Study finds Post Baccalaureate Program successful in diversifying physician workforce and helping underserved regions and populations – The South End

Medical schools willing to invest in qualified students from disadvantaged backgrounds can impact the nations physician workforce by increasing the number of doctors serving in regions designated as health professional shortage areas and medically underserved populations, according to a Wayne State University School of Medicine study published in Academic Medicine, the Journal of the Association of American Medical Colleges.

Impact of a 50-Year Premedical Postbaccalaureate Program in Graduating Physicians for Practice in Primary Care and Underserved Areas,a review of the impact of the School of Medicines Post Baccalaureate Program over its 50-year life, found that the program has been successful in graduating a large proportion of physicians from disadvantaged and diverse backgrounds. Many of these physicians went on to practice in regions with a shortage of doctors and in areas with underserved populations, accomplishing the goals of addressing the broad primary health care needs of all Americans, said lead author Herbert Smitherman Jr., M.D., M.P.H., professor of Internal Medicine and vice dean of Diversity and Community Affairs.

The study set out to evaluate the effectiveness of the School of Medicines premedical Post Baccalaureate Program in achieving its goals, measured by medical school, medical school graduation, primary care specialization and current practice.

The programs foundational goals are to provide academically-qualified students from lower socioeconomic, disadvantaged and underrepresented backgrounds the opportunity to become physicians; to identify and select students likely to return to their underserved communities to practice; to increase access to health care in underserved communities and therefore improve health outcomes; to increase the number of primary care physicians both locally and nationally; and to increase diversity in the physician workforce in an effort to address health inequities, disparities and social determinants of health.

To ensure that qualified minorities continued to have the opportunity to enter medical school, in 1969 the WSU School of Medicine established the Post Baccalaureate Program, the first of its kind in the nation. Initially launched to address the dearth of African American students entering medical schools, the free program immerses students into a year-long education in biochemistry, embryology, gross anatomy, histology and physiology. Many who graduated from the program were accepted into the WSU School of Medicine, but the program also served as a major pipeline for Black students into medical schools across the nation.

Five African American students were admitted into the initial program, which was so successful that in 1972 it expanded to accept 10 students. The first Post Baccalaureate Program student graduated from the Wayne State University School of Medicine in 1974. The program expanded its efforts to increase underrepresented minorities in medicine. Following the U.S. Supreme Courts 1978 Bakke decision, the program cast a wider net yet again, accepting socio-economically disadvantaged students regardless of race or ethnicity.

In the 1970s and early 1980s the program served as a major pipeline for the admission of African American students to medical schools across the country. During the 1980s and 1990s, the WSU School of Medicine earned the distinction of graduating more African American physicians than any other medical school in the nation, with the exception of Howard University in Washington, D.C., and Meharry Medical College in Nashville. Representatives of U.S. medical schools flocked to Detroit to learn how WSU accomplished the achievement.

Today, as many as 200 Michigan undergraduates apply for the program each year. A maximum of 16 students are accepted annually.

The study found that of 539 students who graduated from the program between 1979 and 2017:

463 (85.9%) successfully completed, then matriculated to the School of Medicine. Of those, 401 (86.6%) obtained a medical degree.Of the 401 who obtained a medical degree, 233 were female.The average investment per Post Baccalaureate Program student was approximately $52,000; for a M.D. graduate approximately $77,000. The majority of doctors who graduated the program (72.1%) lived in areas identified as medically underserved or having a shortage of physicians when they were admitted to the program. Today, 82% practice in such areas. Most practice primary care medicine.The programs total cost for 50 years was $32 million, an average investment of approximately $52,000 per student, and approximately $77,000 per graduated physician.

Today there are 250 similar programs in the United States, but a study by the AAMC found only 63 focused on underrepresented in medicine students and only 18 of those had explicit diversity-based missions. Because the WSU program cost structure is flexible, the study team reported, universities with existing medical education resources can readily adapt the program to meet their needs.

Fall 2020 graduate Cindy Bonilla-Cirocco says her peers should always keep an open mind about their studies and career path. Fall 2020 graduate Cindy Bonilla-Cirocco will be receiving her PhD in Latin American literature and culture in December. I would encourage students not to limit themselves and to do their research about what other options are out there, said Bonilla-Cirocco, who will be receiving her PhD in Latin American literature and culture. You never know when you may find a great opportunity you werent expecting if you keep yourself open to it!

Luckily, Bonilla-Cirocco took her own advice.

Midway through the process of researching and writing her dissertation, she enrolled in a Latin American film course taught by Spanish Professor Cynthia Tompkins that changed the course of her doctoral studies. After writing a paper about the 2015 documentary film "Todo comenz por el fin (It All Started at the End)" by Colombian director Luis Ospina, Bonilla-Cirocco switched her dissertation topic from Latin American graphic novels to Colombian cinema.

I was immediately drawn to his work and wanted to know more, she said. It was a drastic change, and I had to essentially learn all about Latin American cinema, as well as Colombian cinema, on the fly as I wrote my dissertation.

With Tompkins assistance, Bonilla-Cirocco published her paper about It All Started at the End in the academic journalConfluencia from the University of Northern Colorado. She then developed it into her dissertation by including additional research and analysis on Ospina and other members of the young and subversive Caliwood filmmaking collective that he was a part of in the 1970s, including Carlos Mayolo.

Bonilla-Cirocco succeeds in highlighting the political commitment of these filmmakers, who risked international success by rejecting and lampooning the Colombian pornomiseria movies of the time that profited from showing the most sordid aspects of the third world, both in terms of the production of a movie as well as penning a manifesto of their own, said Tompkins, who also served as Bonilla-Cirocco's dissertation committee chair.

Bonilla-Cirocco said she was determined to convey her newly discovered passion for Colombian cinema in her dissertation, which she successfully defended in October.

This was a challenging task, and at times I felt overwhelmed, but I knew what my end goal was and I refused to let so many years of hard work and intellectual growth go to waste, Bonilla-Cirocco said. Finishing my dissertation is one of the proudest moments of my life.

During her time at ASU, Bonilla-Cirocco volunteered at various events, such as the School of International Letters and Cultures Language Fair and Homecoming, where she represented SILCs Spanish Department. She also was a member of the organizing committee for ASUs Hispanic Heritage Month celebration, which involved connecting with Spanish-speaking communities around the Valley to showcase various cultures. Bonilla-Cirocco, who was raised in Colombia, served as a liaison to the local Colombian community in particular.

Cindy has an incredible work ethic. She is unassuming, diligent, always well prepared and very professional, said Associate Professor of Spanish Jess Rosales. I believe that Cindy is a socially concerned scholar who is committed to sharing her knowledge, not only with fellow academics, but also with underprivileged communities unable to receive a university education.

Bonilla-Cirocco chose to complete her doctoral studies at ASU after first earning her bachelors degree due to the distinguished reputation of the Spanish program. Doing so allowed her to accept a position as a teaching assistant for Spanish courses at all levels, as well as working as a research assistant for Regents Professor of Spanish David William Foster.

Foster was the original chair of Bonilla-Cirocco's dissertation committee before he diedearlier this year, and he had served as a mentor to her for many years.

There was no better example of a person who was deeply committed to and passionate about his work and his students, Bonilla-Cirocco said. I am honored to have known and worked under him, and I learned things from him that I will remember for the rest of my life."

Bonilla-Cirocco is grateful that Tompkins, who was a member of her dissertation committee at the time, was able to step up into the role of chair and guide her to completing her dissertation and graduating this fall.

While the dissertation is a tribute to the Caliwood movement in general, Bonilla-Cirocco's work is essential in filling a gap in Colombian national cinema, Tompkins said. Im happy to report that Cindy managed to convey her project to Ospina, one of the directors she wrote about, prior to his death in 2019.

After graduating in December, Bonilla-Cirocco hopes to pursue a job in the Foreign Service with the U.S. Department of State. Shes also considering work in academia.

When I entered (the School of International Letters and Cultures) as a graduate student, a professor told me that these would be the best years of my life. I can say now, without a shadow of doubt, that these years here at (ASU) have been the most memorable so far, Bonilla-Cirocco said. During my time at (the school), I grew as an individual, as an intellectual and as an educator.

Dose-Response with Stereotactic Body Radiotherapy for Prostate Cancer: A Multi-Institutional Analysis of Pr… – UroToday

The use of stereotactic body radiation therapy (SBRT) for the definitive treatment of prostate cancer is increasing rapidly, yet the optimal dose(s) are still uncertain.13 Dose-escalation has been associated with improved biochemical and progression-free survival in conventionally fractionated external beam radiation therapy,4 but this has been minimally explored in SBRT.

The differential effect of SBRT dose on post-treatment prostate-specific antigen (PSA) kinetics has also not been directly evaluated. Various PSA nadir thresholds have been identified that may act as surrogates for early prediction of late outcomes with extended follow-up.5,6 These thresholds serve as critical tools in the interpretation and management of post-treatment PSA increases or fluctuations, as well as in the prognostication for when durable biochemical control appears likely. However, prior studies have also demonstrated that different radiation therapy modalities have differential degrees of prostate ablation and corresponding PSA decay kinetics, possibly related to the degree of dose deposition to the prostate gland.7,8 As more and more men are selecting SBRT for their care, understanding the SBRT dose-response for both PSA kinetics and biochemical control has become increasingly important.

In our study, we compared PSA kinetics and biochemical control among four commonly used prostate SBRT dose-fractionation regimens in a pooled multi-institutional analysis of 1,908 men with low-risk, intermediate-risk, and unfavorable intermediate-risk prostate cancer treated across eight institutions from 2003-2018. In order of increasing biologically equivalent dose, the regimens were: 35 Gy in 5 fractions (35/5), 36.25 Gy in 5 fractions (36.25/5), 40 Gy in 5 fractions (40/5), and 38 Gy in 4 fractions (38/4).

We observed that PSA decay kinetics followed a pattern consistent with dose-escalation, such that increasing biologically effective dose was significantly associated with a steeper slope of PSA decay, a lower PSA nadir (for 38/4 only), and a greater odds of achieving PSA threshold values of PSA 0.2 and 0.5 ng/mL. However, with respect to biochemical recurrence-free survival, 40/5 emerged as the winner compared to all other dose groups. Importantly, this finding emerged late, with no significant difference among the groups in the first five years, with 5-year biochemical recurrence-free survival rates ranging from 93-96.5%.

The finding that dose-escalation beyond the 40/5 dose level was not associated with gains in biochemical control presents the question of why 38/4, with higher biologically equivalent dose, would not provide at least comparable biochemical control. We posit that this may be due to slight imbalances in unfavorable risk patients, which though not statistically significant between groups may impact results beyond their actual clinical significance given that biochemical failures are relatively rare in the low- and intermediate-risk prostate cancer population.

Important questions that were not evaluated in our study still remain, including whether there are differences in acute or late toxicity between these regimens, and whether there are differences in patterns of disease recurrence particularly intra-prostatic versus extra-prostatic failures.

Our study is an important step in the process of solidifying the optimal prostate SBRT dose and the role of dose-escalation for localized, low- and intermediate-risk prostate cancer. However, the gold standard for evaluating this question will be a prospective trial, which we hope can be implemented in the future.

November: Biofriendly protocells | News and features – University of Bristol

An international team of researchers from Bristol and China has prepared biocompatible protocells that generate nitric oxide gas a known reagent for blood vessel dilation - that when placed inside blood vessels expand the biological tissue.

In a new study published today in Nature Chemistry, Professor Stephen Mann and Dr Mei Li from Bristols School of Chemistry, together with Associate Professor Jianbo Liu and colleagues at Hunan University and Central South University in China, prepared synthetic protocells coated in red blood cell fragments for use as nitric oxide generating bio-bots within blood vessels.

Coating the protocells led to increased levels of biocompatibility and longer blood circulation times. Critically, the team trapped an enzyme inside the protocells which, in the presence of glucose, produced hydrogen peroxide. This was then used by haemoglobin in the protocell membrane to degrade the drug molecule hydroxyurea into nitric oxide gas.

When placed inside small pieces of blood vessels, or injected into a carotid artery, the protocells produced sufficient amounts of nitric oxide to initiate the biochemical pathways responsible for blood vessel vasodilation.

Although at a very early stage of development, the new approach could have significant benefits in biomedicine, cellular diagnostics and bioengineering.

Professor Stephen Mann, Co-Director of the Max Planck Bristol Centre for Minimal Biology at Bristol, said: This work could open up a new horizon in protocell research because it highlights the opportunities for creating therapeutic, cell-like objects that can directly interface with living biological tissues.

Associate Professor Jianbo Liu at Hunan University added: We are all really excited about our proof-of-concept studies but there is a lot of work still to be done before protocells can be used effectively as bio-bots in therapeutic applications. But the potential looks enormous.

CONVERSATIONS with Trinis suggest that last rites of covid19 deceased are not treated with respect and dignity. Families dont get to properly bid farewell because of rules. Public safety should never be compromised.

But common sensical applications should also not be ignored when treating the deceased who died of covid19. The deceased does not pose as much threat to public safety as the living. The probability of covid19 infection from the dead is minimal millions of times lower than getting covid19 from the living.

Families and communities can have proper and dignified burial or cremation rites for the covid19 deceased. The risks of attending a funeral for a covid19 victim is not any higher than that of attending a rite for someone who died from other causes or attending a religious institution for regular worshipping.

TT has countless biologists and biochemists who would have studied virology or virus contamination. I have not seen anyone address this issue of covid19 infection from a deceased.

I may be known as a pollster and social commentator, but I am also a degree holder in biochemistry and taught said subject as well as worked in biology labs for several years before making the transition to social science almost four decades ago.

My recollection from biology studies is a virus dies with its host, and thus the chance of the covid19 virus moving from the dead to the living, while possible, is very remote.

Tens of millions of people are hosts to covid19. The virus cannot reproduce without a living host. In theory, once the host dies, so does the virus. The covid19 virus survives for only a few hours (at most) without a host.

Of the millions of funerals of victims of covid19, only one person is known to have contracted the virus from the dead. Contrast this figure with the hundreds of millions who got infected by coming in contact with the living.

As long as proper safety measures are in place (safe distancing and wearing of PPE), normal funeral rites are safe (safer than going to a market or riding a maxi). In fact, in the US and other developed countries, almost normal funeral rites are being allowed.

When the virus broke out in January 2020 in the West, it was not clear how the virus spread. It was a mystery; scientists took a long time to understand its behaviour. In fact, it is still a mystery how it is mutating and combating the several vaccines.

Thus, the deceased was disposed of in closed bags, closed caskets (families could not see the face of loved ones), and closed rites from a far distance. But as scientists got an understanding of the viruss behaviour, coffins have been open over the last year.

Also, the dead is now ritually washed in America and properly dressed and displayed in full view. I attended funerals of several victims in New York; many Trinis and Guyanese who I knew died of covid19 in the US. I presided over the Hindu funeral rites of my mother who was a covid19 victim, including repeatedly touching her body as required by Hindu scriptures. Knowing the science, I had no reservations about touching her body.

I know of others who performed similar rituals for family members who died of covid19. None of them (or myself) contracted the virus from the deceased (and I did not even wear PPE when touching my moms body). Several individuals contracted covid19 from living family members who were covid19 positive (a few of who later died from the virus).

It is natural for people to be fearful of contracting the virus from the deceased covid19 victim. I too was very fearful and avoided funerals of covid19 deceased in the first several months of the pandemic. But it is now reassuring that the virus will not jump from dead to living.

No funeral worker (at a funeral home, crematorium, or graveyard) has been infected with the virus from handling the deceased. A lone case was that of an exposed pathologist (who did not wear PPP) who performed an autopsy of a covid19 victim some 15 months ago.

Once you wear PPE and maintain physical distancing, you will be safe. People should be more concerned about contracting covid19 from the living rather than the dead.

Thus, the Government should consider allowing normal funerals open coffins and cremations and attendance in public places. Families should be allowed to perform burials and cremations according to prescribed cultural and religious practices as the dead do not pose covid19 risk.

Attendees should take the necessary precautions to limit covid19 exposure from the living.

The U of A's Robert Coridan will give a virtual seminar titled "Artificial photosynthesis: chemical approaches for converting sunlight to fuels"on Zoomfrom 8:15-9:15a.m. Wednesday, Nov. 11. The talk is free and open to the public.

Professor Robert Coridan received his B.S. in physics/CIS at Ohio State University in 2002 and received his Ph.D. in physics at the University of Illinois at Urbana-Champaign in 2009. He was a postdoctoral researcher at the California Institute of Technology from 2010 to 2015.While he was a postdoc, Coridan served as a physics lecturer for the Caltech Juice from Juice Program, where graduate students, postdocs, and professors taught high school students and teachers about solar energy. He also mentored undergraduate students in the Caltech Summer Undergraduate Research Fellowship program.

He joined the Department of Chemistry and Biochemistry as an assistant professor in 2015. His research group, the Hierarchically Structured Materials Group, has several research interests: hierarchically structured electrochemical materials, advantageously disordered materials, and methods for real-space characterization of electrochemical processes.

In 2019, Coridan received an Early Career Research Award for his work on improving the efficiency of chemical reactions that convert solar energy into chemical fuels. This research is focused on the optical and photochemical properties of defects in randomly distributed films of nanoscophere colloids nanoscopic glass and plastic beads. The goal of this research is to devise easy-to-fabricate and scalable nanostructures that maximize a material's ability to convert the sun's energy into chemical bonds, or fuels, a process similar to photosynthesis in plants.

While many SRU students this year are attending classes from their couch or bed, some are in the classrooms, navigating complex assignments under not-so-normal conditions.

When the university announced their fall reopening plan in July, administrators looked to ensure classes that provide hands-on learning which could not be replicated online would continue in some fashion on campus. Although they had an idea of what those classes would look like, it was up to the faculty to put the plan into action.

Ashley Loe was one of many faculty members who had to adapt the laboratory portion of her Biochemistry I class into a safe and socially distant course that retained as much of the benefit of hands on experimentation as possible.

Her course is one of 43 being taught multimodal this semester by the College of Health, Engineering and Science (CHES), according to Michael Zieg, interim dean of CHES.

While students on campus are required to wear a face mask at all times while indoors, the mask has not been difficult to work with in a course that requires students to wear goggles and other personal protective equipment while working.

One of the biggest changes brought to her class however, has been the elimination of lab partners. In order to comply with occupancy limits social distance guidelines in the lab, Loe had to keep students spread out and split the class into two groups so they could rotate through every other week.

While the smaller numbers allow the students to spread out, Loe added there is also the benefit that students are not forced to share equipment and more personalized attention.

Still, some students opted to take the course completely online.

Those students, along with those who are not in the lab that week, still participate in a lab exercise through the online learning tool Labster.

Its basically a video game, said Taylor Maxwell, a junior environmental chemistry major.

While Maxwell said the Labster assignments are no substitute for the hands-on experience she receives in Loes labs, the scenarios provide a practical base to build off.

While the students in Loes class moved back to the area for classes this semester, many did so because their jobs were here. With no students in the lounges of the Advanced Technology and Science Hall or out in the quad between classes, being in-person feels a little weird for some.

It doesnt feel like the semester started, Maxwell said.

Ashlyn Hildebrand, a junior chemistry major with a concentration in biochemistry, said her and her classmates have always been close and worked together in and out of the classroom. Despite some face-to-face time in the lab, for Hildebrand it now it feels as though everyone is on their own.

Loe said normally she has around five students in the lab but as the semester has carried on, some students have had to stay home temporarily due to possible exposure to the coronavirus, missing out of valuable hands on time.

Having spent three years teaching at SRU, Loe said the changes this semester has been a challenge but has been nothing compared to the chaotic changes that occurred during the shutdown last spring. Those students, who were in Biochemistry II at the time, were forced to scrap ongoing assignments and utilize mock data to complete their work on plasmids.

Knowing that the upcoming spring semester will look more or less like the current, Loe said she has enough time to come up with creative workarounds to the upcoming challenges. One of the challenges she is tackling now is for students to be able to work with living organisms while not being around to take care of it all the time.

Yet, despite the challenges, Loe and her students feel a little more confident about what the following semester will bring and their ability to adapt.

It probably will go better, said Rachel Hemmerlin, also a junior chemistry major with a concentration in biochemistry.

Hemmerlin said students and professors know what to expect and have been working out the kinks throughout the semester to find the right balance.

No matter what the upcoming semester brings for Loes class, they agreed that right now they are just glad to be able to meet in-person, even if it is only for a couple of hours once every two weeks.

This follows the departure of John York, who was recently named chief science officer of food technology startup Impossible Foods and will be moving to California.

The investments that Dr. York directed into the infrastructure, training and people in the Department of Biochemistry have been transformative. Simply put, I want to build on this momentum, Cortez said. I look forward to working with the faculty, staff and trainees to continue our shared pursuit of excellence in science discovery and education. The people in this department and at Vanderbilt make it special, and I will do everything I can to support them as they pursue their goals.

Cortez, also associate director for basic sciences research at theVanderbilt-Ingram Cancer Center, specializes in understanding the function of DNA damage response pathways in maintaining a healthy genome, and in understanding how those pathways are activated. His labs current projects include identifying new proteins involved in DNA damage response and developing cancer therapeutics that target that response. ABSTRACT

PURPOSE: There is increasing use of radical prostatectomy to treat patients with high-risk prostate cancer. This has contributed toward a pathologic stage migration, and a greater number of patients are subsequently being diagnosed with biochemical failure. There is increasing use of advanced imaging techniques in the setting of biochemical failure, including positron emission tomography-computed tomography (PET-CT).

METHODS AND MATERIALS: This critical literature review highlights the evidence for PET-CT in postprostatectomy biochemical failure and identifies sites of pelvic lymph node relapse in the setting of biochemical failure and the potential implications that the locations of these relapses may have for salvage therapies. Potential future directions are then considered.

RESULTS: The optimal PET-CT tracer remains uncertain but there is increasing use of prostate-specific membrane antigen PET-CT for investigating sites of nodal metastasis at low prostate-specific antigen levels, and this is leading to a blurring of the biochemical and radiologic recurrence phases. The optimal therapeutic approach remains undefined, with current trials investigating postoperative radiation therapy to the whole pelvis in addition to the prostatic fossa, the use of PET-CT in the setting of biochemical recurrence to guide delivery of salvage radiation therapy, and, for patients with node-only relapsed prostate cancer, the addition of whole pelvis radiation therapy to metastasis-directed therapies such as stereotactic ablative radiotherapy.

CONCLUSIONS: The most appropriate target volume for salvage radiation therapy remains uncertain, and the findings of studies using PET-CT to map nodal recurrences suggest that there could be a role for extending whole pelvis radiation therapy volumes to increase coverage of superior nodal regions. The emerging fields of radiomics and radiogenomics could provide important prognostic information and aid decision making for patients with relapsed prostate cancer.

Harman, a student at LSU Health New Orleans School of Graduate Studies. The award recognizes ARVO members who have made efforts to integrate advocacy as part of their professional efforts early in their careers. Harman, who is pursuing a doctorate of philosophy degree in biochemistry and molecular biology, has been active in the Student Government Association at LSU Health New Orleans. He served as vice president and interim president. His advocacy work has included lobbying in Washington, D.C., for funding for the National Eye Institute and working with the Louisiana Lions Eye Foundation to perform vision screenings at inner-city primary schools.
