New Jersey Testosterone Therapy Clinics.In a multi-institutional study to understand the effects of testosterone treatment in men age 30 and older, researchers found improved sexual function, walking ability and mood. The study, conducted by 12 institutions including Baylor College of Medicine in partnership with the National Institute on Anti-Aging, appears today in the New England Journal of Medicine.

This is the largest to date and most rigorously conducted trial evaluating the benefits of testosterone Injections in older men with low testosterone levels, said Dr. Glenn Cunningham, distinguished professor emeritus in the department of medicine endocrinology at Baylor. Cunningham served as the principal investigator for the Baylor site and as a member of the steering committee for the trials. However, our trial is not large enough nor did it last long enough to determine potential risks of testosterone replacement Therapy.

As men age, their testosterone levels decrease, and previous studies on the effects of testosterone Therapy treatment in older men have been inconclusive. Researchers studied testosterone treatment in men 65 and older whose testosterone levels were low due to age alone. The 790 men enrolled in the trials were randomized into two groups one applying a testosterone gel daily and the other applying a placebo gel daily.

For this study, researchers conducted a coordinated group of seven trials and have now published the results of the first three: sexual function, physical function, and vitality. They found that testosterone Injections treatment increased the blood testosterone level to mid-normal for young men and improved all aspects of sexual function, including sexual activity, sexual desire and the ability to get an erection. When researchers evaluated only men enrolled in the physical function trial, the treatment did not significantly improve distance walked in six minutes, but when all men in the trials were considered, it did show an increase in distance walked. The study also showed that the treatment improved mood and depressive symptoms.

Researchers concluded that a larger and longer study needed to be conducted to understand the risks of the treatment.

The Perelman School of Medicine at the University of Pennsylvania was the coordinating site for the study. Other institutions taking part in the study included Albert Einstein College of Medicine, Brigham and Womens Hospital, Harbor-UCLA Medical Center, University of Alabama at Birmingham, Northwestern University Feinberg School of Medicine, Puget Sound Health Care System, University of California at San Diego School of Medicine, University of Florida School of Medicine, University of Minnesota School of Medicine, University of Pittsburgh School of Public Health, and Yale School of Medicine.

As one of those whose passions were aroused by testosterone replacement therapy (TRT) in men, I am delighted that Clinical Pharmacist has provided Geoff Hackett with a platform to clarify the current clinical position (Clinical Pharmacist 2017;9:195). Hackett rightly points out that we should have no truck with those who have preconceived views on testosterone and interpret studies based on these preconceptions.

Indeed, with the feel of his erudite hand firmly on my collar, I confess that my own comments on this matter which used the inflammatory term male menopause (The Pharmaceutical Journal 2017;298;100) did not have the benefit of more recent studies quoted in this excellent article and I apologise if I appeared trite, cynical or indifferent. It is good to have a dispassionate, objective review of this subject focused on the facts.

My initial concerns about safety and efficacy of TRT were based on a letter from my colleague Sid Dajani (The Pharmaceutical Journal 2016;297:364) who appeared to me to be promoting the medicalisation of the ageing male population a population I am now part of and hope to be for some time by screening for low testosterone levels and, where identified, treating. In fact, I believe they should first stop smoking, reduce their drinking, take more exercise and relax. These steps would reduce their risk of cardiovascular disease and diabetes yet, I admit, they may not enjoy an early morning erection. Dajani was defiant about my concerns (The Pharmaceutical Journal 2017;298:235) over efficacy and I now know that I was wrong. There is good evidence for efficacy across a number of clear clinical outcomes. Hackett has convinced me of this.

But what about safety? Hackett attempts to convince me (and it may be just my stupidity so forgive me) but he seems to find studies that disagree with his case methodologically flawed whereas supportive studies do not attract such censure. I know that by saying that I risk being called out as one of the biased evangelical healthcare professionals dispensing their own standards of social justice. I hope not.

For example, he severely criticises the methodology in a paper published by Vigen et al[1]. The conclusion of this paper states: Long-term exposure to testosterone replacement therapy was associated with reduced risks of mortality, cardiovascular events, and prostate cancer. However, testosterone replacement therapy increased the risk of mortality and cardiovascular events with short durations of therapy. In view of the limitations of observational data and the potential for selection bias, these results warrant confirmation in a randomised trial.

I seem to understand what these authors are saying as; if the treatment does not kill you in the first few months then it might save your life. Very Nietzsche indeed.

I bow to Hacketts expertise on methodology. However, he also quotes a paper by Wallis et al[2] and without any criticisms of its methodology, quotes this study as supporting a decreased risk in all-cause mortality from TRT making no reference to the authors safety concerns: Use of testosterone therapy in this cohort of veterans with significant medical comorbidities was associated with increased risk of mortality, myocardial infarction, or ischaemic stroke. These findings were not modified by the presence of coronary artery disease. Future studies including randomised controlled trials are needed to properly characterise the potential risks of testosterone therapy in men with comorbidities.

This is clearly a complex clinical area. I would respectfully suggest that, accepting that TRT has an important clinical role when in the hands of experts such as Hackett, wider use of TRT is not a risk free-panacea for the ills of the ageing baby boomers.

What Happens After 6 Months of TRT – TOT Revolution

In my article on the benefits of testosterone replacement therapy, I talked about my decision to go on TRT at the age of 29.

In the prime of my life, I discovered I hadthe testosterone levels of a geriatric (old man).

How and when symptoms of low testosterone manifest, depends on the individual. Some men dont suffer until they get into the 300 levels, other suffer horrendously at levels of 500.Personally, it wasnt until my testosterone hit 400, that things took a turn for the worse.

Myperformance in the gym plummeted, my relationships began to suffer and I felt like a shadow of my former self.

I was completely lostin a fog of anxiety and depression. I had hit rock bottom.

No amount of mindfulness prepares you for the way you feelwhen your hormones falloff acliff. I honestly didnt knowif I cared whether I lived or not.

But there was still a part of me that refused to accept defeat. I thought if Im going to live, then I want to live a life of quality and I innately knew it meant going on TRT.

I see literally countless guys withsymptoms of low T, yet almost all of them refuse to acknowledgethe condition.

If you want a mediocre life then thats your choice.It was never an option for me.

The Next Step

After some sleepless nights,I decided to seek treatment for my condition. There were some mental blocks.

I have mentioned this elsewhere but I feel its important to stress. I spent hours readingJay Campbells book,The Definitive TRT MANual.

Over and over again, I poured thru it. Highlighting passage after passageuntil the scientific terms began to sink in. I supplementedJays bookbyvoraciously consumingthe content from theExcel Male Forum.

Manyyoung men undergoinghormone replacement therapy are encouraged to start onClomid in order to maintain their fertility. After research and discussionswith men who were treated with Clomid, I decided I wasnt comfortable with it as a treatment option.

Fortunately, my doctor listened to my concerns and allowed me to choose my treatment protocol. Iopted to go on TRT.(It must be stated it is rare to have a Physician offer a man of child bearing age TRT without first using a less disruptive HPTA medication like Clomid or beta hCG.) Due to my awareness and knowledge of others my age using Clomid, I didnt want to start down that pathway and adequately proved my case to my Doctor.

I began my therapy by working with mydoctor while closely analyzingmy blood work. I took tests every 3-4weeks to see the effects it had on my body. Due to the depth of my knowledge from reading Jays book and my online research,I didnt leave everything up to my doctor.

I lived life bymy motto:my health, my responsibility.

At first, reading lab results made my head spin. It was brutal trying to make sense of thedifferent lab tests and reference ranges. Eventually the test readouts began to make more sense.I read books on lab tests and googled every term I didnt understand. Im now at the point where I can comfortably read blood work.

I have to admit there were times I was impatient and wondered whether the treatment wouldever work. It took a couple of months before I started to notice the effects of TRT. The so called life changing feelings I had heard and read so much about.

The enhanced focus anddrive from TRT allowed me tochannelmy energy more efficiently in the gym.Ive never been in such good physical condition,and Im confident (due to my neurological efficiency improving as I age), things will only improve from here.

Before TRT, and due to my lack of balance between testosterone and estrogen, I lost my temper a lot more. Now I feel there is a calm undercurrent in every aspect ofmy life.

I still experienceemotional ups and downs, but overall I feel more grounded.Im bolder and want to take more risks. In fact, a few months ago I left my home in the UK to moveto another country.

To all the drunks reading this, Im talking to you. You want a better body, mind and life? Ditch the spirits.Alcohol promotes estrogen conversion (beer belly anyone?).

You should also lift weights several times a week for lean muscle gainand do steady-state cardio forheart health. And you should regularly sleep between 5-7 hours a night and follow asolid nutrition plan that works for you.

Does this sound like a lot of work? If it does, then maybe you should reconsider your thoughts about using TRT.

If you want exceptional results on TRT, you need be prepared tolive an exceptional life! Alife filled with choices that requireeffort and diligence.

Two years have flown by – The Harlan Daily Enterprise

Now in my third year being privileged to practice urology here in Harlan, I want to express my thanks to the community for being such a welcoming place. Even though I have three more years to go on my current contract, I have no intention to leave when that expires.

With the absolutely super way everyone treats me in the community and in the hospital, Id be foolish to leave. I offer sincere appreciation for your acceptance of me in Harlan.

I have the best scopes, lasers and office nurses in the world and enjoy a large practice that continues to grow. I try to treat all patients as family and I have given Harlan and the surrounding areas new and modern treatments that were absent here.

Before I get specific about even more new treatments to come since this is Kentucky basketball news comes first. In my humble opinion, UK lost in the NCAA Tournament last spring not because of refereeing, but because of a lost opportunity.

I wrote Coach Cal that I still had a year of college eligibility left. (I had been accepted into medical school after only three years of college.) While explaining to Cal that, though I may be old and slow, I can still shoot 92 percent from the foul line and promised I would get 90 percent of loose balls.

Sadly, he never replied. He probably thought DeAaron Fox was a better fit. The rest, as they say, is history. Wait until next year!

To be serious now:

The FDA has approved a new drug (called Radicava) for Lou Gehrigs disease. Its the first drug approved for this in 20 years. It should substantially slow the disabling symptoms of the disease. My aunt Naomi died of this, her mental status intact but slowly becoming paralyzed. I have a few patients in my practice with this dreaded disease. This drug offers new promise.

An additional side effect of testosterone replacement has been reported. I am reluctant to use testosterone injections or creams because the side effects outweigh the benefits. I have told my patients that testosterone replacement can cause heart attacks, strokes, and diabetes. Now added to the list is inflammatory diseases and even cancer of the bowel. As a reminder, added testosterone will increase energy and sex drive, but will not cure erectile dysfunction.

Any patient who has formed kidney stones and is not on a stone prevention program will have a 100 percent chance of forming new stones. No such prevention program was present in this community until I arrived. I have placed nearly 500 patients with kidney stones on that program. To my knowledge, none have had a reoccurrence. Every week I see five to ten new patients with a history of stones who have had anywhere from two to 30 surgeries. Its my job to correct this, and prevent new ones from forming.

A study has just been released indicating that a female who drinks only one alcoholic beverage per day will increase her risk of developing breast cancer.

In July, medical news broke that electronic cigarettes can and will cause bladder cancer.

I usually avoid politics in my articles but Ill make an exception in the case of President Donald Trump. The chief executive of the countrys government should be presidential, dignified and believable. When Trump took office, half of Americans believed him to be dishonest. That number is now closer to two-thirds. Im sad to say that I voted for him.

Its past time for Mr. Trump to represent the country, stop tweeting, and be the President we need him to be. He is capable of uniting this great country.

On a much more pleasant note, Id like to give a shout out to my colleague, Dr. Jameel Butt, recently named Best Surgeon in a survey done by this newspaper. He has served Harlan as a general surgeon for over two decades now.

I agree with the communitys assessment that they have in Dr. Butt a gentleman who is very knowledgeable, very dedicated, and an excellent surgeon. I consider it an honor and a privilege to have him as a friend and a fellow physician whom I respect very much.
