Fallin rejects health care exchange, Medicaid expansion

The decision means Oklahoma will not do anything to accommodate the controversial federal health care law known as Obamacare but it also means the state will skip an opportunity to help some 693,000 uninsured Oklahomans, 18.7 percent of the state population, get coverage through the federal government.

After careful consideration, I have today informed U.S. Secretary of Health Kathleen Sebelius that Oklahoma will not pursue the creation of its own health insurance exchange, Fallin said. Furthermore, I have also decided that Oklahoma will not be participating in the Obama Administrations proposed expansion of Medicaid.

Any exchange that is compliant with the federal health care law will necessarily be state-run in name only and would require Oklahoma resources, staff and tax dollars to implement, Fallin said.

It does not benefit Oklahoma taxpayers to actively support and fund a new government program that will ultimately be under the control of the federal government, that is opposed by a clear majority of Oklahomans and that will further the implementation of a law that threatens to erode both the quality of American health care and the fiscal stability of the nation, she said.

A Medicaid expansion would cost the state up to $475 million between now and 2020, with escalating annual expenses in subsequent years, Fallin said.

Other estimates of Medicaid expansion are much lower, and some show the state would actually save money with the move.

In reality, the expansion is a very favorable deal for Oklahoma, said David Blatt, director of the Oklahoma Policy Institute.

The federal government will pay 100 percent of the cost for the first three years and then gradually shift costs to the states with a that amount capping at 10 percent in 2020. The Oklahoma Health Care Authority estimates the states share of Medicaid costs in 2020 would be $28 million to $37 million, an amount that is less than 0.5 percent of current state appropriations, Blatt said.

With savings to the states health, mental health and prison programs and increased tax revenues from new medical professionals needed to cover increased demand that would come with the expansion, the Urban Institute has estimated that Oklahomas budget comes out saving money with the move.

But Fallin said the Medicaid expansion is the wrong direction for the state. It would also further Oklahomas reliance on federal money that may or may not be available in the future given the dire fiscal problems facing the federal government, Fallin said. On a state level, massive new costs associated with Medicaid expansion would require cuts to important government priorities such as education and public safety.

Health care austerity protests in Spain

Spaniards angered by austerity measures, including budget cuts and plans to partly privatise some of their country's cherished national health service, have held a rally in Madrid.

About 10,000 people, including health workers dressed in clinical white and blue, marched from four large hospitals on the outskirts of Madrid to central Puerta del Sol square on Sunday behind banners saying: 'Our public health service is not for sale, it's to be defended'.

Some protesters said they were outraged by Madrid regional government plans to convert a large hospital specialising in rare and infectious diseases into an old people's home and then sell it.

'Our hospital, Carlos III, is a medical reference point. We treat highly infectious illnesses. We serve a lot of patients. Yet they still want to get rid of it,' said nurse Natalia Fernandez, 34.

Javier Fernandez-Laquetty, Madrid councilor in charge of the region's health care, said the measures being implemented seek to achieve greater efficiency and guarantee that taxpayers survive the crisis with first-class hospitals.

'The measures are to ensure, responsibly, that we continue to have a high quality universal public health service, open to all,' he said.

Rally organisers, who called the march 'a white tide', said Madrid's regional health workers would hold four days of strikes on November 26-27 and December 4-5 to criticise the government's actions.

'We have built hospitals and health centres with public money and the government is handing them over to its friends,' said nurse Maria Victoria de Lucas, 52.

Health care and education are administered by Spain's 17 semi-autonomous regions, rather than the central government, and each sets its own budgets and spending plans. Regions account for almost 40 per cent of public spending.

Many regions are struggling as Spain's economy contracts into a double-dip recession triggered by a real estate crash in 2008.

WASHINGTON (AP) Threatened with repeal just weeks ago, President Barack Obama's health care law now appears on track in close to half the states, with others playing catch-up and the administration readying a fallback for states not wishing to participate.

Friday was the original deadline for states to notify Washington if they would play a role in building new health insurance markets through which the uninsured can get coverage starting in 2014. Though the administration granted a month's extension, most states have already made their intentions known.

As of Friday, 23 states plus Washington, D.C., were proceeding; 15 said they'd defer to the federal government to run their markets and 12 were still mulling over their options. The mostly blue group proceeding included five Republican-led states. The undecided included several states that seemed to be moving toward an active role.

"Postelection it's really been `game on,"' said Kelly Barnes, leader of the health care group at the PricewaterhouseCoopers consulting firm.

Republican presidential candidate Mitt Romney had vowed to begin dismantling "Obamacare" his first day in office. But Romney lost, and instead GOP governors are scrambling to see if they can find an accommodation with the administration after two years of fighting the president's signature law.

"The president won the election... and New Jersey is going to comply with the Affordable Care Act," Republican Gov. Chris Christie said Friday. Christie said he still has questions about costs and is weighing his decision. Earlier this year he vetoed legislation creating a state exchange, as the new insurance markets are called.

A check by The Associated Press found that 16 states plus Washington, D.C., want to build their own exchanges. Starting Jan. 1, 2014, individuals, families and small businesses will be able to buy private coverage through an exchange in their state, with most consumers getting government assistance to pay premiums.

The exchanges will also help steer low-income, uninsured people into expanded Medicaid programs in many states.

State awards new health insurance contracts

The state has awarded contracts to Health Alliance HMO and Coventry Health Care HMO to provide health-care coverage to state employees, retirees and their dependents.

The new contracts have an initial term of 3 1/2 years, but the state has the option of an additional five, one-year renewals. The new contracts are set to begin Jan. 1.

Awarding the contracts essentially resolves a controversy that erupted in 2011 when the state rebid health insurance contracts and did not select Health Alliance or Humana, two longtime providers of state health insurance to thousands of employees and retirees.

Instead, the state awarded HMO contracts to Blue Cross and Blue Shield, even though the company didnt contract with health-care providers in some downstate counties. The Department of Healthcare and Family Services said the switch would save the state $1 billion over 10 years.

The change sparked a legislative and legal battle over the states decision. The state eventually agreed to rebid the contracts.

It resolves the issue of the rebidding of the group insurance contracts for state employees and retirees, said DHFS spokesman Mike Claffey. Were happy to move forward.

Blue Cross and Blue Shield will continue to provide HMO coverage for people in the Chicago area.

Claffey did not provide costs for the new contracts, but said the state no longer expects to save the $1 billion over 10 years that it previously expected.

Health Alliance HMO covers 43,141 members and 43,504 dependents, DHFS said. Coventry HMO (formerly Personal Care) covers 5,408 members and 4,608 dependents.

"Just because the federal government says they may do something in the future, even with Obamacare, doesn't mean it's going to be there because that fact is we have a huge deficit and a big fiscal crisis in our nation."

Expanding the state's Medicaid program would come with some state expenses, and that is money that won't be available for other priorities, including education and transportation, she said.

"I can't print money, unlike the federal government," Fallin said. "I have to balance our budget. I have to live within our means."

CHICAGO, Nov. 20, 2012 /PRNewswire/ -- Health Care Service Corporation (HCSC), the nation's largest customer-owned health insurer, and Northwestern University Feinberg School of Medicine announce a collaborative research initiative called Joint ADventure, focused on improving symptoms and quality of life for those with signs of knee osteoarthritis (OA). Funded by a grant from the National Institutes of Health, Joint ADventure aims to prevent work absenteeism, work disability and increased healthcare costs associated with knee OA through a workplace intervention pilot program with HCSC employees.

The obesity epidemic in this country has led to growing numbers of adults with knee osteoarthritis. Nearly 50 percent of all adults and two-thirds of obese adults are expected to develop symptomatic knee OA at some point in their lives. Knee OA is already a major cause of arthritis-related disability, resulting from difficulty doing major activities such as walking, climbing stairs, kneeling or stooping. Because obesity and knee injury (both risk factors for the development of knee OA) are increasing in the US, the lifetime risks and the absolute number of persons with knee OA and consequent disability are projected to rise dramatically.

Joint ADventure, a two-year research initiative designed specifically for adults with chronic knee symptoms, will develop, implement and test a program to address knee symptoms and improve the quality of life for participants with or at risk for developing OA. Approximately 90 HCSC employees will voluntarily participate in a twelve-month intensive physical activity and nutrition intervention which includes weight loss goals, fostered by better nutrition and physical activity benchmarks. The program also includes a lifestyle intervention led by a nurse clinical specialist, fitness expert, dietitian, and an occupational therapist from Northwestern University, which will provide motivational coaching and cohort sessions focused on adopting healthy eating and exercise routines. The participant selection process for the program is currently underway. The intention is to replicate the program's successes with HCSC members and employer groups.

"Our partnership with HCSC is a great example of how the public and private sector can join to work together to find an effective, replicable and sustainable intervention strategy for those with - or at risk for - knee OA to attain and maintain healthy behaviors," said chief investigator Dr. Rowland Chang, M.D., professor of preventive medicine and director of the Institute for Public Health and Medicine at Northwestern's Feinberg School. "Knee OA is a major cause of work disability and absenteeism. Our hope is that the results of this study will provide a blueprint that will lead to fundamental changes in the way lifestyle interventions are delivered to large employed populations."

Results will be measured based on the evaluation of participant positive lifestyle behavior modifications including physical activity, dietary behavior measures and the assessment of OA pain and self-reported health related quality of life improvements.
