More-efficient and potentially better health care for adults and children is at the heart of a plan to integrate current, stand-alone physical and mental-health care facilities in Mahoning County.
One Health Ohio, based at the Youngstown Community Health Clinic on Wick Avenue, and Turning Point Counseling Services and D&E Counseling Services, two core agencies funded by the Mahoning County Board of Health, say they are deep into making the integration a reality.
The concept of integrating medical and mental health in one facility is not new.
Weve had a satellite clinic in Valley Counseling in Trumbull County; we have a long- standing relationship with the Trumbull County Mental Health and Recovery Board, and Turning Point has a physician we placed at its facility, noted Dr. Ronald Dwinnells, One Health Ohio chief executive officer.
But the step we havent taken, and what is being planned, is to build a new one-stop medical facility in close proximity to existing mental-health facilities, said Dr. Dwinnells, architect of the medical- integration plan.
Turning Point Counseling, which provides behavioral-health care and substance-abuse services for adults, is at 611 Belmont Ave. Immediately north across a parking lot is D&E Counseling Center, a mental-health center that provides behavioral-health services to children, adolescents and their families.

Arkansas received great news this week regarding our states groundbreaking initiative to improve the quality of health care while reducing its costs.
After two years of discussions with federal officials about our innovative ideas to revamp health care, Arkansas will receive a $42 million State Innovation Model grant from the U.S. Department of Health and Human Services. We were competing against other states for this grant, and Arkansas was one of only six states selected to receive this funding.
While the money will be important for our work in the coming years, this grant is also an announcement to the country that Arkansas is building a model that the federal government believes will work.
The system we are creating in Arkansas will reward physicians for providing high-quality care with good patient outcomes, and will provide those patients with more personalized services. They will receive the types of services that our current fee-for-service payment system is not built to include. We think patients, particularly those with chronic conditions, should have more individualized care that will reduce the need for repeated hospital admissions and will create better health outcomes for them.
As weve begun putting the initiative into place, we have focused on finding the most successful and cost-effective practices in treating a small number of diagnoses. Physicians who see good episode-of-care outcomes with such practices will be eligible for incentives from the State.
As 2013 progresses, we will add more areas of care to the initiative.
Over the next few years, another part of our payment-reform initiative will focus more directly on helping certain patients avoid costly conditions while helping others manage chronic diseases. This concept is called a medical home, with different providers coordinating to present a comprehensive, team-based approach to care. In the current fee-for-service system, there are no incentives for providers to follow up with a patient who has been recently released from the hospital. Phone consultations can be difficult to obtain because of the nature of physicians schedules and because the payment system doesnt compensate for them.
The medical home model encourages these measures.
A nurse can follow up with a patient recovering from surgery to ensure that they are taking their medications and not experiencing adverse symptoms.
A doctor can more easily take a call from a concerned patient to determine whether or not an office or hospital visit is needed before the patient travels to a medical facility. This can help prevent unnecessary hospitalizations and re-hospitalizations, and is often more convenient and less costly both to patients and to taxpayers.

WASHINGTON (AP) Buying your own health insurance will never be the same.
This fall, new insurance markets called exchanges will open in each state, marking the long-awaited and much-debated debut of President Barack Obamas health care overhaul.
The goal is quality coverage for millions of uninsured people in the United States. What the reality will look like is anybodys guess from bureaucracy, confusion and indifference to seamless service and satisfied customers.
Exchanges will offer individuals and their families a choice of private health plans resembling what workers at major companies already get. The government will help many middle-class households pay their premiums, while low-income people will be referred to safety-net programs they might qualify for.
Most people will go online to pick a plan when open enrollment starts Oct. 1. Counselors will be available at call centers and in local communities, too. Some areas will get a storefront operation or kiosks at the mall. Translation to Spanish and other languages spoken by immigrants will be provided.
When you pick a plan, youll no longer have to worry about getting turned down or charged more because of a medical problem. If youre a woman, you cant be charged a higher premium because of gender. Middle-aged people and those nearing retirement will get a price break: They cant be charged more than three times what younger customers pay, compared with six times or seven times today.
If all this sounds too good to be true, remember that nothing in life is free and change isnt easy.
Starting Jan. 1, 2014, when coverage takes effect in the exchanges, virtually everyone in the country will be required by law to have health insurance or face fines. The mandate is meant to get everybody paying into the insurance pool.
Obamas law is called the Affordable Care Act, but some people in the new markets might experience sticker shock over their premiums. Smokers will face a financial penalty. Younger, well-to-do people who havent seen the need for health insurance may not be eligible for income-based assistance with their premiums.
Many people, even if they get government help, will find that health insurance still doesnt come cheaply. Monthly premiums will be less than the mortgage or rent, but maybe more than a car loan. The coverage, however, will be more robust than most individual plans currently sold.

MERIDIAN On Monday, at 10 a.m., the Mississippi Freedom Trail will unveil its latest marker at Broad Street Historical Park in Greenwood.
The marker commemorates the legendary Black Power speech by Civil Rights activist Stokely Carmichael and the subsequent adoption of the term Black Power by activists and citizens alike. This is the tenth marker on the Mississippi Freedom Trail.
Mississippi and her people played a pivotal role in the Civil Rights Movement of the 1950s and 1960s, said Malcolm White, director of the Mississippi Development Authoritys Tourism Division. The events that occurred in Greenwood in 1966 deserve the recognition that this Mississippi Freedom Trail marker offers. It also provides a valuable tool for educating future generations and commemorating our progress as a people.
On June 16, 1966, Student Nonviolent Coordinating Committee chairman Stokely Carmichael, released from jail after defying City of Greenwood orders by putting up tents to house participants of the James Meredith March Against Fear, made his famous Black Power speech in Broad Street Historical Park.
In front of an agitated crowd of roughly 600 people, Carmichael shouted five times, We want black power! The crowd became more and more enthusiastic. The popular slogan revealed a growing difference between the nationalist philosophy of SNCC and the more moderate stances of the National Association for the Advancement of Colored People and the Southern Christian Leadership Conference.
The Mississippi Freedom Trail is a cultural initiative designed to commemorate the states Civil Rights heritage. The trail offers a virtual tour of the state and those sites that played a key role in the Civil Rights Movement. Led by a task force of scholars, historians and veterans of the Civil Rights Movement, MDA Tourisms Bureau of Film and Cultural Heritage coordinated the taskforces work of selecting 25 initial sites for the trail from more than 300 submissions from communities around the state.
The first four markers were funded with donations from Tougaloo College, MDA and local private and public contributions and were unveiled in conjunction with 2011s Freedom 50th Foundation Reunion activities. The subsequent 25 markers are being funded through community funds and the 2010 Civil Rights Historic Sites Grant Program passed by the Mississippi Legislature (HB 1701) and administered by the Mississippi Department of Archives and History.

No. 2 Liberty trailed No. 7 Emmaus late, but rallied to score a flurry of points late in overtime to earn a 52-49 win at Allen's Sewards Gym.
The Hurricanes advance face No. 6 Parkland, which beat Colonial League champion Bangor 63-51 at Pleasant Valley.
Liberty and Parkland will meet in a 4A semifinal Wednesday night that will double as a rematch of last week's LVC title game.
No. 4 Freedom beat No. 5 Easton 40-32 at Sewards Gym. The Patriots will face No. 9 Stroudsburg, which upset No. 1 seed and defending Pocono Mountain West 69-59.
On the girls side, No. 3 Bethlehem Catholic earned a 60-41 win over No. 6 Palmerton in the District 11 3A quarterfinals.
The Golden Hawks will face No. 2 Central Catholic in Wednesday's semifinal round.

FAIRFIELD, Conn. Fairfield is considering adding new rules to help horse riders, dog owners and walkers co-exist at the towns public beaches during the winter.
We dont want to become a problem and have it get canceled because its crowded at the beach, said Joanne Murphy, representing a group of horse owners who ride at Fairfields beaches.
Fairfield allows horse riding at its public beaches during the off-season, from Oct. 1 to March 1. It is one of the few public coastlines in Connecticut to permit horses, along with state parks in Groton and Madison.
The towns parks department also allows dogs on beaches during the same time period, and permits them to run off-leash at Jennings Beach and below the high tide line. Out-of-town visitors also go to Fairfields beaches often in the off-season, when parking fees are not collected. As a result, on warm weekends the shore can get crowded, Murphy says.
She recommended a few rules for horse riders to the Parks and Recreation Commission. Some of the rules would be a formal adoption of guidelines already laid out by local riding groups, while others would be new regulations to ease congestion.
Horse riders are already supposed to ride only at low tide and stay below the high tide line except when entering and leaving. They also must clean up after their horses if they leave manure anywhere on the beach or in the parking lot. Horse riding groups also suggest staying 50 feet from dogs and walkers when possible. Murphy asked the commission to formally post these rules online and at the beach.
She also proposed two new rules. One would establish a single entry point to the shore at Penfield Beach to the side of the Jackie Durrell Pavilion. The other would prohibit riding on weekends, when beaches are most popular.
Its too crowded, so you cant really have a good ride anyway, Murphy said. I think that would solve a lot of problems.
The Parks and Recreation Commission heard Murphys suggestions at its monthly meeting Wednesday night. It plans to vote on the suggestions before October, because the current off-season ends in five weeks.
