Anti-aging Medicine: Wave of the Future, or Wrong Turn?

Anti-aging medicine is out of our general area of expertise, but it relates to one of our main interests: aging well. Hormone replacement therapy for men is nothing new. Years ago, I remember my father, a medical doctor, talking about the benefits experienced by some of his aging male patients after receiving testosterone injections. More recently, Ive received emails from older readers telling me how much better they feel and how much better their training is going after their doctor prescribed testosterone supplementation. One particularly enthusiastic 61-year-old man told me shortly after he started taking a testosterone pill under his tongue -- his testosterone had been found to be "low normal," and he had experienced a lack of muscle tone and a tendency to shift fat to his waistline, even though he was keeping his body weight stable and training regularly with weights -- that he was experiencing "a vast increase in energy." A month later, he reported that his doctor had switched him to a testosterone patch which "provides the same level of rise and fall of the testosterone level as is normal in young men." He said he was making "phenomenal" gains in body weight and strength. "I have not experienced anything like it since I was 20 years old," he wrote.

This man understood that the long-term effects of testosterone replacement are largely unknown, but assured me that he was being carefully monitored by his doctor for negative side effects. "The physical effects are too good to do without," he added. I still receive positive reports from him from time to time. He is now almost 65 and tells me hes still slowly getting stronger.

But it was not until doctor David Chauvin, an emergency room physician from Ohio, visited with us earlier this year that I realized that anti-aging medicine is a fast-emerging specialty. Immediately before coming to Albuquerque, Dave was in Las Vegas, Nevada, talking with the doctors at the Cenegenics Medical Institute. With close to 600 patients, Cenegenics is probably the countrys largest "age management medicine" practice. Dr. Barry Sears, who developed "The Zone Diet," is a high-profile patient, and three-time Mr. Olympia Frank Zane (58) wrote in the summer 2000 issue of his Building the Body newsletter (www.frankzane.com) that he is taking supplemental thyroid, human growth hormone and testosterone prescribed by Cenegenics Dr. David Leonardi.

[We received an email dated 11/19/2008, from Ann Castro, Director of Public Relations, submitting a letter from Cenegenics CEO Kristy N. Berry. The letter says Cenegenics Medical Institute is the world's largest age management medicine practice, with over 15,000 patients worldwide. In addition, Berry states that Cenegenics is not part of the anti-aging movement. "Unlike our 'anti-aging' counterparts, Cenegenics is pro-aging--regarding aging as a natural process (not a disease) that can be managed," Berry writes. Dave Chauvin brought me a packet of material on anti-aging medicine. One of the things I learned was that the American Academy of Anti-Aging Medicine (A4M) now has 8500 physician members and, according to A4M president Ronald Klatz, MD, DO, the membership is doubling every year. Clearly, this is a trend worth watching.

The article which follows is an outgrowth of my discussions with Dave Chauvin. I asked Dave to give our readers an overview of anti-aging medicine, with special attention to its most controversial therapy, multi-hormone optimization. Many traditional physicians question the wisdom of restoring hormones to the levels of youth; they believe thats too aggressive, at least until we have a better understanding of the long-term consequences. Some physicians, such as well-known alternative medicine proponent Andrew Weil, MD, dont believe in anti-aging medicine; they believe the aging process should be accepted and adapted to with conservative and healthful tactics, not fought. In a recent issue of Modern Maturity magazine, Dr. Weil said, "Healthy aging is having the physical faculties, mental faculties, and energy to do what you want as you move through life and being comfortable with the aging process."

Antiaging Creams: Read This Before You Waste Any More Money

You may (or may not) be surprised by how many women own anti-aging creams who havent even hit 30 yet. Their cabinets lined with little bottles and jars meant to protect them against that first wrinkle.

For women already beyond the age of 40, the anti-aging creams are meant to erase proof that they are indeed getting older.

Whether its a desperate stab at youth or compulsive vanity, anti-aging creams are just one vein of the multi-billion dollar beauty industry.

Its the one that promises women that their creams and lotions will turn back the clock. Advertisements silently wink promises that no one will ever have to know that you are a woman who actually ages.

But for all the smoke, mirrors, and hundreds of dollar spent, its important to ask if these so-called anti-aging creams are worth the money.

According to dermatologists and scientists, the answer seems to be, Mostly, no.

Before you take that mostly as a sign to rummage through a mountain of products, you may want to hear these professionals and their research out:

Few studies have been published in medical journals to show the products work as advertised or are safe to use. And the U.S. Food and Drug Administration doesnt require companies to prove that cosmetic products are safe or effective.

Efficacy is very vague in terms of over-the-counter products, said Dr. Simon Yoo, assistant professor of dermatology at Northwestern Universitys Feinberg School of Medicine. Without any oversight, it is difficult to say whether these do anything.

In other words, these people can claim ANYTHING about their product. And theres no proof that what youre putting on your skin is even safe. They only need to verify that their statements arent FDA validated and mumble certain other disclaimers.

Health Column: Anti-aging strategies

The goal of anti-aging is to slow, stop, or even reverse many of the changes associated with aging. This means staying healthy, active, and vibrant of body and mind, while avoiding the common diseases and infirmities of aging. A cornerstone of anti-aging strategy is to replace the hormones that naturally decline with aging, including the sex hormones estrogen, progesterone, and testosterone, as well as a few adrenal hormones.

HORMONE REPLACEMENT THERAPY (HRT)

The science of aging reveals we are designed to reproduce and to survive. Once accomplished, by about middle age, our genetic replication starts increasing in mistakes, enzymes slow in catalyzing chemical reactions, and hormones decline. Aging begins

HRT is really about preventing disease and maintaining quality of life. Its about keeping the body operating well. Not having optimal hormone levels is like letting a fine automobile just run out of oil, then declaring it is growing old as the red light comes on and it overheats.

After menopause female sex hormone production dramatically plummets, and while the same thing happens to men at andropause, it occurs at a slower rate. One result of hormone decline is physical aging meaning thin bones, loss of muscle mass, fat accumulation and loss of skin elasticity and radiance. For women, vaginal dryness, and for men, erectile dysfunction, are directly related to low-hormone levels.

Mental aging related to low sex hormones includes many symptoms, such as depressed mood, anxiety, insomnia, fatigue, brain fog, lack of motivation, and low sex drive. Increased rates of diseases are associated with low sex hormones, including heart disease, osteoporosis, diabetes, dementia and certain cancers.

The adrenal hormones decline about the same in men and women, and similar to testosterone in men, they drop slowly over time. By age 50 many people are already low on two key adrenal hormones, pregnenolone and DHEA (de-hydro-epi-androsterone).

Pregnenolone is known for its role in mental processing and memory. It is also produced in the brain and has been shown to play a role in the creation of memory as well as protecting the nerve circuits that preserve memory. Scientists believe that the hormone pregnenolone has vast potential for maintaining healthy cognitive function and may be the most potent memory enhancer yet reported.

Research shows that low pregnenolone correlates with Alzheimers dementia. Pregnenolone stimulates the growth of new nerves in the brain and increases a memory enhancing neurotransmitter called acetylcholine. Further studies show that supplementing pregnenolone can reverse memory deficits. It also has the amazing ability to stimulate nerve transmission while some of its metabolites actually calm over-excited nerve tissue. This makes pregnenolone sharpen memory and cognition while helping conditions such as anxiety.

DHEA is made from pregnenolone and has a wealth of benefits including musculoskeletal support, promotion of mental health, immune system regulation and maintaining cardiovascular health. Good for bones, muscle and metabolism, DHEA has been shown to increase bone mineral density, increase muscle mass and stimulate breakdown of fat leading to weight loss.

Global spa, wellness industry estimated at $4.3 trillion

NEW YORK - A growing middle class and consumers' evolving attitudes toward health and travel have fueled a global spa and wellness industry worth an estimated US$3.4 trillion S$4.3 trillion) in 2013, according to a report released on Tuesday.

Nutrition and weight loss, preventative and personalized health, complementary and alternative medicine, and beauty and anti-aging treatments were the biggest growing sectors, the report compiled by the non-profit research centre SRI International showed.

"All across the world we have seen, from Asia to Europe to Africa to North America, more and more people are consciously thinking about healthy food, exercising, looking to nature, getting massages and doing yoga," said Ophelia Yeung, a senior consultant for SRI International who led the study.

Spa treatments and products, alternative and complementary treatments and weight-loss programs once considered beyond the means of many people, she added, are becoming more mainstream with a growing middle class.

While medical care treats illness and disease, wellness is focused on prevention through a variety of healthy habits, nutritional eating, exercise and treatments.

To compile the report researchers looked at wellness sectors ranging from mind and body fitness to beauty and anti-aging, spas and workplace wellness.

The global spa industry generated $94 billion last year, according to the Global Spa and Wellness Economy Monitor report, up from $60 billion in 2007.

With more than 32,000 spas, Europe had the highest revenue of $29.8 billion, followed by the Asia-Pacific region with $18.8 billion and North America with $18.3 billion.

Emerging markets in the Middle East and Africa have been growing the fastest in terms of adding spas. In Asia, China and India are leading growth, while in Europe it is Eastern Europe, Russia and the Baltic states.

Thermal/mineral springs generated $50 billion worldwide but the biggest industry sector was wellness tourism, or travel associated with maintaining or enhancing one's personal well-being and health, which accounted for $494 billion.

A first-of-its-kind scientific study reveals that dermatologists are the groundbreaking innovators in five of five categories of anti-aging injectable treatments for the face.

The evidenced-based study Who are the Pioneers? A Critical Analysis of Innovation and Expertise in Cutaneous Non-Invasive Procedures is examining all published research to determine which medical specialties have made the most advances in cosmetic treatments. The first study segment released today focuses on botulinum toxins and four types of soft-tissue fillers.

This first unbiased analysis of all existing scientific research will help consumers decide which specialist might be best for facial injectable treatments, said study author Omar A. Ibrahimi, M.D., Ph.D., director of the Connecticut Skin Institute in Stamford, Conn., and a member of the American Society for Dermatologic Surgery.

Consumers can be significantly confused because some cosmetic procedures are administered by physicians who are not board-certified dermatologists, and some are even offered by non-physicians, said Ibrahimi. This study is important because expertise and innovation often are judged by who has made the largest contributions to scientific research.

He said the thorough analysis of all peer-reviewed medial research on facial injectables finally answers the question of who are the innovators and the pioneers.

The answer for facial injectables is clearly dermatologists, said Ibrahimi. These data show the overwhelming majority of pioneering research in wrinkle-relaxers and fillers has been done by dermatologists. They are the leading innovators. Patients will know they are in the best hands with a dermatologist.

This first study segment includes botulinum toxins to ease facial lines (i.e., Botox, Dysport and Xeomin) as well as four types of soft-tissue fillers to smooth wrinkles and add facial volume. These include hyaluronic acid fillers (i.e., Juvederm, Restylane, Belotero and Voluma), calcium hydroxyapatite filler (i.e., Radiesse), poly-l lactic acid filler (i.e., Sculptra) and collagen fillers (i.e., Zyderm and Zyplast).

According to the scientific analysis, dermatologists are considered the pioneers in 10 of 10 measures using two scoring systems.

The overall project is using a ranking system for research studies. Those considered to be more valuable generally are cited more frequently by subsequent publications, Ibrahimi said. He and co-authors first sought to identify the most important research studies on injectable procedures, then analyzed the findings to determine the specialties whose members published the most significant research.
