Living the vegan life in Cleveland County – Gaston Gazette

The smells of macaroni and cheese, collard greens and fried "chicken" filled the air on Washington Street Saturday. It was like many cookouts in the south, except this one was vegan.

LeRoy White of Healing, Health and Wellness Center, LLC, hosted one of the stores now regular vegan lunch pop-ups on Saturday. People lined up at the door around noon as the tantalizing smells wafted through the rooms at the shop located at 323 S. Washington St., Shelby.

As customers waited for Chef ZiZi, a Charlotte-based vegan chef, to set-up, they talked with other vendors and White about vegan products, the plant-based lifestyle and more. People got a chance to try vegan cupcakes from The Patty House and Bakery before trying some whipped shea butter perfume from Kisses of Shea. Customers could even grab a nibble to go from Powerful Bites.

It was in March when White and Chef ZiZi got together for the first vegan event at the shop since more and more people have been coming to him to learn about veganism.

"I was surprised that we had over 100 people sign up," White said.

He was shocked, but excited, by the number of people.

"My passion was to get people healthy. To me that is the most wonderful thing," White said.

And for White, his passion stems from his journey with food.

It was a decade ago when White got a wakeup call about his health and moved to a plant-based diet.

"I went to the VA for a check-up. She said I was fat and going to die," White said.

It shook White to his core, so the pastor decided he was going to change. With a host of ailments including diabetes, high blood pressure, arthritis and high cholesterol, White decided to focus on his diet but wasnt too sure what he was doing.

White looked to Gerene Butterfield at the Healing, Health and Wellness Center for help. She told him to go vegan.

"I told her she was crazy, but I did what she said," White said.

He decided to drink smoothies for 30 days. By the time that was done, White was off his diabetes medications and realized veganism was the way to go for him.

As he went through his journey to the all plant-based diet, he noticed his church congregation suffering the same ailments he was slowly starting to see go away.

"I realized it was what we were eating," White said.

Getting vegetarian and vegan products 10 years ago was near difficult in Cleveland County, but White and Butterfield worked together to help others learn more about the plant-based lifestyle. White began by doing educational classes about veganism, emphasizing that the diet consists of more than just salads day-in and day-out.

For several years he talked about his journey to veganism and the changes it made in his life. Then one day, Butterfield asked White if he wanted to take over Healing, Health and Wellness Center.

"To get this building was a blessing," he said.

The Center transformed into a haven for herbalists, people looking to change their lifestyles and those that might just need a friendly chat.

He aims to have products customers might not find in local grocery stores at reasonable prices.

"I know it is a struggle. I know," White said. "We want to make it as convenient as possible for people."

Ten years ago White never dreamed he would be hosting vegan food pop-ups, offering Chef ZiZis line of vegan frozen meals and selling a wide variety of natural organic products and groceries, but now he is dreaming bigger.

His goal is to educate more and more people about living a healthy lifestyle until Cleveland County is no longer number 80 in the state health ratings, but up into the 20s.

21 ways to reduce your Alzheimer’s risk, backed by research – KTVZ

There are 21 known ways to reduce your risk of developing Alzheimers, and 10 of those are backed by strong scientific evidence, new research has revealed.

The preventative steps with the most impact are mentally stimulating activities, including reading and chess; avoiding stress, depression and head trauma; and avoiding diabetes, high blood pressure and weight gain in midlife and beyond, according to a meta-analysis released Monday.

Regular physical exercise, quality sleep, no smoking, maintaining good heart health in later life and including vitamin C in the diet are among the nine preventative actions with slightly weaker but still supportive evidence.

If risk factors can be modified, the evidence suggests that at least one out of every three cases of Alzheimers may be preventable, said neurologist Dr. Richard Isaacson, director of the Alzheimers Prevention Clinic at Weill Cornell Medicine.

People can take different roads to Alzheimers disease, said Isaacson, who was not involved in the study. This paper helps to provide an evidence-based framework for doctors and patients to consider when managing a persons Alzheimers risk.

The study published in the Journal of Neurology, Neurosurgery & Psychiatry.

Two-thirds of the most promising interventions focused on healthy lifestyle changes and targeting risk factors for cardiovascular disease, such as high blood pressure and cholesterol levels.

The study analyzed 395 observational prospective studies and randomized controlled trials. These efforts, the researchers said, were the most comprehensive and large-scale systematic review and meta-analysis for Alzheimers disease to date.

The top 10 preventative actions backed by science that the study found included:

One of the most important and commonly overlooked modifiable factors is a high level of homocysteine in the blood, Isaacson said. In his New York-based preventative clinic, he said he will often suggest B-complex vitamins to combat elevated homocysteine levels.

However, past research has shown that B-vitamin supplementation works better if these people also have sufficient levels of Omega3s, which are a brain-healthy fat found in fatty fish, for example, Isaacson explained.

So, this is just one of the many examples of the complex web of decision-making that goes into the clinical care of people at risk for the disease.

Nine actions that had weaker, but still positive, impacts on reducing the risk of developing Alzheimers were:

Of the 21 study recommendations, the last two were actions that physicians should avoid when working with patients at risk of Alzheimers disease. Strong evidence was found against giving postmenopausal women estrogen replacement therapy to improve cognition.

I wholly agree with this statement but for women in the peri-menopause and early menopause, hormone replacement therapy may sometimes be considered based on a variety of other factors, Isaacson said.

Finally, weaker evidence indicated that acetylcholinesterase inhibitors drugs like donepezil and galantamine, which are often prescribed to treat memory loss in people with diagnosed Alzheimers disease should not be used as a prevention tactic.

Those are drugs for symptomatic people with mild or worse dementia, Isaacson said. Those drugs work for symptoms but dont slow disease progression.

People who are concerned about their risk of developing Alzheimers should work with a doctor trained in the field to develop an individualized plan to reduce their risk, Isaacson said.

Its also important to note that there is not a single magic pill or action that can prevent Alzheimers, he said.

Risk reduction care is not one size fits all it needs to be comprehensive and individually tailored, ideally under a doctors supervision.

The benefits of gua sha have been much touted in recent years. Ostensibly loved by celebrities such as Gwyneth Paltrow and Elle McPherson for its anti-aging qualities, and used by Justin Bieber to ease neck pain, gua sha tools are applied to the face or body for an effective anti-ageing facial, to potentially reduce acne, ease pain and muscle tension, and even lessen the appearance of cellulite.

But is gua sha legit? I asked three experts Francesca Canzano-Franklin, a Chinese Medicine practitioner and founder of Philosophia Botanica; Charlotte Conwy, an acupuncturist and practitioner in Five Element and Traditional Chinese Medicine at acu:east; and Cecily J Braden, founder of CJBand a recognised trainer in the wellness industry about the benefits of gua sha.

Long before gua sha became a fashionable tool, it wasan integral part of Traditional Chinese Medicine for thousands of years, explains Francesca. Gua sha is an ancient practice for energy flow and blood circulation; gua means to scrape, and sha means sand.The term means the act of scraping the skin to remove an energy blockage using a stone made of bian, jade or rose quartz [youll also find tools made from other materials such as obsidian, amethyst and copper]. While scraping the skin may not sound particularly relaxing, gua sha is a gentle process that works as a natural face lift, to reduce pain and get rid of toxins, says Francesca.

Gua sha is a form of massage, whereby you use oil and a smooth-edged tool on the face or body to rub the skin, stroking along the meridian lines, says Charlotte. It boosts circulation studies show by up to 400% and creates micro trauma, boosting collagen and stimulating the lymphatic and immune system. While many of us only know about gua sha facials, its uses extend further. Many of my clients ask for gua sha as its the perfect antidote to tightness in the neck and shoulders, or tension in the jaw or face, says Charlotte.

From a TCM (Traditional Chinese Medicine) perspective, gua sha can help to increase the flow of blood, qi and fluids, opening the pathways that feed the skin, to return vitality to the skin and support a healthy ageing process, says Cecily. But how does that make gua sha anti-ageing on delicate facial skin? Varying pressures and angles of the stones work different layers of the skin with techniques that are specific to lymphatic drainage (surface), fine lines and deep wrinkles (middle layer) and tension in the muscles (deeper layers), explains Cecily. The combination of different techniques and not all movements today are strictly gua sha but a mix of different facial massage techniques ignites a chain reaction within the skin to promote the healthy exchange of nutrients and waste.

Regular face gua sha also helps drain puffiness anddecreases any swelling that has occurred during the night, or as a result of eating salty foods or consuming alcohol, adds Francesca good news if youre trying to counteract the effects of the snacks and glasses of ros youve been enjoying.

Consistency is key and practicing gua sha on a regular basis will prove most beneficial, advises Francesca. Im an advocate of self-care and strongly suggest adding gua sha into your home beauty rituals.Butdont over do it, as the skin on the face is so delicate.

Gua sha can be done daily, says Cecily. However, if youre experiencing any redness, or tenderness, its best to wait until its subsided. Its important to work in a gentle way that allows the skin to respond to the movements and use its natural power to rejuvenate.

The touch and pressure should be very gentle, and you should always sweep the gua sha board across your face in specific motions, advises Francesca. The lighter the touch, the more youre supporting the lymph fluid, while with more pressure you will be lifting, toning and sculpting the facial muscles.

Alwaysstart at the neck areafor optimal lymphatic drainage and move upwards, says Cecily. The gua sha board should be heldat a 15-degree angle (almost flat against the skin). Then gently glide along the skin using sweeping, upward strokes. Each motion should be repeated five times.

When used on the body, studies show that it helps reduce pain, speeds up muscle recovery and improves movement.

There have been various studies on the effectiveness of gua sha to treat pain. In one German study, neck pain severity after one week improved significantly better in the gua sha group compared with the control group. In another study cited in The American Journal of Chinese Medicine,gua sha was found to be an effective treatment for patients with chronic neck and low back pain. Other studies indicate that it may be helpful as a treatment for migraines and mastitis.

However, my personaladvice would be to choosea stone that you feel drawn to. Theres no one model that works best.

Its not just the material you need to consider, but the shape of it, too. You should select a stone based on your expert level; the simpler the tool, the more beginner-friendly it will be. Whereas more advanced stones will have a few sides and textures so you can try different techniques, advises Francesca. Cecily agrees, Its less about the material and more about the unique combination of the stones pressure, angle and edge.Charlotte suggests you dont even need to use stone. In China they would use a variety of tools, including coins and spoons. One modern gua sha expert even swears by jam jar lids!

Dr. Stempien-Otero says many patients who are sick with Covid release enzymes into their blood that show they have damage to their heart.

"If you have chest pressure, discomfort, shortness of breath, call 911 or go to the ER. If you have fainting, call 911 or go to the ER. Do not be afraid that the ER, that youre going to get something from the ER. We have everything setup so that the COVID patients are separate, it is a safe place to go," she says.

Dr. Stempien-Otero says they are testing several different hypotheses at UW, including why patients with high blood pressure and diabetes are more susceptible to Covid-19, even more so than patients with lung disease.

While researchers continue to learn more about Covid every day,Dr. Stempien-Otero says Seattle area residents should take advantage of the summer months.

Living the vegan life in Cleveland County – Shelby Star

The smells of macaroni and cheese, collard greens and fried "chicken" filled the air on Washington Street Saturday. It was like many cookouts in the south, except this one was vegan.

LeRoy White of Healing, Health and Wellness Center, LLC, hosted one of the stores now regular vegan lunch pop-ups on Saturday. People lined up at the door around noon as the tantalizing smells wafted through the rooms at the shop located at 323 S. Washington St., Shelby.

As customers waited for Chef ZiZi, a Charlotte-based vegan chef, to set-up, they talked with other vendors and White about vegan products, the plant-based lifestyle and more. People got a chance to try vegan cupcakes from The Patty House and Bakery before trying some whipped shea butter perfume from Kisses of Shea. Customers could even grab a nibble to go from Powerful Bites.

It was in March when White and Chef ZiZi got together for the first vegan event at the shop since more and more people have been coming to him to learn about veganism.

"I was surprised that we had over 100 people sign up," White said.

He was shocked, but excited, by the number of people.

"My passion was to get people healthy. To me that is the most wonderful thing," White said.

And for White, his passion stems from his journey with food.

It was a decade ago when White got a wakeup call about his health and moved to a plant-based diet.

"I went to the VA for a check-up. She said I was fat and going to die," White said.

It shook White to his core, so the pastor decided he was going to change. With a host of ailments including diabetes, high blood pressure, arthritis and high cholesterol, White decided to focus on his diet but wasnt too sure what he was doing.

White looked to Gerene Butterfield at the Healing, Health and Wellness Center for help. She told him to go vegan.

"I told her she was crazy, but I did what she said," White said.

He decided to drink smoothies for 30 days. By the time that was done, White was off his diabetes medications and realized veganism was the way to go for him.

As he went through his journey to the all plant-based diet, he noticed his church congregation suffering the same ailments he was slowly starting to see go away.

"I realized it was what we were eating," White said.

Getting vegetarian and vegan products 10 years ago was near difficult in Cleveland County, but White and Butterfield worked together to help others learn more about the plant-based lifestyle. White began by doing educational classes about veganism, emphasizing that the diet consists of more than just salads day-in and day-out.

For several years he talked about his journey to veganism and the changes it made in his life. Then one day, Butterfield asked White if he wanted to take over Healing, Health and Wellness Center.

"To get this building was a blessing," he said.

The Center transformed into a haven for herbalists, people looking to change their lifestyles and those that might just need a friendly chat.

He aims to have products customers might not find in local grocery stores at reasonable prices.

"I know it is a struggle. I know," White said. "We want to make it as convenient as possible for people."

Ten years ago White never dreamed he would be hosting vegan food pop-ups, offering Chef ZiZis line of vegan frozen meals and selling a wide variety of natural organic products and groceries, but now he is dreaming bigger.

His goal is to educate more and more people about living a healthy lifestyle until Cleveland County is no longer number 80 in the state health ratings, but up into the 20s.

Diwan, PhD, President and Executive Chairman of the Company, will participate in the "B. Riley FBR Virtual Infectious Disease Summit - Therapeutics Day" on Tuesday, July 21, 2020. Dr. Diwan is invited to participate in Panel #3 at 2020 at 2:10 p.m. ET, entitled "Taming the Severe Disease Presentations". He will briefly discuss the Company's novel nanomedicines platform and the Company's progress in the lead IND-ready candidate for the treatment of shingles rash, NV-HHV-101, as well as in developing a drug candidate against SARS-CoV-2, the cause of COVID-19 global pandemic.

The Company is also working with its regulatory consultants on completing an IND with the US FDA to advance its lead drug candidate NV-HHV-101 into human clinical trials for topical dermal treatment of Shingles rash as the initial indication. In particular, the Company is working on finalizing the clinical trials plan for the anticipated human clinical trials for shingles rash treatment. The Company is also in the process of finalizing clinical trial sites. This process has been adversely affected by the current global COVID-19 pandemic, and in particular, its effects across the USA.

Importantly, nanoviricides are designed to act by a novel mechanism of action, trapping the virus particle like the "Venus-fly-trap" flower does for insects. Antibodies, in contrast, only label the virus for other components of the immune system to take care of. It is well known that the immune system is not functioning properly at least in severe COVID-19 patients.

Additionally, it is well known that viruses escape antibody-drugs via mutations. The Company's "nanoviricide" drug candidates, in contrast, are designed to be broad-spectrum, and therefore virus escape by mutations is expected to be unlikely.

The market size for the treatment of shingles is estimated at approximately one billion dollars by various estimates. These estimates take into account the Shingrix vaccine as well as existing vaccines. About 500,000 to 1 million cases of shingles occur in the USA alone every year.

The market size for our immediate target drugs in the HerpeCide program is variously estimated at billions to tens of billions of dollars. The Company believes that its dermal topical cream for the treatment of shingles rash will be its first drug heading into clinical trials. The Company believes that additional topical treatment candidates in the HerpeCide program, namely, HSV-1 "cold sores" treatment, and HSV-2 "genital ulcers" treatment are expected to follow the shingles candidate into IND-enabling development and then into human clinical trials. These additional candidates are based on NV-HHV-101, thereby maximizing return on investments and shareholder value.

Amazfit Bip S smartwatch review: Price and battery life will smoke the competition – CNET

The Amazfit Bip S isn't the best smartwatch I've ever tested -- not even close. But it's definitely the longest-running one. I wore it for almost 10 days straight without having to charge it once, and that's a lot more than I can say for its pricier competitors. But if battery life alone isn't enough to get your attention, the $69 price tag should seal the deal. (UK prices weren't available, but it's AU$119 in Australia, which is about 65.)

The Bip S is the third variant of the Amazfit Bip (along with the Amazfit Bip Lite). Created by Chinese company Huami in 2018, the original Bip was a fan favorite from the get-go. It did the basic smartwatch tasks for much less than competitors, and it lasted up to 45 days on a single charge. CNET's Scott Stein gave it high praise, calling it one of the best low-key smartwatches around.

The Bip S has better fitness features and a slightly better screen than its predecessor, but it hasn't changed much in the last two years. And while it's still one of the best deals out there, it lags a bit behind others, like the Apple Watch or Galaxy Watch, when it comes to features and connectivity.

The Amazfit Bip S comes in four colors, with a new two-toned pink option.

Save for a few new color options (pink and green) and a brighter display, the Bip S looks like a carbon copy of the original Bip. And by that, I mean it looks like a plastic Apple Watch, with a much thicker bezel and lighter frame.

It's definitely not the most attractive smartwatch I've laid my eyes on, but it's practical and more comfortable to wear than some of the more premium, heavier watches that are bulkier. It's so light that I even forgot I had it on when I slept.

But the biggest price compromise comes at the expense of the screen. It's always on, which is a nice perk, but it's not quite as sharp, bright or responsive as a phone screen. The backlight also didn't always turn on when I needed it to. Sometimes it works on demand, but I had to exaggerate raising my wrist more often than I'd like to get it to turn on. And as mentioned before, it's not as sensitive to touch. I had to swipe or tap the screen a few times to get it to respond. When it does light up or brighten though, it's clear to see in broad daylight.

The Bip S includes a few more watch faces than its predecessors, as well as the ability to customize some of the existing ones.

The original Amazfit Bip covered the basics when it came to fitness tracking, but the Bip S takes it a step further by adding more workouts and better metrics.

It can now log up to 10 different activities including yoga, weights and swimming. Its 5ATM water resistance makes it safe for both pool and open-water swims, and its built-in GPS means you can leave your phone on dry land while you're in the water or out on a run.

Since testing the Amazfit Bip S, I've been able to track my stats accurately. But I have to remember to do it properly. For example, the dial button can be programmed as a shortcut to start workouts, which is convenient. But for running (or other outdoor workouts) I still have to press the screen to confirm GPS connection before actually registering the run. This defeats the purpose of having the shortcut in the first place. The first few times I took it out, I forgot to confirm and it didn't log my run. Because it doesn't have automatic workout detection, you have to start it manually to actually log your workouts -- yet it will pause automatically when it senses that you've stopped moving for a while.

Like its predecessor, the Bip S has continuous heart-rate monitoring. During exercises, it broke down my information into zones on the mobile app to show whether I was in a light, intensive, aerobic, anaerobic or Vo2 max (maximum oxygen consumption) heart-rate zone. This is similar to what other sports watches like Fitbit and Garmin do, but because the Bip S' results didn't always reflect the intensity at which I perceived my workout, I'm skeptical of its accuracy.

The biggest upgrades on the Amazfit Bip S are health and fitness related.

In addition to the usual steps and distance, the Bip S adds a new metric called PAI (Personal Activity Intelligence)., which grades you on how much activity you've done during the day This means that rather than relying on step count or calories burned, the Bip S uses heart rate data (along with basic demographic information) to determine whether or not the user was able to raise their heart rate long enough throughout the day to maintain a healthy lifestyle. When this is achieved, the risk of developing chronic illnesses like heart disease, hypertension and diabetes lowers, according to Huami.

The idea itself is not novel and it's similar to the move ring on the Apple Watch and zone minutes on Fitbit devices -- all of which are activity metrics that go beyond step counting. But it's new to the Bip family, and it was a good way to keep myself honest about how much activity I was doing (or not doing) during the day. Having it actually motivated me to keep going throughout the week.

The biggest deal-breaker for me about the Amazfit is that it didn't really work well as a "smart" watch. Technically it's compatible with iOS and Android, but when I tried to use it with my iPhone, the connection was unreliable. I'd stop receiving phone notifications on the watch, and I'd have to re-pair the app with the phone a few times. A firmware update did help with the connectivity issues, but it didn't completely solve the problem. Seeing my data on the phone app also took a while to sync, too.

Even when the Amazfit displayed my notifications properly, I couldn't do much with them. Because it doesn't have a microphone, dictation is also out of the question, so don't think about programming quick replies.

All in all, besides a few basic apps for the weather and music control (which is a step up from the original Bip), the watch basically mirrors your phone and doesn't have many standalone functions. Even the alarm app has to be programmed on the phone, and there are no mobile payments of any sort.

Even with its spotty connectivity and mediocre smart features, this watch keeps going long after its competitors give up. The fact that I didn't have to sideline it to a charger at the end of the day meant it never left my wrist.

Huami says it can go up to 40 days on a charge if you're doing the bare-bone functions, although realistically I'd expect about 15 days worth of battery life. Or 10 days if you're firing on all cylinders, like myself (max screen brightness and GPS workouts on a regular basis, for example).

Beyond the convenience of not having to charge it at the end of each day, having it on 24-7 meant I tracked my sleep a lot more regularly than I did with other smartwatches.

The Bip S gave me a sleep score in the morning based on a number of different factors, like sleep duration, bedtime and quality of sleep compared to other users. As a mother of two small children, I know my sleep habits have room for improvement, so I was shocked to find that I've been averaging about a 90 (out of 100) every night. So either Amazfit users have deplorable sleep habits, or it's not very good at determining whether I'm in bed nursing the baby or actually asleep. I suspect it might be a combination of both.

But even with my skepticism, having this data over a longer period of time helped me identify the times when I was getting the most restful sleep and it changed my bedtime habits (to an earlier hour) so I could maximize that deep sleep cycle. Whether it actually helps me feel more rested during the day is TBD.

If you're looking for a smartwatch to free you from your phone once in a while, this is not it. In that case you're better off getting an Apple Watch Series 3 or Galaxy Watch Active 2. The Amazfit Bip S is more in line with the Fitbit Charge, a dumb sidekick that tracks your sleep and activity levels and occasion
ally lets you know when someone is trying to reach you. But for some, that's all they really need. And for $69 it's as good of a deal on an entry-level smartwatch as you can get, especially if you're on the fence about getting one in the first place.

After this review, I'm probably going to retire the Bip S for a smarter alternative. But I'm definitely going to miss that enduring battery life. And I agree with CNET's Rick Broida, who said he might even consider going back to it when he travels to savelosing yet another cable to the hotel room power outlet. But we'll see about that once "traveling" becomes a thing again.

In Remission for 10 Years: Long-term Data on CAR-T Therapy – Medscape

When a patient with cancer hears there isn't much left that doctors can do, it always stays fresh in the mind.

Doug Olson was first diagnosed with chronic lymphocytic leukemia (CLL) over 20 years ago, in 1996. For several years, his doctors used the watch-and-wait approach. But then his cancer progressed and needed treatment. By 2010, it had mutated so much that it no longer responded to standard therapy.

He was rapidly running out of options. Back then, the only treatment left was a bone marrow transplant. Without one, his doctors said, he would have 1 or 2 years left to live.

"I was really trying to avoid a bone marrow transplant. You're playing your last card if that doesn't work. It's a pretty rough procedure," Olson told Medscape Medical News.

Looking back, Olson counts himself as lucky for being in the right place, at the right time, with the right doctor. His oncologist was David Porter, MD, the principal investigator on a trial at the University of Pennsylvania that was investigating a brand new approach to treating cancer: chimeric antigen receptor (CAR) T-cell therapy.

CAR T-cell therapy uses a patient's own T cells engineered to express a receptor that targets proteins on cancer cells. CAR T cells are considered "living drugs" because they expand inside the body and stick around for years maybe for a lifetime to fight the cancer if it tries to come back.

"I was certainly intrigued by the approach. It had worked in mice, and it was the sort of thing that looked like it would work," Olson recalled.

Science is not a foreign language to Olson. He holds a PhD in medicinal chemistry, spent most of his career in the in vitro diagnostics industry, and currently acts as chief executive officer of Buhlmann Diagnostics Corp.

So he read the clinical protocol for the first in-human trial of CAR T cells and agreed to become patient number two.

Olson's T cells were harvested, engineered to attack the CD19 antigen found on malignant and normal B lymphocytes, and then were expanded into millions in the lab. After undergoing preconditioning with chemotherapy to minimize rejection and boost the CAR T cells' expansion inside the body, he received several infusions of the new therapy over the course of 3 days.

Nothing really happened for 2 weeks. Then he developed severe flu-like symptoms so bad that he was hospitalized.

Ironically, getting sick was a sign that the CAR T cells were working. Olson was experiencing one of the main short-term effects of CAR T-cell therapy: cytokine release syndrome. Symptoms include extremely high fevers and dangerous drops in blood pressure that can potentially cause end-organ damage.

In the early trials of these products, some patients experienced such a severe reaction that they needed intensive care, and some died. With increasing clinical experience, doctors have learned to control the reaction with the use of steroids and interleukein-6 inhibitors such as tocilizumab (Actemra).

Fortunately for Olson, the reaction passed, and he was eventually discharged.

Then the "aha moment" happened. Four weeks after receiving the CAR T cells, Olson found out that he was cancer free.

"It still gives me shivers," he said. "Dr Porter said, 'Your bone marrow's completely free. We just can't find a cancer cell anywhere.' "

The remission has lasted, and it is now 10 years later.

Long-term data have been accumulating for these novel therapies since Olson's treatment in 2010. This is particularly important for CAR T-cell therapy, because of its longevity. Because these are living cells and are expected to persist in the body for years, there is great interest in longer-term data, especially the risks for toxicity.

The FDA requires clinical follow-up for at least 15 years for patients treated with CAR T-cell therapy or any other genetically modified cells.

So far, most of the experience with CAR T cells comes from anti-CD19-directed therapy, which has shown "remarkable" remission rates in the 50% to 85% range, said Nirali Shah, MD, head of the hematologic malignancies section of the Pediatric Oncology Branch at the National Cancer Institute (NCI).

The most recent results presented at this year's annual meeting of the American Society of Clinical Oncology support earlier efficacy data, she noted. In the longest follow-up to date, researchers reported remissions lasting over 9 years in patients with relapsed/refractory B-cell lymphoma or CLL treated with Kite's axicaptagene cilleucel (Yescarta), one of two anti-CD19-directed CAR T-cell therapies approved by the FDA in 2017 (the other is Novartis' tisagenlecleucel [Kymriah]).

This study included 43 patients and showed an overall remission rate of 76%. Complete remission was achieved in 54% of patients, and 22% had partial remission.

The other focus is long-term safety. Although some of the long-term adverse effects are known and are manageable, others fall into the theoretical realm. In early May 2020, the NCI held a multidisciplinary virtual conference on CAR T-cell therapy "to encourage collaborative research about the subacute and potentially long-term toxicity profile of these treatments."

"We know just a little at this point about late- and long-term effects of CAR-T therapy, because we are relatively early in the era of CAR T cells," said Merav Bar, MD, from the Fred Hutchinson Cancer Research Center in Seattle, Washington.

What is known is that B-cell aplasia represents the most common long-term adverse effect of CAR T-cell therapy. B-cell aplasia results when anti-CD19 CAR-T therapy wipes out healthy B cells as well as the malignant ones responsible for leukemia/lymphoma.

As major players in the immune system, B cells are a key defense against viruses. So B-cell aplasia represents a very specific type of immunosuppression. It is generally less severe than immunosuppression that occurs after organ transplant, which hits the immune system pretty much across the board and carries a much higher risk for infection.

The main concern is what happens when someone with B-cell aplasia encounters a new pathogen, such as SARS-CoV-2.

After infection, B cells generate memory cells, which are not killed off by anti-CD19 therapy and that stick around for life. So a patient such as Olson would still make antibodies that fight infections they experienced before receiving CAR-T therapy, such as childhood chickenpox. But now they are unable to make new memory cells, so these patients receive monthly immunoglobulin infusions to protect against pathogens they have not previously encountered.

Olson takes this in stride and says he isn't overly worried about COVID-19. He follows the recommended precautions for a man his age. He wears a mask, washes his hands frequently, and tries to maintain social distancing. But he doesn't stay locked up in his New Hampshire home.

"I took the attitude when I was diagnosed with cancer that I'm going to live my life," he said. "Quality of life to me is more important than quantity."

Another problem is the possibility of neuropsychiatric toxicity. Past studies have reported a wide range of such toxicities associated with CAR T-cell therapy, including seizures and hallucinations. Most have occurred early in the course of treatment and appear to be short-lived and reversible. However, there remain questions about long-term neuropsychiatric problems.

In a long-term study of 40 patients with relapsed/refractory CLL, non-Hodgkin lymphoma, and ALL, nearly half of patients (47.5%, 19/40) self-reported at least one clinically meaningful negative neuropsychiatric outcome (anxiety, depression, or cognitive difficulty) 1 to 5 years after anti-CD19 CAR T-cell therapy. In addition, 37.5% (15/40) self-reported cognitive difficulties.

"Patients with more severe neurotoxicity showed a trend for more cognitive difficulties afterwards," said Bar, senior author of the study.

However, teasing out the role that CAR T-cell therapy plays in these problems poses a challenge. All of these patients had been heavily pretreated with previous cancer therapy, which has also been associated with neuropsychiatric problems.

"So far, we don't know what caused it," Bar said. "Nevertheless, people need to pay attention to neuropsychiatric symptoms in CAR T-cell therapy. It is important to continue to monitor these patients for these issues."

Another potential problem is graft-vs-host disease (GVHD). This is not uncommon after hematopoietic stem cell transplants. It develops when the donor T cells view antigens on healthy recipient cells as foreign and attack them.

For patients who are treated with CAR T cells, GVHD is mostly a concern among individuals who have previously had a transplant and who are already at increased risk for it.

In a study of late effects among 86 adults treated with anti-CD19 CAR T cells for relapsed/refractory non-Hodgkin lymphoma, Bar and colleagues found that GVHD occurred only among patients who had received a previous donor stem cell transplant. Of these, 20% (3/15) developed GVHD about 28 months after CAR-T therapy.

"The data for CAR T cells causing GVHD really hasn't shown that it's a huge problem, although we have seen it and are continuing to monitor for it," the NCI's Shah commented to Medscape Medical News.

A range of other long-term adverse effects have been reported with CAR-T therapy, including prolonged cytopenias (reduced mature blood cells), myelodysplasia (bone marrow failure), and second malignancies.

In the study with the longest follow-up to date, 16% (7/43) of patients developed second malignancies, which is comparable to data from Bar's study in Seattle (15%, 13/86). The researchers in this study consider this rate to be no higher than expected: these patients had already received extensive chemotherapy, which increases the risk for other cancers, they point out.

However, this brings up theoretical concerns about the long-term effects of gene modification. CAR T cells are engineered using retroviruses (mainly lentiviruses), which randomly insert the CAR genes into the host genome. Doing so may cause mutations that could promote cancer. These lentiviruses also carry the theoretical risk of becoming capable of viral replication once inside the body.

To address these concerns, viruses used to engineer CAR T cells go through comprehensive safety testing. After therapy, patients are checked every few months during the first year and annually after that.

So far, there have been no reports of cancers associated with CAR T-cell therapy.

"Any type of cancer is a very theoretical risk," Bar told Medscape Medical News. "Most likely the malignancies in our study were related to prior treatment that the patients received. None of them had any evidence of replication-competent lentivirus, or any other evidence that the malignancies were related to the CAR T cells."

Another theoretical concern is the possibility of new-onset autoimmune disease, although, once again, no cases have been reported so far.

"We think of it as a theoretic possibility. Whenever you jack up the immune system, autoimmune disease is a potential risk," said Carl June, MD, director of the Center for Cellular Immunotherapies at the University of Pennsylvania.

June was the coprincipal investigator of the trial in which Olson participated. He is also the inventor on patents for CAR T cells licensed by the University of Pennsylvania to Novartis and Tmunity and is a scientific founder with equity in Tmunity.

Still, autoimmunity could occur, and scientists are looking out for it.

"We are continuing to be vigilant in our monitoring for autoimmune disease," Shah added. "We've been doing CAR T-cell therapy since 2012, and I think we have yet to see true autoimmunity beyond GVHD."

In the 10 years since Olson received CAR T-cell therapy, an entire industry has sprung up. Over 100 companies worldwide are now developing CAR T-cell therapies targeting various antigens. These therapies are directed at about 60 different tumor types, including solid tumors. Nearly 200 clinical trials are underway, though most are still in early stages: as of September 2019, only 5% had reached phase 3.

Clinical data show promising results for CAR T-cell therapy directed against CD22 (overexpressed on ALL cells), and BCMA (found on almost all multiple myeloma cells). Yet questions remain as to whether CAR T cells will be as effective if they target antigens other than CD19 or cells other than B lymphocytes. One of the biggest research questions is whether they will be effective against solid tumors.

One research avenue being watched with great interest is the development of universal CAR T cells. So far, such products are at very early stages of development (phase 1 trials), but they are attractive because of the potential advantages they offer over bespoke CAR T cells. Automating the process holds the promise of immediate availability, standardizing production, expanding access, and lowering costs. And because the T cells for this universal product come from healthy donors, they may function better than T cells that have been battered and bruised by past cancer treatments, or even the cancer itself.

However, precisely because they are developed from healthy donor T cells, universal CAR T cells may pose increased risk for GVHD. Scientists are trying to get around this problem by engineering universal CAR T cells that lack the T-cell receptor involved in GVHD.

There are also other concerns. Nature has a penchant for mutation. Engineering CAR T cells without T-cell receptors means the body may no longer detect or reject a universal CAR T cell if it goes rogue. Also, gene insertion in universal CAR-T therapy is targeted rather than random (as in bespoke CAR T cells), which could create off-target effects. Both issues create a theoretical risk of such products inducing an untreatable CAR T-cell therapyassociated cancer.

"The theoretic risk with universal cells is that their safety profile may not be as good for long term," June commented.

From that first trial in which June and Porter used CAR T cells, two of three patients they treated are still alive 10 years later.

Olson is one of these two, and he still undergoes monitoring every 3 months to check for relapse. So far, none of his tests have shown signs of his cancer returning.

After going into remission, Doug spent the next 6 to 9 months regaining his health and strength.

"I figured if I had this amazing treatment that saved my life, I had an obligation to stay alive," he said. "I'd better not die of something like a heart attack!"

He took up long distance running and has completed six half marathons. He became involved in the Leukemia and Lymphoma Society, participating in fund-raising and helping newly diagnosed patients. Over the years, he has also given talks for researchers, people with cancer, and healthcare providers.

Doug is now 73. Today, he marvels at how rapidly the CAR-T field has progressed.

"Twenty years ago, if you had cancer, your prospects weren't nearly as good as these days. In 2010, people still didn't believe in CAR T-cell therapy," he said. "My goal always in telling my story is a message of hope."

How research groups are tackling the problem of biologic drug delivery – Pharmaceutical Technology

Over the past few decades, the rise of biologics has been nothing short of meteoric. Between 2010 and 2017, around a quarter of the new molecular entities approved by the US Food and Drug Administration (FDA) fell into this bracket (63 out of a total of 262). That may not sound like a lot, but since biologics are much more expensive than small molecule drugs, they account for most of the worlds top-selling drugs by revenues. They also represent one of the fastest growing categories in pharma.

Unfortunately, biologics have a major downside when it comes to delivery. Unlike small molecule drugs, which can generally be taken orally, biologics are large, complex entities that degrade rapidly in the gastrointestinal tract. Weighing between 200 and 1,000 times the size of a small molecule drug, they almost always need to be delivered via injection or infusion. This is far less convenient than swallowing a pill, not to mention far more costly.

To take diabetes management as an example, most people with diabetes still rely on multiple daily injections. Inhalable insulin has been tried out (as per Exubera in 2006 and Afrezza in 2014) but both suffered with safety concerns and poor sales volume. Oral insulin the holy grail of diabetes treatment has yet to hit the market.

There are dozens of groups around the world intent on creating oral delivery methods. Some focus on modifying the drugs chemical structure to improve their stability in the body. Others focus on the epithelial barrier in the gut if you can make that more permeable, large molecules will be able to pass through more easily.

To give an example of the first approach, researchers at EFPL in Switzerland are working on a method to deliver peptides orally. In 2018, the team developed double bridged peptides a structure with much more stability than a typical amino acid chain. Unfortunately, despite this added stability, most of them still disintegrated in the digestive tract.

Since then, the researchers have found a way to trawl through a peptide library (billions of random peptide sequences, twisted into the double bridged format) to identify the ones that actually are stable. After they have isolated the surviving candidates, they do further tests to find the ones that bind to the disease target.

Its a bit like searching for a needle in a haystack, and this method makes this easy, said Professor Christian Heinis, the lead researcher.

An example of the latter approach comes from Israeli biotech Chiasma. Its Transient Permeability Enhancer (TPE) aims to protect drug molecules from digestive enzymes, as well as triggering the temporary expansion of tight spaces in that gut barrier.

This gives just enough space for the drug molecule to pass into the bloodstream, but not enough space for viruses and bacteria. Last year, the company completed a phase III trial on oral capsules called Mycapssa, which could be used as a maintenance treatment for acromegaly.

There are also a number of oral delivery approaches involving nanoparticles. As the thinking goes: if you incorporate the drug into tiny transporting particles, those particles will be able to protect it from stomach acid, as well as accurately targeting its delivery.

Researchers at Houston Methodist Research Institute in Texas are trialling one such approach, in which a peptide-based drug is chemically linked to fatty acids, and packaged in a nanoparticle. In mouse studies, the animals absorbed almost a quarter of the drug dosage (a lot, considering the typical oral bioavailability for a biologic stands at around 1%).

We know the human body can absorb fatty acids, so we decided to chemically link biological drug molecules to fatty acids to see how well these drugs are absorbed into the gastrointestinal system. It turns out that our transporter approach was effective, saidDr Haifa Shen, Professor of Nanomedicine at Houston Methodist.

Nanotechnology could have many further applications within drug delivery. In particular, it could be used for delivering drugs across the blood-brain barrier, a network of blood vessels that functions a bit like neurological armour. While this barrier protects the brain from harmful chemicals, it also prevents therapeutic agents from getting through.

Researchers at Cedars-Sinai Medical Center in California are working on a new type of nano-immunotherapy, which could deliver cancer drugs directly into brain tumours. Again, they are at an early stage of research, but mouse studies have shown promising results.

Current clinically proven methods of brain cancer immunotherapy do not ensure that therapeutic drugs cross the blood-brain barrier, said Dr Julia Ljubimova, Professor of Neurosurgery and Biomedical Sciences at Cedars-Sinai. Although our findings were not made in humans, they bring us closer to developing a treatment that might effectively attack brain tumours with systematic drug administration.

Another exciting research avenue is microneedles. The promise is clear a minimally invasive, painless technique that could deliver the drug through the skin.

One example is a smart, adhesive insulin-delivery patch, being developed by bioengineers at UCLA. The patch, which is adhesive and resembles a sticking plaster, is pre-loaded with insulin and releases the medicine once blood sugar levels exceed a certain threshold. The technology has been accepted into the FDAs Emerging Technology Program, and human trials could start within the next few years.

US biotech Rani Therapeutics is combining both approaches microneedles and oral biologics delivery with its so-called robotic pill. This capsule, which could be used to deliver a number of biologics, moves through the stomach intact, before reaching the desired location in the gut. Here, it releases biodegradable microneedles, which inject the biologic.

This delivery system is still at an early stage of development, but in preclinical tests it performed as well as a subcutaneous injection. The company also has one human trial to its name a phase I trial for a drug called octreotide, which is normally injected intravenously.

While these research groups, and many others, are seeing promising results in the lab and early trials, it remains to be seen which new delivery methods will become a clinical reality. Given the challenges associated with existing delivery methods, and the seemingly unstoppable rise of biologics, there is everything to play for.
