A U.S. bankruptcy judge has rejected Hertz's controversial employee incentive plans, calling them "offensive."

After hearing testimony from all sides and allowing for the cross-examination of a few witnesses in a Delaware courtroom Thursday afternoon,Judge MaryWalrath struck them down, going along withthe objectors.

Objectionscame from a group of former executives, who fear the big payouts will jeopardize their promised retirement income from the company, and the U.S. trustee overseeing Hertz'sChapter 11case, who questionedthe real intentions and purpose behind them.

While Walrath spurned the incentive plans, she left the door open for Hertz to come back with revised ones, moving to continue the hearing and her final decision to a later date.

As it fights to drive itself out of bankruptcy, Estero-based Hertz Global Holdings parent of The Hertz Corp. has argued suchincentives are critical for motivating and keeping the employees in its management ranks and necessary to compensate them for their extra hard work in challenging times.

As written, the plans could have resulted in payouts of more than $14.6 millionto hundreds of employees, including its top brass.

In her ruling after anhours-long hearing, Walrath said it seemed offensive to award senior executives bonuses as they're the very same ones who got hefty retention payments a few days before the company'sbankruptcy filing, even though they "weren't entitled to them under the debtors' ownpolicy."

Hertz doled out more than $16.2 million in "cash retention payments" to what it described as "a broad base of key employees at the director level and above," raising some eyebrows.

Under the incentive plan proposed for top executives, a little over a dozen of Hertz's highest-ranking employees could have earned more than $5.4 million in additional bonus pay if Hertz achievedits highest performance goals.

More than 50 others who received retention pay would have been able to collect more money too under a second incentive plan.

Hertz filed for bankruptcy on May 22 after seeing its rental revenuevirtually vanish and the value of its rental fleet plummet with the spread of the coronavirus, leaving its U.S. operations in dire financial straits and unable to meet its heavy debt load.

While Hertz may have slipped into bankruptcy because of the outbreak of COVID-19 the incurable, sometimes fatal disease caused by the coronavirus and not due directly to any wrongdoing or mismanagement, Walrath said it shouldn't mean its leaders are entitled to bonuses because of the unforeseen disaster that's made their lives harder.

"There a lot of people who were affected by COVID," she said. "A lot of people have had their compensationcut as a result of COVID. Many people have been furloughed and lost their jobs completely."

In making her decision, Walrathquestioned the validity of the incentive plans, saying she had "serious problems" with them, including the three levels of metrics Hertz proposed to use as triggers for the payouts.

The judge arguedemployees should not be givenrewards for not allowing Hertz's situation to get any worse or for keeping conditions going in the direction they're already headed as proposed in the plans. Thepayouts, she said, should be based on meeting much harder goals or true "stretches," if they're really designed to be incentives.

"I am very concerned about the tying of these metrics to what the debtor is already doing," she said.

As a judge, she saidshe'd never seen a situation where a company paid out millions in bonuses days beforea bankruptcy filing, only to come back a few months later and ask for thesekinds of incentives post-bankruptcy.

"I think more has to be established on why the employees who got their retentionbonuses and agreed to stay with the company are not going todo their best to make sure this company survives and succeeds," Walrath said.

She questioned why the request for the incentiveshave come so soon, when the company has yet to file a reorganization plan.

Walrath suggested Hertz change its metrics or come up with better arguments for why it chose the ones it did to address a "gut feeling" that the incentives aren'tnecessary or may not be "appropriate at all."

In his testimony, Hertz's attorney Jason Zakia defended the incentive plans, arguing they were crafted in everyone's best interest by consultants and outside directors who would not directly benefit from them.

As for the metrics, he said, none of them would allow the company to be "simply put on autopilot."

Zakia argued the metrics and goals can't be so difficult to meet that they're seen as unachievable or unattainable because they would then "serve no incentivizing purpose whatsoever."

"Indeed, they'd serve no purpose for anyone whatsoever," he said.

"You need to strike a balance between something that is achievable, possible, but something that's incentivizing and motivating, not automatic. That's exactly what happened here," Zakia said.

He emphasized that if the goals were achieved and all the bonuses paid out it would be "good news for everybody."

Even when counting the prior bonuses, Zakia said the compensation for plan participants in no case would have exceeded the median pay offered at industry comparable companies or for comparable jobs.

He described the incentives as a wise expense based on Hertz's business judgment.

Hertz's unsecured creditors' committee supported the plans, arguingthey'd result in a stronger company performance that could ultimately allow the car rental companyto pay back more of what it owes.

"They looked at this very carefully," Zakia said. "This committee is no rubber stamp and they weren't a rubber stamp here."

Thomas Mayer, an attorney representing the unsecured creditors' committee, agreed, saying the group had done a lot of research of its own on the plans and their metrics.

"I don't know how much, if any of these bonuses, will be paid. So we do not view these as layups. We do not view these aspayments for just showing up," he said.

There's a long road to recovery ahead and theplans, Mayer argued, could help the company get out of bankruptcy sooner.

"It is highly unlikely that this debtor is going to be able to exit bankruptcy bythe end of 2020," hesaid.

Representatives with FTI Consulting, which recommended and helped create the incentive plans, defended themat the hearing too, answering tough questions asked by attorneys representing the other side, who drilled down into the nitty-gritty details of them.

LindaRichenderfer, counsel forU.S. TrusteeAndrew Vara, asked pointed questions about how the metrics were determined and where the company's current performancestood against those metrics.

"In some areas we are improving and are ahead of the metrics. Inothers, we still trail behind or have work to do," said Michael Buenzow, a bankruptcy consultant with FTI.

Based on the proposed metrics, he said, thecompany was pacingahead on the sale of its surplus rental cars, for example, but still hada long way to go to meet its goals for operating cash flow, which continues to be in the negative.

Richenderfer also picked apart the comparable analysis used to develop the plans, questioning the peer group the consultants considered in coming up with the scope and amounts for its requested incentives.

Attorney John Demmy, representing the retired executives group, questioned how the performance targets were determined for each of the metrics, suggesting that they seemed too low based on business improvements seen since the early onset of the pandemic.

He pointedly asked Buenzow if any of the proposed metrics could be achieved by management just showing up.

Buenzow answered with a firm "absolutely not."

"For each of themetrics, there is a significant amount of effort that needs to be undertaken by the employees," he said.

The judge and the objectors remained skeptical.

The executives Demmy representsagreed to defer part of their compensation in return for Hertz's promises to pay them back what they're owed in retirement benefits.

The group claims that a few days after Hertz's bankruptcy filing thecompany told them the lifetime benefits they had been receiving were "required to stop."

As a group, the executives estimate they're still owed more than $5.5 million in benefits based on "over 182 years of accumulated service" withHertz.

That's just a small fraction of what Hertz Global owes.

By the end of March, the companyhad racked up more than $24 billion in debt, according to its bankruptcy filing, with only $1 billion of available cash to operate and pay its bills.

The Hertz Corp. operates the Hertz, Dollar andThriftybrands, with thousands of corporate and franchise locations throughout the country.

Parent company Hertz Global also ownsDonlen,an American fleet leasing and management company based in Illinois, which is part of the bankruptcy filings.

The company completed its new multmillion-dollar global headquarters in Estero at the end of 2015. The companyreceived millions in state and local incentives to relocate from New Jersey to Southwest Florida.

Judge rejects Hertz’s bankruptcy bonuses to execs – MarketWatch

A bankruptcy judge blasted Hertz Global Holdings Inc.'s plan to pay top managers up to $14.6 million in bonuses after they collected millions in additional bonuses days before the rental car company filed for bankruptcy.

"It seems offensive to give senior executives bonuses when they were paid" retention compensation immediately before the bankruptcy was filed, Judge Mary Walrath said Thursday in U.S. Bankruptcy Court in Wilmington, Del. The 14 most senior executives could receive as much as $5.4 million.

"The incentive plan has serious problems," she said, asking Hertz to revise it.

The judge also said the financial targets the Hertz executives need to hit to qualify for the bonuses need to be tougher, echoing objections by a federal bankruptcy watchdog

That proposed $14.6 million in potential incentive compensation was on top of the $16.2 million in retention bonuses Hertz, just days before its May bankruptcy filing, said it would pay to about 340 employees in recognition of the uncertainty faced as a result of the novel coronavirus pandemic's impact on travel.

The new incentive plan would pay 309 employees.

The judge's ruling is a victory for a group of retired Hertz executives who criticized the new bonus plan, saying the company's promises to provide them with a secure retirement are being cast aside.

U.S. Trustee Andrew Vara, the government's bankruptcy watchdog, also has criticized Hertz's bonus plan.

The trustee said the company's top executives were reneging on a deal they made to get their retention pay before bankruptcy. Those executives signed agreements waiving their rights to 2020 performance bonuses as a condition of collecting the retention bonuses, Mr. Vara said.

Now in bankruptcy, they were trying to revive the performance-bonus program, with targets set at much lower levels, he has said.

Some key Hertz creditors, however, had backed the company's bonus plan. Those creditors supporting the plan include a bankruptcy committee that includes the International Brotherhood of Teamsters.

A lawyer for Hertz said during the hearing that the incentive plan was developed by independent directors and advisers who had no direct financial stake in the bonuses.

Hertz representatives couldn't be reached for immediate comment after the hearing.

Shareholders of Dallas-based Tuesday Morning Corp. may be in the rare situation of owning some of the retailer after its expected exit from bankruptcy this fall.

Tuesday Morning, a retailer of gifts and home closeouts founded in 1974, filed for bankruptcy May 27 after all of its 700 stores were closed by the pandemic and it had no e-commerce business to make up any of the lost revenue.

U.S. Bankruptcy Court Judge Harlin DeWayne Hale on Friday approved the creation of a committee of equity security holders after denying the request twice before.

The Dallas judge said in his order that he now has greater confidence that there is a substantial likelihood that equity will receive a meaningful distribution."

Tuesday Morning stores have performed well since reopening, along with other retailers that sell home furnishings as Americans reacted to the pandemic by sprucing up their homes. Plus, the retailer has attracted 40 prospective buyers who have signed nondisclosure agreements to access confidential information. Several parties have signed nonbinding letters of intent, according to the judges order.

The judge previously said he would reconsider forming an equity committee if events changed the analysis. Theres little precedent for shareholders of public companies to come out with anything from a Chapter 11 reorganization. Thats because theres a strict hierarchy for who gets paid from the assets. Government taxes, lenders and other creditors, including utilities and merchandise suppliers, bondholders and preferred shareholders, are all paid ahead of common stockholders.

Tuesday Mornings stock price stopped trading on the Nasdaq at 28 cents a share on June 5 and has since been trading on the over-the-counter market, often called the pink sheets. The stock closed Friday at 63 cents a share, up 18 cents, or 40%.

Tuesday Morning said it will file bid procedures by Wednesday and anticipates a final sale hearing at the end of October. Next week, it said, it will also file a plan and disclosure statement to serve as an alternative to the sale process.

Hale said given how quickly Tuesday Morning is moving on the sale process, he believed that it was necessary to appoint an equity committee at this time to ensure that equity holders are able to participate meaningfully in the sale and plan processes in a streamlined and efficient way.

Unlike many retailer companies that were forced into bankruptcy because of debt, Tuesday Morning didnt have a problematic balance sheet. It lacked cash flow with stores closed, and because of uncertainty related to the pandemic, the bank wouldnt extend credit to the company.

As part of its reorganization, Tuesday Morning closed 132 stores through the end of July and 65 additional stores that started closing in August. It also closed a new distribution center in Phoenix. It was already in the process of retrofitting its 1.2 million-square-foot Dallas distribution center.

Dealing with default: workouts, restructuring, bankruptcy and litigation – Crain’s New York Business

CRAIN'S: When should a commercial borrower in default consider a workoutnegotiating with its lender to try to settle the debt with a reduced balance? Which lendersand what kinds of loansare particularly well-suited to that approach?

MALATAK: A borrower whose liabilities exceed assets, or who lacks the cash flow to pay debts when they come due, is effectively insolvent. The borrower then has three choices: (1) ignore the problem and wait for the unavoidable lawsuits and/or possible involuntary bankruptcy, (2) attempt to negotiate a workout with the lender or (3) voluntarily file for bankruptcy. A responsible borrower will not ignore the issue but will first meet with trusted and competent advisersincluding counsel and accountantsto develop a strategy and then meet with the lender in an attempt to outline a solution before the economic situation becomes overwhelming. The workout options can include, among others, a loan modification, a refinancing of the debt by a third-party lender, the borrowers transfer of the property to the lender by deed in lieu of foreclosure, or a sale of the secured property to a third party.

The foregoing approach is better suited when the borrower has only one lender. When multiple lenders are involved, the challenges for a workout grow significantly because they likely have competing interests, and all want access to available cash. Finally, loans that are secured by assets having equity above the amount of the borrowers present indebtedness or where the company and its owners can pledge additional collateral security, or personal guaranties, are more likely to be worked out.

CRAIN'S: How does a commercial borrower looking to restructure decide between negotiating new terms with the current lender and refinancing with a new loan?

MALATAK: Whether a borrower decides between a loan modification with the existing lender or a refinancing through a prospective lender will be determined by the strength of the borrowers relationship with the existing lender and the terms being offered by the existing and prospective lenders. The relationship between a lender and a borrower, like any other business relationship, can be a very amicable and trusted one. If that is the case, and the only negative development was an extraordinary event like Covid-19, the borrower may very well prefer to stay with the existing lender. However, whether it is economically feasible to do so will depend on the borrowers cash flow and the terms being offered by the existing lender versus the terms being offered by the prospective lender.

CRAIN'S: What do commercial borrowers need to understand about their lenders interests when negotiating with them? How does the current public-health and economic crisis affect a lenders willingness to work out a troubled loan?

MALATAK: A borrower needs to understand that a lender must know whether the lender is fully secured by the assets pledged as collateral and personal guaranties, and whether the borrowers business is viable or notand if not, can it be made viable and over what time horizon. As previously mentioned, however, if a borrower has loans with more than one lender, the lenders might have competing interests which should be discernable by understanding the terms of the various loan documents. As for lenders present willingness to work out a distressed loan, lenders are not desensitized to the impact of Covid-19 and they much prefer having a performing loan in their portfolios than a nonperforming loan. The issue, however, is not one of the lenders willingness to work out a distressed loan, but rather their ability to do so, which is still largely guided by pre-Covid-19 external and internal rules and regulations.

CRAIN'S: When is bankruptcy, as opposed to a workout plan, the better option for a business facing default on a commercial loan?

MALATAK: The issue of whether a company should file for bankruptcy and what kind, such as Chapter 7 or 11 (including Subchapter 5 under Chapter 11), requires a complicated factual and legal analysis that should be undertaken with the advice of competent and experienced advisers. As an aside, Subchapter 5 was created to help small businesses reorganize in a quicker and less costly manner and might well be a good option in todays environment. A company should consider as part of its analysis whether it is able to work out its distressed loan on terms that it honestly believes it can satisfy. If not, the company will likely soon find itself in default of the modified loan and without any additional options for a second workout or a third-party refinancing, particularly if the companys fortunes have continued to erode and many other creditors are knocking at the door.

CRAIN'S: If you are the lender, whats the best way to enforce a judgment against a recalcitrant borrower and guarantor(s)?

MALATAK: A lender that obtained a judgment has many legal rights and remedies to enforce it. However, in order for a lender to take full advantage of these rights and remedies, its counsel must be very familiar with them, as well as the procedural nuances surrounding them. Counsel who is knowledgeable in this area of law will press a lenders rights and remedies with precision and timeliness because failure to do so may result in: (1) certain lenders rights and remedies being waived or lost, (2) lenders judgment lien being subordinated to more aggressive competing creditors and/or (3) available assets being overlooked or fraudulently transferred or encumbered (which only invites further litigation). Finally, a lender with a judgment needs to be patient because if its rights and remedies are executed upon in a precise and timely manner, and if there exist assets against which its judgment can be wholly or partially satisfied, it should eventually be satisfied.

Regus throws 6 NYC locations into bankruptcy – The Real Deal

Short-term office company Regus put half a dozen of its New York City workcenters into bankruptcy as the company seeks Chapter 11 protection for upwards of 100 locations across the country.

Regus, which went through a bankruptcy restructuring in 2003 after the dot-com bubble burst, has put roughly 90 locations into Chapter 11 over the past six weeks, filings show.

The company has filed for six workcenters in Manhattan, Brooklyn and Long Island City in the past week.

Four of the locations are in Midtown: Paramount Groups 1325 Sixth Avenue at 53rd Street, Levin Properties 1501 Broadway in Times Square, Brookfield Properties 424-434 West 33rd Street in Manhattan West and EQ Offices 1740 Broadway at 56th Street.

The other centers are at Normandy Real Estate Partners 175 Pearl Street in Dumbo and Jamestown Properties Falchi Building at 31-00 47th Avenue in Long Island City.

A representative for Regus declined to comment. But in the half-year update from its parent company, Switzerland-based IWG, CEO Mark Dixon said the firm will be accelerating its plan to trim 4 percent of its global portfolio in response to Covid-19.

Whilst the Covid-19 pandemic continues, we expect our third quarter to be particularly challenging. We therefore remain sharply focused on maximising further cost savings in the coming months, he wrote in the August report, noting the company is working to build a large cash buffer.

Regus is the largest flex-office provider in the world, with about 10 times as many locations as WeWork. The company, founded in 1989, is largely seen as a barometer for the short-term office market, which has grown significantly in the past few years with WeWorks expansion driven largely by SoftBank.

As recently as last fall, Dixon was crowing that Regus was thriving as its younger, upstart rival was experiencing growing pains.

But the coronavirus has had a severe impact on the short-term office market, with a number of smaller players fizzling out. Regus bankruptcy filings are the most significant indicator yet that the pandemic pain may start to reach the industrys highest levels.

The majority of the sites Regus put into bankruptcy are in urban cores including New York, Chicago and San Francisco areas hard hit by the virus. The number of centers in Chapter 11 represent about 2 percent of the 1,000 locations the company has in the United States and Canada.

The company believes its clients will want to work in suburban offices closer to home, rather than in dense business centers.

Nearly $13 billion worth of commercial-mortgage backed securities loans have exposure to Regus locations, according to a recent report from Kroll Bonds Ratings Agency.

A hotel project pegged as a catalyst for the revitalization of downtown New Rochelle just filed for Chapter 11 bankruptcy.

A company tied to a luxury Wyndham hotel planned for 115 Cedar Street made the bankruptcy filing Sept. 11 in federal court. The hotels developer is Ward Capital Management, a real estate investment firm based in nearby Dobbs Ferry.

The hotel project the largest in Westchester County in 20 years owes creditors $4.67 million, including $715,000 to prolific hotel architect Gene Kaufman. It has assets of $4.1 million, according to the filing.

Ward Capital Management secured New Rochelles approval to build a 24-story, 225-key hotel last summer. The project was part of the citys larger goal to revitalize its downtown area to attract younger residents.

DWayne Prieto, the managing partner and CEO of Ward Capital, is the husband of Jewelle Prieto, founder and co-owner of the now-shuttered restaurant and nightlife spot Don Coqui, which operated at the site for years.

Amenities at the 245,000-square-foot hotel tower planned to include a restaurant and bar, a spa, a rooftop pool and grill, valet parking and conference space. The hotels expected completion date was June 2021.

An attorney representing the hotel, Kevin Nash of Goldberg Weprin Finkel Goldstein, did not immediately return a request for comment. Ward Capital Management declined to comment. Court records show the partners in the project are in litigation; the bankruptcy filing could be a means to resolve their dispute.

Hotels have been slammed by the pandemic, as tourism and business travel have been virtually nonexistent since March.

The Courtyard by Marriott in Herald Square just revealed it will not reopen, and a few weeks ago, the Hilton in Times Square also announced it is closing permanently. The 478-room hotel at 234 West 42nd Street disclosed its plans to permanently shutter in a filing with the New York State Department of Labor.

GNC cancels bankruptcy auction, moves ahead with sale to Harbin – NutraIngredients-usa.com

GNC filed for Chapter 11 bankruptcy in June, with Harbin serving as the stalking horse bidder (the initial agreement puts a value of over $750 million on GNC). Harbins interest was not unexpected after the Chinese pharmaceutical company previously invested $300 million into GNC.

In an 8-K filed with the Securities Exchange Commission (SEC), GNC stated that the stalking horse bid was accepted by the debtors on August 7 and this was approved by the Bankruptcy Court on August 19.

in its Notice of Auction Cancellation filed with the bankruptcy court dated today (September 14), GNC noted that the only qualified bid for the debtors assets was the Stalking Horse bid. (The auction was initially scheduled for tomorrow, September 15.)

On September 17, Judge Karen Owens of the U.S. Bankruptcy Court for the District of Delaware will hear any objections and ultimately approve a sale to the successful bidder.

The completion of the approval process for the sale of the Assets pursuant to the Stalking Horse Agreement is expected to move expeditiously and the Debtors expect to emerge from the Chapter 11 proceedings early in the fourth quarter of 2020, stated GNCs SEC filing.

The potential acquisition was the subject of some last-minute resistance from leading political figures, including Senator Marco Rubio who cited national security concerns in a letter to U.S. Treasury Secretary Steven Mnuchin, and called for a full review by the Committee on Foreign Investment in the United States (CFIUS) of the potential acquisition.

Harbins interest in GNC had already been cleared by CFIUS review in 2018 when the Chinese pharmaceutical company became the supplement players majority stakeholder.

In response to the concerns raised by Sen Rubio, the GNC spokesperson said: Nothing is more important to GNC than the trust of our customers. Our consumer data is safeguarded by rigorous standards and none of it is accessible to any foreign nationals or governments.

The Real Deal reported this week that Regus has put half a dozen of its New York City workcenters into bankruptcy as the company seeks Chapter 11 protection for upwards of 100 locations across the country. Over the past six weeks, the company has added around 90 locations to its Chapter 11 filing. In a recent interview, Mark Dixon, CEO of Regus parent company, IWG, said the firm will be accelerating its plan to trim 4 percent of its global portfolio in response to Covid-19.

Bizjournals reported this week that lawsuits are stacking up against Knotel. According to reports, landlord Hudson Pacific Properties sued the company Wednesday over unpaid rent at a South of Market office building where Knotel has leased space since 2018. Hudson Pacific is seeking damages in excess of $393,498. The lease for that building is set to expire in April of 2027, but just 2 years in the flexible workspace provider is having a hard time making rent. The new suit adds to Knotels already mounting legal battles with landlords in New York, Los Angeles, and Atlanta.

Elderly will be hardest group to protect with vaccine – University World News

Because the immune system naturally weakens with age, vaccine manufacturers, by and large, will not use an elderly population for phase I [trials] because they want to show that its safe and they want to show that its efficacious, John Nicholls, clinical professor of pathology at the University of Hong Kong and coronavirus expert, told University World News.

An analysis of 26 studies from around the world by researchers at Dartmouth College in the United States published last month found that the risk of dying from COVID-19 rises substantially from 0.4% for individuals aged 55 to 15% for those aged 85.

Ageing weakens the immune system making the body more vulnerable to infectious diseases, and intensifies inflammation when infections do occur. The ageing human body also loses the ability to acquire immunity through infection or vaccination, meaning that vaccines effective in younger adults may not protect people over 55.

You basically run out of nave immune cells that react to a pathogen the body has never seen before, said Nicholls.

Vaccine manufacturers may have too much to lose by trialling vaccine candidates on vulnerable populations too soon. They need to produce a vaccine and theyre not going to try it out on a 55- to 70-year-old population because theres definitely less chance of getting an immune reaction and an immune response, said Nicholls.

According to Nicholls, matters are worse for SARS-CoV-2, the coronavirus that causes COVID-19, because it enters human cells through a gateway protein that usually triggers inflammation, which is already elevated in the elderly.

The bodys defence mechanism goes into overdrive. Thats why you get this nasty reaction [with COVID-19] of diffuse lung damage, said Nicholls.

Trials that include the elderly

Experts agree that not enough has been done to find out how the elderly could respond to vaccines, even at the preclinical stage.

Phase I clinical trials test a vaccine candidate for safety and adverse reactions, usually with around 100 participants. The elderly are often left out of clinical trials. They have comorbidities and theres more of a chance that theyll have adverse reactions, Nicholls said.

Two of the vaccine candidate frontrunners, one being developed by the University of Oxford with UK-based pharmaceutical company AstraZeneca, and the other by US biotech giant Moderna, have enrolled adults over age 55 in their trials.

The Oxford programme, which includes older adults in Phase II and Phase III trials but excludes them from Phase I, was paused for a week in September in Phase III after a participant experienced inflammation on both sides of one section of the spinal cord. It has not been confirmed whether the event was due to the vaccine.

So far, Moderna is the only manufacturer to have tested their candidate vaccine on 20 adults aged over 55 in Phase I clinical trials and did so after demonstrating safety in adults aged 18-55.

Risky population groups

Phaik Yeong Cheah, an associate professor at the University of Oxford and head of the department of bioethics and engagement at the Bangkok-based Mahidol Oxford Tropical Medicine Research Unit, explained: Studies get their inclusion and exclusion criteria to be as representative as possible of the population that they want to use the eventual drug or vaccine on, but they also have to narrow it down quite a bit because they dont want to include people who are risky and prone to side effects.

Cheah highlighted the persistent challenge of companies not recruiting participants from high-risk groups for clinical trials. If somebody gets really ill and dies, then youll kill the new drug [or vaccine]. So theres no incentive to try it, she said.

High-risk populations, in addition to the elderly, include young children, pregnant women and those with comorbidities more than one disease present in a person at the same time.

The downside is that when there is a vaccine, it is only valid for the population groups that participated in the clinical trials, Cheah added.

Fears over Guillain-Barr syndrome a rare attack on the nerves by a persons own immune system that can paralyse the body discouraged many people from taking up the influenza vaccine, which may have exacerbated the 2009 H1N1 flu pandemic. Research, including an eight-year study in Taiwan of individuals over 50, has yet to prove an association between the influenza vaccine and the syndrome.

Transmission in the elderly

Nicholls said the types of COVID-19 vaccine currently undergoing trials will reduce cases of severe pneumonia but will not stop transmission, so vaccinated individuals exposed to the virus could transmit it to members of high-risk populations, while themselves being protected from developing the disease. Thats why the vaccine has to be put in together with social distancing, he said.

A vaccine delivered via a nasal spray already used for some existing influenza vaccines would cause the immune system to produce a type of antibody that stays in the nasal passages and intercepts viruses entering through the nose, preventing transmission.

Teams in the United States, Canada and the Netherlands are working on nasal vaccines for COVID-19, but a higher risk of severe inflammation from these vaccines, which are a live attenuated virus, discourages most manufacturers. A live attenuated vaccine is the live [virus] but its basically been tampered with. People are worried that if its not attenuated enough, then it may end up causing disease, said Nicholls.

The good news is that, in the elderly, you can try to boost your immune system, Nicholls said.

A recent review by immunologists at the universities of Palermo and Molise in Italy, Kings College London in the UK, and Harvard University in the US, highlighted evidence that suggested probiotics, micronutrients such as zinc and vitamin E and several chemicals in some foods such as resveratrol in red wine and omega-3 fatty acids in fish oils could give more protection, either by stimulating the immune response and-or lowering inflammation, though these effects will vary with different people.

Staff Spotlight: Karen White Tong | Duke Department of Neurology – Duke Department of Neurology

Karen White Tong first came to our EMG labs as a college sophomore, soon becoming fascinated with the neurological processes behind nerve conduction studies and with her interactions with patients. Now as a clinical research coordinator, shes running a large clinical trial for patients with Parkinsons disease and recruiting patients with spinocerebellar ataxia. For this weeks Spotlight interview, White Tong talks to us about developing long-term relationships with her patients, juggling work and parenting during the age of COVID-19, and her lifelong enjoyment of Duke football and basketball and the outdoors of North Carolina.

What are your current responsibilities within the Department? What does a typical day look like for you?I am a CRC II (clinical research coordinator), and I am currently running a big Parkinsons research trial with early Parkinsons patients that have monthly infusions. I also have a spinocerebellar ataxia trial going on right now that I am still recruiting for. Recruiting for these studies involves a lot of phone calls, looking through medical records and going through extensive screening/consent processes. Typically my studies run 2 years, if not more.

Since I typically see my patients monthly for greater than 2 years, I feel like the patients and I get to know each other really well and we develop friendships that last far beyond when the studies have concluded. Each day for me starts with seeing my first patient of the day, usually starting at 8 am. My study visits can last anywhere from 1 hour to 4 hours so afternoons are usually spent going through all my data and entering it for sponsor perusal. I typically prepare for my study visits in advance and have appointments, paperwork, lab kits, etc all ready to go on visit days.

How has the COVID-19 epidemic affected your job and personal life? Whats one positive strategy or resource youve found that helps you cope?So far, COVID-19 has not affected my job as my studies are considered essential and I have had every single patient come in for their visits since March. Personally, COVID has affected me much more on the home. In March, my daughters school went virtual and I had to juggle coming into work every day and staying home with her to make sure she did her virtual classes. We also didnt go out to eat anymore or go to the movies. My daughter played soccer and that was canceled as well as spring/summer swim team. Luckily, I live in the country and can take walks in the woods with our yellow lab Kula or fish on our pond in order to get fresh air.

How and when did you first come to the Neurology Department? I came to the Neurology Department when I was a sophomore in college here at Duke. I worked part time in the EMG Lab and eventually full time. I started working with Drs. Donald Sanders and Janice Massey. I was an EMG/NCS technologist and performed nerve conduction studies on patients. Doing this helped me learn about the various neurological disease processes and fascinated me.

Whats the biggest change in your work and at Duke in general since you first came here?One of the biggest changes in my work since I came to Neurology is the number of patients I see on a daily/weekly basis. In the EMG Lab I saw up to 8 patients a day and now I see 1 to maybe 2 patients a day. I still know and work with a number of neurologists that I knew when I first started here at Duke.

What other passions or hobbies do you have outside of the Department?I love going to see/watch Duke football and basketball games. I have been doing that since I was a child. I also love watching my daughter play soccer, which is mostly year long and I love watching her swim for her swim team in the spring and summer. I enjoy taking walks/hiking on our property in Hillsborough, we have miles of trails in the woods and a stream to wade in. We can also fish in the pond behind our house.

Cortical Plasticity Charted by fMRI and TMS Shows… : Neurology Today – LWW Journals

A case report, involving a 49-year-old nurse who had lost her left, non-dominant arm below the elbow and underwent allotransplantation, uses both fMRI and transcranial magnetic stimulation to highlight how the brain works to restore cortical representation on a once amputated limb after a hand transplant.

A new paper sheds light on how the brain works to restore cortical representation of a previously amputated limb after a hand transplant.

The study, published online ahead of print in Neurology on August 13, is the first to combine fMRI and transcranial magnetic stimulation (TMS) to examine the neurophysiological changes in the somatosensory and primary motor cortices before and after surgery to transplant a human hand.

Prior to the surgery, cortical areas that had once represented the patient's hand were usurped by the biceps. Within a few months of the transplant surgery, howeverand before any restoration of functional capability was evidentthe study found that those cortical regions resumed representation of the hand. Moreover, cortical inhibition levels, which had been low prior to the transplant, gradually approached normal during the months following it.

The restoration of lost inhibition after [a] hand transplant is a sign of functional recovery after transplant, the paper concluded. The finding that cortical plastic changes occurred at early stages suggest that it may drive recovery and is an important factor in successful recovery of function in the transplanted hand.

The paper was coauthored by neurologists and surgeons at the University of Toronto, where the procedure was performed, as well as at the University of Michigan and the National Institute of Neurological Disorders and Stroke.

Reduction of cortical inhibition appears to be critical to enabling the plasticity necessary for the brain to shift somatosensory regions, said the senior author of the study Robert Chen, MD, professor of medicine at the University of Toronto and senior scientist at the Krembil Research Institute.

We assume this is a method of the brain to spread representation into the area not being used, Dr. Chen said.

Neuroscientists and surgeons who have previously led studies of cortical changes following hand transplantation said they welcomed the new findings, although some expressed disappointment at the brevity of the report.

I don't believe anyone has previously described imaging of the brain before and after the transplant, said Jon Kaas, PhD, the Gertrude Conaway Vanderbilt Chair and Distinguished Centennial Professor of Psychology at Vanderbilt University. But, he added, They're so brief about it. It would be much more interesting if they had elaborated more.

The case report involved a 49-year-old nurse who had lost her left, non-dominant arm below the elbow in an automobile accident in 2005. She underwent allotransplantation on January 7, 2016.

Dr. Chen and colleagues conducted a longitudinal study to evaluate cortical plastic changes beginning four months before and up to six months after the surgery. They performed somatosensory mapping using fMRI with electrical cutaneous over the upper arm and thumb. They also conducted TMS mapping to evaluate the changes in motor cortical representation.

The fMRI mapping showed that representation for [the] upper arm on the transplant side was located anterior-laterally compared to the intact side before surgery and moved posterior-medially at 6 months after surgery while that for the intact side was stable over time.

The resting motor threshold stimulus intensity for the biceps, the authors reported, were higher than those for abductor pollicis brevis muscle in the intact arm while the measurements for biceps muscle on the transplant side were similar to those for the intact abductor pollicis brevis muscle before surgery and gradually increased after surgery.

TMS of the biceps muscle on the transplant side showed that it was at a more anterior-lateral location before surgery and moved gradually in the posterior-medial direction after surgery, the study found.

The inhibition between hemispheres was absent before surgery but gradually increased afterward, the paper reported, from the symmetric position of motor hotspots for intact muscle on the transplant side (mirrored point) to the intact side. Short-interval intracortical inhibition on the transplant side was likewise absent before surgery and increased gradually after.

The restoration of lost interhemispheric interaction after surgery suggest that pyramidal neurons mediating transcallosal and corticospinal projections undergo plastic changes in a similar manner although the two groups of neurons located in different cortical layers, the paper concluded. Our finding that cortical inhibition was decreased in the amputated state was consistent with the opinion that gamma-aminobutyric-acid mediated cortical inhibitory circuit acts as a gate keeper in the induction of cortical plasticity.

The short length of the paper, Dr. Chen said, was due to the journal's length limit for the single case report; supplementary information is available at http://bit.ly/corticalSUP.

Steven McCabe, MD, one of the surgeons who performed the transplant, told Neurology Today that the patient has regained substantial use of the hand.

She has good motor recovery with grade 5/5 wrist extension and flexion, Dr. McCabe stated in an email. She has full finger flexion with the exception of the index which has reduced flexion of the metacarpophalangeal joint. She has full finger extension with no claw deformity. She has some sensory recovery with the ability to accurately localize light touch to each digit but no two-point discrimination.

His group is planning to write up a formal five-year report, Dr. McCabe added.

Decades of research have established the brain's plasticity in response to loss or restoration of limbs, first in animals and then in humans. In the 1980s, Dr. Kaas coauthored pioneering papers describing somatosensory reorganization in the non-human primate brain in response to the repair of peripheral nerves which had previously been severed.

In 2009, French researchers published one of the first papers to describe the restoration of hand-muscle representations in the human motor cortex following a hand allograft. A 2014 paper in Nature Communications reported that somatosensory reorganization following spinal cord injury is due not to cortical mechanisms but to changes at the level of the brainstem nuclei.

This new case report supports the growing body of literature on the ability of the brain to reverse plastic reorganization years later, said the senior author of the 2014 paper, Neeraj Jain, PhD, professor and director of the National Brain Research Centre in Manesar, India.

Vilayanur S. Ramachandran MD, PhD, professor of psychology and neurosciences and director of the Center for Brain and Cognition at the University of California, San Diego, said the new paper demonstrates yet again that there are no fixed connections in the brain.

The old model of the brain is fatally flawed, Dr. Ramachandran said. It was once generally assumed that the brain is made of highly specialized, task-specific autonomous modules arranged in a serial hierarchy starting from the sensory modules to the motor output. But in the last two decades it has become clear that we are dealing not with static maps but with dynamic, fluctuating mosaics of neural activitymore like a termite mound than a computer.

William Gaetz, PhD, research associate professor of radiology at the Perelman School of Medicine at the University of Pennsylvania, said the new paper was the first to establish a baseline of neural organization prior to the surgery.

That's the nicest part of the study, that they had a baseline and then followed up after, said Dr. Gaetz, who was the first author of a 2018 paper describing cortical reorganization following bilateral hand transplant in a child. It is an impressive demonstration of large-scale reorganization, and particularly interesting to see from someone in their late forties.

Dr. Gaetz expressed concern, however, about the risk-benefit ratio of performing an allotransplant on the non-dominant hand of a person who was not previously on anti-rejection drugs. The child described in his 2018 paper had lost both hands due to sepsis and was already on anti-rejection drugs following a kidney transplant.

You have to balance any benefit with the costs, which are not trivial, Dr. Gaetz said. Immunosuppressive drugs increase the risk of cancer and infection. And we know that transplanted organs tend to degrade after a decade or so. This recipient will likely outlive her transplant. Cortical Thickness and Plasma Proteins Correlate with… : Neurology Today – LWW Journals

Workers at the site of the 9-11 World Trade Center (WTC) attacks in 2001 showed a reduction in cortical thickness among cognitively impaired responders both with and without post-traumatic stress disorder. Another investigation identified proteins in plasma that can differentiate between those with only post-traumatic stress disorder (PTSD), only with mild cognitive impairment, and those with both conditions.

Two large related studies of workers at the site of the 9-11 World Trade Center (WTC) attacks in 2001 showed distinct brain changes and levels of proteins in plasma in those who were cognitively impaired years later. Both studies were described at the Alzheimer's Association International Conference held virtually in July.

In one study, the largest neuroimaging study of the workers to date, researchers observed reduced cortical thickness. The reduction in cortical thickness was observed in cognitively impaired responders both with and without post-traumatic stress disorder, said the study author Sean Clouston, PhD, associate professor of public health at Stony Brook Medicine.

And a second related study involving proteomic analysis of WTC respondersconducted by a group led by Benjamin Luft, MD, director of the Stony Brook World Trade Center Wellness Programidentified proteins in plasma that can differentiate between those with only post-traumatic stress disorder (PTSD), those only with mild cognitive impairment, and those with both conditions.

Studies have suggested that inhalation of fine particulate matter, such as that at the WTC site, could cause neurodegeneration. No studies had used neuroimaging to assess these responders, but they are at more risk of cognitive impairment than the general population.

In the imaging study, researchers conducted T1-MPRAGE, a kind of MRI on 99 responders, 51 of whom were cognitively unimpaired and 48 who were impairedand calculated cortical thickness in 34 regions of interest.

The gray matter volume for those who were cognitively impaired was 603 cubic centimeters, compared with 629 cubic centimeters for the cognitively unimpaired (p=0.03). Whole-brain average cortical thickness was 2.41 mm among the impaired, compared with 2.48 among the unimpaired (p=0.0003). No differences were found in average cortical thickness when researchers compared PTSD to non-PTSD groups. Significant reductions in cortical thickness were found in 23 of the 34 regions analyzed.

Researchers also compared cortical thickness to the norms seen in the published literature. They found significant reductions among those who were cognitively unimpaired in seven of the regions, with the greatest reductions in the entorhinal cortex. In impaired responders, significant reductions in thickness were in 14 of the 34 regions.

Our healthy World Trade Center responders may also be in the earlier stages of disease in some cases, Dr. Clouston said in his presentation.

In the proteomics study, researchers analyzed plasma from 181 responders34 with PTSD and mild cognitive impairment (MCI), 39 with only PTSD, 27 with only MCI, and 81 controls with neither one. They identified 16 proteins that were associated with PTSD and MCI together, 20 associated with PTSD alone, and 24 associated with MCI alone.

Those associated with both disorders together included Neurocan core protein, Brevican core protein, Cathepsin S, and othersall of which have known correlates in the Alzheimer's and psychiatricand in particular, schizophreniaresearch field, Dr. Clouston said.

The researchers also created a multi-protein composite risk score that was reasonably accurate, identifying PTSD-MCI, PTSD alone, and MCI alone. The areas under the curve for those scores were 0.84, 0.77, and 0.83, respectively.

To our knowledge, the current study was the largest to profile a targeted set of proteins involved in the neurobiologic processes, Dr. Clouston said. The significant associations across these three case-group analyses suggested that shared biological mechanisms may be involved in the two disorders. If findings from the multi-protein composite score are replicated in independent samples, it has the potential to add a new tool to help classify both post-traumatic stress disorder and mild cognitive impairment.

Charles Hall, PhD, professor of epidemiology and public health at Albert Einstein College of Medicine who has studied the neurologic effects of WTC responders, said the findings represent a significant step forward. Up until now, researchers had found cognitive impairment effects that were strongly associated with PTSD. Still, the question remained whether this link was an artifact of the neuropsychological instruments used to assess patients.

These studies are important in that they show that there may be a biological substrate to this, he said. These are not large studies, it's going to require a lot more work, but this certainly should motivate more work.

Much research has been done over the past several years on associations between air pollution and cognition. Still, it has tended to rely on regional ambient air quality measurements that are not very good at quantifying individual levels of exposure, Dr. Hall said. Although the World Trade Center exposure to particulate matter is a much larger exposure than most people would ever see in a lifetime, these findings should encourage researchers to continue that line of research.

Here we have a huge exposure that is showing not just effect on cognition, but a biological substrate for some of the impairment, he said. So as far as the general population is concerned, I think this should stimulate more research in general population cohorts on the effect of particulate matter, with better exposure measures.

Dr. Hall and colleagues recently had a paper accepted to the International Journal of Environmental Research and Public Health that found that PTSD symptoms mediated the association between WTC exposure and subjective cognitive concerns. In contrast, the Stony Brook findings on cortical thickness found reductions both in those with and without PTSD.

Dr. Hall said the difference might be attributable to the way PTSD was treated in the studies. In his research, PTSD symptoms were considered along a continuum, while in the Stony Brook study, participants were either considered to have PTSD or they weren't.

Marcia Ratner, PhD, a behavioral neuroscientist and toxicologist who studies the neurological effects of occupational exposures, said the studies report interesting results which need to be replicated.

She said there are many potential confounders that were not controlled for in the studies, including comorbid medical conditions and medications taken. She added that PTSD alters levels of neuroactive steroids that can influence memory function. Furthermore, she said, mild cognitive impairment and dementia are stressful, and elevated cortisol levels have been associated with dementia.

A major limitation of this study is that the findings were not related to serum concentrations of cortisol, she said. An important question, she said, is how many patients had received treatment for their symptoms and how many were nave. After breaking groups down by disorder, the sample sizes are small, she added.

In short, she said, the findings from these two studies are interesting, but more work is needed to fully elucidate the relationships between PTSD and CI in this population.

Using a three-dimensional model of engineered human tissue, researchers replicated the pathological effects of the blood-brain barrier under ischemic conditions and used drugs to prevent or lessen the damage. The lab model will be used to test new therapies, their potential neurotoxicity, and make it easier to understand how the blood brain barrier works, experts say.

Using a three-dimensional model of engineered human tissue with six different cell types, researchers at Wake Forest School of Medicine replicated the pathological effects of the blood-brain barrier (BBB) under ischemic conditions and used drugs to prevent or reduce the damage.

The spheroids have all the qualities of human BBB functionwith expression of tight junctions, adherens junctions, adherens junction-associated proteins, and cell-specific markers. They also have the same proportion of human microvascular endothelial cells, neurons, astrocytes, microglia, pericytes, and oligodendrocytes.

The lab model will be used to test new drugs in the pipeline and their potential for neurotoxicity. The model will also make it easier to understand how the BBB works to protect the brain and to model human brain diseases, experts told Neurology Today.

We developed a much-needed model of an entirely natural BBB and show that we can reverse many of the problems that occur in acute stroke, said senior author Anthony J. Atala, MD, director of the Wake Forest Institute for Regenerative Medicine.

In our three-dimensional model, every cell contributes. The development of tissue-engineered three-dimensional brain tissue equivalents can help advance the science toward better treatments and improve patients' lives. The study was published online on June 17 in Nature Scientific Reports.

Much of the day-to-day work on these studies was carried out by the paper's first author, Goodwell Nzou, PhD, who created the model during his doctoral studies at Wake Forest.

To study the model, the scientists decided to test what goes wrong with the BBB during stroke. They depleted the organoids of oxygen and observed an inflammatory cascade set in motion to stop the damaging chain of events that occurs during stroke.

By knowing at the cellular level what is going on, we can start thinking about therapies to stop this process, explained Dr. Atala.

Not surprisingly, the more oxygen removed from the cellular environment, the more damage there wasincluding an increase in proinflammatory and oxidative stress molecules. The investigators looked closely at the neuroinflammatory cascade and observed that the length of exposure, not just the amount of oxygen reduction, seemed to play a role.

The cytokines that were released disrupted the integrity of the BBB, and over time the cells began to lose their capacity to function normally. The scientists were able to measure the chemicals during the cytokine storm: chemokines and cytokines, heat shock proteins, transport proteins, tight junctions, and basement membrane protein expression.

Next, they used anti-inflammatory agentssecoisolariciresinol diglucoside and 2-arachidonoyl glycerolto reduce the effects of inflammation and the other pathological processes underway.

The treatment was effective at ameliorating the damage and improved tight junction distribution, said Dr. Atala.

The investigators also tested a free radical scavenger and anti-inflammatory endocannabinoid that have been shown to model inflammation and cell death. Again, it worked to alleviate the response to the hypoxic environment.

Dr. Atala, a regenerative medicine specialist, believes that this disruption of the BBB may be used as a crack in the door to get targeted treatments through. When we used these agents we were able to manage the response and observed positive effects, said Dr. Atala. By using this model that replicates in many ways the human physiological response to hypoxia we can now look at agents that can impact things in a positive way.

The critical role played by the BBB in maintaining homeostasis within the central nervous system makes it clear why BBB breakdown is evident in many neurological disorders, they wrote in the paper. During ischemic stroke, in particular BBB breakdown leads to edema and hemorrhage.

These pathologic consequences worsen secondary brain injury and significantly contribute to cognitive impairment. Hence pathological effects of hypoxia on BBB function may be critical in understanding strategic therapeutic targets for BBB maintenance and recovery during and after neurologic injury, the scientists wrote.

The scientists are now designing studies of other pathological conditions, including Alzheimer's disease, Parkinson's disease, amyotrophic lateral sclerosis, and Huntington's disease. Dr. Atala said that neuroimaging studies have shown BBB breakdown and inflammatory processes underway in many neurodegenerative conditions.

The development of three-dimensional organoid models of the neurovascular unit offers a unique opportunity to study the effectiveness of novel therapeutics for neurological diseases in a cell-based system that takes into account intercellular communication, said Patrick Ronaldson, PhD, an associate professor of pharmacology, physiological sciences, and neuroscience at the University of Arizona College of Medicine.

Intercellular communication between endothelial cells, glial cells, mural cells, and neurons is a property that is often lacking in many of the in vitro model systems that are routinely used for CNS drug discovery and development.

Dr. Ronaldson added that three-dimensional neurovascular unit organoid models can also enable the identification of novel compounds that can protect the vasculature from further injury. Such utility may facilitate critical breakthroughs in treatment of neurological diseases where blood-brain barrier dysfunction and neurovascular injury are known pathological characteristics, such as ischemic stroke, traumatic brain injury, and Alzheimer's disease.

Despite this advance, however, he said there are many unanswered questions regarding the application of this three dimensional organoid model to drug discovery and development. These questions specifically relate to the magnitude of blood-brain barrier leak observed in the organoids and the ability to study drug transport at the neurovascular unit in a multi-transporter environment.

This study evaluated BBB permeability using only large molecular weight tracers such as immunoglobulins, albumin, and fluorescently-labeled dextrans, he explained.

This is a missed opportunity because permeability to smaller molecules could still persist. Such a leak of small molecules can occur even if large molecular permeability is restricted and can contribute to exacerbation of neurovascular pathology or neurotoxicity. Understanding whether therapeutics can protect against small molecule paracellular permeability is critical in the advancement of these compounds to the clinic.

Additionally, the researchers only evaluated organoid expression of one therapeutically relevant transporter (that is, P-glycoprotein, also known as MDR1), Dr. Ronaldson said.

There are many other transporters such as breast cancer resistance protein and organic anion transporting polypeptides that are critical determinants of drug disposition at the neurovascular unit, he continued. For this three-dimensional organoid model to be truly applicable to neurological drug discovery and development, it is important to evaluate the expression and function of the many other transporters that are known to be clinically relevant.

Costantino Iadecola, MD, the Anne Parrish Titzell professor of neurology and director and chair of the Feil Family Brain and Mind Research Institute at Weill Cornell Medicine, said: The findings are anticipated based on previous stroke studies: the impact of hypoxia on BBB constituents, cytokine production, oxidative stress, etc., and the protective effects of free radical scavengers, anti-inflammatory cannabinoids. All these events and protective approaches were already known and validated in animal models but failed in humans.

There is little mechanistic novelty and therapeutic advance [with this paper], he said, but the positive aspect is that the model could be used for higher throughput screens for new drugs.

But, Dr. Iadecola added, one caveat with the current study is that only a particular segment of the cerebral vasculature was examined (capillaries and related cells) and that only hypoxia was tested. This is not the same as ischemia produced by an arterial occlusion in vivo, which is associated with intravascular clotting, distal microembolism, infiltration of inflammatory cells, no-reflow phenomenon, and more, he said.

Zika-Chikungunya dual infection linked to stroke: Research – Outbreak News Today

A deadly combination of two mosquito-borne viruses may be a trigger for stroke, new research published in theThe Lancet Neurologyhas found.

University of Liverpool researchers and Brazilian collaborators have been investigating the link between neurological disease and infection with the viruses Zika and chikungunya. These viruses, which mostly circulate in the tropics, cause large outbreaks of rash and fever in places like Brazil and India. Zika is widely known to cause brain damage in babies following infection in pregnancy, but the new research shows it can also cause nervous system disease in adults.

The study of 201 adults with new onset neurological disease, treated in Brazil during the 2015Zika and 2016 chikungunya epidemics, is the largest of its kind to describe the neurological features of infection for several arboviruses circulating at the same time.

The new research shows that each virus can cause a range of neurological problems. Zika was especially likely to cause Guillain-Barre syndrome, in which the nerves in the arms and legs are damaged. Chikungunya was more likely to cause inflammation and swelling in the brain (encephalitis) and spinal cord (myelitis). However, stroke, which could be caused by either virus alone, was more likely to occur in patients infected with the two viruses together.

Stroke occurs when one of the arteries supplying blood to the brain becomes blocked. The risk of stroke is known to be increased after some types of viral infection, like varicella zoster virus, which causes chickenpox and shingles, and HIV. Stroke is also being recognised increasingly as a complication of COVID-19. This has important implications for the investigation and management of patients with viral infection, as well as for understanding the mechanisms of disease.

In total 1410 patients were screened and 201 recruited over a two-year period at Hospital da Restaurao in Recife, Brazil. Comprehensive PCR and antibody testing for viruses was carried out in Fiocruz laboratories.

Of the 201 patients admitted with suspected neurological disease linked to Zika, chikungunya or both, 148 had confirmation of infection on laboratory testing, around a third of whom had infection with more than one virus.

The median age of patients was 48, and just over half the patients were female. Only around 10% patients had fully recovered at discharge, with many having ongoing issues like weakness, seizures, and problems in brain function.

Of the stroke patients, who were aged 67 on average, around two thirds had infection with more than one virus. Many of the people who had a stroke had other stroke risk factors, such as high blood pressure, indicating that stroke following Zika and chikungunya viral infection may most often be seen in those who are already high risk.

Dr Maria Lcia Brito Ferreira, neurologist and head of department at Hospital da Restaurao, leading the Brazilian team said: Zika infection most often causes a syndrome of rash and fever without many long-term consequences, but these neurological complications although rare can require intensive care support in hospital, often result in disability, and may cause death.

Dr Suzannah Lant, a Clinical Research Fellow at the University of Liverpool, who worked on the study explained: Our study highlights the potential effects of viral infection on the brain, with complications like stroke. This is relevant to Zika and chikungunya, but also to our understanding of other viruses, such as COVID-19, which is increasingly being linked to neurological complications.

Senior author Professor Tom Solomon, Director of the National Institute for Health Research Health Protection Research Unit in Emerging and Zoonotic Infections at the University of Liverpool said: Although the worlds attention is currently focused on COVID-19, other viruses that recently emerged, such as Zika and chikungunya, are continuing to circulate and cause problems. We need to understand more about why some viruses trigger stroke, so that we can try and prevent this happening in the future.

Clippers talent cant overcome their lack of chemistry – The Boston Globe

In the spotlight the most was coach Doc Rivers, who left Boston because he didnt want to endure another rebuilding plan and wanted the challenge of leading the Clippers to unprecedented prosperity.

Instead, the Clippers have blown two 3-1 playoff series leads during his tenure.

You could just see the difference in the two teams; theyve been together and we havent, Rivers said. "We didnt meet [expectations]. Thats the bottom line. Im the coach and Ill take any blame for it. If we had [met expectations] in my opinion, wed still be playing. And although our numbers said we were a good defensive team, I dont think we realized that part of our game at all.

It was hard to watch because Ive got a good group of guys, I really do. Obviously, I could have done something more. I always think its me, no matter what. Ill look at this and try to figure it out. But its very disappointing.

The Clippers admitted they were never as cohesive as the outside public perceived. Williams left the bubble to go to a funeral and then had to spend extra time quarantining after he was photographed attending a strip club. Harrell left the team for nearly a month to attend a funeral. Ivica Zubac and Landry Shamet contracted COVID-19.

George acknowledged he was dealing with depression because of being sequestered in the bubble. Honestly, the Clippers were a talented mess that was just hoping their prowess would be good enough to overcome their weaknesses.

We understood that this was going to take some time, Williams said. A lot of the issues that we ran into, talent bailed us out, chemistry didnt. In this series, it failed us. This is our first year together. Were a highly talented group and we came up short.

"Chemistry is something youve got to build, and I thought we were moving in the right direction at the end of the year before the COVID thing happened.

While Rivers had title aspirations, it was apparent his players didnt share that sense of urgency.

It was a great first season with Kawhi. Were still learning, figuring each other out, George said. But I thought we had a great year playing off each other. We can only improve off that. But you want that pressure. First time Ive been on a team thats not been considered the underdog, so it was different."

Instead of lamenting a blown 3-1 lead, George appeared to believe next season was more of the target year. George and Leonard have player options in their contracts for the 2021-22 season.

We have to be positive going forward, George said. Thats all we have right now. Were just positive and optimistic about it.

But its apparent too much pressure was placed on George, Leonard, and the Clippers to reach new heights since that July 2019 night Leonard agreed to sign, and the Clippers acquired George.

Its obvious pressure to live up to the title expectation, but as a player you want that, George said. But its not a cop-out. We didnt live up to that expectation. But I think internally weve always felt this is not a championship-or-bust year for us. We can only get better the more we stay together. The more chemistry for this team, the better. I really think thats the tale of the tape.

CALL OF DUTY

Public display of affection

The NBA invited a handful of public address announcers to call games in the bubble despite the lack of fans. The league wanted to replicate a game atmosphere.

Four accepted the offer: Olivier Sedra of the Nets, Kyle Speller of the Nuggets, Tim Sinclair of the Pacers, and Eric Smith of the Clippers. They had to come to Orlando, quarantine, and call multiple games per day.

Its an unusual responsibility in an unusual time. These PA announcers are used to calling games for their home team three nights a week. But these four were assigned to call every scrimmage, seeding game, and playoff matchup. For three of the announcers who spoke with the Globe, it was an honor to show their skills on the biggest stage.

Id like to think I do have a responsibility to the NBA, if they need me or call on me for something, Im humbled by it, Sedra said. I could have never imagined this growing up."

Sedra came to Brooklyn from the Cavaliers, where his baritone voice could be heard saying, LeBron James! after a thunderous dunk or stepback 3-pointer. Sedra is one of the more highly regarded PA announcers in the NBA, and he has was delighted by the opportunity to sharpen his pipes to say, LeBron James again when he called Lakers games in the bubble.

Its going to be fun saying his name again, Sedra said. Its definitely morphed from saying his last name or rolling the L in his first or rumbling the R and theres gaps between his first and last name. I guess a player like that, a type of iconic player, allows you to do things like that."

Sedra had a heart-to-heart conversation with his 9-year-old son, Adrian, who lives in Cleveland, about dad having the opportunity to call NBA games over several weeks.

Its funny, when its my son, I kind of have to see how he feels when I move along, Sedra said. So I got the word and presented it to him in a way where I said, I think this could happen, to feel him out. And his eyes went wide open and he said, Really? And to see that in him, you cant get me talking about that.

Sedra had a meeting with the league and told Adrian he would call him afterward with the official word.

I called him up and said, Yo, I got it, and he was so giddy, Sedra said. And I knew I was good. Just his reaction, his facial expression, and what he admitted to me is what I needed to see and hear and feel. So I was like, Lets roll.

The challenge for Sedra and his colleagues was learning the names of every player in the bubble, how they like that name enunciated, and creating an inflection for each depending on whether they were the home or road team.

Ive had opportunities to call different games in different places, and in this case I try to make a connection with each team, he said. Some are true and genuine and personal. And teams that I dont know anybody, you just try to talk to them before the game.

WORK TO DO

No offseason for Raptors Ujiri

The Raptors were eliminated by the Celtics in seven games and saw their reign as champions end. But the organization has no plans to rest on its laurels, especially with several pressing issues this summer.

First is the status of general manager Masai Ujiri, one of the most coveted executives in the NBA. He is entering the final year of his contract and there will be teams that will throw lucrative offers at Ujiri to rebuild their franchises.

Ujiri said he has not had contract extension discussions, but that did not stop him from extending Nick Nurse, the reigning NBA Coach of the Year. Ujiri said it was his priority to extend the teams leadership under him, and hes in the process of getting new contracts for other front office members.

Next are the players, and the Raptors have a handful of free agents, including Fred VanVleet, Serge Ibaka, Marc Gasol, and Rondae Hollis-Jefferson. VanVleet teamed with All-Star Kyle Lowry to make up one of the best shooting backcourts in the NBA. Re-signing the 26-year-old VanVleet will be the teams first priority.

You want to start with whoever our free agents are, Ujiri said. "Theres a priority for Fred. We also have to look at the game and see where the game is going. We are all waiting on the cap to see where the numbers fall. We also have to look at free agency in 2021. We have to look at the short-term future and the long-term future.

"This is where we really have to buckle down and make an assessment of what bubble basketball was against what the outside-the-bubble basketball was. This is our time. You support as much as you want during the bubble, during the playoffs. But now is the time to work.

Im very confident in our culture and, yes, Fred is a priority, a giant priority. Marc Gasol brings so much to our organization. Serge is a priority. We really have to attack this head-on.

One of
the most disappointing aspects of the Raptors' loss to the Celtics was the performance of All-Star Pascal Siakam, who averaged 14.9 points, 7.4 rebounds, and just 38.2 percent shooting from the field. Siakam also missed 28 of 32 3-point attempts in the series. Siakam never appeared comfortable playing in the bubble.

We talked extensively with Pascal. Everybody stood by Pascal, Ujiri said. Theres something about that bubble that brought all of us together. I know Pascal, sometimes we need this almost like a kick in the butt. Hes going to bounce back 100 percent. He felt like he let the team down. Lets not forget, Pascal helped us win a championship. Hes going to bounce back.

Ujiri acknowledged it was difficult being sequestered for weeks.

The mental aspect was hard for me, I cant even imagine the players, he said. "See the people youre competing against each other every day. I saw Brad Stevens every time I went into the elevator or went outside. I could only imagine with the players. The mental aspect of being alone. You dont know what people are going through outside thats beyond your control.

I think it brought some camaraderie to the NBA. I got to know a lot of people.

The night the Raptors won the championship should have been the greatest night of Ujiris life. Instead, it was marred by an incident with an Alameda County (Calif.) sherriffs deputy, who initiated an altercation as Ujiri tried to walk onto the floor at Oracle Arena to celebrate the win over the Warriors.

Video was released last month showing the deputy, Alan Strickland, making contact with Ujiri and never allowing him to reveal his credential. NBA officials and a representative from Nike intervened, and Ujiri was eventually allowed on floor, but not without having a shoving match with the deputy. Strickland has filed a lawsuit against Ujiri.

I had my own incident and everybody has dealt with it. There is racism everywhere. Lets face it, theres racism in Canada. This is a global pandemic, Ujiri said. "This was very hard for me. This is the part of the bubble I struggled. When the video came out I didnt sleep for a few days. You question yourself. As time goes on you begin to doubt yourself, and I doubted myself.

I watch all these documentaries and I say, Why dont you just say what happened? I began to doubt myself. You have a lot of time to think. I started to think, what if this had gone the total wrong way? You think about everything. You think about racism and where its going. There was time I wondered whether Black lives matter to only Black people.

Ujuri sat in on some of the player meetings after the game boycotts last month. He said he appreciated their efforts and sentiments.

I know there was definitely a time where my team, we talked about the boycott, he said. We talked about the mental fatigue. We talked about Jacob Blake and you could tell there was a mental drain on these players and everybody was tired and we needed to pause. I commend LeBron [James], all these guys, Chris Paul, Jaylen Brown, all these guys. Because they stepped up and said something.

With the Clippers' collapse, the question is whether the reputations of Tyronn Lue and Sam Cassell, assistants seeking head coaching jobs, took a hit. Lue was viewed as a top candidate in Philadelphia, but that was before Mike DAntoni decided to leave Houston, and apparently the 76ers job is his if he wants it. The Pelicans and Pacers are also looking for head coaches, but the New Orleans job is likely more suitable for Lue, who coached the Cavaliers to the NBA championship in 2016. The Pelicans want to win now and need a defensive-minded coach who can mold their younger core. Lue sounds like a perfect fit. Cassell has been with the Clippers for several years and is ready for his first top job. But if youre Houston, Oklahoma City, Indiana, or Chicago, do you take a chance on a first-time head coach? All four franchises are looking for coaches who can establish a culture and win immediately. The offseason coaching search will be interesting and has already gotten off to a surprising start with the Nets' hiring of Steve Nash . . . The Kings hired Rockets assistant GM Monte McNair as their GM and kept former Pistons executive Joe Dumars as a consultant. Former GM Vlade Divac stepped down after the Kings again failed to make the postseason. Kings fans havent forgotten Divac passed on Luka Doncic in the draft in favor of Marvin Bagley III, who has been oft-injured during his first two seasons. Also, coach Luke Walton turned former lottery pick Buddy Hield into a sixth man and the former Oklahoma standout is not happy with that role . . . There was a huge moment this past week for Roxbury native Shey Peddy, the Mercury forward who drained the winning 3-pointer at the buzzer to beat the defending WNBA champion Mystics and advance to the second round. Peddy, 31, starred at Melrose High School. She entered the WNBA bubble as a member of the Mystics but was waived. Peddy was quickly signed by the Mercury and she made the biggest shot of her career against her former team.

Q&A: Nobel Laureate in Chemistry Dr. Frances Arnold speaks to The Hustler about her inspiration and advice to students – The Vanderbilt Hustler

On Sept. 15, Dr. Frances Arnold, a Nobel Laureate in Chemistry and the Linus Pauling Professor of Chemical Engineering at California Institute of Technology, gave the fall John R. and Donna S. Hall Engineering Lecture to over 700 participants over Zoom.

The lecture covered the concept of directed evolution, for which Arnold was awarded the 2018 Nobel Prize in Chemistry. Directed evolution uses lab technologies that create mutations in a pre-selected gene. These genes are then used to manufacture the mutated enzymes, which are then tested for function. Enzymes that help facilitate the reaction are selected and the process continues until the scientist is satisfied with the results.

The Hustler spoke with Dr. Frances Arnold on a Sept. 15 Zoom call about how she overcomes roadblocks, what inspires her and her advice to students.

Vanderbilt Hustler: What is a concept that still amazes you to this day?

Dr. Frances Arnold: Evolution, I think its amazing that such a simple engineering process can solve such complex problems. Were just at the beginning of this whole idea that you can reprogram the biological world, and use evolution to do it. Its mind-boggling. Youre lucky youre young. Youre going to see such incredible advances, such amazing things that you can do with biology.

When you face a roadblock in a project, what is the first thing you do?

It depends how big the roadblock is. If its an important problem, and Ive got to get over it, I find other ways to solve the same problems. Just as often I say, its not really worth getting over that roadblock, lets turn right and see whats over there.

Especially for engineers, this is one of the treasures of being an engineer. Were trying to make things that nobody has made before. Were trying to understand where we can go with, say, reprogramming the biological world. So, if you cant do one thing, there are 500,000 other things that you can do, so I think its important to adapt to roadblocks.

Could you talk about what motivated you to go into chemical engineering? You got your undergraduate degree in mechanical engineering and aerospace engineering at Princeton; what guided you after you had that strong foundation?

When I was a mechanical engineer, I wanted to engineer the most complicated things on the planet, and to me, that was an airplane or rocket ship. I took this job in Brazil and was looking at ethanol fuels and engineering something like biofuels. I realized that human engineering complexity is the tiniest fraction of the complexity of the biological world and not nearly as elegant. I love how a bird flies. There are very different solutions to the same problem, but the biological solutions are so amazing.

So, I decided to go into chemical engineering as a graduate student, mainly to try to do biofuels research, but of course, that was the end of that. Reagan was the new president. Cars were getting long again, and no one cared about energy efficiency. But it was the beginning of the DNA revolution, so I said, there is a whole new world of biological engineering that Im going to be part of. Chemical engineering was a great way to combine chemistry, biology and engineering.

At that time, was chemical engineering fairly separated from the biological sciences?

Chemical engineering has a long history of dealing with biological processes, but they werent doing it using genetic engineering, they were doing it at the process design level ethanol plants and agriculture and food science and things like that. But what happened starting in the later [19]70s and early [19]80s is that these new techniques of recombinant DNA technology started to become available.

I and other people realized that you could solve engineering problems at the level of the catalyst. If you could design a better catalyst, you could solve all sorts of process problems. We took the long history of chemical engineering and grew this new protein engineering out of it.

What do you see as the future of the enzymes you are making?

I dream of the day that all the synthetic chemists will be replaced by bacteria. (laughs) It makes me popular in the chemistry circles. Think about it, if you could just genetically encode all these transformations, you could take renewable resources to anything you want.

You worked at the forefront of the variety of disciplines, chemistry, biology, engineering as well as agriculture. Is there anything you are more interested in learning more about?

Im learning more about everything everyday. I take on new jobs, Im on the board of directors of Alphabet [Google parent company], so Im learning about antitrust suits. The world is a fascinating place. Theres lots of science, but theres lots of other interesting and important things to learn about. Im always listening to seminars and getting ideas, it never stops.

Beyond science, in college, what were some of the things that formulated the perspectives you have today?

That was my problem. I became a mechanical engineer because that had the fewest number of requirements. Then I could take Russian, Italian, French, economics and development. At Princeton, its a very liberal arts school, but also a good engineering school. I was able to learn a lot of different things and explore different ideas.

I think thats very important as a young person because you never know where your inspiration will come from. I also took a lot of time off. I lived in Spain, Italy, Brazil. You never know when something is going to be useful to you later on.

One problem with the educational system is that it is more eye-closing than eye-opening.

How are you able to go beyond calculations and keep an open mind in the sciences and engineering?

All I have to do is remember all the people who told me that what I was doing was a total waste of time. One group said it was impossible; another it wasnt worth doing anyway. These were top scientists, and they really thought they knew. So, I think we all should be humble in thinking what is worth doing and thinking we know the answers because all sorts of crazy surprises come up.

[Students] go and think everything is known. Will there be room for me? Will I be able to make fundamental advances? I felt that same way when I was an undergrad and yet there is so much that is unknown. There is so much gold out there to discover and uncover if you dont close your eyes to it. One problem with the educational system is that it is more eye-closing than eye-opening.

What has been your favorite class to teach?

I love teaching my biomolecular engineering class. I have a debate with a rational designer, another faculty member at Caltech, who rationally designs complex molecules and I am the evolutionist. We just throw tomatoes at each other across the room, and students love it. They see the debate in real-time and then they have to make a decision of who is right and who is wrong. Are they both right? When do I use one method versus the other? Thats a fun one.

Do you look more to biology or chemistry when facing a problem?

It really depends. If we are looking for new chemistry to do a chemical reaction that a human has invented, you start with chemistry. What is the mechanism? Is there a protein that looks remotely like this? So, you are inspired by the chemistry. On the other hand, if you want to understand how evolution works, there is no chemistry in that. It is much more in the biology side.

I run a big research group now and almost all the problems are brought in by students and postdocs. My job is to be an editor of ideas rather than the generator of ideas. They are all going to have their own training and way of looking at it and I try not to close their eyes.

At what point did you become interested in preserving the environment?

I was already interested in alternative energy coming out of college. That was part of the environmental pr
oblem, but it was more about stability and sustainability. How can we become independent from the Middle East? So, it was also political. As my career has gone on, I see the tremendous destruction that we are doing to the planet. It just becomes worse and worse, so I have become much more interested in trying to limit environmental degradation.

How are you able to carry out such wide-ranging projects?

They are not my ideas. The insect pheromone (Provivi) was my former students who said, I want to take these methods I learned at Caltech and use them on a really important problem. So, that came out of that. That is the wonderful thing about having a technology that is simple to use. Directed evolution is really simple and fundamentally powerful.

All sorts of people will use it for really interesting things, and then I can take at least some tiny amount of credit. Evolution is everything in the biological world. It is not surprising that it has many applications in biotechnology.

How did you get interested in teaching and what inspires you to continue teaching?

I wanted to have no boss. That was the driving force to get me into academics. I have had lots of jobs over my lifetime, and the one thing I really didnt like was having a boss. I went into academics so that I could do what I was interested in. It is a lot of responsibility because you have to do something interesting, otherwise you get booted out.

I wanted to plot my own course. I probably wouldnt have chosen to teach, but I really like working with grad students. I like working with a team and I learned how to do that over the course of my career, so that teaching became more enjoyable as I got better at it.

Do you have advice for undergrad students pursuing engineering or chemistry?

Id like them to know that the horizon is wide open. The field is wide open. There is so much more to do and discover. It is a great way to use your creativity to solve big problemsto do something good for the planet.
