iBio and Planet Biotechnology Enter into Exclusive Worldwide License Agreement for the Development of a COVID-19 Therapeutic – GlobeNewswire

NEW YORK, Aug. 28, 2020 (GLOBE NEWSWIRE) -- iBio, Inc. (NYSE AMERICAN:IBIO) (iBio or the Company), a biotechnology innovator and biologics contract manufacturing organization, today announced that it has entered into an exclusive worldwide license agreement with Planet Biotechnology Inc. (Planet) for the development of Planets COVID-19 therapeutic candidate, ACE2-Fc.

ACE2-Fc is a recombinant protein comprised of human angiotensin converting enzyme 2 (ACE2) fused to a human immunoglobulin G Fc fragment (Fc). As an immunoadhesin, ACE2-Fc targets the coronavirus virions directly by using the ACE2 extracellular domain as a decoy to bind the spike protein and block infection of healthy cells, while the fused Fc domain prolongs the life of the protein in circulation. The design is expected to bring the benefit of a traditional neutralizing antibody while prospectively limiting the potential for viral escape given that ACE2 is also the target receptor for coronavirus for cell entry. Planets in vitro studies demonstrated that its ACE2-Fc blocks SARS-CoV-2 virus from infecting Vero E6 cells.

Under the terms of the agreement, iBio obtained an exclusive license to Planets ACE2-Fc. Planet is eligible to receive certain pre-specified payments upon achievement of clinical development milestones.

Australia can take the lead in biotechnology revolution | The Strategist – The Strategist

The Australian government needs to develop a bold vision and strategic plan to create the data-driven healthcare system and bioeconomy of the future.

Biotechnology dates back over 6,000years to the domestication of wheat and the use of yeast for fermentation, but has entered the province of human design and invention as a consequence of the gene-cloning, gene-manipulation and genome-sequencing revolutions of the past 50years.

While often thought of in terms of drug development and genetic engineering, biotechnology is fundamentally becoming an information industry with a universe of applications.

Humans have acquired the ability to read the genetic programming and analyse the structure of the molecules of lifehow cells work, how were different from each other, and what makes a lime different from a lemon. Life has evolved the most exquisite nanomachinesprotein machines that can bind oxygen and almost any other type of molecule; trap photons to turn carbon dioxide and water into carbohydrates; detect electric fields or sound waves and turn them into visual images; pump ions; facilitate the entry of viruses, bacteria and parasites into cells; and make molecular motors that spin or contract to trap prey, exert force and move.

The extraordinary advances in DNA sequencing are leading to an avalanche of genomic information. Genetic engineering can now be done with high speed and precision. The pace of change is accelerating, and biological technologies are intersecting with optical technologies, nanotechnologies, advanced computing and artificial intelligence to create possibilities that were, if not beyond imagination, well beyond feasibility just a few years ago.

The advent of population-scale genomic and smart sensor data will transform health care, health economics, medical research and drug development. It will also transform the national economy and create digital products that can be exported to the world. To achieve this, the Australian government needs a strategic plan for acquiring genomic, clinical, pharmaceutical, sensor and patient self-reporting data and for providing trusted online evidence-based analysis of that information and advice to healthcare practitioners and citizens.

Governments also need to convince citizens of the personal and national value of big data in health care, and the privacy and security of that data, using a model that combines political leadership with subject-matter expertise.

The Australian government should establish a secure national repository for genomic and phenotypic data for research and healthcare use. This de-identified data should be made available to Australian researchers and health system managers. It could be provided to external parties, such as pharmaceutical companies, under appropriate conditions in exchange for early access to expensive treatments or shared benefits.

The Australian government should establish a central unit to assemble and supply evidence-based, well-curated and continuously updated genomic analysis, integrated with clinical and other data, and linked to national treatment guidelines, for decision support at the point of care.

The cost of doing this will be trivial compared with other costs in the healthcare system, and trivial in relation to the cost reductions and healthcare improvements that it will enable. This work could be funded from the Medical Research Future Fund and publicprivate partnerships.

Smart sensors should be made available through Medicare and routinely installed in hospitals, clinics and other healthcare settings, such as aged-care homes.

Conversion to electronic health records should be mandatory for all healthcare providers receiving government support or reimbursement in Australia, as recommended by the Australian Academy of Technology and Engineering.

Genetic tests should be progressively upgraded to whole-genome sequencing to build the national genomic estate. That would convert diagnostic expenses into an enduring strategic asset.

All suitably accredited health professionals, including general practitioners, should be able to obtain a genomic report, pharmacogenomic advice, early warning of an incipient disease, or any combination of that information, about a patient (with patient approval) upon request, and bulletins issued where relevant. Citizens should also be entitled to access such information in the interests of caring for their own health, provided it is conservative and actionable.

Security and police agencies should establish internal expertise in biotechnology and biodata analysis. They should have access to national genomic information under defined circumstances and subject to judicial approval and oversight.

Major investment funds and relevant government departments should consider establishing bio-intelligence units to keep abreast of opportunities and incorporate those opportunities into their strategic planning. Australian research funding agencies and the CSIRO should continue to invest in advanced genetic engineering and vaccine and drug development. Superannuation funds should be encouraged to invest at least 1% of their resources into domestic start-up and early-phase high-tech and digital enterprises, especially in health and biotechnologies.

Universities should provide training in computer programming and big-data analysis as a core component of all science and engineering degrees.

Phase 1 Trial Suggests PD01A Is Safe in Parkinson Disease Treatment – Neurology Advisor

Repeated administrations of PD01A were found to be safe and well tolerated over an extended time period in patients with Parkinson disease (PD), according to phase 1 study results published in Lancet Neurology.

PD01A is an immunotherapy that targets oligomeric -synuclein, a potential driver of neuronal dysfunction in patients with PD. Study researchers sought to evaluate the safety and tolerability of PD01A in patients with PD. A total of 24 patients with PD aged between 45 and 65 years (mean age, 54.9 years) were recruited from a single center in Austria and enrolled into this first-in-human phase 1 trial of PD01A.

All patients were on a stable treatment regimen for PD for 3 months prior to enrollment. Patients were randomly assigned to receive 4 subcutaneous administrations of either 15 g (n=12) or 75 g (n=12) PD01A that were injected into the upper arms. These patients were followed up for 52 weeks and then an additional 39 weeks.

After the 39-week follow-up, patients were randomly assigned to receive the first booster immunization with PD01A at 15 g (n=10) or 75 g (n=12). The follow-up period after the second randomization was 24 weeks. Patients then received a second booster of 75 g PD01A and were followed up for another 52 weeks. Study researchers examined the primary outcomes safety and tolerability with patient diary data and investigator assessments at each visit. These outcome assessments focused on all local or systemic treatment-emergent adverse events (TEAEs).

Approximately 87% of patients (n=21) received all 6 administrations of PD01A and completed between 221 and 259 weeks in the study. All patients in the study experienced 1 adverse event (AE), but the majority of these events were deemed unrelated to treatment from this study. Local injection site reactions, while transient, were the only treatment-related AE reported in this study (n=23). A total of 500 TEAEs were reported by all patients across the follow-up period. The systemic TEAEs possibly related to treatment and reported in 2 patients were fatigue (n=4), headache (n=3), myalgia (n=3), muscle rigidity (n=2), and tremor (n=2).

Investigators observed no abnormalities on MRI after baseline; however, they observed 1 treatment-unrelated microhemorrhage event in the 15 g group. In the 15 g dose group, the geometric group mean titer of antibodies against the immunizing peptide PD01 increased from a baseline of 1:46 to 1:3580 at 12 weeks. In the 75 g dose group, the geometric group mean titer of antibodies increased from 1:76 at baseline to 1:2462 at week 12. Over a 2-year period, antibody titers returned to baseline. These titers were capable of being reactivated following a booster immunization from week 116 onwards, subsequently reaching geometric group mean titers of up to 1:20218.

Limitations of the study included the small sample size, the lack of a placebo group, and the inclusion of patients from a single center in Austria, which may reduce generalizability of the findings.

Investigators concluded that their findings of a sustained -synuclein-specific antibody response, suggests immunization with PD01A might be a promising strategy for long-term management of PD.

The path to seizure freedom can be a long and frustrating journey. We often have to try different medications or combine several treatments before finding a suitable treatment with minimal side effects. 1,2 For this reason, it is essential that we try to get our patients on the right treatment plan right from the start. The challenge HCPs and patients face is that each patients needs are different and reaching the right dose of an anti-seizure medication (ASM) is a journey unique to every patient. Understanding the barriers patients often face can steer us to the right path for a smoother journey.

The Challenge of Dosing

One of the biggest challenges in treating epilepsy is the process of getting patients to their target dose of anti-seizure medication by increasing the dose incrementally (called titration) as quickly and safely as possible while minimizing side effects. However, reaching the target dose is a journey, and each patients journey is different.1,3 During this process, the number of doctor visits increases for the patient as the care team monitors how well the drug is working and how side effects are tolerated.1,4 Each patients response to treatment is also different and unfortunately, we cant know if a drug is effective for someone until their titration is complete. Meanwhile, the financial and time burdens associated with these additional visits and testing can add stress for patients and their families.1,3,4

Another significant part of the seizure freedom journey is finding the right dose frequency (dosage) for a patient that also fits the needs of their lifestyle. For many medications, the higher the dose, the more chances for adverse events to interfere with quality of life.5 At the same time, the complexity of the regimen, such as multiple drugs or multiple doses per day, may lead to poor compliance. Knowing details of a patients day-to-day living, such as a change in diet or a new work schedule, can be incredibly useful when making treatment decisions and considering dosing schedules right from the start.

Balancing the Effects of Dose Titration

Juggling multiple medications and adjusting to treatment schedules can be stressful for a patient. There is also the related worry that a seizure can occur while waiting for the new medication to reach its target dose.

Medications requiring shorter titration periods may help avoid uncontrolled seizures by getting patients to their target dose faster.6 Additionally, drugs that stay in the patients body longer, sometimes referred to as a longer half-life, may help in the inevitable case of missed doses.7,8 These drugs require less frequent dosage, are associated with better adherence and may mitigate concentration fluctuations in the body.7,9

As health care professionals, it is important that we consider a dosing plan as we explore an appropriate treatment regimen for each patient. Our goal is to get patients on the most effective dose of medication with the simplest regimen for them as quickly as possible while minimizing side effects.1 But to achieve that, we need to engage our patients in a clear, ongoing discussion about their lifestyle, causes of missed doses, tolerance for side effects, and potential solutions that make sense for that individual.

By having these discussions, we can help reduce the burden of repeat office visits, minimize the risk of ongoing seizures, and lower overall healthcare costs, all of which leads to an overall better patient experiencesomething we should all strive for in our practices.1,7

Neurologists Are Treating Alzheimer’s Disease by Sending Electrical Currents Deep in the Brain – Science Times

Bill Gates and other philanthropists from the US have granted the Imperial College London and the UK Dementia Research Institute a $1.5 million grant to put to the test their new technology for Alzheimer's disease.

The neurologists chose 24 Alzheimer's disease patients in their early stages to participate in the therapy. Experts have high hopes for this trial after dozens of dementia drugshave failed tests.

Neurologists from Imperial College London and the UK Dementia Research Institute received a $1.5 million grant from Bill Gates and other US philanthropists to start treating Alzheimer's diseaseby sending electrical currents deep in the brain.

The technology is called the temporal interference brain stimulation, which will be participated by 24 patients with early-stage Alzheimer's disease. Electrodes will be attached to their scalp during the trial that will last for two weeks with daily hour-long sessions.

The electrodes will be sending two harmless-high-frequency electric currents, 2,000 Hz and 2,005 Hz, into the brain. When these two slightly different frequencies meet, they create a third current that is a low-frequency wave of 5 Hz.

Neurologists are hoping that this third electric current will make a difference in the treatment of dementia. The frequency will be triggered in an area deep in the brain responsible for new memories, called the hippocampus.

Hopefully, the process would revive the mitochondria in that area of the brain. Mitochondriais the powerhouse of a cell that becomes damaged when a person has Alzheimer's disease.

Unlike the two original beams, the third frequency is not too high a frequency to interfere with the healthy brain tissue. It will only have a similar rate at which the brain cells fire that allows them to spark the affected neurons back to life.

The researchers also tested their new technology on healthy patients. They found an increased blood flow in the brain and improved facial-recognition results.

But it will be the first time to test the new technology on patients with Alzheimer's disease that is scheduled in January next year.

The new technology which aims to treat Alzheimer's disease will target the mitochondria that are damaged by the disease. According to researcher Dr. Nir Grossman, more and more evidence has come to light that mitochondrial dysfunction has a vital role in the progression of Alzheimer's disease.

He added that this is an essential milestone in their research after years of studying and working on breakthrough technology.

An estimated 850,000 people in the United Kingdom have dementia, and among these numbers around 500,000 have Alzheimer's disease.

Bill Gates, Mikey Hoag, and the US Alzheimer's Association have given 16 grants to different institutions including this new trial, with a total of $60 million as part of their Cloud Program.

Gates said that finding a treatment for dementia needs increased and continued research after witnessing first-hand what dementia could do to people.

He has been in the forefront of defining clinical, electrophysiological, and IVIG responsiveness of multifocal motor neuropathy (MMN), a disease close to his heart.He has published extensively on chemotherapy-induced polyneuropathies (CIPN) from taxol, vincristine, suramin, thalidomide, and bortezomib. These papers have emphasized the dose-dependent nature of neuropathy and patterns of length-dependent distal symmetric neuropathy versus the nonlength dependent neuropathy.

"I continue to be involved in trials of myasthenia gravis, amyotrophic lateral sclerosis (ALS), and amyloid neuropathy. Electrophysiology remains the cornerstone of all my research and I have worked on several projects including motor unit estimates, motor unit number index, neuronal excitability studies, and electrophysiological methodological studies in ALS," Dr. Chaudhry said.

Dr. Chaudhry has been active in the AANEM for many years. He served as chair for both the AANEM CME Advisory Committee and the Journal Committee. He was a member of the Neuromuscular Vision Task Force, Membership & Ethics committee, and has served on the Muscle & Nerve Editorial Board. Dr. Chaudhry was also actively involved with ABEM, serving as an examiner for the ABEM certification exams in 2002, 2003, and 2007- 2013 and as the ABEM Secretary. AANEM is important to Dr. Chaudhry.

AANEM is a special organization of the physicians, by the physicians, and for the physicians practicing neuromuscular and electrodiagnostic medicine, said Dr. Chaudhry. AANEM meetings make me feel completely at home, providing the perfect opportunity to lean on, teach and interact with colleagues practicing in your field."

Dr. Chaudhry is currently a Professor of Neurology at John Hopkins University School of Medicine and Director of Neurology EMG Laboratory at the John Hopkins Hospital. At Johns Hopkins, he has also served as the Vice Chair of Clinical Affairs and Director of the Neurology Outpatient Center (2002-2013), and Director of Clinical Neurophysiology/Neuromuscular Fellowships (1996-2005), and Director of Neuromuscular Division (2001-2004). Dr. Chaudhry has been a member of AANEM since 1989 and has presented at over 20 sessions and symposiums at AANEM Annual Meetings since 1997. Dr. Chaudhry received his medical degree from the All-India Institute of Medical Services in New Delhi, India. He became house officer in internal medicine at Preston Hospital & Tynemouth Victoria Jubilee Infirmary in North Shields, England and senior house officer in internal medicine at Llanelli Hospital in Wales, United Kingdom. He obtained membership in the Royal College of Physicians (M.R.C.P.), equivalent to board certification in internal medicine. He went on to complete a residency in neurology at the University of Tennessee Center for Health Sciences and the University of Alabama in Birmingham. He was the chief resident during his last year of training. He then trained at Johns Hopkins, receiving subspecialty training in clinical and research aspects of neuromuscular diseases. Dr. Chaudhry has also been active with the American Academy of Neurology (AAN) serving on the board of directors from 2007 to 2011 and the chair of NM section of AAN (2001-2005). It was under his leadership that the Accreditation Council for Graduate Medical Education (ACGME) subspecialty NM examination was created.

In the Age of Digital Medicine, This Humble Medical Tool Hangs On – Medscape

Receiving a diagnosis in 2020 at least one made at a medical center outfitted with the latest clinical gadgetry might include a scan that divides your body into a bread loaf of high-resolution digital slices. Your DNA might be fed through a gene sequencer that spits out your mortal code in a matter of hours. Even your smartphone is being used to uncover health problems and could maybe even help in detecting COVID-19 symptoms.

Yet nearly 130 years since its inception, after decades of science has mapped out our neuronal pathways, a simple knob of rubber with a metal handle remains one of medicine's most essential tools. I'm referring to the cheap, portable, easy-to-use reflex hammer.

This unassuming device can be invaluable in diagnosing nervous and muscular disorders, and in determining whether a patient's pathology lies in the brain or elsewhere in the body. It can also help curtail healthcare spending by preventing unnecessary, often expensive testing. Yet like so many major medical and scientific discoveries, the reflex hammer has humble origins in this case, the basement of a Viennese hotel.

The inn was run by the father of Leopold Auenbrugger, an 18th century doctor who is considered to be among the founders of modern medicine. To gauge how much wine was left for customers, hotel employees would thump casks with their hands and listen for a dull thud or hollow tympany. Auenbrugger realized that the same technique, now called "percussing," could be applied to the human torso to, say, determine how much fluid had built up around a diseased heart. He wrote as much in his 1761 paper "New invention to detect diseases hidden deep within the chest."

Leopold Auenbrugger (here with his wife, Marianne) is regarded as one of the founders of modern medicine, having applied the idea of detecting disease by sound.

Thought to be more accurate than the human hand, it wasn't long before percussion hammers were being designed to more precisely diagnose disease. Competition ensued.

Scottish physician Sir David Barry's model, released in the 1820s, was the first. German doctor Max A. Wintrich's came shortly after and was more popular but was not without its critics: "[Wintrich's hammer] is inconvenient to hold, it is rigid ... it required education to use it, and even then it does not fulfill its purposes," a rival inventor commented.

As neurologist Douglas J. Lanska wrote in a 1989 paper on the many types of reflex hammers, "Some were T-shaped or L-shaped, others resembled battle axes, tomahawks, or even magic wands." He adds that no material was off-limits: wood, ebony, whale bone, brass, lead, even "velvet-covered worsted" (a type of yarn).

As percussion hammer warfare waged on, doctors and scientists were also beginning to understand the concept of reflexes, or involuntary, near-immediate responses to stimuli that occur before any sensory information reaches the brain. Muscular jerks. Blinking. Sneezing. Gagging. All of these are automatic feedback loops between sensory and motor neurons that help us navigate our environment and protect us from danger.

In 1875, German neurologists Heinrich Erb and Carl Friedrich Otto Westphal were among the first to realize that eliciting a reflex by briskly tapping the tendons of major muscles might be useful. They felt that the knee jerk or "patellar-tendon" reflex in particular could help assess nerve function.

Anton Wintrich introduced this percussion hammer model in 1841.

Hammers specifically suited to test reflexes were soon developed, the first of which had the now classic shape we're accustomed to, a thin metal handle with a triangular rubber head. Designed by American physician John Madison Taylor in Philadelphia in 1888 and modified ever since by many the simple device was heavy enough to elicit reflexes and had round edges to ease impact. An entry-level model runs just $6.99 on Amazon.

The Krauss hammer, developed by German-American physician William Christopher Krauss, was designed around the same time. It had two rounded heads: a large one for knees and a smaller one for biceps. Dr Ernst L.O. Trmner's did too, but it tapered to a thin end to assess skin reflexes. There were also the Queen Square hammer, the Babinski hammer, the Buck hammer, and the Berliner hammer. The Stookey hammer flaunted a camel hair brush to get a better sense of touch sensation. The list goes on.

Daniella C. Sisniega is a neurology resident at Mt. Sinai Medical Center. At the 2017 American Academy of Neurology annual meeting, while still a medical student, she presented a poster explicating the reflex hammer's past. "I'm fascinated by how the reflex hammer started out as a percussion hammer but was [then] adapted to elicit reflexes and has been in every neurologist's tool box ever since," she says.

"I also did not know that the little rubber triangle was the first reflex hammer. I feel like I owe it an apology!" she joked, referring to the often lackluster quality of the inexpensive Taylors.

"The little tomahawk is included in the kit everyone receives when they enter medical school," she recalls. "The rubber is cheap and very light, while the other hammers are heavier on the head so that you can use the 'swing' of the hammer as opposed to the strength of the strike to test the reflex."

While welcoming technological advances in medicine, Mt. Sinai School of Medicine neurologist and multiple sclerosis expert Stephen Krieger doesnt hedge on the role of the reflex hammer in modern medical diagnosis.

"We could argue about the nuances of the hammer the Queen Square, the Tomahawk, plastic handle, metal handle, weighted, flexible or rigid but the hammer itself is always in the hand. Reflexes tell the story of neurologic diseases of all sorts," he says.

Krieger explains how disorders of the brain, like a stroke or brain tumor, result in hyperactive reflexes, while conditions affecting muscles and peripheral nerves usually result in reduced or nonexistent reflexes. Reduced reflexes, for example, are a common symptom of back pain due to degenerative disk disease.

Dr Andrew Wilner, a long-time Medscape contributor and staff neurologist at a county hospital in Memphis, Tennessee, recounts the story of one of his patients, who had back pain, weakness, and numbness of the legs. Wilner was leaning toward a diagnosis of either Guillain-Barr syndrome (GBS) an autoimmune disorder of peripheral nerves or a myelopathy, an injury of some kind to the spinal cord. Both conditions can lead to medical emergencies, but each requires drastically different treatment.

"The reflex hammer was arguably our most important tool in narrowing down the differential diagnosis," he says. "Had we found diminished or absent deep tendon reflexes, GBS would have been more likely. As it turned out, the patient had brisk pathological knee jerks, pointing to a lesion in the brain or spinal cord."

On the basis of these findings, Wilner ordered an imaging study of the patient's spinal cord, where a lesion was found as opposed to pursuing the costly tests involved in a GBS diagnosis.

Wilner feels that the simple art of interviewing and examining a patient can get overshadowed by the myriad new diagnostic technologies. When it comes to clinical tools, he feels that sometimes basic is better.

"Technology is glorious," admits Krieger, "and [it] will teach us things about patients that we could never have known or imagined. But the simple, elegant, inexpensive, almost plebeian swing of the reflex hammer has a cost-benefit ratio that I think no advanced technology will likely ever match."

New Research Finds Troubling Link Between COVID-19 and Neuropsychiatric Health – Psychiatric Times

From the beginning of the COVID-19 outbreak, clinicians have known that the virus can cause acute respiratory distress. Anew study suggests it may have neuropsychiatric effects as well.

That is according to a team of British scientists, who recently published a paper on neuropsychiatric complications stemming from COVID-19. In an online edition of The Lancet, the researchers reported an array of symptoms, from strokes to altered mental states like new-onset psychosis.1 Surprisingly, they discovered that acute alterations in mental status were disproportionately overrepresented in younger patients.

Up until now, published reports on COVID-19s neuropsychiatric effects have been largely anecdotal, limited to individual cases or a small case series. The British study is the first nationwide survey, which was based on 153 cases submitted by clinicians across the United Kingdom. It was undertaken to explore the proportion of neurological and psychiatric complications that affect the central nervous system versus the peripheral nervous system, as well as who was most at risk for these symptoms.

The researchers found a number of psychiatric complications. Altered mental status was the second most common presentation, they wrote. Of patients with complete datasets, 31% received a psychiatric diagnosis. Clinicians reported 4 cases of affective disorders, 6 cases of dementia-like symptoms, and 10 cases of new-onset psychosis. There was also 1 case of catatonia and another of mania. Only 2 patients had exacerbations of existing enduring mental illness.

According to the reports authors, altered mental states are common in cases of severe infection, but this symptom typically predominates in older groups.The COVID-19 data told a different story. Almost half of the patients with psychiatric complications from COVID-19 were 59 years or younger.

The authors offered at least 1 possible explanation. The large number of patients with altered mental status might reflect increased access to neuropsychiatry or psychiatry review for young patients, they wrote. But the exact relationship between COVID-19 and mental health remains a mystery, especially where younger individuals are concerned. The authors insist that confirmation of the link between COVID-19 and new acute psychiatric or neuropsychiatric complications in younger patients will require detailed prospective longitudinal studies.

Although further study may be necessary to draw firm conclusions, one thing is certain: Severe neurological and neuropsychiatric presentations associated with COVID-19 have become increasingly apparent.
