Detecting cancers — from tiny bits of tumor DNA in blood

When cancer blooms in the body, tiny bits of tumor DNA can be found in the blood. Cancer specialists would love it if these DNA fragments could one day be used in noninvasive diagnostic tests -- liquid biopsies -- that are relatively inexpensive and sensitive. There's a lot of work going on in this area right now.

One team of researchers reported a step toward that goal in a paper published Wednesday in the journal Science Translational Medicine. They used a strategy that can detect many different mutations in some key genes known to be involved in cancer even though the pieces of DNA from them were present in the blood plasma at low levels. Such a test, they say, would not have to be tailor-made for each cancer patient because it can look at a lot of different mutations at once, and that would make it cheaper and more practical.

The researchers, of Cambridge, England, showed that their strategy could track the progression of disease in advanced ovarian and breast cancer patients fairly accurately. They could see when a patient responded to treatment (plasma levels of key DNA fragments fell) and when they stopped responding to treatment (plasma levels of the DNA fragments started to rise again).

In a case that illustrates how they think their technology could be used, the scientists described a patient whod had tumors in the bowel and ovary. She had surgery, and responded well to it. Five years later, however, she developed a mass in the pelvis and the doctors werent sure which tumor it had come from. It was not biopsied because that was deemed too dangerous, so doctors proceeded with their best bet for a course of treatment and the patient did, in fact, respond to it.

The authors of the paper did an after-the-fact genetic analysis of the patients plasma and tumors. They found that the bowel and ovarian tumors had different genetic mutations in them and that the patients plasma at the time of relapse contained the mutations corresponding to the bowel tumor, not the ovarian tumor. Had these results been available, uncertainty and treatment delays may have been avoided, as well as the risk of prescribing chemotherapy for an inappropriate tumor site, wrote Tim Forshew, of the Cancer Research UK Cambridge Research Institute, and his colleagues.

There are a variety of ways that such technology could be helpful one day in cancer treatment, the scientists say:

Doctors could see what mutations were behind a patients cancer and when new mutations were added as time went by and the cancer mutated further. Cancer, as its often been said, isnt a single disease: There are many different ways that cells of the body can go rogue and start growing out of control and spreading.

Whats more, analysis of plasma would offer a noninvasive whole body look at all the cancer growing in a persons body. Since cancers mutate and change over time, one tumor in the same body could contain mutations not present in another one. With a plasma screen, bits of DNA from all of them would be floating around and be detected.

Because a test like this could help cancer doctors know just which genes are responsible for Person As cancer versus Person Bs cancer, it might help them decide which drugs and therapies to give a patient. (Some drugs are helpful for some types of cancer and not others.) As new mutations arose, they could change the therapy if appropriate.

Doctors could track how well therapy was working, and test to see if the cancer was returning in patients who had been responding to treatment.

Researchers complete the first epigenome in Europe

A study led by Manel Esteller, director of the Epigenetics and Cancer Biology Program at the Bellvitge Biomedical Research Institute (IDIBELL), professor of genetics at the University of Barcelona and ICREA researcher, has completed the first epigenome in Europe. The finding is published in the latest issue of the international scientific journal Epigenetics.

The genome of all cells in the human body is the same for all of them, regardless their aspect and functions. Therefore, genome cannot fully explain the activity of tissues and organs and their disorders in complex diseases like cancer. It takes a further explanation. Part of this explanation is provided by epigenetics, a field of biology that studies the heredity activity of DNA that does not involve changes in its sequence. That is, if genetics is the alphabet, epigenetics is the spelling that guides the activity of our cells.

Methylation

Epigenetics refers to chemical changes in our genetic material and proteins that regulate it. The best-known epigenetic mark is the methylation, the addition of a methyl chemical group (-CH3) in our DNA. The epigenome consists of all the epigenetic marks of a living being.

The authors of the study have completed the epigenomes for all brands of methylation of DNA from white blood cells of two girls: a healthy one and a patient suffering from a rare genetic disease called Immunodeficiency, Centromere instability and Facial anomalies syndrome (ICF). This disease is caused by a mutation in a gene that causes the addition of a methyl chemical group in its DNA.

The analysis performed by the researchers reveals that the patient has an epigenomic defect that causes fragility of chromosomes, which thus can easily be broken. In addition, the study shows that the patient has a wrong epigenetic control of many genes related to the response against infection, which causes a severe immune deficiency.

The study coordinator, Manel Esteller, emphasizes that due to this study, "we now know what happens in this type of rare diseases and we can start thinking about strategies for new treatments based on this knowledge."

Dr. Esteller is an international leader in the field of epigenetics. His work has been crucial to show that all human tumours have in common a specific chemical alteration: the hypermethylation of tumour suppressor genes. Weight gain associated with pregnancy has very real components. On average it's made up of about 7.5 pounds baby, 7 pounds maternal stores of protein, fat and other nutrients, 4 pounds extra blood, 4 pounds other body fluids, 2 pounds uterus growth, 2 pounds breast tissue, 1.5 pounds placenta, and 2 pounds amniotic fluid.

Healthy weight gain is based on pre-pregnancy weight and body mass index, or BMI. A woman who is underweight (BMI less than 18.5) would need to gain 28 to 40 pounds. Those at a healthy weight, BMI 18.5 to 24.9, should gain 25 to 35 pounds.

An overweight woman (BMI 25 to 29.9) should gain 15 to 25 pounds, but if she's obese (BMI higher than 30) only 11 to 20 pounds.

Personality Genes May Affect Your Longevity

The 1993 movie Grumpy Old Men played up a common stereotype about those who live well beyond their golden years. With lines like, "Do me a favor. Put your lip over your head... and swallow," they're not the nicest lot. But a study published in the May 2012 issue of Aging found that those who lived to be 100 generally had a "positive attitude towards life and emotional expression." It also found that those positive traits are located in your genes.

But the study is a long way from proving that a positive outlook causes these centenarians to live longer, said Dr. Nir Barzilai, one of the study's researchers. "There are personality changes and physiological changes that make a lot of elderly people become just more agreeable personalities," Barzilai said. "It has nothing to do with what got them to be 100."

Researchers looked at a group of more than 500 Ashkenazi Jews age 95 to 107, and 700 of their offspring. They're a favorite in longevity studies because they're genetically homogenous (40 percent of Ashkenazim derive from just four Jewish mothers). By creating a personality trait index focusing on levels of neuroticism, positive attitudes toward life and emotional expression, researchers found that the group was less likely to have high levels of neuroticism and were more likely to be extroverts. The same was true of their children, creating an argument for personality genes.

"My own interpretation from the study is that, yes, this is the personality were seeing [but] we still have to figure out if its important," Barzilai said. "The most interesting thing for me is to see if the longevity genes influence the personality."

Yet the health benefits of a sunny disposition can't be denied, the study notes:

The study's findings hew closely to what other researchers have found regarding the link between personality genes and longevity. The George Centenarian Study, the Swedish Centenarian Study and a study done in Japan found that centenarians had a more positive outlook than those in their respective mixed-age population.

The next step for the researchers is a study that will look at people who are the offspring of centenarians and those who aren't "to see the association between longevity phenotype and genotype and personality," Barzilai said. "Then well see if personality is part of living longer. Well watch them until theyll die to see if theyre personality changed or if it was personality that was part of what was important for longevity."

For readers who think they should change their attitudes so they can live longer, Barzilai has this to say: "Yes, everybody [should] become nicer," he laughed. But those are his thoughts "just as a citizen though, not as a responsible scientist."

Council committees discuss suspending longevity pay

PARKERSBURG - City council members, acting in committees, unanimously agreed to suspend the employee longevity pay program for another year.

The committees also agreed to look at across-the-board pay raises for employees and a reclassification of positions that could result in base pay raises greater than the annual longevity raises traditionally doled out.

"We want a system our employees can count on," said personnel committee chairwoman Sharon Lynch. "We want to retain our employees."

"We want to do something better than longevity," Mayor Bob Newell said.

Both the personnel and finance committees met Tuesday and agreed to the continued suspension of longevity pay for city employees. The suspension will not affect the previously approved 20-cent-an-hour pay increase approved by council earlier this year.

Discussion bogged down between the committees with regard to across-the-board pay raises. Members of the personnel committee wanted to see how much money they have to work with for possible base pay raises for employees.

Lynch submitted possibilities that included an increased base rate, plus a 1 percent increase on the base. The committee could reach no consensus to act.

Members of the finance committee voted to postpone discussion on base wages until the personnel committee had a figure.

"How much money do we need?" finance committee chairman Jim Reed asked.

"We are all in agreement that this needs to be done," said finance committee member Nancy Wilcox. "We need to know where the revenue is coming from forever."

INDIANAPOLIS Pacers coach Frank Vogel hopes to steal the script from his NFL neighbors.

Five days after Indiana ended its longest playoff run in seven years, Vogel told reporters he believes if the progress continues at this rate, the Pacers could become the NBAs version of the Colts perennial championship contenders for nearly a decade.

I think weve built something thats sustainable here around a young group of guys, and I think were going to have a chance every year for the next five to seven years, Vogel said Tuesday. I think thats what the Colts had for about eight to 10 years where every year might not be a Super Bowl year, but they were always in the mix.

Pacers fans would certainly take it after enduring the misery of the previous six seasons.

Following the 2004 brawl in Detroit and the subsequent retirement of star guard Reggie Miller, the combination of legal woes and playoff failures left loyal fans dismayed. Many stayed home and opted to follow a Colts team that won seven division crowns, two AFC titles and a Super Bowl championship between 2003 and 2010.

Now, the tables may be turning again with the Colts rebuilding and the Pacers emerging as one of the NBAs best young teams.

This season proved just how good the Pacers could be.

Indiana finished with the third-best record in the Eastern Conference (42-24) and closed the regular season with 12 wins in 15 games. It rebounded from an inexplicable stumble in Game 1 to win four straight against Orlando, giving the Pacers their first playoff series win in seven years. Then, after taking a 2-1 lead against Miami in Round 2 and being on the verge of pulling a huge upset, Indiana fans spent May tracking developments at both the Indianapolis 500 and Bankers Life Fieldhouse with equal vigor, just like the good old days.

And playoff games were actually sold out.

Despite losing the last three games to Miami, this seasons success has renewed hope in Indianapolis.

Defense to explore DNA, mental health in Utica woman’s death

Even though police say Craig Ingersolls DNA links him to the body of a Utica woman who was killed more than five years ago, that doesnt necessarily mean he sexually assaulted and strangled her, his defense attorney said Wednesday.

As Ingersoll, 30, of Herkimer Road, spends the next month considering whether he will plead guilty in connection to the 2007 death of 29-year-old Jennifer Bennett, Assistant Public Defender Luke Nebush said the circumstances surrounding Ingersolls DNA will be open to discussion.

Where the DNA was found is also a critical issue in this case, Nebush said, without clarifying exactly where Ingersolls genetic material was located on either Bennetts body or clothing, or anywhere else.

Nebush also would not comment when asked whether the circumstances of Bennetts death leave any room to be interpreted as an innocent accident.

Ingersoll pleaded not guilty Wednesday morning in Oneida County Court after he was indicted last week on charges of second-degree murder and first-degree criminal sexual act.

He is due back in front of Judge Barry M. Donalty on Tuesday, June 26, to either accept or reject the prosecutions plea offer.

In the meantime, Nebush said he expects to further review additional evidence provided by Assistant District Attorney Laurie Lisi and to look further into Ingersolls psychiatric background.

Thats something were going to explore, to determine whether any psychiatric issues actually exist, Nebush said.

Ingersoll is accused of killing Bennett sometime between Jan. 7 and 9 in 2007 while using forcible compulsion during a sexual assault on the woman.

After Oneida County sheriffs investigators spent years trying to identify unknown Bennetts killer, Ingersoll unexpectedly became the prime suspect several months ago after a statewide DNA database for previously convicted criminals matched Ingersolls DNA with DNA that was found with Bennetts body behind a Deerfield Fire Department pavilion on Jan. 11, 2007.

Switzer, an emeritus professor of biochemistry at the University of Illinois, begins with a quote (from Victor Davis Hanson's own book on farming) that "the American yeoman farmer is doomed," and describes the internal and external forces that led to the demise of his family's farm in northwest Illinois.

The story of the Allison-Switzer farm (named for Switzer's maternal grandparents, who bought the 121-acre property in 1916, and his father and mother, who took it over after her parents retired in 1946) is just one of millions of such stories, Switzer writes.

"In 1900, 42 percent of the U.S. population lived on farms; by 1990 that number had dwindled to less than 2 percent," he says in the book's prologue. This transition occurred largely as a result of economic and technological changes made possible by the aggressively optimistic borrowing, investing and expansion that some farmers were willing to embrace in the latter half of the 20th century. Many other farmers, who had stared down economic catastrophe in the 1920s and '30s, were unwilling to take on new big risks, and their farms generally gave way to the forces favoring consolidation and the mass-production of agricultural commodities. Switzer's book is not a treatise on the evolution of American farming, however.

"The characters in this story are not statistical stick figures illustrating the decline of a Midwestern family farm," he writes. "They are my family. The details of their lives provide an intimate portrait of a once common way of life, now almost entirely vanished from the American countryside."

This portrait includes details normally left out of family memoirs: his maternal grandmother's hostility to her daughter's intellectual and educational aspirations; his grandfather's recurrent narcolepsy, a lifelong handicap brought on by severe heatstroke suffered while working in the fields as a teenager; Switzer's mother's depression and unhappiness with farm life; and his father's inability to recruit his sons to the profession.

The book also offers an account of the changes that occurred over the 76 years the family owned the farm, from the early days of kerosene lamps, hand milking and horse-drawn plows, to the gradual though never fully realized modernization of equipment and farming techniques.

Switzer begins with the gritty details of his grandparents' daily life. Charlie and Mabel Allison milked their cows twice daily in a drafty barn. They lived in an oversized and poorly insulated farmhouse with no modern conveniences. They grew corn, hay, oats and barley to feed their livestock and themselves. Charlie carted fresh milk to a nearby cheese factory every morning. Mabel kept a vegetable garden and orchard, and canned produce for the winter. The couple raised chickens and sold their eggs.

Servicemembers who work with a financial planner cite financial know-how and moral fiber as the top qualities to look for when picking a planner, according to the First Command Financial Behaviors Index.

Recent survey findings from the First Command Financial Behaviors Index reveal that roughly three quarters of middle-class military families (senior NCOs and commissioned officers in pay grades E-6 and above with household incomes of at least $50,000) picked trustworthy, knowledgeable and honest as the most important attributes they seek in a planner.

Financial planning is a profession built on personal relationships with people you trust, said Scott Spiker, CEO of First Command Financial Services, Inc. These survey findings underscore the unique combination of economic and emotional assistance provided by financial planners as they coach servicemembers and their families in the fiscal behaviors associated with financial security and success.

The top attributes military families look for in a financial planner are:

Notably, the cost of planning services is not a significant consideration when choosing a planner. Just three out of 10 respondents picked low price for services as the most important attribute to look for in a planner.

Financial planning is not a commodity that consumers shop for based on price alone, Spiker said. Consumers are willing to pay for planning services when they feel the planner has technical expertise and personal integrity. Through one-on-one service, financial planners provide a sense of security to men and women in uniform as they pursue their long-term goals and life-time dreams.

Study of the Day: A Diet Loaded With Sugar Makes Rats Dumber

New research in the Journal of Physiology suggests that fructose disrupts learning and memory, but omega-3 fatty acids can help.

PROBLEM: The average American consumes roughly 47 pounds of cane sugar and 35 pounds of high-fructose corn syrup per year, mostly through processed foods like soft drinks, condiments, and desserts. Though previous studies have shown how these sweeteners harm the body through its role in diabetes, obesity, and fatty liver, it was unclear if they had impact on mental processes.

METHODOLOGY: University of California, Los Angeles, researchers Rahul Agrawal and Fernando Gomez-Pinilla trained two groups of rats on a maze twice daily for five days before serving them a fructose solution as drinking water for six weeks. The second group also received omega-3 fatty acids, which protect against damage to the synapses or the chemical connections between brain cells that enable memory and learning. After this experimental diet period, the researchers tested the rats' ability to recall the visual landmarks the scientists installed to help them escape the maze.

RESULTS: The second group of rats remembered the correct route and were able to exit much faster than the rats that did not receive omega-3 fatty acids in the form of docosahexaenoic acid (DHA) and flaxseed oil. A closer look at the brains of the DHA-deprived mammals showed a decline in synaptic activity and signs of resistance to insulin, a hormone that controls blood sugar and regulates synaptic function.

CONCLUSION: A high-fructose diet sabotages learning and memory, but omega-3 fatty acids can partially offset the damage.

IMPLICATION: Gomez-Pinilla says consuming one gram of DHA per day through foods rich in omega-3 fatty acids like salmon, walnuts, and flaxseeds can protect the brain against fructose's dumbing effects. "It's like saving money in the bank," he says in a statement. "You want to build a reserve for your brain to tap when it requires extra fuel to fight off future diseases."

Nutrition program offers breakfast, lunch to kids

WEATHERFORD Weatherford ISD is making it easier on parents this summer, by offering their annual Summer Meals program, beginning June 4.

Funded through a grant and paid for by the state, the program offers free breakfast and lunch to kids and youth up to the age of 18.

Some kids may not get the nutritional meals they need in the summer, especially if the parents are at work, Jerolyn Goodman, a registered dietitian and director of the WISD child nutrition services, said. The neat thing is that they dont even have to be Weatherford students or live in town. Anyone up to 18 can come and eat for free.

The program will be held at Ikard Elementary, 100 Ikard Lane, and runs through Aug. 10. Services will not be available July 4.

Breakfast will be served Monday through Friday, from 8 to 9 a.m., and lunch from 11 a.m. to 12:30 p.m.

For the longest time, we were the only place in Parker County that offered it, but there really hasnt been a big turnout, Goodman said. We really want people to come out.

Breakfast is available for accompanying adults for $2, and lunch for $3.25.

Its a good way, if youre out and about, to just stop by and feed your kids a nutritious lunch for free, Goodman said.

With a variety of meals, Goodman said that pizza and hot dogs are the top favorites, noting that kids can count on pizza every Thursday.

In addition to the meals, Kids Unite programs will also be going on, with anyone welcome to attend.

Foods, nutrition professor named to UGA Foundation Professorship

Richard D. Lewis, professor of foods and nutrition in the University College of Family and Consumer Sciences, has been named to the UGA Foundation Professorship in Family and Consumer Sciences.

Since joining the College of Family and Consumer Sciences in 1986, Dr. Lewis has excelled in teaching, research and outreach, said FACS Dean Linda K. Fox. His research on topics related to bone strength have been on the cutting edge, and he has incorporated the newest research findings into the courses he regularly teaches on life-cycle nutrition and medical nutrition therapyas well as seeking opportunities for students to engage in service-learning projects.

Richard D. Lewis, professor of foods and nutrition was named to the UGA Foundation Professorship in Family and Consumer Sciences.

Lewis spent his first five years at UGA as a nutrition and health specialist with UGA Cooperative Extension, working specifically on programs related to weight control for adults and healthy lifestyles for children. In 1992, his focus shifted to teaching and research, and he began exploring the impact of gymnastics on female bone strength.

Soon after I joined the university, I began providing nutritional guidance to our athletes on a volunteer basis, Lewis said. When we decided to explore this research question (of gymnastics and bone strength), it was generally thought that the excessive exercise and food restriction that is often found in gymnastics would have a negative impact on bone strength. On the contrary, our research showed that college gymnasts had very dense bones.

Lewiss research was one of only two projects on the topic published nationally in 1995, putting him at the forefront of the field. Among the unique aspects of the study was the use of non-gymnast controls.

We made sure that every gymnast was matched with regard to their age, height, weight and other factors to ensure that we could relate the difference in bone strength to gymnastics rather than other factors, he said.

Lewis has gone on to measure bone strength in retired competitive gymnasts at the ages of 35 and 45. He is now beginning to look at 55-year-old former gymnasts to gauge whether higher bone strength continues into their menopausal years. He also has looked at children as young as four to determine when bone strength differences begin.

His other research includes pioneering a federally funded study into the role of vitamin D in developing bone strength in children, a topic he will lecture on at a conference in Switzerland, and examining the use of soy isoflavones to counteract menopause-related bone loss in women.

His current research focuses on the role a common virus may play in bone strength among obese children. He also is working with colleagues on additional projects related to UGAs obesity initiative.

GW researchers discover biomarker for advanced bile duct fibrosis and bile duct cancer

WASHINGTON (May 29, 2012) GW Researchers, Jeffrey M. Bethony, Ph.D., associate professor of Microbiology, Immunology, and Tropical Medicine, and Paul Brindley, Ph.D., professor of Microbiology, Immunology, and Tropical Medicine at GW School of Medicine and Health Sciences, along with colleagues from Khon Kaen University in Thailand have determined that plasma Interleukin-6 (IL-6) levels are an sensitive and specific biomarker for the detection of the advanced bile duct fibrosis and bile duct cancer that comes from chronic infection with the Asian Liver Fluke (Opisthorchis viverrini). The research, titled, "Elevated Plasma IL-6 Associates with Increased Risk of Advanced Fibrosis and Cholangiocarcinoma in Individuals Infected by Opisthorchis viverrini," was recently published in the journal, PLoS Neglected Tropical Diseases.

This discovery is a critical advance for the Northeastern (Isaan) region of Thailand and other areas of Southeast Asia, where an estimated 10 million people are currently infected with Asian Liver Fluke, which is the leading cause of cancer in the region. This food-borne parasite encysts in the fins, skin, and musculature of fish and infection occurs when people ingest this raw or uncooked fish. While the infection can be cured by chemotherapy, environmental and cultural factors of the region strongly favors rapid reinfection, with bile duct cancer as a common outcome. Northeastern Thailand has the highest levels bile duct cancer in the world. The determination that plasma IL-6 levels are associated with bile duct fibrosis (a precursor state to bile duct cancer) and early bile duct cancer were done by examining participants in the Khon Kaen Cholangiocarcinoma Cohort, the largest cancer cohort of its type in the world, and an NIH sponsored project in collaboration with George Washington University and Khon Kaen University. In this study, researchers visited nearly 4,000 individuals resident in rural areas of Thailand, where the parasite is endemic to conduct ultrasound studies to detect advanced bile duct fibrosis and early bile duct cancer and then link the ultrasound results with levels of plasma IL-6. This research was done in collaboration with Drs. Banchob Sripa, Bandit Thinkhamrop, Eimorn Mairiang, and Thewarach Laha from the Khon Kaen University School of Medicine and School of Public Health.

As a result of the identifying this biomarker, the Thai Ministry of Public Health has implemented testing for levels of plasma IL-6 for individuals between 20 and 60 in endemic regions of Isaan Thailand to identify individuals at risk for bile duct cancer. Through this test, it is hoped that bile duct cancer can be detected earlier and at a more treatable stage.

"Typically, bile duct cancer has a late presentation, and so a very high mortality rate. Now that we've identified the IL-6 biomarker, our hope is that earlier detection is possible and earlier treatment saves more lives," said Jeffrey M. Bethony, Ph.D., associate professor of Microbiology, Immunology, and Tropical Medicine at the GW School of Medicine and Health Sciences (SMHS).

In collaboration with Dr. Jason Mulvenna of the Queensland Institute of Medical Research in Brisbane Australia, the researchers are also using quantitative proteomics approaches to identify biomarkers that could be used in an even more sensitive and specific test for the early detection of this infection-associated bile duct cancer. The current discovery was possible through funding from an RO1 grant (R01CA155297-01) from the National Cancer Institute (NCI) of the National Institutes of Health (NIH).
