OKLAHOMA CITY (AP) Looming decisions for Gov. Mary Fallin on how Oklahoma will respond to the sweeping federal health care law are prompting an energetic, behind-the-scenes lobbying effort by hospitals, insurance companies, business and industry groups, and other constituencies that will be affected by provisions of the law.

Fallin is expected to announce within the next week her position on whether the state will move ahead with setting up a state-based online health insurance marketplace, or exchange, required under the law. Oklahoma policymakers also must decide whether the state will expand its Medicaid eligibility to provide coverage to thousands of low-income, uninsured citizens.

Fallin has yet to stake out a position on either proposal and faces a delicate political balancing act in a state where Republicans have bitterly resisted the requirements of the new federal health care law. On the one hand, hospital officials and chamber groups are pushing for both a state-based exchange and an expansion of Medicaid. But both of those ideas are fiercely opposed by tea party and other grass-roots activists who have been fighting implementation of the law since its passage in 2010.

"It's a real challenge for the governor because what it's done is put her right in between two major constituent groups inside the Republican Party," said Keith Gaddie, a professor of political science at the University of Oklahoma.

The Tulsa Metro Chamber, which represents more than 3,000 members, is among those lobbying the governor to support both a state-operated insurance exchange and an expansion of Medicaid.

"Certainly health care is one of the two largest employment sectors in Tulsa, so we're very cognizant of the jobs health care provides in our community," said Susan Harris, a senior vice president at the chamber who works on health policy. "Also, we need a healthy workforce. Healthy workers get better jobs, make more money and take care of themselves. It's better for the whole community if we've got a healthy workforce."

Fallin herself opposed the law when she was a member of Congress and even dangled a "Don't Tread on Me" flag before a tea party rally at the U.S. Capitol during the health care debate in 2010. Fallin and other Republican leaders initially hoped the U.S. Supreme Court would overturn the law, and when that didn't happen, she withheld a decision on whether to proceed until after the election, hoping Mitt Romney would be elected and either help overturn or the slow the implementation.

Oklahoma was among the states that over the law, and even after the Supreme Court's decision, Attorney General Scott Pruitt amended his lawsuit to challenge its implementation of the law.

"There is political risk in quitting resistance, but after you fail three times you really have to reassess whether or not you're going to prevail in this fight," Gaddie said. "At some point you just have to leave the battlefield and go home."

Fallin spokesman Alex Weintz said Friday the governor is still exploring the state's option as they relate to both the creation of a state exchange and the expansion of Medicaid.

FAIRBANKS Disability compensation, health care, dental care and mental health counseling are just some of the services available to veterans in Alaska.

Unfortunately, out of the close to 77,000 veterans in Alaska, only around 30,000 have registered to take advantage of the services available to them.

In recognition of Veterans Day, the State of Alaska Department of Health and Social Services Rural Veterans Health Access Program, in collaboration with the Department of Military and Veterans Affairs and the U.S. Department of Veterans Affairs Alaska VA Health Care System, are rolling out a state-wide education campaign to encourage Alaska veterans to enroll in VA benefits. The goal of the campaign is to increase awareness about VA benefits provided by the VA and to increase access to care and services for veterans.

Posters with tear-off informational cards are being distributed throughout the state to different social and support organizations and establishments where veterans are known to gather.

Information on the cards include steps to apply for disability compensation, health care, counseling, mental health services, pension benefits, burial benefits, and vision and dental treatment.

Health care benefits

Applying for enrollment is now easier and faster than ever. Regulations have changed enabling the VA to relax income restrictions on enrollment for health benefits. Any veteran who has served in the active military, naval or air service, and separated under any condition other than dishonorable, may qualify for VA health care benefits. Current and former members of the Reserves or National Guard who were called to active duty (other than for training only) by a federal order and completed the full period for which they were called or ordered to active duty may be eligible for VA health care.

Every VA medical center has a team ready to welcome Operation Enduring Freedom, Operation Iraqi Freedom and Operation New Dawn service members and to help coordinate their care.

That's when a structural budget deficit caused by $180 million in future retiree health care costs will rear its ugly head again and threaten to drive Pontiac into bankruptcy, according to the citys emergency financial manager, mayor and finance director.

Days after voters repealed Michigans emergency manager law and Pontiac voters overwhelmingly rejected a millage to pay for retiree health care, city officials are proposing the privatization of a city pension plan thats 150 percent funded, then taking the leftover 50 percent and placing it in a trust account earmarked for retiree health care.

One municipal finance expert said its a groundbreaking idea.

I have never seen, in my memory, a situation where a (pension) issuer is trying to do it retroactively: Taking existing pension benefits and privatizing (them); I have not seen it. Its an interesting idea, said Dick Larkin, director of credit analysis at the Iselin, N.J.-based investment firm HJ Sims. Larkin is the former chief municipal rating officer at Standard & Poors.

Mayor Leon Jukowski said the city has pension obligations to about 1,000 retirees. Current city employees are on a defined contribution plan, so the citys pension plans are closed, meaning no employees will join them in the future.

We think that we have more money than we need to cover those obligations, so we (could) go into the market and buy an annuity equal to the retirement benefit due for every person who has a benefit coming. If we do that, we set them all up with an annuity that covers their retirement obligation, (and) theres money left over that the city can use to cover retiree health care, the mayor said.

Emergency Financial Manager Lou Schimmel said: If they dont eliminate retiree health care or (go through) with the privatization plan ... the only option left for this city is bankruptcy." Continued...

Health Care REIT Inc. (HCN), a real estate investment trust (:REIT), reported third quarter 2012 FFO (funds from operations) of 75 cents per share compared with 85 cents in the year-earlier quarter. The decrease in year-over-year FFO per share was primarily attributable to increased number of outstanding shares in the reported quarter.

Q. What is Kaiser Permanentes position on the Affordable Care Act?

Mr. Lum: The rising cost of health care and a large uninsured population is a big and complicated problem. Kaiser Permanentes position is that we must expand access to affordable insurance coverage through improving the efficiency and quality of care. We also believe that people should have a clear choice of health plans, and insurance companies should compete on the quality of services rather than their underwriting tactics to avoid risk. The ACA has many provisions that support our view. The main discussion about the ACA has been on its insurance reform features and now on the introduction of the California State exchange, Covered California, in 2013. The exchange will be the key vehicle for expanding coverage, offering choice, and making quality transparent to consumers.

Q. How is the market preparing for the introduction of Covered California, the new state exchange?

Mr. Lum: The State of California took an early leadership role in starting the development of the exchange. Our insurance market is also very familiar with exchanges. CaliforniaChoice, a private exchange, has been an important part of the small business market for many years. Many small businesses also bought insurance coverage through PacAdvantage, another California exchange that since closed. Despite the early start and our states experience with exchanges, theres a lot to do to by October 2013 when Covered California opens its doors to enroll people for January 2014 coverage. Covered California needs to put into place all the processes, people, and technology to help many thousands of people understand the cost and choices of different insurance plans. Then Covered California and the health plans need to work together to enroll people, so that Kaiser Permanente and other health plans can issue and collect monthly premiums and provide services. Theres a tremendous amount of work that is happening right now to get ready.

Arkansas moves forward on health overhaul

LITTLE ROCK, Ark. (AP) -- Arkansas officials say the state is moving forward with implementing parts of the federal health care law after an election that provided clarity on the federal level but uncertainty in the state Legislature about the political future of the controversial legislation.

President Barack Obama's re-election on Tuesday night came in the same election where Republicans won control of both chambers of the state Legislature partly based on opposition to his federal health care overhaul. Officials with Gov. Mike Beebe's administration say the election at the national level provides some certainty on the future of the law as it takes effect in the state.

The next step for Arkansas comes this week, when Beebe is expected to formally tell Obama administration officials that the state plans on partnering with the federal government on setting up a health insurance exchange, in which millions of households and small businesses will shop for private coverage. A spokesman for Beebe's office said the governor planned to notify the U.S. Department of Health and Human Services by the end of the week.

The partnership option allows states to handle consumer relations and oversight of health plans, while the federal government does the heavy lifting, taking care of enrollment, and figuring out any taxpayer help that consumers may be entitled to. Beebe had opted for the partnership model after Republicans last year opposed efforts for the state to set up its own exchange.

"We will pursue the next best thing we can to maintain as much state input as possible," Beebe spokesman Matt DeCample said.

Arkansas has already received $27.7 million in grants from the federal government for planning for the exchange partnership, including an $18.6 million grant that's still pending approval from state lawmakers, according to Cynthia Crone, who leads the Insurance Department's Health Benefits Exchange Partnership division.

Crone said the presidential election may help in making the case to lawmakers who wanted to wait and see whether Obama would remain in office before moving forward on the exchange partnership, which allows the state to have some say in its setup.

The bigger question looming for the state is whether lawmakers will support an expansion of Medicaid's eligibility under the federal health care law. Beebe has said he supports the expansion, which would add about 250,000 people to the state's Medicaid rolls, but noted that it'll take the support of 3/4th of the House and Senate. With Republicans who are opposed or hesitant about the idea, that may be too high of a vote threshold in the GOP-controlled Legislature next year.

If approved, the expansion would offer coverage to everyone making up to 138 percent of the federal poverty level. That works out to about $15,400 for an individual or $30,650 for a family of four. Most of those who would be added to the Medicaid rolls are low-income adults without children.

Beebe has said that the election doesn't change his agenda, including the Medicaid expansion, and noted that winning support for it was an uphill challenge even before Republicans won control of the state Legislature.

In front of the Port Townsend Food Coop, Gunther Dohse talks about signing the I-522 petition, being a coop member and healthy living. I-522 is an initiative to the Washington State legislature to establish mandatory labeling of foods produced through genetic engineering. As recommended by the Secretary of State, the LabelitWA group needs to turn in at least 320000 signatures to ensure they have 241153 valid signatures by December 31, 2012 in order to get on the November 2013 ballot. I-522, "The People #39;s Right to Know Genetically Engineered Food Act," is simple. The initiative would require food sold in retail outlets to be labeled if the ingredients are produced through genetic engineering.
