Mississauga rising: Why health care here is better than in Toronto

Live slow, die old in Mississauga!

Doesnt quite have that bumper-sticker ring, but it could very well be the new branding strategy for the sixth-largest city in Canada.

While hospital row on University Avenue is justifiably famous, theres an outpost of high-quality health care in Mississauga: Credit Valley Hospital and Trillium Health Centre.

This week, as part of Mississauga Rising a month-long series that explores what Hogtown could learn from its westerly neighbour The Globe and Mail considers why the largest 905 burg might be a smarter place to get chemotherapy, hip surgery or treatment following a stroke, despite the prevailing wisdom that Toronto offers the best health care in the country.

Through a landmark hospital-comparison tool developed this year by the Canadian Institute for Health Information, Credit Valley (which merged in December with Trillium Health Centre) ranks at the top of the pack among all 14 large community hospitals in the GTA (to clarify: the five teaching hospitals in Toronto, which see much higher patient loads, were rated in a different category).

Based on data from 2010 and 2011, the tool shows that a patients odds of dying in hospital within 30 days following a stroke are much lower at Credit Valley, compared to the provincial and national average. The hospital also beat most of its counterparts when it came to low readmission rates in the 90 days after hip and knee replacement.

And the hospitals becoming a magnet for top talent. In September, it poached Dante Morra, revered internal-medicine specialist and the co-founder of the Centre for Innovation in Complex Care at the University Health Network. Hes Credit Valley-Trilliums new chief of staff.

At the helm of the newly merged hospital is Michelle DiEmanuele, who left Toronto and a range of top gigs in 2008 to become Credit Valleys CEO. We caught up with her in her office.

COLUMBIA A blow to the knee meant more than pain and discomfort for law student Katherine Sheffield it meant $3,000 in medical bills, not including a future surgery.

Because of the Affordable Care Act, Sheffield, 24, will be covered under her stepfathers insurance plan until she's 26. Before the health care act, young people could be removed from their parents insurance plans as early as 19.

The actual (cost of) surgery will be outrageous, Sheffield said. So yeah, as an indebted 24-year-old, Im pretty darn thankful for insurance.

Sheffield said health care will be on her mind when she goes to the polls Tuesday.

Health care is a hot-button issue this election season, with most discussions focusing on the Affordable Care Act. When asked how large a role health care would play at the polls on Nov. 6, Columbia residents had a variety of responses.

Nationally, health care is ranked second after jobs and the economy in the top spot on the list of issues that would influence people's voting decision, according to a study by the Harvard School of Public Health.

That's the highest health care has ranked as a presidential election issue since 1992, according to researchers.

AMENDMENT 1

Health Care Services

What it would do: Amend the state constitution to prohibit any Florida law or governmental rule that would require any person or employer to have or provide health care coverage. Also would prohibit any law abolishing the private market for health care coverage and prohibit any penalties or taxes for paying directly for health care services.

Lawmakers put the amendment on the ballot before the U.S. Supreme Court ruling that upholding the individual mandate portion of the federal Affordable Care Act that requires most Americans to buy health-care coverage. That ruling means states cant opt out of the requirement.

AMENDMENT 2

Veterans Disabled Due to Combat Injury; Homestead Property Tax Discount.

What it would do: Amend the constitution to expand the availability of the extra $25,000 homestead exemption. Veterans disabled by a combat injury and who were not Florida residents when they entered the military would be eligible for the exemption. Applies to assessments for local government and school district taxes; would take effect Jan. 1.

Nerurkar on health care worries

Jamshedpur, Nov. 3: Tata Steel managing director H.M. Nerurkar conceded that health facilities available in the steel city were not enough to meet the growing demands of its burgeoning population.

Addressing a gathering during the groundbreaking ceremony of a nursing school to mark the golden jubilee of Mercy Hospital at Baridih this evening, Nerurkar said the steel major was conscious of this need and was willing to extend all help in improving health care facilities in the city and its outskirts.

"It is in pursuit of this effort that we have extended financial contribution to Sisters of Charity of Jesus and Mary for opening a nursing school on their premises and also in upgrading their infrastructure time and again. This hospital serves the marginalised and we are happy that through the year it has carved a niche for rendering hospitality with care and service to the poor," he said.

The managing director said Tata Main Hospital (TMH) was one of the most trusted hospitals of the region but more such heal hubs were needed to meet the growing needs of the masses.

"We have TMH and a government hospital (read MGM Medical College and Hospital) and a couple of other industrial hospitals in the city. But it is not enough for the people. We need more super-speciality hospitals for the people of the region," said Nerurkar.

The managing director's remarks were significant in light of Tata Steel signing a pact with Manipal Hospitals Group for a medical college and nursing school. The search was on for a suitable plot for this purpose.

The Tata Steel managing director also played a crucial role in Calcutta-based Medica Superspecialty Hospital signing a MoU with Kantilal Gandhi Memorial Hospital in Bistupur for operation and maintenance of the Jamshedpur based hospital.

Nerurkar felt that shortage of qualified nurses was also plaguing the heal hubs in the region and hoped that the nursing school would help in ensuring a bank of good nurses.

"Majority of nurses after putting in some years of service in the city go outside due to lucrative options and there is dearth of good nurses here. Such nursing schools will ease this shortage to a great extent," he said.

For the record, Tata Steel has chipped in with Rs 5 crore for the three-storied nursing school at Mercy Hospital.

Instead, more than 3 million people in Florida from infants to the elderly who get their health care under Medicaid will be affected in different ways depending on whether President Barack Obama is re-elected or voters choose GOP challenger Mitt Romney.

About 16 percent of people in Florida receive health care under Medicaid, the federal-state program for the poor and disabled. More than one-quarter of Florida's children, half of all baby deliveries and 63 percent of nursing home days in the state are covered by the program.

Significant attention in the campaign has focused on how best to preserve Medicare, the federal health care program for people age 65 and over. But Obama and Romney are also pushing widely divergent plans for dealing with Medicaid.

Obama wants to expand Medicaid as a key component of his Affordable Care Act, providing care to more people by raising the eligibility limits. Romney, who wants to repeal the health care law, has advocated converting Medicaid into a grant program that would give states much more flexibility in deciding who should be covered by the system.

Like other proponents of the approach, Romney says the grant system would allow the federal and state governments to better control rising costs.

State leaders across the country have balked at the rising bite that Medicaid takes out of their budgets, especially after the Great Recession and plummeting tax revenues have forced numerous cutbacks. Florida's Medicaid costs alone have more than doubled since 2001, with more than 1 million more people joining the state's programs since 2007.

Among other measures, Florida has tested and is pushing for approval to expand statewide a managed care program to rein in costs.

But critics warn the block grant approach which would essentially cap costs with a factor for inflation and population would result in deep cuts in services, impacting some of Florida's most vulnerable residents, ranging from children in families struggling with little income to frail seniors in nursing homes.

A study last month by the Kaiser Family Foundation shows Florida could lose as much as $33.3 billion over the first 10 years of a block grant program based on a plan passed by the U.S. House and back by Romney's running mate, U.S. Rep. Paul Ryan, R-Wisconsin. Nationwide, the block grant program would cut Medicaid spending by $810 billion in the same period.

As Mitt Romney and President Barack Obama pack in some last-minute campaigning before Tuesdays election, polls show voters are split just about down the middle on who they prefer: Romney, who has promised to do everything he can do repeal the 2010 health reform law, and Obama, who says its benefits are just beginning to take hold.

But while the presidential race gets most of theattention,the choices voters make to fill governors mansions and state legislatures mayhave just as big an effect on what kind of health coverage they will have in coming years.

Thats in part because the Affordable Care Act sets it up that way, but even more so because the Supreme Court says its up to states to decide whether and how to expand the Medicaid health insurance plan for the poor.

One thing the voters should be aware of is what are their governors are going to be doing. Will more people have access to Medicaid or access to a state-run exchange? says John Poelman of healthcare consulting firm Leavitt Partners and a former health policy analyst at the Department of Health and Human Services.

Two states at the two extremes of health care coverageare Texas, with a free-market, bare-bones approach, and Vermont, which is unabashedly going for a European-style, government-supported system.

Brandi DeFrank is one of the 6.3 million people in Texas who lack health insurance. That'sa quarter of the state's population and the highest percentage of uninsured peoplein the country.

Like millions of women across the country, DeFrank,20, was fully covered under Medicaid, the state-federal health insurance plan for low-income people, while she was pregnant. The birth of her 3-month-old son, Gabriel, was also covered, but after that, her own coverage ended. The baby remains on Medicaid -- all states make some provision for children whose parents lack insurance -- but now DeFrank is on her own and gambling that she wont get sick.
