Mirrione formed the Harmony Power Foundation, an organization that is dedicated to "standing up to bullying and standing for human equality." Mirrione eventually scored a huge victory when the Elizabeth Public School District decided to incorporate his Harmony Power Awards program into its anti-bullying curriculum at no additional cost to the district.

Currently, Mirrione is calling for the creation of a "national anti-bullying liaison" in Washington D.C. in order to help people of all ages overcome emotional, mental and physical abuse from bullying.

"In the midst of this current political transition, we have a unique opportunity in front of us as a country," Mirrione said. "We can reaffirm our commitment to our children to raise them in a world where they don't need to be afraid where they can go about their life and become the best person they can be."

"By doing so, we might actually see there is more that unites us rather than divides us," he added.

The 10th District includes the following municipalities: Bloomfield, East Orange, Glen Ridge, Irvington, Maplewood, Montclair, Newark, Orange, South Orange, West Orange, Bayonne, Jersey City, Hillside, Linden, Rahway, Roselle, Roselle Park and Union Township.

Malachi O’Doherty: Sammy Wilson is a libertarian… but only when it suits him – Belfast Telegraph

It's tempting to wonder if history is made by stupid people as much as by clever people. The reputation of former adviser to Boris Johnson, Dominic Cummings, was enhanced by a television drama which presented him as a deep thinker and a deft communicator, two gifts that don't always go together.

ot that there weren't other voices ready to explain how bumping yourself out of the single market was effectively imposing sanctions on yourself, the sort of treatment we usually reserve for rogue states.

One of the qualities of a truly great leader must surely be the ability to hold fast against all derision when you are sure you are right. It is also the mark of a fanatic.

We only get to find out which term applies when the story is over and history has passed its judgment.

Fortunately, there are tests we can apply to political figures while they are still alive. Arlene Foster agitated for Brexit without foreseeing the danger of a weakening of the Union. It's hard to see how posterity will vindicate that.

Maybe foreign capital will now pour into Northern Ireland and we'll all be so rich in 10 years' time that we'll be called the Orange Tiger.

We got a nice insight into the workings of the mind of one of our conviction politicians on Any Questions on Radio 4 last week. Sammy Wilson came out as seriously sceptical of the efforts to curtail the Covid-19 virus.

He said the Government and advisers had succumbed to "Project Fear". This is a phrase that was coined to dismiss the warnings about how bad Brexit could be. Sammy applied it to the reaction to the virus. Now it's the handy phrase for mocking any doubts about any policy.

Sammy scoffed at the "deplorable way" in which old people have been left "cowering in their homes", because of measures to control a pandemic which has affected very few of us.

He didn't exactly say, "Give me liberty or give me death", but he did say that we are being kept in a state of perpetual fear to prepare us to accept curtailments on our liberty, as if he thinks the curtailment of liberty is the core objective.

His solution would have been to "protect the vulnerable and let others get on with their lives". He didn't say how that could be done without curtailing the liberties of older people. (It can't.)

Sammy is 67 years old. He is one of the vulnerable himself. I'm a bit worried about how red his cheeks are. He is a portly man.

The implications of what he says are that he himself should be removed from society, out of reach of a virus that could kill him, and that people who are less likely to die should be free to blithely infect themselves and each other.

So, on the one hand, he is saying that old people are cowering in their homes and, on the other, that that's where they would be anyway if he was in charge.

He is demanding freedom from curtailments and then endorsing curtailments.

Sammy rants a lot and yet one of his repeat themes is that we are all getting over-exercised about something or other. Like the chances of a united Ireland.

That question was a prompt for further self-contradiction. He said that the Government handling of the virus has demonstrated the merits of being part of the United Kingdom, a bigger and richer country.

I should have been on that programme. Somebody should have been there to point out that the Irish Republic's infection levels are proportionately about a quarter of those in Northern Ireland.

And how come these measures, which he dismissed minutes earlier as "deplorable", are now evidence of the merits of the Union?

Sammy builds up arguments on different issues and doesn't check whether they contradict each other. Then he did it again.

There was a question about whether electric cars will ever be affordable. Sammy said the Government's Green plan was "Stalinist".

This from the man who wanted the vulnerable to be sectioned off from the rest of society. He said he drives a diesel van and that people should have the option of driving whatever car they think they need.

So, one minute he is the social engineer who will lock up the vulnerable and the next he is a free market libertarian who would let anyone drive whatever they liked, regardless of the impact on the environment. He's a libertarian when it suits him.

If the threat is a virus, then the response should be targeted and thorough.

And if it is climate change, then everyone should do as they please.

We should have more of our local politicians on Any Questions in the hope that they will unpack their thinking, or lack of it, as candidly as Sammy did.

In the style of the programme, there is often a light question at the end. This was the week in which Barbara Windsor died. One of the clips played over and over again in the news reports showed her as the landlady in EastEnders, ordering someone out of her pub.

So, who would Sammy order out of his pub? The Chief Medical Officer. Sammy didn't remember his name. That's how much attention he has been paying to him.

The case against Professor Chris Whitty (write it on your cuff, Sammy) is that he ordered the pubs closed without having gathered sufficient evidence of the extent to which Covid-19 might be spread in them.

Some things have to be taken on faith and it seems to me that one of the easier ones to accept is that drunk people mingling in a bar and bumping against each other and shouting and blathering and squaring up to each other are more likely to spread infection than people sitting down to a meal, well spaced from each other.

You get the feeling that Sammy, when we were hit with a pandemic, would have spent a year gathering data on how it spread before taking measures against it.

I hope Sammy has a happy and restful retirement and that it starts soon. In fact, I hope the same for a lot of our politicians.

But one thing we have learnt in this strange year is that daft as some of our politicians are, they are not exceptional. There are others as daft everywhere.

EASTON Lori Roman does not hold back in her assessments of governments ramping up their enforcements of COVID-19 orders on small businesses and churches.

That includes tip lines set up by Gov. Larry Hogan as well as some local health departments and police departments encouraging residents to report neighbors and businesses who are not complying with coronavirus orders related to masks, social distancing or rules on capacities and gatherings.

We see a lot of state and county governments actively encouraging residents to snitch and each other, said Roman, who is president of the conservative American Constitutional Rights Union.

That is Orwellian, said Roman.

The conservative group started helping a Maryland church this month after it was issued an Order of Immediate Compliance by a local health department after receiving a complaint about whether social distancing and COVID mask rules.

The Community Methodist Church in Pasadena was issued the warning by the Anne Arundel County Health Department after receiving a complaint about whether face coverings were being worn during services and there was handshaking, high-fives and fist bumps by the congregation.

The Anne Arundel Health Department told The Star Democrat earlier this month that the church was not fined or sanctioned and that the complaint was closed after a follow-up visit by a health inspector.

Still, Roman does not like the idea of county health inspectors or police coming to churches to enforce COVID-19 orders.

Churches have very special protections in the Constitution and worship should not be infringed upon, she said in an interview with The Star Democrat.

The Florida-based conservative libertarian group is ramping up its advocacy for churches and small businesses in Maryland, including on the Eastern Shore, Roman said.

She also worries about local health departments in Maryland and other states presuming churches, restaurants and small businesses are guilty of violating COVID orders simply because they received a complaint.

Thats un-American, she said.

In response to rising reported COVID-19 cases, deaths and hospitalizations after the November election, Hogan pressed for more enforcement of the states pandemic orders requiring masks be worn indoors in public places as well as outdoors.

The governor and public health officials say the COVID orders and enforcement will help curb the spread of the virus.

Maryland has reported 5,152 deaths attributed to the coronavirus this year. There are currently 1,702 patients hospitalized for COVID statewide, according to Maryland Department of Health figures on Thursday, Dec. 17.

Roman said the COVID tip lines create a dangerous precedent for civil and religious liberties. What Orwellian world have we come to where a government official shows up at a church because a snitch said two church members shook hands, she said.

The governor also dispatched the Maryland State Police and pressed local police to do compliance checks to see if restaurants, bars and other businesses were abiding by coronavirus orders.

A COVID tip line was also established by the state police.

Top state health officials including Acting Deputy Secretary for Public Health Services Dr. Jinlene Chan and senior state medical advisor Dr. David Marcozzi have said masks and social distancing will still be required into next year even as they urge residents to get new COVID vaccines.

That is because there will be residents who will not want to get vaccinated.

There has been increasing resistance to state and local orders across the country restricting church capacities and shutting down restaurant dining while other types of businesses remain open and other types of gatherings such as progressive political protests have not been restricted or chastised.

The U.S. Supreme Court ruled 5-4 against an order from New York Gov. Andrew Cuomo that restricted religious services but not some other businesses.

Roman also points to Maryland and other states releasing some prison inmates early to protect them from COVID while local health departments are warning churches about social distancing.

Its an upside down world, she said.

Depression is the leading cause of disability in the United States among people ages 15 to 44. While there are many effective treatments for depression, first-line approaches such as antidepressants and psychotherapy do not work for everyone. In fact, approximately two-thirds of people with depression dont get adequate relief from the first antidepressant they try. After two months of treatment, at least some symptoms will remain for these individuals, and each subsequent medication tried is actually less likely to help than the one prior.

What can people with depression do when they do not respond to first-line treatments? For several decades, electroconvulsive therapy (ECT or shock therapy) was the gold standard for treatment-resistant depression. In fact, ECT is still considered to be the most potent and effective treatment for this condition, and it continues to be used regularly across the country. For many people with depression, however, ECT can be too difficult to tolerate due to side effects on memory and cognition. For those individuals and the many others who have had an inadequate response to medications and therapy alone, there is a newer treatment option called transcranial magnetic stimulation (TMS).

Transcranial magnetic stimulation, or TMS, is a noninvasive form of brain stimulation. TMS devices operate completely outside of the body and affect central nervous system activity by applying powerful magnetic fields to specific areas of the brain that we know are involved in depression. TMS doesnt require anesthesia and it is generally exceptionally well tolerated as compared to the side effects often seen with medications and ECT. The most common side effect is headache during or after treatment. A rare but serious side effect is seizures, and TMS may not be appropriate for people at high risk such as those with epilepsy, a history of head injury, or other serious neurologic issues.

Approximately 50% to 60% of people with depression who have tried and failed to receive benefit from medications experience a clinically meaningful response with TMS. About one-third of these individuals experience a full remission, meaning that their symptoms go away completely. It is important to acknowledge that these results, while encouraging, are not permanent. Like most other treatments for mood disorders, there is a high recurrence rate. However, most TMS patients feel better for many months after treatment stops, with the average length of response being a little more than a year. Some will opt to come back for subsequent rounds of treatment. For individuals who do not respond to TMS, ECT may still be effective and is often worth considering.

TMS therapy is an intensive treatment option requiring sessions that occur five days a week for several weeks. Each session may last anywhere from 20 to 50 minutes, depending on the device and clinical protocol being used. When patients arrive, they may briefly check in with a technician or doctor and then begin the stimulation process. The technician will determine the ideal stimulation intensity and anatomical target by taking advantage of a landmark in the brain called the motor cortex. By first targeting this part of the brain, the team can determine where best to locate the stimulation coil as it relates to that individuals brain and how intensely it must fire in order to achieve adequate stimulation. Calculations are then applied to translate this data toward finding the dorsolateral prefrontal cortex, the brain target with the greatest evidence of clinical effectiveness and an area known to be involved in depression. Though one session may be enough to change the brains level of excitability, relief isnt usually noticeable until the third, fourth, fifth, or even sixth week of treatment.

TMS is being studied extensively across disorders and even disciplines with the hope that it will evolve into new treatments for neurological disorders, pain management, and physical rehabilitation in addition to psychiatry. There are currently large clinical trials looking at the effectiveness of TMS in conditions such as pediatric depression, bipolar disorder, obsessive-compulsive disorder, smoking cessation, and post-traumatic stress disorder. While promising avenues for research, TMS for these conditions is not yet approved and would be considered off-label.

Transcranial magnetic stimulation (TMS) is a noninvasive procedure that uses magnetic fields to stimulate nerve cells in the brain to improve symptoms of depression. TMS is typically used when other depression treatments haven't been effective.

This treatment for depression involves delivering repetitive magnetic pulses, so it's called repetitive TMS or rTMS.

In transcranial magnetic stimulation (TMS), an electromagnetic coil placed against the scalp creates a magnetic field that stimulates certain areas of the brain.

During an rTMS session, an electromagnetic coil is placed against your scalp near your forehead. The electromagnet painlessly delivers a magnetic pulse that stimulates nerve cells in the region of your brain involved in mood control and depression. It's thought to activate regions of the brain that have decreased activity in depression.

Though the biology of why rTMS works isn't completely understood, the stimulation appears to impact how the brain is working, which in turn seems to ease depression symptoms and improve mood.

There are different ways to perform the procedure, and techniques may change as experts learn more about the most effective ways to perform treatments.

Depression is a treatable condition, but for some people, standard treatments aren't effective. Repetitive TMS is typically used when standard treatments such as medications and talk therapy (psychotherapy) don't work.

Repetitive TMS is a noninvasive form of brain stimulation used for depression. Unlike vagus nerve stimulation or deep brain stimulation, rTMS does not require surgery or implantation of electrodes. And, unlike electroconvulsive therapy (ECT), rTMS doesn't cause seizures or require sedation with anesthesia.

Generally, rTMS is considered safe and well-tolerated. However, it can cause some side effects.

Side effects are generally mild to moderate and improve shortly after an individual session and decrease over time with additional sessions. They may include:

Before considering treatment, check with your health insurance company to see whether rTMS is covered. Your policy may not cover it.

Repetitive TMS is usually done in a doctor's office or clinic. It requires a series of treatment sessions to be effective. Generally, sessions are carried out daily, five times a week for four to six weeks.

Before treatment begins, your doctor will need to identify the best place to put the magnets on your head and the best dose of magnetic energy for you. Your first appointment typically lasts about 60 minutes.

You can return to your normal daily activities after your treatment. Typically, between treatments, you can expect to work and drive.

If rTMS works for you, your depression symptoms may improve or go away completely. Symptom relief may take a few weeks of treatment.

The effectiveness of rTMS may improve as researchers learn more about techniques, the number of stimulations required and the best sites on the brain to stimulate.

After completion of an rTMS treatment series, standard care for depression such as medication and psychotherapy may be recommended as ongoing treatment.

It's not yet known if maintenance rTMS sessions will benefit your depression. This involves continuing treatment when you are symptom-free with the hope that it will prevent the return of symptoms.

However, if your depression improves with rTMS, and then later you have another episode of symptoms, your rTMS treatment can be repeated. This is called re-induction. Some insurance companies will cover re-induction.

Transcranial magnetic stimulation (TMS) is a noninvasive form of brain stimulation in which a changing magnetic field is used to cause electric current at a specific area of the brain through electromagnetic induction. An electric pulse generator, or stimulator, is connected to a magnetic coil, which in turn is connected to the scalp. The stimulator generates a changing electric current within the coil which induces a magnetic field; this field then causes a second inductance of inverted electric charge within the brain itself.[1]:3[2]

TMS has shown diagnostic and therapeutic potential in the central nervous system with a wide variety of disease states in neurology and mental health, with research still evolving.[3][4][5][6][7][8][9][10]

Adverse effects of TMS are rare, and include fainting and seizure.[11] Other potential issues include discomfort, pain, hypomania, cognitive change, hearing loss, and inadvertent current induction in implanted devices such as pacemakers or defibrillators.[11]

TMS is non-invasive, and does not require surgery or electrode implantation. Its use can be divided into diagnostic and therapeutic applications. Effects vary based on frequency and intensity of the magnetic pulses as well as the length of the train, which affects the total number of pulses given.[citation needed] TMS treatments are now approved by the FDA in the USA and by NICE in the UK for the treatment of depression and are predominantly provided by private clinics.

TMS can be used clinically to measure activity and function of specific brain circuits in humans, most commonly with single or paired magnetic pulses.[3] The most widely accepted use is in measuring the connection between the primary motor cortex of the central nervous system and the peripheral nervous system to evaluate damage related to past or progressive neurologic insult.[3][13][14][15]

Repetitive high frequency TMS (rTMS) has shown diagnostic and therapeutic potential with the central nervous system in a variety of disease states, particularly in the fields of neurology and mental health.[3][4][5][7][8][9][10]

Although TMS is generally regarded as safe, risks are increased for therapeutic rTMS compared to single or paired diagnostic TMS.[16] Adverse effects generally increase with higher frequency stimulation.[11]

The greatest immediate risk from TMS is fainting, though this is uncommon. Seizures have been reported, but are rare.[11][17][18] Other adverse effects include short term discomfort, pain, brief episodes of hypomania, cognitive change, hearing loss, impaired working memory, and the induction of electrical currents in implanted devices such as cardiac pacemakers.[11]

During the procedure, a magnetic coil is positioned at the head of the person receiving the treatment using anatomical landmarks on the skull, in particular the inion and nasion.[12] The coil is then connected to a pulse generator, or stimulator, that delivers electric current to the coil.[2]

TMS uses electromagnetic induction to generate an electric current across the scalp and skull.[19][20] A plastic-enclosed coil of wire is held next to the skull and when activated, produces a magnetic field oriented orthogonal to the plane of the coil. The magnetic field can then be directed to induce an inverted electric current in the brain that activates nearby nerve cells in a manner similar to a current applied superficially at the cortical surface.[21]

The magnetic field is about the same strength as an MRI, and the pulse generally reaches no more than 5 centimeters into the brain, unless using a modified coil and technique for deeper stimulation.[20]

From the BiotSavart law,

it has been shown that a current through a wire generates a magnetic field around that wire. Transcranial magnetic stimulation is achieved by quickly discharging current from a large capacitor into a coil to produce pulsed magnetic fields between 2 and 3 Tesla in strength.[22] Directing the magnetic field pulse at a targeted area in the brain causes a localized electrical current which can then either depolarize or hyperpolarize neurons at that site. The magnetic flux generated by the current causes its own electric field, as explained by the Maxwell-Faraday equation,

This electric field causes a change in transmembrane currents resulting in depolarization or hyperpolarization of neurons, causing them to be more or less excitable, respectively.[22]

Deep TMS can reach up to 6cm into the brain to stimulate deeper layers of the motor cortex, such as that which controls leg motion. The path of this current can be difficult to model because the brain is irregularly shaped with variable internal density and water content, leading to a nonuniform magnetic field strength and conduction throughout its tissues.[23]

The effects of TMS can be divided based on frequency, duration and intensity (amplitude) of stimulation:[24]

Most devices use a coil shaped like a figure-eight to deliver a shallow magnetic field that affects more superficial neurons in the brain.[28] Differences in magnetic coil design should be considered when comparing results, with important elements including the type of material, geometry and specific characteristics of the associated magnetic pulse.

The core material may be either a magnetically inert substrate ('air core'), or a solid, ferromagnetically active material ('solid core'). Solid cores result in more efficient transfer of electrical energy to a magnetic field and reduce energy loss to heat, and so can be operated with the higher volume of therapy protocols without interruption due to overheating. Varying the geometric shape of the coil itself can cause variations in focality, shape, and depth of penetration. Differences in coil material and its power supply also affect magnetic pulse width and duration.[29]

A number of different types of coils exist, each of which produce different magnetic fields. The round coil is the original used in TMS. Later, the figure-eight (butterfly) coil was developed to provide a more focal pattern of activation in the brain, and the four-leaf coil for focal stimulation of peripheral nerves. The double-cone coil conforms more to the shape of the head.[30] The Hesed (H-core), circular crown and double cone coils allow more widespread activation and a deeper magnetic penetration. They are supposed to impact deeper areas in the motor cortex and cerebellum controlling the legs and pelvic floor, for example, though the increased depth comes at the cost of a less focused magnetic pulse.[11]

Luigi Galvani (1737-1798) undertook research on the effects of electricity on the body in the late-eighteenth century and laid the foundations for the field of electrophysiology.[31] In the 1830s Michael Faraday (1791-1867) discovered that an electrical current had a corresponding magnetic field, and that changing one could induce its counterpart.[32]

Work to directly stimulate the human brain with electricity started in the late 1800s, and by the 1930s the Italian physicians Cerletti and Bini had developed electroconvulsive therapy (ECT).[31] ECT became widely used to treat mental illness, and ultimately overused, as it began to be seen as a panacea. This led to a backlash in the 1970s.[31]

In 1980 Merton and Morton successfully used transcranial electrical stimulation (TES) to stimulate the motor cortex. However, this process was very uncomfortable, and subsequently Anthony T. Barker began to search for an alternative to TES.[33] He began exploring the use of magnetic fields to alter electrical signaling within the brain, and the first stable TMS devices were developed in 1985.[31][32] They were originally intended[by whom?] as diagnostic and research devices, with evaluation of their therapeutic potential being a later development.[31][32] The United States' FDA first approved TMS devices in October 2008.[31]

TMS has shown potential with neurologic conditions such as Alzheimer's disease,[4] amyotrophic lateral sclerosis,[4][34] persistent vegetative states,[4] epilepsy,[4][35] stroke related disability,[4][11][14][15][36][37] tinnitus,[4][38] multiple sclerosis,[4] schizophrenia,[4][10] and traumatic brain injury.[39]

With Parkinson's disease, early results suggest that low frequency stimulation may have an effect on medication associated dyskinesia, and that high frequency stimulation improves motor function.[40] The most effective treatment protocols appear to involve high frequency stimulation of the motor cortex, particularly on the dominant side,[41] but with more variable results for treatment of the dorsolateral prefrontal cortex.[42] It is less effective than electroconvulsive therapy for motor symptoms, though both appear to have utility.[43][44][45] Cerebellar stimulation has also shown potential for the treatment of levodopa associated dyskinesia.[46]

In psychiatry, it has shown potential with anxiety disorders, including panic disorder[47] and obsessive-compulsive disorder (OCD).[4] The most promising areas to target for OCD appear to be the orbitofrontal cortex and the supplementary motor area.[48] Older protocols that targeted the prefrontal dorsal cortex were less successful.[49] It has also been studied with autism,[50] substance abuse,[4] addiction,[4][51] and posttraumatic stress disorder (PTSD).[4] For treatment-resistant major depressive disorder, high-frequency (HF) rTMS of the left dorsolateral prefrontal cortex (DLPFC) appears effective and low-frequency (LF) rTMS of the right DLPFC has probable efficacy.[4][5][7][8][9]

TMS can also be used to map functional connectivity between the cerebellum and other areas of the brain.[52]

Mimicking the physical discomfort of rTMS with placebo to discern its true effect is a challenging issue in research.[4][11][53][54] It is difficult to establish a convincing placebo for TMS during controlled trials in conscious individuals due to the neck pain, headache and twitching in the scalp or upper face associated with the intervention.[4][11] In addition, placebo manipulations can affect brain sugar metabolism and MEPs, which may confound results.[55] This problem is exacerbated when using subjective measures of improvement.[11] Placebo responses in trials of rTMS in major depression are negatively associated with refractoriness to treatment.[56]

A 2011 review found that most studies did not report unblinding. In the minority that did, participants in real and sham rTMS groups were not significantly different in their ability to correctly guess their therapy, though there was a trend for participants in the real group to more often guess correctly.[57]

TMS research in animal studies is limited due to its early FDA approval for treatment-resistant depression, limiting development of animal specific magnetic coils.[58]

Nexstim obtained 510(k) FDA clearance for the assessment of the primary motor cortex for pre-procedural planning in December 2009[59] and for neurosurgical planning in June 2011.[60]

In 2008, the US Food and Drug Administration authorized the use of rTMS as a treatment for depression that has not improved with other measures.[61][62] A number of deep TMS have received FDA 510k clearance to market for use in adults with treatment resistant major depressive disorders.[63][64][65][66][67] The Royal Australian and New Zealand College of Psychiatrists has endorsed rTMS for treatment resistant major depressive disorder (MDD).[68]

The use of single-pulse TMS was approved by the FDA for treatment of migraines in December 2013.[69] It is approved as a Class II medical device under the "de novo pathway".[70][71]

In the European Economic Area, various versions of Deep TMS H-coils have CE marking for Alzheimer's disease,[72] autism,[72] bipolar disorder,[73] epilepsy [74] chronic pain[73] major depressive disorder[73] Parkinson's disease,[41][75] posttraumatic stress disorder (PTSD),[73] schizophrenia (negative symptoms)[73] and to aid smoking cessation.[72] One review found tentative benefit for cognitive enhancement in healthy people.[76]

In August 2018, the US Food and Drug Administration authorized the use of TMS in the treatment of obsessive-compulsive disorder (OCD).[77]

In 2013, several commercial health insurance plans in the United States, including Anthem, Health Net, and Blue Cross Blue Shield of Nebraska and of Rhode Island, covered TMS for the treatment of depression for the first time.[78][79][80][81] In contrast, UnitedHealthcare issued a medical policy for TMS in 2013 that stated there is insufficient evidence that the procedure is beneficial for health outcomes in patients with depression. UnitedHealthcare noted that methodological concerns raised about the scientific evidence studying TMS for depression include small sample size, lack of a validated sham comparison in randomized controlled studies, and variable uses of outcome measures.[82] Other commercial insurance plans whose 2013 medical coverage policies stated that the role of TMS in the treatment of depression and other disorders had not been clearly established or remained investigational included Aetna, Cigna and Regence.[83][84][85]

Policies for Medicare coverage vary among local jurisdictions within the Medicare system,[86] and Medicare coverage for TMS has varied among jurisdictions and with time. For example:

The United Kingdom's National Institute for Health and Care Excellence (NICE) issues guidance to the National Health Service (NHS) in England, Wales, Scotland and Northern Ireland. NICE guidance does not cover whether or not the NHS should fund a procedure. Local NHS bodies (primary care trusts and hospital trusts) make decisions about funding after considering the clinical effectiveness of the procedure and whether the procedure represents value for money for the NHS.[94]

NICE evaluated TMS for severe depression (IPG 242) in 2007, and subsequently considered TMS for reassessment in January 2011 but did not change its evaluation.[95] The Institute found that TMS is safe, but there is insufficient evidence for its efficacy.[95]

In January 2014, NICE reported the results of an evaluation of TMS for treating and preventing migraine (IPG 477). NICE found that short-term TMS is safe but there is insufficient evidence to evaluate safety for long-term and frequent uses. It found that evidence on the efficacy of TMS for the treatment of migraine is limited in quantity, that evidence for the prevention of migraine is limited in both quality and quantity.[96]

Subsequently, in 2015, NICE approved the use of TMS for the treatment of depression in the UK and IPG542 replaced IPG242.[97] NICE said "The evidence on repetitive transcranial magnetic stimulation for depression shows no major safety concerns. The evidence on its efficacy in the short-term is adequate, although the clinical response is variable. Repetitive transcranial magnetic stimulation for depression may be used with normal arrangements for clinical governance and audit."

A single TMS session for depressive disorders averages US$350. A full course of treatment could cost between US$6,000 and US$12,000, depending on the number of treatments.[98][26]

Manufacturers of the devices include Brainsway, Deymed, MagVenture, Mag&More, Magstim, Nexstim, Neuronetics, Neurosoft.[citation needed] Current UK providers include a few NHS Trusts and a private operator, Smart TMS. In Cyprus FDA approved treatments for depression and OCD are provided by Cyprus rTMS [99].Also the Cyprus Technological University uses rTMS in research [100]

What The Heck Is TMS TherapyAnd Is It A Legit Treatment For Depression? – Scary Mommy

Transcranial Magnetic Stimulation (TMS) therapy sounds intimidating. The name alone is a mouthful and the visual it createsskulls and magnetsis potentially alarming. But in reality, TMS therapy is neither intimidating nor alarming. Its a treatment for treatment-resistant depression that has been FDA approved since 2013. It might sound woo-woo, but theres real science behind it and real people who have benefited from it.

TMS therapy is a noninvasive form of brain stimulation used when frontline treatments, including antidepressants and psychotherapy, prove ineffective for a patient. The treatment is delivered by a device that generates highly concentrated, magnetic fields which are rapidly switched on and off.

An electromagnetic coil is placed over the area of the brain that we know is involved in depression. The coils create repeating magnetic fields which can generate electrical activity in the nerve cells, stimulating the target area, as well as other connected areas of the brain.

Treatment sessions typically last 30-40 minutes, but can vary in length. Most patients receive treatment five days a week for four to six weeks, and many may continue for another few weeks to taper off the treatment.

TMS therapy is prescribed by a TMS physician, but the treatment is usually administered by a TMS technician. During the first session, the physician determines the patients initial motor threshold. This is done by administering several brief pulses and determining the minimum amount of power necessary to make the patients thumb twitch. The motor threshold is used to personalize the treatment settings and figure out what amount of energy is needed for brain cell stimulation.

Treatment begins once the coil is placed based on all the patients measurements. Patients will hear loud clicking sounds and may feel tapping. Patients may begin to experience relief after one session, though for most, relief becomes noticeable after the third week of treatment, or even not until the sixth week of treatment, in some cases.

TMS therapy is appropriate for folks for whom antidepressant medications proved inadequate. 50% to 60% of people with depression who have tried and failed to receive benefit from medications experience a clinically meaningful response with TMS, according to Adam P. Stern, MD, director of psychiatric applications at Beth Israel Deaconess Medical Centers Berenson-Allen Center for Noninvasive Brain Stimulation.

TMS therapy would not be appropriate for patients with metal in their heads, including aneurysm clips or coils, stents in the neck or brain, metallic implants, shrapnel or bullet fragments in or near the head, facial tattoos with metallic or magnetic-sensitive ink, or any other metal object in or near the head. Braces and dental fillings are excluded from this list and considered safe during treatment.

TMS is noninvasive, which means its not surgical, and non-systemic and targeted, which means it does not affect other areas of the body like a medication might. TMS doesnt require anesthesia or a hospital stay. Its generally well tolerated and typically patients can drive home and return to their usual activities after.

The most common side effect reported in about half of patients is headache, usually mild. The headache can be treated with over-the-counter medication and usually lessens over the course of treatment. A smaller number of patients may experience hearing troubles due to the noise produced by the device, but this is likely a temporary effectand patients are generally advised to wear ear plugs while receiving treatment.

The most serious side effect of TMS therapy is seizures. But its important to note that this risk is exceedingly low.As a result, patients who are prone to seizures or who suffer from epilepsy, a history of head injury, or other serious neurologic issues, should exercise caution when deciding whether TMS is right for them.

Unfortunately, the results of TMS are not permanent. However, many patients find that they feel better for a little more than a year after treatment. Many may choose to return for additional rounds of treatment at that time.

The FDA has recently approved TMS therapy for adult patients with obsessive-compulsive disorder (OCD). The therapy has also been studied and is showing promising results for treating PTSD, neuropathic pain, and fibromyalgia. The FDA recently cleared the treatment for short-term smoking cessation in adults.

Depression is one of the most common mental conditions in the United Statesand can impair a persons life as much as any other chronic disease. TMS is just one tool researchers and clinicians are developing to help folks suffering from depression. The treatment is FDA approved, safe, and largely effective. Its not a miracle treatmentno treatment is yetbut it is a giant leap in the right direction for many patients.

This year the Teton Middle School Student Council and Latinos in Action are devoted to keeping all the kids at TMS engaged, involved, and having fun, COVID be darned.

Even though were going through the pandemic, school feels more united, because were doing a lot more activities that make us feel engaged, said council member Sage Allen.

The mission of the two clubs is to form a cohesive student leadership team. The council is more focused on fun, while Latinos in Action is a service group, but theres plenty of overlap in their projects.

These clubs are a pretty good representation of the student body, council advisor Jennie Beach said. They have the full spectrum of interests and try to engage the entire student body. I think its pretty awesome.

In the past few months alone, the clubs have organized a community clean-up that doubled as a welcoming party for new sixth graders, decorated the halls for Halloween, held contests, and created tutorials in English and Spanish on how to use the different programs required for online learning, so that both students and parents feel more comfortable tackling schoolwork.

Latinos in Action just held its second annual stocking stuffer drive and presented 68 stockings full of gifts for kids to Subs for Santa on Dec. 12. The student council, with donations from the Victor Valley Grocer and Arnold Farms, is assembling holiday meals for families. And just to keep the mood light in the halls of the middle school, the grades are engaged in an intense battle for who can decorate the best classroom door.

Eighth grader Alexis Hernandez masterminded the idea of Pay to Play, Latinos in Actions new fundraising game that has already enabled them to buy stockings for Subs for Santa, and lets kids blow off some steam at the same time. Students can buy $1 raffle tickets and the winner is allowed to play (pre-approved) video games for a brief period during the day, usually an experience that is verboten at school.

Kayleen Diaz, also in eighth grade, explained that some kids feel more comfortable asking friends for help than going to a teacher, so she has proposed setting up study groups that offer peer-to-peer tutoring.

Theyre fantastic on having an idea and then moving forward quickly with it, Beach said. It really makes a difference.

But, Leo Cuellar noted, Latinos in Action and student council arent just implementing their own ideas; other kids at the school are always welcome to offer suggestions. We definitely like to have all the students input, so its fun for the whole school, he said.

This school year has been difficult, so were trying to focus on the positive and make things fun, Gracie Kincaid added. I think thats what we all need right now.

ABSTRACT

Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is a global pandemic which has induced unprecedented ramifications, severely affecting our society due to the long incubation time, unpredictably high prevalence and lack of effective vaccines. One of the interesting notions is that there is an association between COVID-19 and cancer. Cancer patients seem to exhibit exacerbated conditions and a higher mortality rate when exposed to the virus. Therefore, vaccines are the promising solution to minimise the problem amongst cancer patients threatened by the new viral strains. However, there are still limitations to be considered, including the efficacy of COVID vaccines for immunocompromised individuals, possible interactions between the vaccine and cancer, and personalised medicine. Not only to eradicate the pandemic, but also to make it more effective for immunocompromised patients who are suffering from cancer, a successful vaccine platform is required through the implementation of nanotechnology which can also enable scalable manufacturing and worldwide distribution along with its faster and precise delivery. In this review, we summarise the current understanding of COVID-19 with clinical perspectives, highlighting the association between COVID-19 and cancer, followed by a vaccine development for this association using nanotechnology. We suggest different administration methods for the COVID-19 vaccine formulation options. This study will contribute to paving the way towards the prevention and treatment of COVID-19, especially for the immunocompromised individuals.

Microsoft and the professional services network Ernst & Young (EY) have announced that they have expanded the Xbox Enterprise Blockchain Platform to provide a financial system of record for gaming rights and royalties management.

The blockchain-based system will be used by the software giant to enable its Xbox gaming partners and network of artists, musicians, writers and other video game content creators to gain increased visibility into the tracking, management and payment processing for royalty contracts.

EY expects its other B2B enterprise clients to follow suit and create similar blockchain platforms to help them automate contract-related calculations and processing.

The firm's global blockchain leader Paul Brody provided further insight on how enterprises could benefit from implementing blockchain technology in a press release, saying:

Blockchains could well become the glue that digitizes interactions between enterprises. This go-live represents another big step on that path, extending the level of automation and cycle time compression all the way from digitizing the contract to posting financial accruals. Blockchain solutions like this help raise the bar for enterprise integration, from point-to-point integration to ecosystem-level automation.

With its expanded blockchain platform, Microsoft will be able to accelerate contact creation using AI based on Microsoft Azure.

The Xbox Enterprise Blockchain Platform is also now able to seamlessly generate and integrate statements and invoices with enterprise resource planning (ERP) applications to process and record royalties with increased speed, visibility and transparency. Accounting entries can even be generated from the blockchain platform into ERP applications as well.

The expanded solution reduces processing time by 99 percent with 100 percent near real-time calculation of royalties using digital contracts across game development partners. The Xbox Enterprise Blockchain Platform has also been tested to support performance under a high volume of usage and is capable of processing 2m transactions per day.

GM of global finance operations at Microsoft Luke Fewel explained how the recent go-live shows the potential for blockchain and smart contract technology, saying:

In this go-live, we successfully generated the first round of partner payments utilizing blockchain and smart contract technology. This expanded solution will help streamline financial and operational processes with the ability to scale, reduce heavy manual overhead and improve the experience for Microsofts gaming partners.
