Republicans in the Tennessee Legislature are skeptical the federal government will respect the autonomy of "state-run" exchanges, so it doesn #39;t make much sense to spend a lot of time devising a system. The so-called exchanges outlined in the federal Affordable Care Act are supposed to be operational by the beginning of 2014. Poor and middle class customers approved to shop the exchanges will be eligible for government subsidies to help pay premiums. In this Nov. 13, 2012 interview, Senate Government Operations chairman Mike Bell, a Republican from southeast Tennessee, said he opposes the state getting involved in the business of running health insurance exchanges as envisioned by Obamacare legislation passed two years ago. Bell said he is "still holding out hope" that Republicans in Congress can roadblock funding for Obama #39;s health care law "and thereby delay or possibly even kill it at the federal level." WASHINGTON (AP) - In a momentous ruling touching virtually every American, the Supreme Court narrowly upheld President Barack Obama's historic health care overhaul Thursday with the unlikely help of conservative Chief Justice John Roberts.

But the decision also gave Republicans unexpected ammunition to energize supporters in the battle for the White House and to fight "Obamacare" as a new tax on people who don't obtain health insurance.

Roberts' vote, along with those of the court's four liberal justices, preserved the largest expansion of the nation's social safety net in more than 45 years, including the hotly debated core requirement that nearly everyone have health insurance or pay a penalty. The aim is to extend coverage to more than 30 million people who now are uninsured

The 5-4 decision meant the huge overhaul, still taking effect, could proceed and pick up momentum over the next several years, with an impact on the way that countless Americans receive and pay for their personal medical care.

The ruling handed Obama a campaign-season victory in rejecting arguments that Congress went too far in approving the plan. However, Republicans quickly indicated they would try to use the decision against him.

At the White House, Obama declared, "Whatever the politics, today's decision was a victory for people all over this country." Blocks away, GOP presidential candidate Mitt Romney renewed his criticism of the overhaul, calling it "bad law" and promising to work to repeal it if elected in November.

Demonstrators for and against the law crowded the grounds outside the Supreme Court Building on Capitol Hill as Roberts, sitting at the center of the nine black-robed justices inside, announced the decision to a packed courtroom.

Breaking with the other conservative justices, Roberts read the judgment that allows the law to go forward. He explained at length the court's view of the insurance mandate as a valid exercise of Congress' authority to "lay and collect taxes." The administration estimates that roughly 4 million people will pay the penalty rather than buy insurance.

Congress called the payment a penalty, not a tax, but Roberts said the court would not get hung up on labels. Among other indications it is a tax, Roberts said, "the payment is collected solely by the IRS through the normal means of taxation."

"Because the Constitution permits such a tax, it is not our role to forbid it, or to pass upon its wisdom or fairness," Roberts said.

PEABODY Representative Steve Walsh and former Lynn mayor Thomas Costin illustrated the melding of two generations of politicians advocating for universal health care at a benefit for the Lynn Community Health Center Thursday night.

Wisdom to Wellness: Celebrating Visionaries in Healthcare, held at the Peabody Marriott hotel, honored Costin and Dr. Mark Doherty, Executive Director of The DentaQuest Institute, for their contributions and efforts toward the LCHC.

As keynote speaker, Walsh, D-Lynn, spoke about the problems in health care that he aimed to change with the healthcare reform bill that he spearheaded as chairman of the Healthcare Finance Committee in Massachusetts. Walsh said he used to only approach the healthcare budget in a very plastic, very vanilla way when making cuts until his son became critically ill in March 2011.

Walsh said after that incident, he realized, When a loved one gets sick, we dont say, What does this cost will my insurance cover this? We say, I want the best, I dont care what it costs, get my loved one better.

Walsh went on to say that the problems in the healthcare industry are that there are too many hidden costs, that primary care isnt available to patients, and that information technology is lacking, making it difficult to share medical records. Walsh said healthcare centers like the one in Lynn are actually ahead of the times, offering good health care at available hours and at a low cost. We wrote this bill to make sure if anyone is in the position I was in, you dont have to make the choice between getting the best health care or not, he said.

Walsh said the bill was written so it had room for improvement. We left it up to all of you to continue to innovate, to continue to lead, to continue to do the things you do, he said to a crowd of over 100 of Lynn Community Health Center supporters and workers.

Walsh seemed to echo Costins sentiment of encouraging others to join the fight for health care. Costin, former General Postmaster and the youngest mayor and city councilor to have served in Lynn, worked closely with his friend Sen. Edward Kennedy on the Affordable Care Act, his passion being the recognition of alcoholism as a disease. Costin said in the beginning, he had thought, If I could help one person, then I would be successful. Now, he said the Lynn Community Health Center has helped over 20 million people in 41 years, and its been done because one senator put his mind to it, his heart to it and his body to it.

Costin ended his remarks by saying in order for healthcare reform to take place, We have to listen, we have to watch, we have to act with our heart and soul, and if we can do that, Ted Kennedy will always be with us.

Saying that the future of health care will be shaped by collaborative relationships, officials from Toledo-based ProMedica and HCR ManorCare announced plans Thursday to jointly build a skilled nursing and rehabilitation center.

The multimillion-dollar facility will be built on the ProMedica Flower Hospital campus in Sylvania, and ultimately replace ProMedicas 200-bed Lake Park rehab facility.

Officials said the goal of the new facility is to improve how patients make the transition from regular hospitals to their own homes.

Lake Park has served the community very well for almost 50 years, but this is an opportunity to write that next chaper and build a state-of-the-art facility," said Randy Schimmoeller, senior vice president of operations for ProMedica Continuing Care Services Corp.

The proposed 120-bed facility would have 24-hour skilled nursing care, a special gym, more private rooms equipped with flat-screen televisions, and more on-site personal care services, including a barber shop and beauty shop. HCR ManorCare recently opened similar facilities in Michigan, New Jersey, and Washington state and has a project in the works near Cleveland.

Officials said the center will be more like a home and less like a hospital.

When you look at skilled nursing facilities, todays standard is more like a home setting, a home-type model. One hundred twenty beds is kind of the standard, Mr. Schimmoeller said. We want this to be a facility that has that feel, and as we go forward well develop transitional plans for patients and manage it in a way that were meeting all the needs of the patients.

By John Green - The Hutchinson News - jgreen@hutchnews.com Two Catholic-owned healthcare facilities in western Kansas - St. Catherine Hospital in Garden City and St. Rose Ambulatory and Surgery Center in Great Bend - will soon become part of Colorado's largest health care organization.

They will join Denver-based Centura Health Initiatives, a massive health care consortium formed in 1996 that is jointly operated by Catholic Health Initiatives (CHI) and Adventist Health System, a mission of the Seventh Day Adventist Church.

The change, local directors said, won't result in any immediate differences notable to the public other than changes in colors of the care facilities signs, from teal to blue. It will, however, enable local health care providers to access expanded specialized and expert care from other entities in the organization, and allow the organizations to better weather expected changes in health care regulation and funding.

"The organizational structure at St. Catherine's and St. Rose, in terms of governance structure, will remain intact," said Scott Taylor, president and CEO of CHI Kansas. "My boss changes... There may be dotted line reporting changes in terms of senior officers. But within the organization itself, there is no change in reporting structure. Board authorities at each facility remain the same with the Catholic Health Initiatives."

Officials expect no reductions in employment at either facility, either, Taylor said. In fact, some operations the institution previously scheduled to outsource to third party companies will now remain internal.

"They'll actually get to stay with their current employers, where formally they were scheduled to transition out Jan. 1," he said.

The organization will begin work on the integration immediately, with systems at St. Catherine and St. Rose transitioned to Centura Health in the coming months.

Consumer benefits

Centura Health is Colorado's largest family of hospitals and health care services and one of the state's largest private employers, operating 13 hospitals, seven senior living communities and home care and hospice services.

The consolidation "will benefit consumers throughout the region by facilitating shared best practices, linked systems of care, transparent exchanges of health records, and, ultimately, improved and more-consistent clinical quality across the two states and region," according to press release from CHI.

A liberal think-tank closely allied with the Obama administration is proposing a health care spending plan it says could save hundreds of billions of dollars in entitlement spending without hurting middle- and low-income patients.

The plan from the Center for American Progress comes as Congress prepares for a battle royal over the so-called fiscal cliff. That's the combination of tax increases and spending cuts that happen automatically unless lawmakers and the president can reach a deal before the end of the year.

Democrats have been demanding higher taxes on the rich, while Republicans say they won't budge without big changes to programs like Medicare and Medicaid.

Senate Minority Leader Mitch McConnell didn't mince any words in his first floor speech of the lame-duck session Tuesday afternoon: "Republicans like me have said for more than a year now that we're open to new revenue in exchange for meaningful reforms to the entitlement programs that are the primary drivers of our debt."

And meaningful reform is exactly what the authors of the CAP plan said they are proposing.

"If entitlement reform is simply code for dismantling Medicare and Social Security, we're not at the table, and we should oppose that with everything we've got," said Ezekiel Emanuel, a senior fellow with the group who helped put the proposal together. "On the other hand," he added, "if entitlement reform is, 'We're going to transform the system, modernize it so it can deliver high-quality, lower-cost care,' that's where we are. And that's what we think we've got here."

Now, the Center for American Progress isn't just any group. It's considered something of the White House's reserve corps. Emanuel, for example, is the brother of former White House Chief of Staff Rahm Emanuel and a former White House health staffer himself. CAP President Neera Tanden is also a former White House health staffer.

And this new plan is notable not just for potential savings of a half-trillion dollars but for what some of those savings are. They include many things that build on savings included in the Affordable Care Act, like cutting waste and encouraging more efficient care, as well as lowering payments to some providers. But there are also some controversial items. Like asking wealthier Medicare beneficiaries to pay more out of pocket for their care. And limiting the tax-free status of health insurance for people who earn more than $250,000 a year.

People planning for retirement tend to budget for the fun stuff, like cruises with grandkids or golf trips in the Sunbelt, and ignore the less fun stuffespecially the costs of post-retirement health care.

And when boomers find out how much theyre likely to need to cover those medical coststypically, about a quarter of a million dollars, the experts saythey might wish they hadnt asked. All those plans for cruises go out the window if you dont have adequate coverage, said Alexis Abramson, a New York based gerontology expert. But understanding what kinds of costs and calculations lie behind that number can make it less intimidating, and help you prepare.

Fidelity Investments, which oversees some 12 million 401(k) accounts, has been a bellwether in setting expectations on this topic. This spring, for example, it released a study saying that an average 65-year-old couple retiring in 2012 would need to have saved up $240,000 to pay for out-of-pocket health-care costs in retirement.

And thats $240,000 in todays dollars, so a couple retiring in 10 years would need the inflated-adjusted equivalent in the year 2022. (In its 11 years of doing this study, Fidelity has found the rate of health-care inflation to average 6% per year; assuming that rate stayed constant, a 2022 retiree would need about $430,000 set aside.)

Some experts arent comfortable with using a number like Fidelitys as a benchmark. A single number can be misleading in this context, said Paul Fronstin, director of the health research and education program at the Employee Benefit Research Institute (EBRI): If you plan to be average, you have a 1% chance of being right.

Still, Fronstin and other critics say it helps to have a starting point when it comes to figuring out retirement health-care costs. (EBRI put out its own series of estimates last month that segmented retirees based on their drug expenses.) If nothing else, such numbers drive home the point that Medicare doesnt cover everything.

In fact, of all the money spent on health care for people 65 and over, Medicare accounts for just 59%, according to an EBRI analysis of actual expenditures; out-of-pocket spending covered 13% and private insurance paid for 14% (the rest was covered by Medicaid and other sources)
