Broome County remembers the life of Officer Lee Barta after 25 years – WBNG-TV

(WBNG/WNBF) -- Broome County is remembering the life and service of Patrolman Lee Barta as the 25th anniversary of his death approaches this August.

Barta was shot and killed while on duty, pursuing an absconder in August of 1995. The suspect committed suicide after killing Barta.

The Broome County community keeps Barta's memory alive with the Lee Barta Community Center on the Northside of Binghamton, in the neighborhood where he lost his life.

The center is a part of the Healthy Lifestyles Coalition. According to their website, the mission of the Lee Barta Community Center is to collaborate with the community to inspire residents to live healthfully and achieve the benefits of a healthy lifestyle.

The website says the center was opened in 2008 and underwent an expansion in 2018 that included a number of new amenities including a public computer lab, an exercise room, and a CHOW pantry.

One of Barta's sons is now a part of the City of Binghamton Police force, and the community center continues to give back to its residents, keeping Lee Barta's memory alive.

Breast cancer survivor runs first 5K with friends and family since diagnosis – The Columbus Dispatch

On Saturday morning, Patti Gilligan ran her first 5-kilometer road race since her breast cancer diagnosis two years ago. More than 20 of her neighbors, friends and family ran with her in support.

Two years ago, Patti Gilligan was diagnosed with breast cancer, setting her on a journey that involved extensive treatment and surgery and deprived her of many of lifes comforts.

Running was one of her favorite activities before her diagnosis. It was both an opportunity to spend time with like-minded friends and to blow off steam, but the cancer left her unable to participate in rigorous exercise.

So running a 5-kilometer race with a group of roughly 20 friends, family members, and neighbors from her Lewis Center community Saturday felt like a revelation.

"This is the first time Ive run a true 5K" since her diagnosis, said Gilligan, who is now cancer-free. "It is absolutely exhilarating."

And two years after a diagnosis that threw a monkey wrench into her life plans, the run gave her a much-needed feeling of normalcy.

"I feel like cancer took so much of my life, but this is something I can control and I can get back to," Gilligan said.

A little rain briefly hampered otherwise ideal running weather. Cloud cover kept the sun at bay and temperatures hovered below 70 degrees most of the morning.

The run wasnt easy, Gilligan said, but she drew on her friends and her experience to bring her through to the finish line.

"I just kept remembering, when I wanted to quit, how badly I wanted to run when I was going through chemotherapy, and after surgeries," Gilligan said. "It pushed me to get through it."

This years Race for the Cure, which raises money for the Susan G. Komen breast cancer foundation, was virtual because of the coronavirus outbreak. Participants ran on their own and logged their time. Determined runners and cancer survivors such as Gilligan organized their own smaller version of the popular race.

Patti Gilligans husband, Matt Gilligan, said running and otherwise being active was always an important part of their family life.

"We want to live a long time and be healthy," he said. "But we also want to set a good example for our kids." The couple has two children, a 12-year-old son, Brady, and a 16-year-old daughter, Caroline.

Their efforts to encourage a healthy lifestyle seem to have paid off. Caroline left for a cross country camp shortly after the morning run.

Shannon Hoppert, a long-time friend of Gilligans, ran with her on Saturday.

Gilligan helped Hoppert get started as a runner. The two women bonded and used running to escape the stress of work and parenthood. The Lewis Center woman eventually talked Hoppert into running a half-marathon in 2009, and theyve run four more since then.

Gilligan maintained a positive attitude through her diagnosis and the cancer treatment, Hoppert said, providing inspiration to her friends. It was thrilling to have her friend back for a race on Saturday morning, she said.

"Its been too long," Hoppert said. "Its always been something thats been our release from real life. Its awesome to have her back."

Gilligans neighbors on Overland Trail in Lewis Center were there for her throughout her battle with cancer and many jumped at the chance to run a 5K with her.

"Its about community," Matt Gilligan said. "This group of people and this neighborhood have been very supportive throughout."

That support helped Gilligan through the cancer treatment. Her family created a Facebook page for supporters and described receiving inspirational messages.

"There are people here who came and prayed over me before I had my surgeries and treatments," Gilligan said. "Its just a reminder to me, every time I look at these people, of what theyve done for me and how supported and loved I am."

If you are buying a car, house, or making any other large purchase, it is human nature to feel like you got great value and stretched your hard-earned money as far as you could.

The definition of the word deal is an agreement entered into by 2 or more parties for their mutual benefit, especially in a political or business context

This is what we do when someone comes on board as a member to Providence Fit Body Boot Camp.

We make a deal that we will coach them to reach their health and fitness goals in exchange for their commitment to be consistent, disciplined, and engaged with the program.

Lots of mutual benefits there.

The member gets their health and fitness back, and we can continue to do business as an organization.

However, I have an even better idea, and I came up with another acronym to describe this idea.

Make a DEAL with yourself & forget a second party!!

Heres how it goes:

D- Dedicate your life to being fit and healthy.

Yes, you heard that correctly.

Dedicate your life, because if you dont, your life will be cut short, or even worse, be filled with sickness, pain and misery.

When you do not take care of yourself on a daily basis, not only will the weight creep up around your waistline, and every other undesirable area of your body, you will do a tremendous amount of damage inside your body that you probably wont notice until its too late.

Do you know anyone with diabetes, heart disease, high cholesterol, cancer, arthritis, or some gastric issues such as acid reflux or stomach ulcers?

You probably do, and if you dont eat clean, exercise, sleep enough or drink enough water, these symptoms are bound to find you too.

Ya see, these are all preventable ailments that can be avoided through your lifestyle habits.

You may not immediately notice the damage you are doing, because it accumulates over time, however as I said earlier, you wont notice until its too late. (On a side note, once you start practicing living a healthy lifestyle, you will notice and feel the difference IMMEDIATELY!!)

E- Eliminate the toxic components that will derail your goals and dreams.

This includes everything from the food you eat to the people you hang around with to the way you spend your time to the thoughts that you let occupy your mind.

When you eat toxic food full of artificial ingredients, sugar, and empty calories, you are doing yourself a huge disservice. If your body cant break down the food you consume and utilize the nutrients for cellular repair and the regeneration of muscle growth, you are consuming empty calories that will just end up as fat storage in the body. A general rule I follow is that if you cant read or pronounce the ingredients, you shouldnt be eating it.

You also want to eliminate the people that dont add value to your life. If you cant eliminate them, then you need to edit your relationship with them. Toxic people are contagious, and when you find yourself in the company of someone that always complains, gossips, cries victim, and points the finger at everyone else, eventually that will drain you mentally, emotionally, and spiritually, so stay away at all cost.

The last thing to eliminate is the toxic thought process that I cant do this, or Im not a fitness person, because the more you think this way, the more your thoughts will dictate your action, or lack of action. Think positive, and know that if it works for others, it will work for you too.

A- Answer the bell every day. Especially when you dont feel like it.

I have been personally working out for over 30 years now, and I cant remember a time that I would rather workout than do something easy like just hang out or go to the beach.

You will always find something that you would rather do than the workout, however, there arent too many things that are more important.

Its work. Hard work that takes effort and discipline, that most people arent willing to do and do consistently.

Its time consuming, and painful at times, but there isnt a better feeling of accomplishment than when you complete a workout or great week of eating clean.

Eating clean is another task that requires extra work and discipline, alongside the exercise component. You need to shop, meal prep, and most importantly be able to say no to eating the toxic food that will take you off your path.

Be ready to show up and work, because the rent is due every day. Rain, snow, 100 degrees, it doesnt matter because you have a responsibility to yourself to get the job done regardless of the conditions.

L-Listen and learn from people with more experience than you. If you are not sure what you need to do to accomplish your goals, dont suffer in silence, ask for help.

Most of our members werent fitness people when they came to us.

If they had the motivation and know-how to achieve their goals, they wouldnt need us.

One 19th-century artist’s effort to grapple with tuberculosis resonates during COVID-19 – The Conversation US

Like everyone else, artists have been challenged by new conditions and routines since the start of the COVID-19 pandemic. Many have had to adjust what they make as well as how and where they work, coming up with innovative ways to be productive in makeshift studios with limited supplies and in relative isolation.

One thing is certain, though: In response to daily headlines of devastating illness, suffering and death, the need for creative expression and meaningful reflection on loss remains essential.

For the past several years, Ive been researching the impact of disease on late 19th-century American artists. At the time, medical science was ill-equipped to manage rising rates of communicable disease, leaving art to help fill a need to comprehend and process illness.

One of the artists featured in my forthcoming book on art and disease is the painter Abbott Thayer, whose life and work underwent dramatic change following the death of his wife from tuberculosis. For the grieving painter, art functioned as a kind of medicine.

In the late 18th century, tuberculosis started to be tinged with romanticism; it was thought of as an illness that could lead to elevated consciousness, creative insight and intellectual acuity. The poet John Keats and the pianist Frdric Chopin both died young from tuberculosis, cementing its reputation as an affliction of artists.

An early biographer of Robert Louis Stevenson argued that tuberculosis enhanced the writers talent, and in a sculptural relief depicting Stevenson during a stay in New York City, Augustus Saint-Gaudens portrays the bohemian writer with long hair and a cigarette in hand, looking alert and productive, despite being propped up by a stack of pillows in bed. As one critic observed, the relief captured Stevensons picturesque unfitness, as though illness heightened his allure.

If the effects of the disease were poorly understood, so too was the way in which it spread.

For hundreds of years, the cause of disease was believed to be miasmas, or foul-smelling air. Eventually, in the 1880s, medical science realized invisible microorganisms were the source of contagion, and that germs could be quietly passed from person to person. Unlike miasmas, which could be identified through smell, germs moved undetected through crowded cities. They were everywhere.

By the time the wife of painter Abbott Thayer succumbed to the disease in 1891, germ theory was widely accepted and would have been familiar to the artist, who was the son of a physician and public health expert. Fearing his three young children would be next, he sought out a healthy environment a place with plenty of fresh air and surrounded by nature, where the family could eat nutritious meals, roam freely outdoors and get plenty of rest.

The Thayers werent the only family looking for therapeutic settings. The 1870s marked the start of the sanatorium movement, in which individuals who had tuberculosis, or thought they might, were able to steel themselves against the illness in medically supervised, open-air compounds often near the mountains, desert or the sea. At the time, tuberculosis was the cause of roughly one in seven deaths in the U.S.

The life Thayer created for him and his children in Dublin, New Hampshire, was modeled on this type of facility. Their home, at the base of Mount Monadnock, gave the family ample opportunities to be immersed in fresh mountain air, which was then thought to be the purest type of air.

On a typical day, Thayer spent his morning painting and then climbed Monadnock or took long trail walks with his family. These outdoor activities encouraged the kind of deep breathing believed to free toxins from contaminated lungs.

The Thayers also slept outdoors in individualized lean-tos a three-sided shelter that allowed them to breathe fresh air throughout the night. Thayer also invented a breath catcher a device worn around the nose and mouth, not unlike the protective masks of today whichprevented the bodys noxious exhalations from freezing onto bedding at night, according to the thinking of the time. He also wore a special kind of wool underwear marketed for its protective qualities against disease in a further attempt to avoid germs.

While Thayer was working to protect the health of his family, his art underwent a shift.

Early in his career, Thayer mostly painted landscapes and portraits. But following the illness of his wife Kate, Thayer turned his own children Mary, Gerald and Gladys into the primary subjects of his work.

In the first of these, Angel, he painted his eldest child Mary as a heavenly creature, whose pale, chalky skin underscored by her white robe and wings conveys a fragility evoking the effects of tuberculosis.

The painting brings together the contradiction of a healthy daughter and sickly mother, collapsing the promise of wholesome youth and the fear of bodily disintegration.

In A Virgin of 189293, Thayer depicted all three children standing outside. The clouds, which emerge from Marys shoulders as wings, allude to Thayers earlier depiction of her in Angel and thus to her role as a stand-in for his late wife.

Given the way in which Kates illness focused the familys attention on nature and health, it seems significant, too, that the children, shown barefoot and windswept, walk vigorously and purposefully. Their classical clothing pays tribute to the ancient Greeks, celebrated in Thayers time for their commitment to physical fitness and outdoor living.

Immersed in a therapeutic environment while perhaps on one of their treks up Monadnock, Thayers children embody the life their father embraced. They become models of healthy outdoor living in an era of contagious disease.

The image may look antiquated, but it resonates today.

Both tuberculosis and COVID-19 target the lungs. Symptoms for both diseases include shortness of breath and coughing. There was no effective way to treat tuberculosis until the development of streptomycin in the 1940s, so prevention and perseverance during Thayers time as with COVID-19 often involved good hygiene and healthy living. Like Mary, Gerald and Gladys, we are still taking walks in nature in an effort to escape the psychological and physical limitations of quarantine.

Today, filling our lungs with fresh air remains a reassuring sign of health just as it did more than a century ago.

But isnt that counterproductive?

So you want to boost your immune system, but youre not sure what to eat. Samantha M. Coogan, director of the Didactic Program in Nutrition and Dietetics at UNLV, has some recommendations. Next time youre at the grocery store, load up on the following:

red, orange and yellow produce

citrus fruits

berries

green, leafy vegetables

squashes

eggplant

garlic

onion

leeks

seafood

green tea

Coogan says the production of free radicals is a normal occurrence, and explains why nutrition before and after a workout is critical to help fight off those free radicals after your exercise session, and to help recover and repair the stressed and damaged muscle tissue.

Again, Coogan assures this is all completely normal, and even beneficial. The more we expose our bodies to that type of external stimulus, it requires the body to react every time, she says. Each time, it will adapt more quickly and readily.

Finding a safe way to exercise during the pandemic can present its difficulties, especially in the summer when living in the desert. Lifting weights at home, following a yoga instruction video on YouTube and hiking are just a few ways to incorporate activity into your daily routine.

Another advantage of exercise: its role in alleviating stress, which can wreak havoc on the immune system.

Neurotransmitters and stress hormones are released in direct response to external stimuli, whether it be physical stresslike exercise or physical traumaor mental and emotional stress, Coogan says. Cortisol tends to be one of the biggest culprits when it comes to stress, which also hinders weight management or loss, because cortisol increases fat around the organs, known as visceral fat. While other stress hormones like epinephrine and norepinephrine are responsible for the fight or flight response, cortisol is the main obstacle when it comes to weight maintenance and our stress response, Coogan says.

Exercise for the mind

People are more stressed now than ever before, but even during a pandemic, we can incorporate stress-relieving activities into our daily lives.

One way to reduce stressful responses: Give yourself something on which to focus. Stimulating your brain can help stave off stress, especially if you try something new, Coogan says, although she clarifies that doing something familiar can be just as effective.

Find something unrelated to work, school, the pandemic or whats going on around the world right now, and find something that brings you happiness and puts a smile on your face, Coogan says. The health expert recommends video games, a comforting TV show or movie, reading, meditation, puzzles, yoga, brain teasers, painting, adult coloring books, an instrument, knitting or learning a new skill.

Coogan says that during the pandemic, she picked up embroidery as a new hobby. Im so in love with it, she says. I am a very detail-oriented person, and getting lost in the intricacies of the patterns and the way the threads come together to form this beautiful work of art produces very happy hormones and brings me a lot of joy and satisfaction in the work Ive done. I can get lost in it for hours as I give all my attention to the detail of the pattern, with my favorite music playing or a comedy special on in the background.

Coogan says the activity doesnt have to be intricate or complicatedor produce anything. If it has a positive benefit to you, keep doing it, she says. If youve mastered that boss level 100 times, but each time you do it, it makes you feel good, then keep doing it. If sitting on the couch for nine hours straight binge-watching See with Jason Momoa brings you joy and fulfillment for that day, despite no productivity in the traditional sense, thats also OK, Coogan says, adding that she may or may not have done that herself.

Vegging out, she says, is still a way of taking control of stress. Just dont let yourself get lost in your own silence or your own head, Coogan says, warning that can trigger even more stress. Its all about moderation and finding healthy habits.

But how can we each individually take responsibility for our own health so the effects of a widespread outbreak don't affect us and our loved ones with great intensity.

First, we need to know what a healthy being looks like. One who takes dozens of pills daily is most likely treating symptoms and not underlying causes. By starting with underlying causes, we can build our health one step at a time-or more appropriately one vegetable, grain, fruit, bean, and nut at a time. Yes, healthy eating is the cornerstone of a healthy body. Twinkies and potato chips, sodas (diet or otherwise) do not support our wellbeing. In fact these and many other foods do just the opposite. But our health care system seems to be okay with that as few health care professionals are taught much about human nutrition though that is the one thing their patients rely on daily to live.

Additionally, our healthcare system treats diseases after they occur instead of nurturing healthy living so that many of these diseases can be avoided. We have a medical model that supports the use of pharmaceuticals to cure all ills, when in fact those very same pharmaceuticals may be masking underlying causes and may even create illnesses of their own. Pharmaceutical companies don't make money if you can get your "medicine" from the produce aisle or farmers market, so they are invested in getting you hooked on all sorts of pills, potions, and procedures.

We learned the sad truth that tobacco kills long after many people died from smoking-related diseases; do we have to wait years to learn how deadly animal-based foods are to our lives, do thousands/millions need to die before we realize that three times a day we are actively destroying ourselves? Where in India’s COVID-19 Response Is the Moral Compass That Guides Governance? – The Wire Science

A number of incidents in the history of health and medical science, from the Nuremberg doctors trials and the Tuskegee experiments to contested clinical trials and therapeutic regimes, have expanded and refined our approach to handling the complexities of health and medical research. And as we broaden our idea of what it means to be human, we gain a deeper understanding of the nature of our interconnected lives, as also the sense of what is ethical and just.

Ethics is a multidisciplinary field, and its standards, like science, are shaped by the emerging evidence, said Amar Jesani, editor of the Indian Journal of Medical Ethics and a member of many consultative bodies on bioethics. That is why ethics guidelines evolve; they are not pre-fixed.

Every advancement in medicine brings a new set of ethical dilemmas, while a changing world forces us to confront newer challenges. The invention of ventilators pushed us to think about end-of-life decisions and the definition of brain death. The outbreak of cholera in a congested city nudged us to reconsider urban planning. The discovery of vaccines has been accompanied by concerns of safety and disclosure during clinical trials, and acceptability, accessibility and affordability.

Over the years, changing ideas of fairness and equity have been codified into various foundational documents that have generated the guiding principles of biomedical research and health practice. The Belmont report in 1979 laid down the principles of respect for persons, beneficence 1 and justice, particularly in relation to vulnerable groups.

In India, activists also contributed to the growth of bioethics by raising questions about research that was not done for instance, in the wake of the Bhopal gas tragedy, Anant Bhan, a bioethics and global health policy researcher and former president of the International Association of Bioethics, said. Since the 1990s, we have been asking questions about who sets the research agenda, the ways in which drug trials are approved and conducted, and questioning global pharmaceutical patent regimes.

According to Bhan, public health crises like the one we are now facing raise new ethical dilemmas that prompt multilateral bodies, including the WHO, and national bodies that govern public health and research to reformulate guidelines. But the guidelines often dont cover many issues in between the silos of medical practice, clinical research and public health not to speak of numerous allied activities that the state governs.

Where is our talisman?

The COVID-19 pandemic was preceded by a chain of public health emergencies, including epidemics in 2002, 2009, 2012 and 2014. They offered opportunities to learn how to plan for future outbreaks, and reflection by governments and multilateral agencies.

So the questions we are asking now about resource allocation, the need for stringent lockdowns and enforced quarantines, information dissemination and transparency, access to care and protections for those who provide it are not new. Yet we find ourselves on the back foot.

The American bioethics professor Joseph Fins called the COVID-19 pandemic a stress test for bioethics, acknowledging that even ethicists ignored a tale of inequity unfolding around us. Despite early pronouncements that this was a pandemic that would touch everyone equally, its clear today that this isnt the case. And its become abundantly clear that the absence of an ethical approach to decision-making has widened this gap.

In our pandemic, ethics need to be considered in every aspect of crisis management, at every stage immediate steps to contain and prevent; medium-term strategies of care and compensation; and long-term plans for well-being and rehabilitation. One might expect that ethics are the foundation for any action, by the state and by the individual.

However, there is today a tired acknowledgment that the failures of management have been undergirded by a lack of ethical and moral vision, a refusal to think in terms of Gandhis talisman that asks us to weigh every action in terms of how it would benefit the weakest, the most vulnerable.

Consider the ethics of the most visible response to the pandemic: the lockdown. At the broadest level, the Indian governments response seems to have been guided not by a deep commitment to the wellbeing of all its people but by what Debraj Ray and S. Subramanian call a politics of visibility: draconian on high-profile measures such as lockdown, weak on the measures that are less easily observed. They describe the governments actions as constituting nothing more than a nationwide project of virtue signalling.

What I observed since the crisis started, Jesani said, was that there was absolutely no discussion or consideration of ethics in the kind of response that was formulated in India Did the government really consult public health experts?

One key ethical concern, in his view, is that there was no time given for the most vulnerable groups in the country to prepare for the lockdowns consequences, revealing a huge empathy gap between those who made policy and those who suffered it.

Jesani also called attention to how the lockdown and the ways in which it was enforced contravened the Siracusa principles, a covenant drafted in 1985 by the International Council of Jurists. It details how states need to weigh human rights considerations when they impose restrictions to deal with public health emergencies.

But instead of a public health response, the governments approach ended up making it a law and order problem, Bhan said.

According to Maxwell Smith and Diego Silva, The nuance necessary in planning and preparing for how isolation will be deployed in an ethical manner depending on the type of infectious disease and potential pandemic makes it difficult to plan for such differences unless one explicitly considers them in advance. A commentary on the British Medical Journal blog by ethicist David Shaw addressed the need to triage ethical issues during a pandemic.

In the Indian lockdown, one of the most restrictive in the world, there was no indication that the government had considered such issues, and the few measures taken after the migrants crisis unraveled followed public interest suits initiated by civil society. B.R. Shamanna, a professor of public health and epidemiology at the University of Hyderabad, acknowledged that the pandemic has thrown up new issues in health governance. The need for portability of government schemes across state lines was particularly in evidence as migrants were unable to access benefits in their adoptive towns.

I was surprised that the government went for a pan-India lockdown, Jesani continued. To be very frank, [the decision makers] were so obsessed with the virus, they forgot the people completely.

Health NGOs and community organisations stepped into the yawning gap created by the governments decision, patching together relief efforts in the absence of state direction. We need circumscribed lockdowns, and we need to think of support systems. That would have been the ethical way, Jesani said.

Lives on the front line

The front line has become a clich but in the public imagination, its the site of our war against the virus. (Some have argued against the use of the war metaphor as detrimental to a cooperative mindset while another group has pointed to its value in mobilising common cause.) The front line has been the centre of attention and where the ethics of care has been debated to some extent.

Practitioners encounter the difficult decisions of whom to test, whom to treat and where to focus attention and resources. Stories from Italy, where doctors were forced to make decisions about whom to ventilate demonstrated the difficulty of triaging in resource-constrained situations. Medical workers are supported when a disinterested party like a hospital ethics committee offers guidance on such issues in a culturally sensitive manner, but few hospitals in India have such committees.

For years, the duty to care has been taken for granted. No one thought about the need to protect those who provided medical care; the entire focus was on the patient, said Justice T.N.C. Rangarajan, a retired judge of the Andhra Pradesh high court who chairs several institutional ethics committees.

The umbrella of ethics needs to consider workplace protections even for those who have a duty to care often seen as a responsibility they must endure without complaint. Sandhya Srinivasan, consulting editor of the Indian Journal of Medical Ethics and longtime health rights activist, has argued that an ethical reconsideration requires us to recognise that healthcare service providers are not just doctors.

As agitating ASHA workers in Karnataka and elsewhere remind us, those at the very front of the line are often ancillary workers, whose rights and needs are often ignored. Srinivasan described the work of the Centre for Bioethics and Culture in Pakistan: it has worked to expand the definition of health worker to include cleaners, attendants, nursing aides and community health volunteers, among others.

Another ethical issue Srinivasan raised relates to medical decision-making, where doctors are still required to draw on the evidence base but often have to resist pressure from government and patients to try untested therapies.

The tentative nature of knowledge around the novel coronavirus has led to a situation where medical practitioners have to accept the limits of their ability to treat its infection, and ethics demands open and truthful communication with patients and caregivers. Few hospitals have clear communication protocols and there is little training in ethical physician-patient interaction.

This is complicated by the diversity of the patient population and the tendency to adopt a paternalistic attitude towards patients from marginalised groups, often bypassing consent requirements even in experimental treatments. This has been happening both in clinical and community health settings. Jesani cited the Mumbai municipalitys unilateral decision to dose all of Dharavis residents with hydroxychloroquine as a prophylactic despite the contrary evidence of its efficacy.

Ultimately, with the intervention of NGOs and health activists, they were able to contain the spread through the participation of the community, with open communication within a month, Jesani said.

The pandemic has also made even more visible the political, cultural and economic fault-lines that exist in healthcare delivery in India, with the government unable to effectively commandeer the resources of a burgeoning private health sector even in a situation labelled an emergency. Capping the cost of tests, ensuring standards, making beds available for the treatment of COVID-19 all happened under a pall of confusion and much back-and-forth, instead of being part of a carefully thought-out protocol.

In general, there has been little direction from government bodies on practising ethical healthcare in a pandemic. It has fallen to civil society groups such as Jan Swasthya Abhiyan (JSA) and the Peoples Health Movement to spotlight ethical gaps and raise public awareness about issues related to inequity. Jesani recalled the JSAs public interest suit demanding access to hospitals and raising questions about testing protocols, and pushing the government to make the private sector more accountable.

If the nation is in crisis, you can take away my individual rights, but you cannot do anything to the private hospitals? How can the private health sector continue to focus on profits at such a time? he asked.

He also pointed to the disregard for non-COVID-19 related health conditions that require ongoing care but which is now difficult to access. The international health community has raised this issue as well, with David Shaw pointing to the millions of cancer patients that may have been missed in Western hospitals taken over by the pandemic. In India, we can only guess at the large numbers of chronically ill patients whom the lockdown sidelined.

A pandemic preparedness plan rooted in a broad vision of ethical care would have envisioned such needs, and provided for them.

Hope and expediency

In April, the Indian Council of Medical Research (ICMR) issued new guidelines culled mostly from a 2015 document for research during the COVID-19 pandemic. Meant to cover clinical trials of new drug therapies, including repurposed drugs and new vaccine candidates, the document draws on lessons from previous infectious disease outbreaks vis--vis consent, protection of health workers and vulnerable populations.

However, given the somewhat chequered history of clinical trials for new therapies, particularly vaccines, in India, activists have been suspicious of the promise of an early vaccine and government efforts to fast-track tests. In the name of an emergency you cant take shortcuts to generate evidence, as Bhan said.

Shamanna also warned of what has come to be called vaccine nationalism, falling into a competitive mindset that frames vaccine development as a race that will be won by the country that puts most resources into the game at the earliest stages. Of course, in such a situation, decision-making has to be relatively quick, yet scientific integrity has to be maintained, he said.

However, others are more hopeful about changes the pandemic has prompted in the research ethics space. In the 25 years that I have been involved in institutional ethics committees, I have seen the thinking become more complex and nuanced, Justice Rangarajan said. In the early days we focused only on obvious harm to the patient, but now we look at protocols in much more detail, obviously more conscious of the broader human rights discourse. Other areas where he believes there has been progress are biosafety and institutional liability.

Shamanna also sees some positive developments in health governance. The pandemic has forced ICMR to finally come out with a formal set of guidelines for telemedicine something we need now that so much non-COVID-19 care is happening remotely, he said. He added the pandemic had pushed ethics committees to consider important procedural questions like expedited reviews, using digital platforms for meetings and simultaneously approving phase 2 and 3 clinical trials all somewhat contentious issues requiring debate.

Now what?

When the pandemic passes, what if anything will be different? How will we view the world and its interconnections, which have now become so apparent? Will we acknowledge ethical shortcomings and prepare lessons for the future, when disease X strikes? Will we shift our thinking of all cadres of health workers, such as ASHAs, as instrumental resources to be deployed as needed and dismissed with nothing more than the discordant banging of pots and pans? Or will we train a compassionate imagination on the impact of showy policies on the most vulnerable and then revise the way we do things?

Im skeptical, Bhan said. For change to happen, we need a deeper engagement with the politics of health, the social and cultural determinants and the kind of reflective decision making that is needed for this is rare.

Srinivasan said with some cynicism that we should not even assume that ethics plays any role at policy and governance levels, but she holds some hope from activists and the occasional good actor from within the system, who is willing to stand on principle and do the right thing.

She is buoyed by some positive developments in the ethics of practice and research, pushed by NGOs and taken up by visionary bureaucrats, such as accessible anti-retroviral therapy for HIV patients, availability of generic drugs and greater scrutiny of clinical trials.

This said, its hard to not be cynical, and impossible to ignore the lack of ethics in all we have seen, mostly on the part of those who have pledged to protect and serve. Even as I write this, I see a news alert that several state governments have slashed health budgets.

In a recent essay in the Journal of the American Medical Association, Donald Berwick asked an uncomfortable question: How do humans invest in their own vitality and longevity? In wealthy nations, science points to social causes, but most economic investments are nowhere near those causes. Instead, he continued, imagine for a moment that the moral law within commanded [a] shared endeavour for securing the health of communities.

What would it take for such an imagination and commitment to take hold in Indias corridors of power?

Cognitive Super Agers Defy Decline in Brainpower | NIA – National Institute on Aging

Although its normal for brainpower to decline as people age, its not inevitable, studies show. Some people remain cognitively sharp into their 80s, 90s, and beyond, defying the common assumption that cognitive decline is a natural part of aging.

These lucky few, called cognitive super agers, perform demonstrably better on memory tests, such as remembering past events or recalling a list of words, compared with other adults their age. NIA-supported researchers are exploring the factors that set these people apart so the knowledge can be used to help others prevent or reverse age-related cognitive decline.

There is a tendency to equate aging with a decline in mental function, said Molly Wagster, Ph.D., chief of the Behavioral and Systems Neuroscience Branch in NIAs Division of Neuroscience. We want to study these cognitive super agers, who clearly break the expectation of cognitive decline, and unravel their secrets.

Atrophy is the strongest correlate of age, said Claudia Kawas, M.D., of the University of California (UC), Irvine. She leads the longitudinal 90+ Study of more than 1,800 people age 90 years and older the fastest growing sector of the U.S. population.

According to Kawas, a 90-year-old brain typically weighs 1,100 to 1,200 grams, and this is 100 or more grams less than the typical 40-year-old brain. Brain shrinkage mostly affects the prefrontal cortex and hippocampus, and the cerebral cortex, which is important for complex thought processes. Over time, the brain can also be affected by declining levels of neurotransmitters, changing hormones, deteriorating blood vessels, and impaired circulation of blood glucose. These brain changes can affect thinking, making it harder to recall words and names, focus on tasks, and process new information.

Physically, the brains of cognitive super agers seem to defy wear and tear better than the average brain. Researchers at Northwestern Universitys Mesulam Center for Cognitive Neurology and Alzheimers Disease in Chicago have tentatively identified a brain signature for cognitive super agers, which they define as people age 80 years and older who have performance on memory tests comparable to people two to three decades younger. The researchers conducted magnetic resonance imaging (MRI) scans on 12 cognitive super agers, 10 cognitively normal peers, and 14 cognitively normal people in the 50- to 65-year age range. Comparisons revealed that the cingulate cortex, a brain region considered important for the integration of information related to memory, attention, cognitive control, and motivation was thicker in super agers than in their same-age peers and showed no atrophy compared with the same brain region of the middle agers. In fact, a specific region of the anterior cingulate cortex was significantly thicker in the brains of cognitive super agers than in middle agers brains.

In a separate study, the Northwestern researchers found that super agers brains contained a much higher density of a particular type of cell called von Economo neurons, which are linked to social intelligence and awareness. Their brains had more of these neurons even than the brains of younger adults.

In a third investigation, the Northwestern researchers used MRI to measure brain volume over an 18-month period in a small group of study participants age 80 and older. They measured a 2.24% average annual volume loss in the cognitively normal adults versus a 1.06% loss in the super agers. In short, the super agers brains shrank at a significantly slower rate.

At Massachusetts General Hospital, Boston, researchers are studying younger super agers people between age 60 and 80 who have memory recall abilities similar to those of 18- to 32-year-olds. Theyve identified distinctive thicknesses in two neural networks that connect parts of the brain important to memory function. The thickness is nearly the same as in the brains of younger adults. The researchers also found that super agers have a bigger hippocampus than other adults their age.

Researchers with UC Irvines 90+ Study have identified a curious and counterintuitive feature of the super ager brain: sometimes it shows the pathologies that characterize Alzheimers disease and related dementias.

Everyone thinks theres this really strong correlation if you have plaques and tangles, you have dementia, and if you dont have plaques and tangles, you shouldnt have dementia, said Kawas. Through both autopsies and PET scans of the brains of the oldest old, investigators have found that both of those things are often not true.

Researchers are exploring several theories to explain why some peoples cognitive abilities stay intact to the end of life. Perhaps they start out in life with larger, stronger brains. Or perhaps their brains somehow change to compensate for agings damaging effects. Another theory being pursued is whether their brains have stronger defenses against the assaults of aging.

A persons environment may be a factor. Human and animal studies by NIA-supported scientists have contributed to the growing body of evidence that enriching experiences, such as advanced education and mind-challenging occupations, can help brains last longer.

Various exposures throughout the lifetime might help people maintain their brains better or maintain their cognition in the face of age- or disease-related brain changes, explained Yaakov Stern, Ph.D., of the Taub Institute for Research on Alzheimers Disease and the Aging Brain at Columbia University Medical College, New York.

Other research focuses on genetic explanations. In an ongoing study of the Longevity Genes Project at Albert Einstein College of Medicine, New York, investigators are comparing the genetic profiles of children of healthy centenarians with the genetic profiles of the children of parents who did not live as long. They are looking for genes that might support strong neural networks and offer protection against mental and physical decline.

Social factors also could play a role in healthy cognitive aging. In one of Northwestern Universitys studies, investigators gave a 42-item questionnaire on psychological well-being to 31 cognitive super agers and 19 cognitively normal peers. The cognitive super agers reported more friends and family connections, a finding that builds on past research showing links between psychological well-being and lower risk of Alzheimers.

Researchers are grappling to explain why some peoples brains resist physical decline and why other peoples brains show physical signs of age- and disease-related deterioration yet continue to function well. Two theories that are being explored revolve around the concepts of cognitive reserve and brain maintenance. Cognitive reserve is the idea that some brains are strong enough to fend off the assaults of aging and disease. Brain maintenance is the idea that some brains have extra power to keep working well even in the face of aging and disease.

As UC Irvines Claudia Kawas, M.D., explained, You can super age by not getting Alzheimers pathology, or you can super age by getting it and somehow managing to not get sick with it.

Yaakov Stern, Ph.D., of Columbia University, uses a computer analogy: Some peoples brains have good hardware that doesnt break down as easily, while others have software that can write around problem areas in the hardware.

What specific changes in the brain and body provoke cognitive decline or, conversely, stave it off? A team of scientists in NIAs Intramural Research Program, led by Peter Rapp, Ph.D., chief of the Neurocognitive Aging Section, is seeking an answer through a project called STARRRS, for Successful Trajectories of Aging: Reserve and Resilience in RatS.

Rats are useful for studying the effects of aging because they are short-lived, and their brains do not accumulate the hallmark plaque and tangle pathology of Alzheimers. The breed of rats used for STARRRS has natural variability; some maintain their cognitive function as they age, and some dont. This makes them particularly useful for comparative studies.

Researchers are observing the behaviors of these rats over their lifetimes and using neuroimaging, tissue sampling (obtained through noninvasive means, so as not to interfere with behavior), and other methods to track whats going on in the brain.

This is a rare and wonderful opportunity to follow individual animals and relate any changes in cognitive performance to changes going on elsewhere in the body and in the brain, said Wagster. STARRS should give us insights into potential targets for prevention and intervention, as well as when it would be most appropriate to intervene.

In April 2017, NIA coordinated the Cognitive Aging Summit III, which was supported by the McKnight Brain Research Foundation (MBRF) through a generous grant to the Foundation for NIH (FNIH). The summit brought together a multidisciplinary group of investigators with shared interest in research on age-related cognitive decline and cognitive reserve and resilience, as compared to cognitive impairment or dementia. Participants identified opportunities for expanding our knowledge in this area, as well as gaps in our knowledge. One of the opportunities highlighted resulted in the STARRRS initiative.

Another new initiative that stemmed from the summit and that also is underway Collaboratory on Research Definitions for Cognitive Reserve and Resilience brings together investigators from around the globe to develop a consensus on uniform definitions for terms such as cognitive reserve, brain maintenance, resilience, and compensation so that the research community will have a common language and understanding for reporting of results. Use of uniform definitions offers many benefits, including the pooling of data across studies to allow for adequately powered analyses and better interpretation of data.

Its one thing to say, life exposures might increase reserve, says Stern, who serves as the primary investigator for the collaboratory charged with the definitions task. Its another thing to say, we propose that the types of occupation people have can allow them to cope better with amyloid pathology. We can measure amyloid, we can measure cognition, and we can show how life exposure actually moderates between the two. In other words, we can turn a concept into a very concrete recipe for analysis.

In February 2020, NIA issued an additional funding opportunity that stemmed from the Cognitive Aging Summit for support of research to understand factors that promote sustained cognitive health in older age: RFA-AG-21-015, Network for Identification, Evaluation, and Tracking of Older Persons with Superior Cognitive Performance for Their Chronological Age. Although chronological age itself remains the strongest predictor of age-related cognitive decline and many forms of dementia, including Alzheimers, it has become clear there are protective factors against these outcomes that are poorly understood. Awarded grants will support aggregation of sufficient numbers of these cognitive super agers to advance the fields understanding of factors that promote sustained cognitive health and those that are not of primary importance. Working in part with funds contributed by MBRF in conjunction with FNIH, NIA invites applications to identify, evaluate, and track individuals with superior cognitive performance for their advanced age across multiple sites. The deadline for submission of applications is October 1, 2020.

Further research on cognitive super agers could lead to strategies that everyone can use in younger years, in midlife, and in older age to maintain thinking and memory skills. Just as important, it could also provide insights into how dementias such as Alzheimers might be prevented or reversed and provide critically important information for the identification of targets for interventions. And by identifying the factors that affect brain health, this research might one day be used to reduce disparities in brain-enriching opportunities so that everyone has a chance to keep their brain at maximum power throughout life.

Australian-first commission explores community building through art and architecture – Mirage News

The University of Queenslands iconic sandstone towers will be reflected in the colours of Athens-based artist and architect, Andreas Angelidakiss latest high-profile project.

DEMOS (Sandstone) will be launched at The University of Queensland Art Museum on 3 August and comprises 50 large, lightweight, vinyl-covered blocks which will be reconfigured to form seating, a stage, study spaces, walls, monuments, archways and ruins.

UQ Art Museum Senior Curator Peta Rake said that while the term demos refers to the Athenian foundations of democracy, where only some citizens were allowed to speak, Angelidakiss project disrupts that legacy.

Throughout 2020/21, UQ Art Museums creative program Union is considering how people can come together and think through ideas via the lens of common purpose and collective action, she said.

DEMOS is the perfect vehicle for us to do that conceptually to contemplate how we inhabit different spaces and architectures in our 2020 context, and how we make safe community spaces from a social distance that create sites of human exchange whether its conversations, meetings, demonstrations or protests.

Were excited to be working with Andreas and to be the first Australian institution to commission this body of work thats been shown at a number of significant international venues, particularly as it encourages campus engagement.

UQ Art Museum Director Dr Campbell Gray said the colour of Angelidakiss work commissioned for the UQ Art Museum is Australian sandstone and reflects the sandstone synonymous with UQ.

DEMOS approaches architectural and colonial legacies through satire, Dr Gray said.

While the appearance of marble and concrete surfaces imply longevity these blocks are actually light enough to lift.

The fact that the blocks can be manipulated, moved and changed, despite their heavy appearance, causes us to ask questions about the histories and conventions of UQ and of universities in general.

It also prompts us to consider the nature of art museums environments where typically, precious things are presented and people are required not to touch them.

In the case of DEMOS, were inviting people to engage with the work, to rearrange it, to think about it and to participate with it, which is an exciting prospect for students on campus too.

Within the spaces and structures DEMOS offers, students from any discipline can engage with the work in endless ways to explore ideas and conversations.

DEMOS will be ongoing at UQ Art Museum from 3 August.

Active SWV expands into Kanawha, Boone counties | News – The Fayette Tribune

Active Southern West Virginia (Active SWV) has expanded its programming to include Boone and Kanawha counties. Having successfully served Fayette, Nicholas, Raleigh and Summers counties since 2015, Active SWV has also been working with neighboring communities along the Route 3 corridor and the upper Kanawha River valley.

Active SWV is a non-profit providing wellness activities for the residents of southern West Virginia, offering programs led by trained volunteers within communities where they live. The impact of these programs will improve health outcomes in underserved communities, workplaces, and schools, resulting in a vibrant, diverse economy. A healthy population is crucial to attracting new businesses and jobs to our region, program officials say.

The West Virginia Development Office supports the expansion of Active SWV programming into Kanawha and Boone counties. The WVDO recognizes the connection between a healthy workforce and job creation, Michael Graney, executive director of the WVDO, said in a press release. Active SWVs focus on improving community health will help attract new industry and diversify our states economy.

Dr. Eloise Elliott, a member of the Active SWV board of directors and the Ware Distinguished Professor in the College of Physical Activity and Sport Sciences (CPASS) at West Virginia University, said the organizations programming reaches those of all ages in the state in an effort to improve the health of the regions residents.

I am pleased to support the outstanding work of Active Southern WV, and its expansion into Boone and Kanawha counties, Elliott said.
