Campaign launched to raise awareness of lung cancer symptoms – Mancunian Matters

A new campaign to raise awareness around the symptoms of lung cancer has launched in Greater Manchester.

The Do It For Yourself campaign, launched on Monday by MSD UK in collaboration with leading cancer charities, urges anyone who has had a cough for more than three weeks to contact their GP.

It follows a survey of adults in the region that found 45% put off seeing their GP since the start of the pandemic.

GP Dr Hilary Jones said: While these symptoms in recent times can be due to COVID-19, this is not always the cause and increasing understanding around this is critical.

With lung cancer specifically, early diagnosis is incredibly important so its essential that people are both aware of and know how to act on key symptoms.

Lung cancer is the third most common cancer in the UK, accounting for 13% of all cancer cases.

Around 47,000 people are diagnosed with the condition every year in the UK.

There are usually no signs or symptoms in the early stages of lung cancer, but as the condition develops the main symptoms include: a persistent cough; coughing up blood; persistent breathlessness; unexplained tiredness and weight loss; an ache or pain when breathing or coughing.

Clinicians in some areas have reported a steep decline in lung cancer referrals, increasing the risk of late-stage cancer diagnosis.

Lorraine Creech, Head of Nursing Mesothelioma UK, said: People may not want to be a burden on the NHS or think that their symptoms will go away, which means they arent putting themselves first.

Its so important, now more than ever, that we come together to change this perception and encourage people to take action and look after their health.

Hospitals and GP surgeries have introduced a range of measures to see patients safely, such as initial phone consultations and frequent hand sanitising.

In April 2019 almost 200,000 people in England were referred to a consultant by their GPs for suspected cancer; in April 2020 that figure fell to 79,573.

Research published in the British Medical Journal last month showed that delaying cancer treatment by just four weeks increases the risk of death by up to 10%.

Paula Chadwick, Chief Executive of the Roy Castle Lung Cancer Foundation said: Its important we remember that not every cough is a Covid cough and that local communities and GPs work together to encourage patients to contact them to discuss any health concerns or we risk undoing much of the progress that has been achieved over the past decade.

CEO ‘died of mesothelioma – but it’s not clear if he ever came into contact with asbestos’ – Buckinhamshire Free Press

A retired CEO died after a short battle with mesothelioma but his death remains a mystery as it is not clear if he ever came into contact with asbestos, an inquest has heard.

Peter Day, from Amersham, was the Chief Executive Officer of an IT company before he retired.

After coming back from a holiday in Florida with chest pains earlier this year, the 76-year-old was eventually diagnosed with metastatic biphasic pleural mesothelioma primarily caused by exposure to asbestos.

Surgery was not an option after his diagnosis, and he was instead given palliative treatment by Rennie Grove. He tragically passed away with his wife and daughter at his side at home in Clifton Road on July 19.

An inquest into the death of Mr Day, held at Buckinghamshire Coroners Court in Beaconsfield on September 2, heard that as a former chief executive of a software company, it was not clear how he may have been exposed to asbestos.

Senior coroner for Bucks Crispin Butler initially recorded a narrative conclusion, which simply sets out the facts of a death, in a hearing on Wednesday.

But after later hearing a statement from Mr Day's family that had not been taken into account at the time of the inquest, Mr Butler has now concluded that he died as a result of industrial disease.

According to Companies House, Mr Day was also involved with the Wildfowl & Wetlands Trust (WWT), a leading conservation charity.

In a tribute posted on the WWT website, colleagues said Mr Day was one of its most passionate and devoted supporters, as a volunteer for more than 20 years at the London Wetland Centre, as a trustee and as chair of the council.

He had reportedly obtained his PhD in plant molecular, phylogenetic and population genetics at Queen Mary, University of London and the Royal Botanic Gardens, Kew, just two years ago.

The tribute added: Peter made huge efforts to really understand WWT. He travelled extensively to sites and wider conservation projects not just for organised events but in an informal capacity, which was appreciated by many WWT staff, volunteers and our supporters.

He held a strong vision for a better environment, could always be relied on as a source of sound and reliable advice and held a natural ability to convey encouragement to everyone he met.

He was also very supportive to the management team during the recent pandemic.

He will be hugely missed by all staff and volunteers. We send our deepest sympathy to his family.

Asbestos cases often turn on a paper trail as the most reliable evidence at trial, where memories of potential exposure and their sources from decades earlier can be hazy. Todd Suddleson used such a paper trail as the key to a strong closing argument that helped clear CertainTeed in a $5M-plus asbestos trial in 2018.Francisco Herrera v. CertainTeed Corp., CV2014-09632.

Francisco Herrera contended he developed mesothelioma after cutting asbestos-containing cement water pipes made by CertainTeed Corp. while he worked for SunAir Co. in the 1970s.

At trial, the plaintiff sought to paint CertainTeed as a major supplier of asbestos-containing pipe to SunAir while Herrera worked for the company. However, the defense countered Herrera failed to show he was actually exposed to CertainTeed pipe.

During his closing, Suddleson, of DeHay & Elliston LLP, walked jurors through a search of 42,000 invoices he said helped show CertainTeed pipe wasnt connected to Herreras mesothelioma.

Of those 42,000 CertainTeed invoices during the time Herrera worked at SunAir, Suddleson said only seven invoices, for two jobs, were for SunAir. Between the two jobs, there were only 33 fully-machined pieces (of pipe ordered), Suddleson told jurors, holding up a piece of pipe about a foot long. These are the ones that would be cut, so you can get a decent idea on the maximum number of cuts based on the number of [fully-machined pieces] that were ordered.

Suddleson argued there was insufficient evidence to show Herrera even worked on the two jobs for which CertainTeed supplied that pipe, and he pushed back on the plaintiffs criticism of the invoice review. They had the audacity to challenge our review of the invoices. They were like They actually had attorneys do it. Oh my! How can that possibly be credible?

Well, thats the only way to do it, and weve done it, Suddleson added.

We invited the plaintiffs to come look for themselves, Suddleson said, painting CertainTeed as the party with nothing to hide. Did they? No. Because its much easier to just question whether we found all the invoices than it is to actually do the work themselves.

Suddlesons closing argument, and his detailed accounting of the paper trial, helped deliver a defense verdict in the case.

Two recently approved therapies for mesothelioma may soon team up in a study.

Novocure, the manufacturer of the tumor treating fields device NovoTTF-100L, announced a partnership with Merck, the manufacturer of the popular immunotherapy drug Keytruda. The two leaders in oncology will collaborate on a non-small cell lung cancer clinical trial.

If that goes well, the next step might be a mesothelioma study.

Keytruda, the brand name for pembrolizumab, received approval early in 2020 from the United States Food and Drug Administration (FDA) for specific pleural mesothelioma cases. The treatment is available for patients who express high levels of PD-L1, the cancerous protein that Keytruda can neutralize.

The NovoTTF-100L System device, which was rebranded as Optune Lua, passed the FDAs rigorous approval test in 2019. It was the first FDA approval for mesothelioma in 15 years. Optune Lua is available for pleural mesothelioma patients who cannot undergo surgery. It was approved to be combined with chemotherapy.

Optune Lua sends low-intensity electric waves through the cancer location in your body. Cancer cells are sensitive to these fields, which are tuned to specific frequencies. The waves disrupt cell division, prevent the tumors from growing and eventually cause the cells to die. Patients barely feel the waves initially, and they eventually grow accustomed to them.

Pleural mesothelioma patients wear the device on their chest for most of the day and night. They can even sleep with the device, which is another reason why Optune Lua is one of the least invasive therapies for mesothelioma.

Not only is the device easy and safe to use, but the life expectancy and survival results for mesothelioma patients are exceptional. The median overall survival from one study was 18.2 months. Nearly two-thirds were alive after one year, which is a much better rate than chemotherapy on its own.

Since its FDA approval just one year ago, the Optune Lua device is offered at hospitals across the country. The number of institutions with access to this treatment continues to grow.

Keytruda is the brand name for pembrolizumab, an immunotherapy checkpoint inhibitor drug. Keytruda explicitly stifles the connection between two protein receptors: PD-1 on the bodys cancer-fighting T-cells; and PD-L1 on mesothelioma cells.

When these protein receptors connect, T-cells ignore mesothelioma cells in their quest to locate diseases and protect the body. Keytruda breaks or blocks the connection, which aids the immune systems natural defenses.

In a recent study, 34% of mesothelioma patients with high levels of PD-L1 survived for two years thanks to Keytruda. Nearly 20% had progression-free survival (meaning their disease did not grow or spread) for two years.

The non-small cell lung cancer study is a phase 2 trial explicitly for PD-L1-positive patients. Itll enroll up to 66 participants and begin before the end of 2020. The researchers will examine the response rate, overall survival, progression-free survival and disease-control rate.

William Doyle, the executive chairman of Novocure, said that scientific studies suggest that the use of tumor treating fields together with an anti-PD-1 therapy can potentially augment the immune response, resulting in improved tumor control.

Non-small cell lung cancer and pleural mesothelioma are similar in numerous ways. First, the two cancers develop in the same general region: pleural mesothelioma in the thin tissue lining between the lungs and chest (the pleura); non-small cell lung cancer in the tissue of the lungs. Pleural mesothelioma can spread to the lungs, and treatment sometimes involves removing a lung.

The two diseases also resemble one another. Adenocarcinoma is a type of non-small cell lung cancer. Doctors often confuse it and pleural mesothelioma when looking at scans.

Theyre also linked to the same culprit: asbestos. Inhaling or swallowing loose, sharp asbestos fragments can lead to cellular irritation, either on lung tissue or in the pleura. Asbestos is the only known cause of pleural mesothelioma, and it is one of multiple potential reasons for non-small cell lung cancer.

The similarities between the two cancers suggest that a Keytruda-Optune Lua combination could also work for mesothelioma. If the non-small cell lung cancer trial is a success, then the partnerships scope should expand.

Doyle hinted at the possibility of testing the pairing on more cancer types. He called the collaboration an important combination study as we strive to extend survival in some of the most aggressive forms of cancer.

Terry OKeefe and his wife Diane love to hike and travel in their RV. The Grove City couple was hiking in the Midwest a year and a half ago when Terry began experiencing alarming symptoms: shortness of breath, racing heart, fainting.

Back in Pennsylvania, doctors made the diagnosis: malignant pleural mesothelioma, a rare and aggressive cancer that attacks the lining of the lungs, abdomen and heart. Median survival is only about one year. Mesothelioma is strongly connected to asbestos exposure, and Terry was exposed to the substance during his years working as a boiler operator in the U.S. Navy.

Terry underwent surgery and chemotherapy, both standard treatments for mesothelioma. Then Zachary D. Horne, MD, a radiation oncologist who practices at Allegheny Health Network Cancer Institutes center in Beaver County, as well as at Allegheny General and West Penn hospitals, offered Terry a chance to try a new option.

Optune Lua, a wearable, portable device that delivers low-grade electrical fields to the tumors, is the first new FDA-approved treatment for mesothelioma in 15 years. AHN Cancer Institute is the only center in the region offering Optune Lua for mesothelioma, and Terry was the second patient in Pennsylvania to receive the treatment.

Mesothelioma is a devastating diagnosis. We are thrilled to offer our patients Optune Lua, a treatment that can potentially extend their lives and improve their quality of life, Dr. Horne said. The treatment is non-invasive and delivers treatment directly to the tumor, and so most patients experience only minor side effects.

Optune Lua users can continue with normal daily activities during treatment. Small adhesive patches called transducer arrays are placed on the patients chest and back, and are connected by wire to a battery that the patient carries either in a backpack or across the shoulder. The electrical fields work by disrupting the process by which cancer cells make copies of themselves.

In a clinical trial of 80 patients with unresectable or metastatic and previously untreated malignant pleural mesothelioma who were treated with Optune Lua plus chemotherapy, median survival was 18.2 months, and 97 percent of patients saw their mesothelioma either shrink or stop growing.

As for Terry, the lower part of his lung tumor has completely disappeared, while the upper part has grown only slightly. Dr. Horne is adjusting the patches so that more of the electrical fields are directed to the upper lung, and doctors are also adding a newer immunotherapy to Terrys regime. Terry and his wife arent back to RVing yet, but are enjoying short hikes near their home in the company of their two dogs

EPA Draft Risk Assessment on Asbestos Flawed and could be Problematic for Automotive Friction Defendants – Lexology

The United States Environmental Protection Agency (EPA) is currently conducting a chemical risk evaluation of asbestos. The EPAs 310-page Draft Risk Evaluation for Asbestos (the Draft) identified risks for a variety of potential asbestos exposures. The Draft found that acceptable cancer risks were exceeded for much work with automotive brakes and clutches. If these findings are adopted in the final evaluation, defendants can expect asbestos plaintiffs counsel to trumpet the evaluation to juries, even though (as described below and detailed in the attached article) the analysis is flawed and inconsistent with both the scientific literature and legal standards regarding cancer causation.

The Draft looked at exposures to occupational mechanics, bystanders, and do-it-yourself mechanics. The only asbestos fiber type the EPA evaluated was commercial chrysotile, and the only potential causative diseases the EPA evaluated were mesothelioma and lung cancer. The EPA chose to evaluate acceptable cancer risks to individuals based on an Inhalation Unit Risk of 0.16 per fiber/cc.

The Draft found that acceptable cancer risks were exceeded for brake and clutch installation and removal for occupational mechanics at both high-end and central tendency exposure levels and for occupational non-users at all high-end levels. For brake repair and replacement performed indoors with compressed air, the Draft found that acceptable risk levels were exceeded for do-it-yourself mechanics and bystanders of do-it-yourself mechanics at high-end and central tendency levels. For brake work performed outdoors, the Draft found that acceptable risks levels were exceeded for do-it-yourself mechanics at high-end tendency levels when mechanic work was performed 30 minutes per day with 62 years of cumulative exposure starting at age 16.

While the final risk assessment has not been published, the conclusions found in the current Draft lead to concern that these findings will cause confusion or even improperly influence jurors in future asbestos trials. A finding by the EPA that working with and around asbestos-containing brakes and clutches leads to an unacceptable risk of mesothelioma and lung cancer would be used in the opening statements of every asbestos plaintiffs attorney in the country. Although the assessment is based on many assumptions that are not realistic, excludes large swaths of relevant scientific and medical studies, and makes conclusions regarding cancer causation that are entirely out of line with all current legal standards, it would be complicated and difficult to explain in necessary detail to a jury. It would simply be another factoid that will be used to persuade jurors that there is no safe level of chrysotile asbestos exposure and that all exposures cause disease. Surely, many hours of drafting and many tons of ink will be spilt in attempts to keep the findings of the EPA out of the courtroom through pre-trial motions to exclude and motions in limine. This is a future that will soon become all too real if there are not considerable changes made between the current Draft and the EPAs final risk evaluation.
