CLEMSON, S.C. When a beautiful pup has a dental problem, its time to call a veterinarian. But when it runs in the family, researchers take the case.

Clemson University researcher Leigh Anne Clark and her graduate student, Sydney Abrams, study inherited diseases in canines. They have worked previously on conditions that affect Shetland Sheepdogs, those fluffy Sheltie herding dogs that closely resemble a miniature version of Lassie. The pair collaborated on a paper titled Variants in FtsJ RNA 2-O-Methyltransferase 3 and Growth Hormone 1 are associated with small body size and a dental anomaly in dogs, which was published Sept. 21 in PNAS, the official journal of the National Academy of Sciences (NAS).

After Sheltie breeders sought answers about maxillary canine-tooth mesioversion, Clark and Abrams subsequent research led to a groundbreaking discovery.

We wanted to find the genetic basis for lance canines and ended up discovering that its related to the dogs body size, said Abrams, who is lead author on the paper. This is one of the first times a trait in dogs has been attributed directly to genes influencing growth.

The condition is known among Sheltie breeders as lance canine, added Clark, an associate professor in the College of Sciences Department of Genetics and Biochemistry. Thats because that canine tooth points forward, like a lance or spear, instead of pointing down like it should.

The tooth can be extracted or fixed with orthodontics, but that doesnt address its cause. If left untreated, it can cause ulcerations on the lip or prevent the dog from closing its mouth. It can also lead to periodontal disease.

It was having an impact on the breed because when you compete in conformation trials, the teeth are one of the things they look at, Clark said. You must have your canine tooth and it has to be pointing down otherwise, its a fault.

Dogs with lance canine can become pets, of course, but they are not shown.

If youre not shown, you are less likely to be bred, Clark said. These dogs are getting pulled from the breeding pool and youre losing genetic diversity over a crooked tooth.

Because the condition is rarely seen outside the Sheltie breed, Clark said she was sure it was heritable. Getting to its source would help breeders work toward eliminating it.

We took a genome-wide approach looking at genetic markers that span the entire genome hoping to find one or more of these markers associated with lance canine, she said.

This was no small task. Abrams began this study while she was still an undergraduate.

I started working on dental abnormalities in Shelties in 2017 as an undergraduate and received funding from the Clemson University Honors College, Abrams said. This enabled me to continue my research at a high level.

It took years to collect these samples, Clark added. Its slow and tedious to get enough samples to do these kinds of studies.

The dogs whose samples were used could not share common grandparents, so Abrams collected more than 200 samples and ultimately used 78 of those in genome-wide research.

I contacted owners from all over the world to collect the samples, said Abrams, who received her Bachelor of Science in genetics in 2019 before joining Clarks laboratory as a graduate student. I even got to present a portion of this work at a canine and feline genetics conference in Bern (Switzerland) in May 2019.

Abrams and Clarks efforts paid off in a surprising way.

We did find an association on chromosome 9, Clark said. In this region of the chromosome, there were about 30 genes. We generated sequence from a Sheltie with both canine teeth affected. We looked at those 30 genes for a variation that might have an effect. We found two variations in two different genes that looked like they could be problematic. One of the genes was growth hormone.

Because that gene is necessary for normal growth, Clark and Abrams went back to the study population and asked the owners to submit their dogs height and weight.

What we discovered is that dogs with lance canine are, on average, one inch shorter and six pounds lighter than unaffected Shetland Sheepdogs, Clark said.

And in a small breed that averages about 20 pounds and is 13 to 16 inches at the shoulder, this variation is significant.

Nobody had noticed before, Clark said.

But owners quickly had an aha moment as they made the connection.

Next, we looked at these two variations in the genomes of 224 other breeds and wild canids, Clark said. But they were only in toy breeds. They are all tiny dogs. The consequence of the growth hormone gene mutation is a mutant protein that is exactly the same as the one found in humans with pituitary dwarfism.

There is more work to be done, including exploration of why some canines are prone to missing teeth and what connection that might have to other health issues.

Missing teeth is interesting because it is in all breeds, but is more common in small breeds, Clark said. Dogs with two lance canines are significantly more likely to have missing teeth than a dog that has one lance canine. Theres some relationship there.

In addition to Abrams and Clark, other authors include Alexandra Hawks, Jacquelyn Evans and Jennifer Mason from the Clemson Universitys Department of Genetics and Biochemistry; Thomas Famula from the University of California, Davis; Mary Mahaffey from the University of Georgia; and Gary S. Johnson from the University of Missouri.

Abrams is supported by the Collie Health Foundation, and one of her latest projects is to study the genetics of epilepsy.

Anita Qualls and Aditya Wood: Feeding the Frontlines – University of Georgia

Anita Qualls and Aditya Sood dont remember when they met.

Maybe it was during a group gathering in Myers Hall, where they both lived freshman year. It could have been in one of the many biology classes they shared. Regardless of the starting point, Qualls BS 19 and Sood BS 19 both agree that they became close friends while taking an evolutionary biology course their senior year.

So in the spring, when Qualls was looking for a way to support those fighting the COVID-19 pandemic, Sood was one the first people she contacted. The nonprofit they formed has helped ease the burden of thousands of frontline workers across the state. And at a time when collaboration is more important than ever, they are helping bring the UGA community closer together.

When the pandemic hit, Sood was in his second semester of medical school at Emory. Qualls was in Cambridge, England, as UGAs first-ever Churchill Scholar. The prestigious scholarship gave her the opportunity to study immunology before entering medical school at the University of California, San Francisco this fall.

After Qualls was forced to return to the States in mid-March, she actively sought ways to contribute to the fight against COVID-19. One of the friends she reached out to was Cali Callaway BS 17, MS 17, a third-year medical student at Cornell. She was involved in an effort to deliver restaurant-made meals to health care workers around New York, called NYP Healthcare Heroes.

The concept created a partnership between the organization and local restaurants who made and then delivered fresh meals to health care workers in hospitals and medical clinics around the state. To minimize touchpoints, Feed the Frontlines volunteers arent directly involved with food preparation or delivery. Instead, they handle the logistics of connecting with health care providers and providing funding to restaurant partners.

When Qualls, who was quarantining in Hiawassee but lives in Johns Creek, learned about Feed the Frontlines, she set up a similar effort in Athens. But she didnt want to stop there, so she contacted Sood, who lives in Alpharetta.

We realized there was a need, not just with healthcare workers who were struggling but also with local restaurants that had no source of income but massive rents to pay. We realized we could join these two together, not just in Athens but across all of Georgia, Sood says.

What followed was a masterclass in networking and coordination. Qualls and Sood cast a wide net and reeled in dozens of friends and acquaintances from across Georgia who, like them, were trying to figure out a way to make an impact.

Feed the Frontlines Georgia grew from four people to 80 in just two and a half weeks, and it now serves more than 40 hospitals and clinics across the state. More than 1,000 meals are delivered statewide every week. Qualls estimates that 90% of the volunteers are current UGA students or alumni.

We were amazed at the interest from our friends to be involved, Qualls says. (A list of UGA alumni and students who work with Feed the Frontlines Georgia is listed below.)

Weve never worked on something that has been so fast paced and growing like this, she continues. Weve both run organizations before, and weve both been in leadership. But this was really unique because of the timeline and because of the eagerness of every person in our organization.

Feed the Frontlines Georgia serves not only cities like Athens, Atlanta, and Columbus, but it also provides crucial support to rural areas like Berrien County (east of Tifton) and Lanier County (near Valdosta).

To date, they have raised more than $75,000, which has kept dozens of restaurants afloat. Qualls and Sood say Feed the Frontlines Georgia will continue as long as there is a need, even after they return to school in the fall if necessary.

Systolic Blood Pressure and Orthostatic Hypotension Are Linked to Dementia – Neurology Advisor

Systolic orthostatic hypotension (OHYPO) and variability in visit-to-visit systolic blood pressure (BP) postural changes may be associated with increased risk of dementia, according to study results published in Neurology.

BP postural changes affects a significant percent of the elderly population, and various pharmacologic/non-pharmacologic interventions may improve orthostatic symptoms. Additionally, while previous studies have indicated the association between OHYPO and cardiovascular outcomes, much less is understood about the relationship of OHYPO with cognitive ones. Study researchers consequently sought to evaluate whether OHYPO and variability in visit-to-visit BP postural changes were associated with dementia.

The study included older adults (mean age, 73 years; 53% women) who were pooled from the Health, Aging, Body Composition cohort study (n=2131). In this cohort, changes in orthostatic BP were assessed at baseline and at 1, 3, and 5 years. Patients were considered to have OHYPO if they experienced a decrease of at least 15 mmHg in systolic or at least 7 mmHg in diastolic BP after standing from a sitting position for a minimum of 1 out of 3 clinic visits. Both systolic and diastolic OHYPO were assessed separately.

Variability in BP postural changes were also examined over time. Additionally, the investigators examined the prevalence of incident dementia over a 12-year period after baseline (total follow up, 17 years) by assessing dementia medication use, changes in Modified Mini-Mental State (1.5 standard deviation decline), or records of hospitalization with dementia.

A total of 309 (14.5%) patients had OHYPO, 192 (9.0%) had systolic OHYPO, 132 (6.2%) had diastolic OHYPO, and 462 (21.7%) developed incident dementia over 12 years. Systolic OHYPO correlated with greater dementia risk in a multivariable adjusted analysis (adjusted hazard ratio [HR], 1.37; 95% CI, 1.01-1.88). No significant association was found between diastolic OHYPO and increased risk of dementia (adjusted HR, 0.92; 95% CI, 0.60-1.40). Variability in systolic BP postural changes also correlated with an increased risk of dementia, particularly in the highest tertile of variability (adjusted HR, 1.35; 95% CI, 1.06-1.71).

Limitations of this study included its observational design, the lack of a formal and structured clinical assessment for dementia, and the reliance on a relatively small number of orthostatic BP measures to assess variability in BP postural changes.

The study researchers concluded that the management of orthostatic systolic BP and its variability, including with an optimization of antihypertensive treatment, could be a promising interventional target in preserving cognitive function among older adults.

ABSTRACT

Since its original report in January 2020, the coronavirus disease 2019 (COVID-19) due to Severe Acute Respiratory Syndrome coronavirus 2 (SARS-CoV-2) infection has rapidly become one of the deadliest global pandemics. Early reports indicate possible neurological manifestations associated with COVID-19, with symptoms ranging from mild to severe, highly variable prevalence rates, and uncertainty regarding causal or coincidental occurrence of symptoms. As neurological involvement of any systemic disease is frequently associated with adverse effects on morbidity and mortality, obtaining accurate and consistent global data on the extent to which COVID-19 may impact the nervous system is urgently needed. To address this need, investigators from the Neurocritical Care Society launched the Global Consortium Study of Neurological Dysfunction in COVID-19 (GCS-NeuroCOVID). The GCS-NeuroCOVID consortium rapidly implemented a Tier 1, pragmatic study to establish phenotypes and prevalence of neurological manifestations of COVID-19. A key component of this global collaboration is development and application of common data elements (CDEs) and definitions to facilitate rigorous and systematic data collection across resource settings. Integration of these elements is critical to reduce heterogeneity of data and allow for future high-quality meta-analyses. The GCS-NeuroCOVID consortium specifically designed these elements to be feasible for clinician investigators during a global pandemic when healthcare systems are likely overwhelmed and resources for research may be limited. Elements include pediatric components and translated versions to facilitate collaboration and data capture in Latin America, one of the epicenters of this global outbreak. In this manuscript, we share the specific data elements, definitions, and rationale for the adult and pediatric CDEs for Tier 1 of the GCS-NeuroCOVID consortium, as well as the translated versions adapted for use in Latin America. Global efforts are underway to further harmonize CDEs with other large consortia studying neurological and general aspects of COVID-19 infections. Ultimately, the GCS-NeuroCOVID consortium network provides a critical infrastructure to systematically capture data in current and future unanticipated disasters and disease outbreaks.

CSF UCH-L1 Biomarker Predictive of Long-Term Disease Progression in MS – Neurology Advisor

Baseline levels of the biomarker ubiquitin C-terminal hydrolase L1 (UCH-L1) measured in cerebral spinal fluid (CSF) are predictive of long-term multiple sclerosis (MS) progression, according to study results presented at the 8th Joint American Committee for Treatment and Research in Multiple Sclerosis (ACTRIMS) and European Committee for Treatment and Research in Multiple Sclerosis (ECTRIMS) MSVirtual2020 event, held September 11-13, 2020.

The development of increasingly powerful MS treatments has highlighted the need for prognostic biomarkers. The investigators of this prospective cohort study sought to compare the long-term prognostic value of 4 proteins found in paired serum and CSF samples collected following MS diagnosis. Study researchers identified 67 patients who had serum collected within 5 years of their first MS symptom onset and with over 15 years of routine clinical follow-up. Using digital immunoassay, Neurofilament light Chain (NfL), Glial Fibrillary Acidic Protein (GFAP), Tau, and UCH-L1 levels were measured in serum and CSF samples from participants with MS and matched controls. Study outcomes of biomarker performance evaluated by the researchers consisted of conversion to progressive MS phenotype and achieving greater than or equal to 4 on the Expanded Disability Status Scale (EDSS).

In 3 of 4 candidate markers, baseline CSF levels were higher in participants with MS compared with controls: NfL (624 vs 277 pg/mL, respectively; P =.0001), GFAP (6900 vs 694 pg/mL, respectively; P <.0001), and Tau (15.4 vs 8.12 pg/mL, respectively; P =.0001). There was no such difference in UCH-L1 levels. In serum samples, differences between MS and control groups, although less noticeable, were found in baseline measures of NfL (10.1 vs 7.3 pg/mL, respectively; P =.0037) and GFAP (68 vs 51 pg/mL, respectively; P =.0011), but not in Tau or UCH-L1 marker levels. In receiver operating characteristic curve analyses, UCH-L1 levels in CSF samples were most predictive of developing an EDSS greater than or equal to 4 after 15 years of follow-up (AUC, 0.72; P =.003), followed by NfL levels in serum (AUC, 0.70; P=.012) and GFAP levels in CSF (AUC, 0.66; P =.03). UCH-L1 levels in CSF were also most predictive of developing a progressive MS phenotype (AUC, 0.69; P =.0097), followed by GFAP levels in CSF (AUC, 0.66; P =.024) and NfL levels in serum (AUC, 0.64; P =.057). Levels of GFAP, Tau, and UCH-L1 in serum samples, as well as Tau levels in CSF samples, were not predictive of a progressive MS phenotype nor an EDSS score of greater than or equal to 4.

Study researchers concluded that baseline CSF UCH-L1 marker levels were associated with long-term outcomes in MS and that this biomarker was more predictive of EDSS worsening and conversion to a progressive phenotype than well-established markers NfL and GFAP.

UB medical school graduate dies of COVID – University at Buffalo The Spectrum

Adeline Fagan, a second-year OB-GYN resident and 2019 graduate of the UB Jacobs School of Medicine, died on Saturday of a brain bleed after battling COVID. She was 28.

Fagan is being remembered as a beautiful and passionate doctor who wanted to help as many people as she could. The Syracuse native was a fighter who is now inspiring others to give back.

Fagan's story is also a cautionary tale of the dangers of COVID.

On July 8, Fagan worked a 12-hour shift treating COVID patients in the ER. She felt fine and was excited to see patients at the start of her shift, but sometime during the evening, she started experiencing flu-like symptoms.

Fagan spent the next few weeks in the hospital, as doctors tried treating her with several different respiratory therapies and dozens of drugs. When Fagan didnt respond positively to these treatments, she was enrolled in an experimental drug trial.

But before her family could see if this new drug was effective, Fagan's lungs stopped supporting her.

On Aug. 3, Fagan was intubated and placed on a ventilator. On Aug. 4, she was placed on extracorporeal membrane oxygenation (ECMO), a machine that pumps blood from the body, oxygenates it and pumps it back into the body.

Over the past few days, Fagan's family became optimistic that Adeline was turning a corner. On Friday night, a nurse called her parents and sounded quite upbeat when she said that Adeline was decannulated and no longer on the ECMO machine.

On Saturday night, Fagan was doing wonderfully. At 10 p.m., her family called the hospital and received a positive report. But 30 minutes later, they got a call back. A nurse had come into Fagans room for a routine task and noticed that she wasnt responsive.

Fagan had suffered a massive brain bleed.

She was given a one in a million chance for survival.

We spent the remaining minutes hugging, comforting, and talking to Adeline. And then the world stopped Fagans family wrote on their GoFundMe page.

Fagan was in her second year of residency in obstetrics and gynecology. She was memorialized by former professors and classmates in a story posted to the Jacobs School of Medicine website Monday.

Addie decided on a career in obstetrics and gynecology fairly late in medical school, but once she made up her mind she was willing to do whatever it took to pursue her goal, said Vanessa M. Barnabei, a professor of obstetrics and gynecology. She had an amazingly positive attitude and infectious smile and her passing will deprive thousands of women of her empathy and commitment to their health.

Fagan's father, Brent, kept an online journal detailing his daughter's battle with the virus.

The first semester of medical school aims to lay a foundation of knowledge on which medical students can grow into physicians. Among the key foundational pieces laid in those opening months of training is a familiarity and understanding of the human body, often introduced through anatomy.

The COVID-19 pandemic has shifted much of the opening months of medical school from in-person learning to online learning for many preclinical medical students. Still, at some schools, when it comes to anatomy, there is no digital replacement for the real thing.

At the Heritage College, cadaveric anatomy is a priority, said Isaac J. Kirstein, DO, a dean at the Cleveland campus of Ohio University Heritage College of Osteopathic Medicine (OUHCOM), which is a member school of theAMAsAccelerating Change in Medical Education Consortium. There are two key reasons for this. First, the cadaver is the medical students first patient; the students engage with the humanism of the experience, and it forms their professional identity. Second, to truly learn osteopathic principles requires an understanding of anatomy that simulation alone cannot provide.

Learn five tips to survive first-year anatomy.

Opting to offer in-person anatomy meant putting in place several safety protocols. Students spend less time in labsroughly a half day per weekand learn in smaller groups. Instead of performing dissections, cadavers are dissected prior to students arriving in the lab.

Theres a lot to be said for learning the art of dissection to help students pick up some basic techniques, Dr. Kirstein said. We are confident that can be learned during their second year.

OUHCOM first-year medical students began anatomy in late August. Even without dissection, working with a cadaver, rather than attempting to learn the body via some sort of simulation medium, has proven valuable.

Its really important that we learn the 3D aspect to a body versus simply learning from a textbook, said Ashley Aslo, a first-year student at OU. Trying to conceptualize in our brains, its really important to understand that theres layering. And on the other hand, not just science but also being introduced to our first patient and understanding that these people donated their bodies for us to learn fromlearning how to respect them was really important. By doing anatomy in person, we have learned all of those aspects.

As far as any potential safety concerns students might have, Aslo said the systems put in place have largely negated them for her.

Its a scary time, Aslo said, but she believes the university has done an amazing job of adhering by social distancing rules while providing the opportunity to get a good education.

Medical schools will continue to adjust to disruptions related to COVID-19, balancing safety concerns with ensuring that students are appropriately trained to care for patients, families and communities.

Foster School of Medicine to welcome Class of 2024 with White Coat Ceremony – El Paso Herald-Post

The Foster School of Medicine class of 2024 will receive their firstwhitecoats as part of the time-honoredWhiteCoatCeremony on Saturday.

Due to the COVID-19 pandemic, this will be the first year that the schools future doctors will not have their coats placed on them by deans of the medical school. Instead, each of the 116 students in the class of 2024 will approach the podium and give a short message of gratitude to those who have helped them reach this milestone.

Gordon Woods, M.D., associate professor of Internal Medicine and college mentor, will give the keynote address.

At this years ceremony, the Foster School of Medicine continues its own tradition with its twelfth class. The Class of 2024 will be welcomed by faculty and staff, the El Paso community and fellow Texas Tech University Health Sciences Center El Paso students.

TheWhiteCoatCeremonyis a traditional rite of passage for entering medical students. Started in 1993 by Arnold P. Gold and his foundation, theceremonyis designed to remind students of the humanism inherent in their chosen field.

Since its inception at the Columbia University College ofPhysicians and Surgeons, thewhitecoattradition has expanded to97% of medical schools throughout the U.S. and Canada.

What is medical education research? – Baylor College of Medicine News

In order for us to train medical students to be the best physicians they can be, we have to be the best medical educators that we can be. That includes keeping up with our colleagues around the country on best practices and sharing our own experiences. This is why medical education research is so important.

When I say that I do medical education research, I get a lot of funny looks. Many people dont understand what it is or what it looks like, and they dont understand that its important research, just like basic science or clinical research.

There is a science to the neurobiology of how we actually learn, and learning takes place in many environments, including the classroom and the clinic. We publish guidelines and protocols for best practices in medical education, just as clinicians publish the newest diagnosis and treatment protocols.

We also perform trials comparing one method of teaching to another, and study how our education interventions effect outcomes.

The very first goal in our Baylor College of Medicine School of Medicine strategic plan is to establish Baylor College of Medicine School of Medicine as an international leader in medical education innovation and pedagogy and our medical education scholarship is one of the ways that we will get there.

Its important to us that our learners are exposed to the latest technologies, learning tools, and pedagogy because that training will ultimately help them provide the best care to patients. Its impossible to teach students everything there is to know about medicine. I believe it should not be our aim to even try.

I believe that we should focus much more attention on teaching students how to critically appraise, apply knowledge and become consciously competent master adaptive learners.

For example, one recent finding about medical education showed us that learners do better when they are active participants in the learning process. There is now far too much evidence that shows lecture-based learning is not effective.

As a result, we have been transforming the first 18 months of our curriculum. We have increased the amount of active learning in every class, utilizing techniques such as flipped classrooms and team-based learning. New courses have been introduced that are 100% active learning.

I have to admit that one of my favorite things about my job is indeed the research aspect of what I do implementing the latest science in teaching and learning and studying its effects. I love curling up with my academic journals and reading them cover to cover, and I actually love writing papers.

I have collaborated on numerous studies with colleagues from around the country. I find that working with other institutions validates the problems that we all face in medical education. They are not unique and its great to problem solve together.

One recent challenge that we faced during the COVID pandemic was how we could continue to effectively teach our students who were pulled from the clinical learning environment. After consulting with counterparts across the country, we found that we needed to increase our usage of high quality, interactive, online patient cases.

Utilizing these cases that provide the opportunity for critical thinking should not be limited to our clinical students they should be the foundation of how we teach students from the first day of medical school. Adult learning theory supports that even students with no medical training can work through clinical cases albeit at a different level than senior students.

Doing medical education research is part of how educators get promoted up the ranks just like in basic science/clinical science, and we disseminate our findings in scholarly fashions the same way through posters and abstracts at national conferences, as well as in publications.

One difference is while there are opportunities for grants, there are not quite as many available for pure medical education research. Most are in much smaller amounts. However, much medical education research could be incorporated into the basic science and clinical research grants that are submitted at health sciences universities.

I try to mentor all junior faculty educators to study what they do. Make sure to turn that abstract into a paper and be persistent when submitting work for publication. It may not get accepted the first time around, but persistence really does win out.

We need to be more cognizant about publishing what we do so that others can learn from it. There is no point in every institution recreating the wheel and my goal is that Baylor will be creating those wheels and looked at as the leader in medical education.

$25 million gift for TCU and UNTHSC School of Medicine – Fort Worth Business Press

The late Anne Marion and the Burnett Foundation have made a $25 million gift to establish The Anne W. Marion Endowment in support of the operations of the TCU and UNTHSC School of Medicine in perpetuity.

This transformational gift will provide funds to support students, faculty and programming for the medical school, the school said in a news release.

With a mission to transform health care by inspiring Empathetic Scholars, the gift is a catalyst that will enable the School of Medicine to be a leader in medical education, delivery of care and scholarship and necessary skills for complex health care issues, including the challenges of providing care during the COVID-19 pandemic.

I am inspired by the vision of the School of Medicine to transform medical education. This school is bringing considerable advances and innovations that are reshaping curriculum and preparing its graduates to better serve the community. I am pleased to make this gift, Mrs. Marion said before her death in February.

Mrs. Marions family ties to the Fort Worth community date back nearly a century and include a long history of supporting the priorities of the city and its institutions. The Burnett Foundation has been a generous patron of the city investing significant resources to enhance the community in myriad ways.

I am beyond thrilled and grateful for this incredibly generous gift, said Stuart D. Flynn, M.D., dean of the School of Medicine. This historic gift will help us empower current and new generations of talented and diverse students and faculty to address the complex health care issues challenging our world. The foundations generosity supports our efforts to create physicians who are highly skilled and knowledgeable yet compassionate care givers.

The TCU and UNTHSC School of Medicine received preliminary accreditation by the Liaison Committee on Medical Education (LCME) in 2018 and started its first class of 60 students in July 2019. This support comes at a time when the medical school is beginning preparations for the next step of LCME approval, provisional accreditation.

The gift will support the operations of the school, which offers a novel educational experience, tailored to how students learn best, capitalizing on teamwork and active application sessions and assisting students to retain the knowledge they are learning, with the patient always at the center, the news release said.

This new medical school is training future physicians who will not only care for patients, but will provide care, empathetic communication with patients, and the critical thinking skills to lead in an industry that is always evolving and innovating, TCU Chancellor Victor J. Boschini, Jr. said in the announcement. This gift will contribute greatly to current and future faculty and students. I am humbled and thankful that our late colleague and dear friend Anne Marion and the Burnett Foundation embraced our goal of creating Empathetic Scholars with this generous gift to support the School of Medicine.

UNTHSC President Michael R. Williams said at a time when innovation in medical education is not only needed, but also demanded, the gift will ensure that the M.D. School continues to grow its innovative curriculum.

Why Some Cancers May Respond Poorly to Key Drugs Discovered – UT News | The University of Texas at Austin

AUSTIN, Texas Patients with BRCA1/2 mutations are at higher risk for breast, ovarian and prostate cancers that can be aggressive when they develop and, in many cases, resistant to lifesaving drugs. Now scientists at The University of Texas at Austin and Ajou University in South Korea have identified a driver of the drug resistance that can make a life or death difference for patients with these cancers.

A major issue with cancer treatments is the development of resistance, said Kyle Miller, a UT Austin associate professor of molecular biosciences. When treatments stop working for patients, its incredibly demoralizing and its been a huge drive in research to understand these resistance mechanisms.

In a paper published today in the journal Molecular Cell, the researchers describe a protein that may help doctors predict which patients will become resistant to a class of drugs frequently used to treat BRCA 1/2-deficient tumors. The finding could help create more effective treatment plans for their patients.

The scientists identified that a protein called PCAF promotes DNA damage in BRCA 1/2-mutated cancer cells. Patients with low levels of this protein are likely to have poor outcomes and develop resistance to a type of drug that is used to treat BRCA-deficient tumors, called a PARP inhibitor.

PARP inhibitors are an important breakthrough in treating these aggressive cancers, Miller said. What we found is that when levels of PCAF are low, it actually protects the cancer cells from this drug. By testing biopsy samples, doctors may be able to tell using PCAF as a molecular marker for PARP inhibitor responses what treatment may work best for a patient.

Fortunately, there is already another class of drugs on the market, called HDAC inhibitors, that can boost the effectiveness of the PCAF protein. HDAC inhibitors and PARP inhibitors have the potential to be prescribed as a combination therapy.

Previous studies have shown that these two drugs work well together, Miller said. We believe weve found the reason why.

It is possible to test for PCAF levels in biopsy or tissue samples, Miller said, and in the future, the test could be included on a standard panel for cancer testing.

But unlocking the workings of PCAF doesnt just offer clues to combatting cancer. Because this protein is responsible for modifying chromatin, the stuff that organizes 6 feet of DNA in each of our cells so that it fits into its nuclear volume, PCAF also may offer important clues about cell replication.

The focus in my lab is on understanding chromatin and its impact on replicating DNA, protecting DNA and controlling access to DNA, Miller said. Our goal is to understand how every molecule is interacting inside our cells, as this gives clues to what is going wrong in human diseases.

Jae Jin Kim and Seo Yun Lee from the Miller lab were first authors on the paper, and Blerta Xhemalce, an associate professor of molecular biosciences at UT Austin, and Ji-Hye Choi and Hyun Goo Woo of Ajou University contributed to the research.

The research was funded by the National Cancer Institute, the National Institutes of Health, the American Cancer Society, the Department of Defense Congressionally Directed Medical Research Program Breast Cancer Breakthrough Award and the National Research Foundation of Korea. Miller is a CPRIT scholar with the Cancer Prevention & Research Institute of Texas, a member of the Livestrong Cancer Institutes of Dell Medical School and a member of the Dan Duncan Cancer Center at the Baylor College of Medicine.

Teaching hospitals help businesses and schools safely reopen and stay open – AAMC

The first time a staffer at The Florida Aquarium tested positive for the virus that causes COVID-19, anxiety shivered through the executive team. The chief operating officer called a medical doctorat nearby Tampa General Hospital (TGH) early one morning (I think he woke her up, says aquarium CEO Roger Germann) asking what to do: Shut down? Tell some employees to quarantine? Test them all?

Seetha Lakshmi, MD, an epidemiologist, asked a series of questions to assess the risk of a COVID-19 outbreak which appeared to be minimal, because the employee had limited contact with workers and visitors and had recently been off for several days. Lakshmi guided the aquarium on informing staff about the test result, tracing the employees on-site contacts, and continuing its procedures to prevent coronavirus infections.

She calmed us down, Germann says.

That kind of personal attention is golden for businesses and schools trying to operate safely during the pandemic and more and more of them are getting that assistance from university hospitals that created consulting services to help organizations protect their staff and customers from the virus.

Branded with such uplifting names as Reopening America, Safe to Return, and Healthy Restart, these health system initiatives partner with local businesses to create COVID-19 safety plans, conduct walk-throughs on-site to assess risks and establish protocols, run events to educate employees about the virus, provide expedited virus testing for staff, and update leaders about how the latest COVID-19 findings might affect their safety policies.

The partnerships give hospitals new ways to apply what they learned from protecting their staff and patients from COVID-19 to meet an expanding public health need outside their doors.

We see this as an extension of our role to slow the spread of the virus in the community, says Joan Zoltanski, MD, who oversees the Healthy Restart initiative at University Hospitals in Cleveland.

Since the start of the pandemic, doctors and other staffers at hospitals around the country have been fielding calls from local businesses, schools, and government agencies about how to stay open or reopen safely. Many staff have been happy to help, but in some places the volume of requests convinced administrators they needed more than a take-it-as-it-comes approach.

At our morning meetings, we kept hearing, This company reached out to this doctor, this school and this church reached out to whoever they knew in the system to try to understand best practices, Zoltanski recalls. We were all trying to answer these questions organization by organization. Our CEO said, Weve got to put some structure around this.

The process worked in somewhat the opposite direction at Ochsner Health in Louisiana: Donny Lawrence, vice president of business development, says his team contacted companies for which Ochsner already provides employee health services, asking if they needed help dealing with COVID-19.

It was overwhelming, Lawrence says of the response. With little to no information available about the virus back then, they basically said, We dont know whats going on with this thing.

Although medical schools and teaching hospitals routinely provide free COVID-19 resources for the public through website resources, live webinars, and conference calls some organizations want a more personal relationship customized to their needs. And they are willing to compensate the health systems for the staff time required to provide that.

Some of them [businesses] didnt even know where to begin because theres so much information out there, Zoltanski says.

To cut through the information overload, the health systems offer a range of consultation arrangements, typically led by high-level subject matter experts such as chiefs in epidemiology, infectious disease, and environmental hygiene backed by a bench of specialists from throughout the systems who jump in as needed to address details. Their clients range from single-site establishments and local sports teams (University Hospitals works with the Cleveland Browns) to sprawling businesses and school systems (New Jerseys Hackensack Meridian Health (HMH) advises the Bell Works research and retail hub and the Rumson School District).

While the primary goal is to contain the virus, another is to make staff and visitors confident about how these sites are containing the virus.

The hope is to give people a level of comfort that these businesses didnt come up with the opening plans on their own, says Michael Geiger, vice president of Care Transformation Services at HMH, which created the Reopening America initiative. They [schools and businesses] can say they built these plans with the endorsement of New Jerseys largest health care network.

The confusion and concerns of businesses and schools mirror those of the public, then go beyond.

There are lots of questions about masks versus social distancing, says Peggy Thompson, RN, director of infection prevention at Tampa General Hospital, which offers TGH Prevention Response Outreach. Do you have to do both? What if you have people who cant wear masks because of breathing problems?

Many businesses are wary about mandating masks and social distancing for customers, as such mandates have become flashpoints for debates about personal liberty. Before it reopened after a two-month precautionary shutdown this spring, The Florida Aquarium drew up a safety plan that did not require guests to wear masks then added the requirement at the recommendation of TGH.

Being a science-based organization, we decided that that was the right thing to do, even if it cost us some visitors, says Germann, the CEO.

On the other hand, aquarium leaders were initially anxious about whether they could keep visitors far enough apart throughout the facility, where the foot traffic sometimes flows through spaces too tight for passers-by to remain six feet apart.

They said that if somebody just passes somebody else, if they have the masks on, they are probably going to be okay, Germann says. They helped take some of the anxiety off us, by not overthinking some things.

As for the coronavirus tests it seems the more that someone reads about them, the more confused they get.

Its a lot of education about tests, says Geiger at HMH. The sensitivity of tests; the accuracy; when you use one test versus another. How often do I need to test [the staff]? What happens if somebody comes back positive?

Testing is one of several complicated subjects for which health experts explain the science, but they often advise business leaders to avoid getting mired in the minutia of it all. Pull back, they advise, to stick to the core principals of the safety plan.

What we are constantly reinforcing is the basics, Geiger says. Hand washing. Social distancing. Wear a mask.

While that advice goes for individuals as well, businesses and schools confront issues that most people dont think about like indoor air flow. How often should air be circulated through the heating and air conditioning? What filters block the coronavirus? Those questions create a demand not for doctors, but for experts in facility design and common spaces.

One of our most in-demand people is our director of safety; shes an industrial hygienist, says Zoltanski at University Hospitals. She thinks about air quality and filtration. How many times do you have to turn the air over in an operating room to make sure its clean? Whats the right filter for a virus versus a bacteria?

Air quality is one of the many concerns that make schools those that have brought back students and teachers more challenging to safeguard than most businesses.

Schools are complicated, says Lawrence at Ochsner Health. They have employees responsible for the safety of themselves and others; they have kids whose behavior can be difficult to control; and theyve got parents in the mix with ideas about what schools should and should not require of their children.

Some of the health systems assign specialists from their pediatric departments to help schools draft and enforce protocols.

Theres been a lot of apprehension around masking. How could you possibly think a first grader would wear a mask? Zoltanski says. We were able to say, They do it in our hospital every day. We share how we get them to do it our challenges and how we overcome them.

Ultimately, the relationship between the health systems and the businesses and schools they consult goes beyond the recommendations they put on paper.

For starters, the health systems stress that there are no sure answers to all COVID-19 questions, and some answers will change as scientists continue to learn about the virus.

Were not talking about risk elimination, says Lawrence at Ochsner Health. Were going to tell you what is safe, safer, and safest. You as a business owner can decide which portion of the risk you want to take on.

That message is important as business and school leaders weigh the implications of various safety procedures, including what they want to impose on their staff, students, and customers, how much money they want to spend, and how some procedures might affect their operations.

We reinforce to our clients that our expertise is health care, Geiger says. We dont pretend to know the intricacies of their industry.

For example, The Florida Aquarium did not implement a TGH suggestion to take the temperature of all visitors. The reliability of that procedure to detect a COVID-19 infection remains too uncertain to justify imposing that additional process on staff and guests, Germann says.

Of course, scientific knowledge about that might change in which case clients can contact someone at the health system for advice. That continuing relationship can be as valuable to clients as the initial planning.

I view it as an ongoing partnership, says Thompson at TGH. They will reach out with questions long after the initial contract has been fulfilled. Maybe they run into issues they hadnt anticipated.

As part of that relationship, TGH and HMH each provide display materials for businesses to show how they developed their safety procedures in partnership with either of those health systems. The materials, which include social distance markings on floors, signs for windows, and badges for websites, add another level of confidence for staff and visitors.

These projects make health care workers feel better as well.

Its been really rewarding, Thompson says. To feel that I can make a difference in outcomes in preventing a disease from spreading brings a new experience of professional satisfaction.
