"Sensory evolution provides the biologic engine of creativity." says Ivan Schwab. Born and raised in rural West Virginia, with the local woods and streams as a mentor, Ivan R. Schwab, MDFACS developed an interest in natural history and evolutionary biology. He attended West Virginia University and graduated with a degree in biology from the Eberly College of Arts and Sciences in 1969 followed by a medical degree from WVU in 1973. Completing his residency at Pacific Medical Center in San Francisco and following three fellowships in Ophthalmology, Dr. Schwab became a professor of Ophthalmology at the University of California, Davis. He has been active with the American Academy of Ophthalmology, several professional societies, and has served as a Board Director on the American Board of Ophthalmology. He has an active clinical practice, and is engaged in research on ocular surface disease. As a result of his research interests, he was the first person n the United States to transplant bioengineered tissue to the ocular surface in 1999. With an interest in comparative optics and physiology stimulated by his interest in natural history, Professor Schwab has published a book on the evolution of th eye entitled "Evolution #39;s Witness: How Eyes Evolved." We can see that underlying our physical structure is the mind, which not only controls our thought processes but helps assist us in carrying out day-to-day activities such as respiration, circulation, digestion and elimination. The mind and the body work in conjunction with one another to regulate our physiology. In order for the mind to act appropriately to assist the physical body, we must use our senses as information gatherers. We can think of the mind as a computer and the senses as the data which gets entered into the computer. Smell and taste are two important senses th at aid in the digestive process. When the mind registers that a particular food is entering the gastrointestinal tract, it directs the body to act accordingly by releasing various digestive enzymes. However, if we overindulge the taste buds with too much of a certain taste, such as sweet, we may find that the ability of the mind to perceive the sweet taste is impaired; and thereby the body becomes challenged in its ability to process sweet foods. Following damage to the airway epithelium, a repair process of dedifferentiation, regenerative proliferation and redifferentiation takes place that is invariably accompanied by mucus hypersecretion as a key element in the host defence mechanism. In chronic respiratory diseases, however, excessive mucus production leads to a pathological state with increased risk of infection, hospitalization and morbidity. An understanding of the mechanisms that underlie and maintain this hypersecretory phenotype is therefore crucial for the development of rational approaches to therapy.Despite a high and increasing prevalence and cost to healthcare services and society, mucus hypersecretion in chronic respiratory disease has received little attention until recently, probably because of the difficulties inherent in studying this pathology.
