Dean of LSU medical school named second finalist for KU Medical Center executive job

The second candidate named this month for the top position at the Kansas University Medical Center is the dean of the School of Medicine at the Louisiana State University Health Sciences Center in New Orleans.

Steve Nelson, who has been on the faculty at LSU since 1984, will be one of three finalists for the executive vice chancellor job at the medical center. He will take part in a public town-hall meeting from 5 p.m. to 6 p.m. Thursday in the G013 School of Nursing Auditorium on the KUMC campus.

Nelson has been dean of the LSU medical school in New Orleans since 2007, and he has been the president of the LSU Healthcare Network since 2009.

He earned a medical degree from McGill University in Montreal in 1978. Like the first candidate named for the KUMC job, Norman Beauchamp, he has a connection to Johns Hopkins University in Baltimore; he completed his residency and two fellowships at the university's hospital, as well as a fellowship at its School of Hygiene and Public Health. His specialty has been pulmonary medicine, though he also leads a Comprehensive Alcohol Research Center at the LSU Health Sciences Center in New Orleans.

KU released his name and career information in advance of his visit on a webpage set up to track the executive vice chancellor search.

Beauchamp, the first of three finalists for the job, visited the KUMC campus last week. The third candidate, whose name has not yet been revealed, is scheduled to appear at a town-hall meeting Tuesday.

UC Regents request funds to reboot projects

SAN FRANCISCO Two UC campuses received important endorsements Thursday for long-stalled projects: a new medical school at Riverside and a major classroom building at Merced.

The UC regents included a proposed $15 million to help run the medical school and $45 million for the Merced building in their 2013-14 budget request to the governor and Legislature. The regents said they were more optimistic than in the past about their chances since state tax revenues are improving.

Meanwhile, about 60 student protesters demanding that any new UC revenue be used to freeze tuition or roll it back blocked an intersection for several hours near the UC San Francisco facility where the regents were meeting. One person was cited for vandalism after allegedly banging and damaging a car trying to get past, according to San Francisco police.

Inside the meeting, a dozen chanting protesters briefly disrupted the regents' discussion. They were escorted out by police without arrests.

UC Santa Cruz freshman Jeanne Baldzikowski said she regretted skipping school for the street rally "knowing I'm paying for the classes, but maybe I will have to skip a lot of classes in the future if I can't afford them."

Last year, national accreditors refused to allow the UC Riverside medical school to open because it lacked state funding. But the accrediting agency reversed itself last month and granted preliminary approval after Riverside administrators garnered $100 million in private and public money. The school, which is to enroll its first 50 students next summer, will need about $15 million extra annually from the state to expand and win full accreditation, UC officials said.

The youngest among UC's 10 campuses, 7-year-old UC Merced, said the lecture hall building will increase overall classroom space by 50% and help enrollment grow from its current 5,700 to 10,000 by the decade's end. With a room shortage, increasing numbers of classes are taught as early as 7:30 a.m. or after 9 p.m. Funding was denied in past years but the outlook improved recently after Gov. Jerry Brown approved $4.7 million for its design and planning; now the request for construction will be on Brown's desk.

State financing for the Merced building is "absolutely our highest priority for construction," said Patrick Lenz, the UC system's vice president for budget and capital resources. In all, UC is seeking $788 million in state funding for 39 projects, including some renovations and seismic improvements.

The regents also approved an agreement for UC Davis Medical Center to help run the 202-bed Dameron Hospital in Stockton and took preliminary steps to help boost income from UC patents and investments in start-up firms based on UC faculty members' inventions.

Judge rejects effort to prevent medical school proposition from taking effect

A federal judge Thursday rejected arguments that a Nov. 6 ballot proposition seeking higher property taxes for a medical school in Austin and other health care projects should be stopped from taking effect.

U.S. District Judge Lee Yeakel indicated in his ruling that he did not think plaintiffs would ultimately succeed in their lawsuit, which seeks to invalidate Proposition 1, and he refused their request for a preliminary injunction to stop the votes from being canvassed a technical but key step in finalizing the vote.

Central Health is now free to canvass the votes on Monday at noon, as planned, and negotiate contracts for the medical school with the University of Texas and a new teaching hospital to be built by the Seton Healthcare Family, Central Healths lawyer Jim Cousar said.

This is the ruling we hoped for, Cousar said.

The suit by Prop 1 opponents, led by Don Zimmerman, treasurer of the Travis County Taxpayers Union political action committee, claimed that the ballot language was improper and confusing, especially to minority voters. As such it violated the Voting Rights Act, designed to guard against discrimination at the polls, the suit said.

The court holds that plaintiffs have failed to establish a substantial likelihood of prevailing on the merits (and) have an almost impossible burden to demonstrate that the Voting Rights Act provides them the opportunity to challenge the language of Proposition 1, Yeakel said in his 11-page decision.

Stephen Casey, the lawyer for Zimmerman, his PAC and two minority voters, said he is exploring options, including the possibility of appealing to the Fifth U.S. Circuit Court of Appeals. He argued before Yeakel Wednesday afternoon that the ballot language amounted to cheerleading and that minority voters, whom he said have lower reading comprehension than whites, were more likely to find the wording confusing.

We believe weve got a strong case, Casey said. We obviously disagree with the judge.

Casey noted that Yeakel did not throw out the case and left pending Cousars motion to dismiss the case. Yeakels ruling, however, made clear that the judge has serious concerns about the claims Casey made.

Travis County voters handily approved Prop 1, 55 percent to 45 percent. It raises the property tax Central Health imposes for health care services from 7.89 cents to 12.9 cents, causing average tax bills to rise by $107 in 2014.

Newswise New Brunswick, NJ -- Providing quality patient-centered health care and controlling costs, all while adapting to health care reform, is the goal of a new accountable care organization (ACO) being created by the University of Medicine and Dentistry of New Jersey (UMDNJ)-Robert Wood Johnson Medical School. Researchers from the school report in a study published in Novembers issue of Health Affairs on the three-phase development of the ACO to date, the challenges it has faced and solutions to those challenges, and enabling factors that have contributed to its success so far. The lessons shared in the study can be used to assist other academic health centers and health care organizations in establishing new models of care that hold participating providers and systems responsible for managing care and controlling expenses. Health care reform means that academic health centers must change so they may preserve and strengthen their position as leaders in health care delivery, education and research, said Alfred F. Tallia, MD, MPH, professor and chair of family medicine and community health at Robert Wood Johnson Medical School. Clinical care supports most academic health centers; in order to maintain that capacity, medical schools must embrace and experiment with new efficient designs of health care delivery and payment. Academic health centers such as ours have a unique opportunity to reinvent themselves and lead the discovery and testing of new systems of health care organization and delivery. Accountable care organizations bring together in one system the different providers capable of caring for people from cradle to grave, and cover everything from advanced outpatient primary care to innovative cutting edge inpatient tertiary and quaternary care.

Drawing on 15 years of published research on transforming primary care practices conducted by the Family Medicine and Community Health division of research, the medical school initiated the process of creating the ACO in 2009. It established an integrated medical group, the Robert Wood Johnson Medical Group, with a common information platform and governance. The Robert Wood Johnson Medical Group implemented advanced primary care facilities, called Patient Centered Medical Homes (PCMH), in its outpatient primary care offices. This PCMH model of primary care is built around care delivery teams of physicians, nurses and mental health providers, as well as other health professionals who identify, coordinate, and treat acute and chronic conditions in high-risk populations.

By managing health care concerns of high-risk populations, we can reduce avoidable hospitalizations, and recurrent and expensive visits to the emergency room for health issues that can be more efficiently treated in outpatient settings, said Dr. Tallia. However, for accountable care organizations to be successful, they must provide high quality individualized patient care across the entire spectrum of health care, and that will lead to better outcomes.

The first phase of development included discussions with multiple stakeholders including providers -- both individuals and organizations -- such as hospitals, insurers, patients, consumer groups, government payers and policy makers, and researchers. The discussions resulted in an initial plan that includeds governance, infrastructure needs, clinical guidelines, and revenue and expense predictions.

Seeking feedback the plans developers shared it with local health care and government leaders, as well as hospital executives, during the second phase of creating the ACO, in order to ascertain their priorities, gain their support, and learn about challenges that needed to be overcome.

We cannot do it alone, said Dr. Tallia. Collaboration is essential to accountable care to ensure that we can keep people healthy, while also caring for them when they are sick.

The third phase is underway, as Robert Wood Johnson Medical School seeks the necessary start-up funding from private and public investors, as well as partner organizations in the Accountable Care Organization.

Obtaining financial support is one of the barriers the medical school has encountered in the creation of its ACO. Cultural differences and priorities among stakeholders and partners, as well as a lack of legal precedent involving ACOs, also have been obstacles. According to Dr. Tallia, resistance to change and skepticism about health care reform are consistent barriers that the medical school has worked to overcome to achieve a successful accountable care organization.
