Loncastuximab Tesirine Provides Alternative for Broad Range of Patients With DLBCL – OncLive

On April 23, 2021, the FDA granted accelerated approval to the CD19directed antibody and alkylating agent conjugate for the treatment of adult patients with relapsed/refractory large B-cell lymphoma, including diffuse large B-cell lymphoma (DLBCL) not otherwise specified, DLBCL arising from low-grade lymphoma, and high-grade B-cell lymphoma, following 2 or more lines of systemic therapy.1
The decision was based on data from the single-arm phase 2 LOTIS-2 trial (NCT03589469), in which the agent elicited an overall response rate (ORR) of 48.3% (95% CI, 39.9%-56.7%) in 145 patients with DLBCL. This included a complete response (CR) rate of 24.1% (95% CI, 17.4%-31.9%) and a median duration of response (DOR) of 10.3 months (95% CI, 6.9-not estimable).2
In an interview with OncLive, Paolo F. Caimi, MD, an associate professor of medicine in the Division of Hematology and Oncology at Case Comprehensive Cancer Center at Case Western Reserve University and medical director of the Clinical Trials Unit at University Hospitals Seidman Cancer Center, both in Cleveland, Ohio, discussed what lies ahead for loncastuximab tesirine and how it fits into the rapidly changing treatment landscape of DLBCL.
Caimi: This trial was focused on relapsed/refractory DLBCL and included adult patients who had at least 2 prior lines of therapy. It included not only those with regular DLBCL but also patients with double-hit lymphoma, which is termed high-grade B-cell lymphoma with MYC and BCL2 and/or BCL6 rearrangements, as well as those who had primary mediastinal B-cell lymphoma. Patients had to have acceptable functional and hematologic status and had to be within normal limits in terms of renal function and liver function. They also had to have a positive CD19 expression as demonstrated by biopsy if they had undergone prior chimeric antigen receptor (CAR) T-cell therapy; however, patients with prior CAR T-cell therapy were not excluded. The study did exclude patients with bulky disease.
Loncastuximab tesirine was active with an ORR of 48% and a CR rate of 24%. It is notable that as a single agent, it elicited response rates that other agents [did not] achieve in combination. Secondly, there was a good DOR of over 10 months, and the DOR was not reached for those who had a CR. In addition, the population was high risk. We presented at [the 62nd American Society of Hematology Annual Meeting and Exposition], and we are going to present a subgroup analysis [at the 2021 American Society of Clinical Oncology Virtual Scientific Program] showing how different risk factors didnt affect how the drug worked.
Paolo F. Caimi, MD In general, loncastuximab tesirine is a well-tolerated drug. Compared with other drugs that are approved [in this setting], the adverse effects are somewhat different. Loncastuximab tesirine has described elevations of -glutamyltransferase as a frequent adverse effect. [Patients also experienced] mild to moderate hematologic toxicity. Additionally, there were portions of populations [with lymphoma] who had thrombocytopenia, approximately 18%, neutropenia in approximately 26%, and there were occasional cases with fluid retention and edema.
Photosensitivity including a rash or other skin damage with exposure to the sun [was also observed]. Practitioners need to monitor [results of] liver function tests and hematologic function tests, and change or adjust dosing for patients who have fluid retention. I also advise patients to stay away from the sun as much as possible, use sunblock, and [wear] additional clothing.
How does this approval advance theDLBCL treatmentparadigm?
Weve seen several new agents approved for DLBCL in the past 24 months, including some CAR T-cell therapies. This is the first antibody immunoconjugate drug targeting CD19 that is approved as an off-the-shelf agent, that is active as a single agent, and that is well tolerated. In addition, loncastuximab tesirine really doesnt cause neuropathy, which is a common [adverse effect] with other agents.
With loncastuximab tesirine we have another drug that has single-agent activity and has the potential for combinations[I hope] this will be studied. Loncastuximab tesirine has significant value in the treatment of DLBCL, [particularly for those] with severe neuropathy, those who may not tolerate CAR T-cell therapy, or those you may think will need treatment relatively quickly.
[Next steps for loncastuximab tesirine include] delineating the activity as a single agent, which will be important. Single-agent activity promises the potential for combination, and this is something that I think will be studied in the near future to see whether we can elicit further activity by using combinations with other new agents, particularly targeted ones. Also, for loncastuximab tesirine in general, as weve seen with other anti-CD19 with CAR T-cell therapies, the potential for activity in other lymphoma and B-cell lymphoma subtypes is significant. Further examination of its role for patients with mantle cell lymphoma [in combination with ibrutinib (Imbruvica) in a phase 1/2 trial (NCT03684694)] and follicular lymphoma will be relevant.

‘Good’ Bacteria Show Promise for Clinical Treatment of Crohn’s Disease, Ulcerative Colitis | Newsroom – UNC Health and UNC School of Medicine

Balfour Sartor, MD, Midget Distinguished Professor of Medicine, Microbiology and Immunology, is the senior author of a study that shows how a novel consortium of bacteria that live in the digestive tracts of healthy individuals can be used to prevent and treat aggressive colitis in humanized mouse models.
CHAPEL HILL, NC A new study published in Nature Communications demonstrates that a consortium of bacteria designed to complement missing or underrepresented functions in the imbalanced microbiome of inflammatory bowel disease (IBD) patients, prevented and treated chronic immune-mediated colitis in humanized mouse models. The studys senior author, Balfour Sartor, MD, Midget Distinguished Professor of Medicine, Microbiology and Immunology, Co-Director of the UNC Multidisciplinary IBD Center, said the results are encouraging for future use treating Crohns disease and ulcerative colitis patients.
The idea with this treatment is to restore the normal function of the protective bacteria in the gut, targeting the source of IBD, instead of treating its symptoms with traditional immunosuppressants that can cause side effects like infections or tumors, Sartor said.
The live bacteria consortia, called GUT-103 and GUT-108, were developed by biotech firm Gusto Global. GUT-103 is comprised of 17 strains of bacteria that work together to protect and feed each other. GUT-108 is a refined version of GUT-103, using 11 human isolates related to the 17 strains. These combinations permit the bacteria to stay in the colon for an extended amount of time, as opposed to other probiotics that are not capable of living in the gut and pass through the system quickly.
GUT-103 and GUT-108 were given orally three times a week to germ-free mice (no bacteria present) that had been specially developed and treated with specific human bacteria, creating a humanized mouse model. The therapeutic bacteria consortia worked by addressing upstream targets, rather than targeting a single cytokine to block downstream inflammation responses, and reversed established inflammation.
It also decreased pathobionts bacteria that can cause harm while expanding resident protective bacteria, and produced metabolites promoting mucosal healing and immunoregulatory responses, Sartor said. Simply put the treatment increased the good guys and decreased the bad guys.

This inhalation machine can be installed at temples, dargahs, mosques, churches and public places where people can freely take the steam.

Doctor Waheed said that the same ingredients i.e. 4 tsp of kalonji, 4 tsp of methi dana, 10-15 cloves, 10-15 black peppercorns, small bunch of mint leaves, 50-100 grams of ginger, small piece of dalchini, adrak around 50 grams and podina leaves are boiled in one litre of water and this can be had as a decoction. However, in steam therapy, the same ingredients are used along with unani medicine Triyak E Azam.
Asked if any trials or study is done to find its efficacy he said that patients recovery is the clinical evidence. Most of these patients were not monitored at any hospital and were being treated at home. There was a good feedback from patients and reduction in symptoms besides improvement in oxygen level were good enough to see the progress
He however cautioned people not to take decoctions in excess. In normal cases two to three cups of decoction can be had during a day. While in cases of COVID-19, it can be had 4 to 5 times and steam inhalation can be done thrice. This therapy he claims builds immunity, reduces inflammation.
He says Indian traditional methods play a pivotal role to augment preventive measures provided in the guidelines by the Ministry of Health and Family Welfare (MoHFW). The current understanding of COVID-19 indicates that good immune status is vital to prevention and to safeguard from disease progression.

What Is Reiki And How Does It Work? – Forbes

Reiki may sound new age to some, but its anything but new. In fact, its a form of energy healing with roots in ancient cultures. Reiki uses whats known as universal life force energyki or qito improve peoples lives and ailments.
The term reiki comes from the Japanese words rei, meaning universal, and ki, which means vital life force energy that flows through all living things. Some practitioners describe reiki as acupuncture without the needles.
Reiki is a complementary health approach in which practitioners place their hands on or just above different areas of the body. Its based on an Eastern medicine belief that living beings have energy fields that support their health and vitality.
Energy blocks, on the other hand, impede innate flow of energy, causing not only health problems, but also negative life circumstances like financial troubles or relationship problems, according to belief. Energy blocks of any living being can be treated, according to belief, which is why some practitioners also work on animals and plants. Reiki practitioners focus on sensing energy blocks and moving the energy for the greatest good of the client.
There are at least 30 slightly different branches of reiki, but Usui is by far the most popular form. Usui is the method practiced and taught in Japan as early as the 1900s by Makao Usui, who is credited as the founder of modern reiki.
Usui reiki came to the West via Hawaii in the 1930s thanks to a Japanese-American woman named Hawayo Takata. She traveled back to her native Japan to seek healing for her own physical and mental ailments, including a lung condition, asthma and a nervous breakdown after the death of her husband. One of Makao Usuis students, Dr. Chujiro Hayashi, taught reiki to Takata. Her healing experience was so profound that Takata returned to Hawaii and became the first person to teach reiki in the West.
However, due to the abundance of anti-Japanese and anti-Asian sentiments in the United States around World War II, Takata shied away from using the name reiki and instead referred to her establishment on the island of Kauai as a healing studio, according to researcher Nat Newton, Ph.D., a Usui reiki master in Orange County, California.
Takata modified some of the teachings for her English-speaking students and ended up training 22 students in reiki as we know it today, says Newton. Today, thousands of people worldwide are trained in Usui and similar reiki methods.
While reiki can easily be done in just about any setting, a typical reiki session takes place in a relaxing environment or treatment room, akin to massage therapy. Newton holds some of her reiki sessions in well-ventilated outdoor spaces. Meanwhile, many reiki practitioners do remote healing where they arent even in the same building as the client.
In a typical treatment room setting with a reiki practitioner and a client, the client begins face up and fully clothed. They might briefly discuss their ailment or situation they hope to heal. The session might begin with a brief meditation, and then the practitioner uses various hand movements to lightly touch or hover over various parts of the clients body while they are face up and face down. Throughout the session, the practitioner focuses specifically on their intent for healing.
We usually think about activating whatever is in the highest good for the client, says Newton. Sometimes well visualize the client as healed. But always well set a positive intention for whatever they need at the moment that would serve their highest good. We dont know what that is and dont presume to know. We let reiki do what it needs to do.
A typical session lasts 30 minutes to an hour, and a client-practitioner discussion uses a good amount of that time, according to Newton. After the session, I always spend time talking to my clients about what came up for them and perhaps also what I intuited, she says.
The most well-documented benefits of reiki revolve around the relaxation response, which practitioners say invokes the bodys natural healing process.
Dr. Rachel Lampert, M.D., a professor of medicine at Yale School of Medicine, and her colleagues studied 37 patients after having a heart attack. The patients were randomized into three groups: patients who simply rested, those who received a single session of reiki treatment from a nurse trained in reiki and those who listened to relaxing music. The researchers measured activity of the autonomic nervous system (ANS), which regulates heart rate, breathing, blood pressure and digestion.
Lamperts team zoomed in on heart rate variability (HRV), a measure of the pattern of heartbeats thats controlled by the ANS. The higher the HRV after a heart attack, the better the outlook for the patient, explains Lampert.
In the Yale study, patients who received reiki had a higher HRV and improved emotional state. Our study was a very nice demonstration that doing things that are relaxing has benefits, says Lampert. We showed increased activity of the healthy arm of the nervous system.
Researchers at Harvard Medical School followed 99 patients at multiple sites to determine the effects of a single reiki session. The studya single arm effectiveness study published in 2019 in the Journal of Alternative and Complementary Medicinefound statistically significant improvements in anxiety and depression symptoms, as well as pain and nausea.
Additional research suggests reiki helps with depression and insomnia. A 2012 study in the Indian Journal of Positive Psychology looked at 40 women who suffered from depression and anxiety. Half of the group received a reiki treatment twice a week for 10 weeks and the other half received no treatment. The women who received reiki saw significant improvements in both their depression symptoms and sleep quality.
A range of studies address reikis role in pain alleviation after knee surgery, the restoration of range of motion in injured shoulders, post-cesarean section recovery, hypertension management and the improvement of quality of life for patients with rheumatoid arthritis or patients undergoing various cancer treatments.
Portuguese researcher Zilda Alarcao and her colleagues looked at the impact of reiki treatment versus sham or fake reiki in two groups of patients with blood cancer. Each group contained 58 patients who received an hour-long treatment once a week for four weekseither someone trained to administer reiki worked on them or someone pretending to do reiki (sham reiki) spent an hour with them.
The researchers found the patients who received real reiki showed significantly more improvements than the other group in general, physical, environmental and social dimensions of quality of life using the Portuguese version of the World Health Organizations Quality of Life survey (WHOQoL-Bref), a well-regarded research tool that measures pain and other quality of life issues after undergoing an intervention. They published their results in 2016 in the European Journal of Integrative Medicine.
Multiple studies report no adverse effects of reiki, likely because its a noninvasive treatment. Practitioners note clients usually report feeling relaxed after a session, but some feel energized. Depending on the nature of the problem, Newton says some clients also may feel more emotional or have other intense feelings in the days or weeks following a session.
While there is a professional code of ethics that governs reiki practitioners, there are no licensing boards like there are for doctors, nurses or even hair stylists. Reiki master is the highest level of training, but theres a wide range of training and expertise among practitioners. To ensure the best result, practitioners advise clients to do their homeworkask a practitioner about their training and credentials, but also find someone you connect with and trust.
One potential risk is that clients can misunderstand the role reiki should play in a treatment plan. The code of ethics of the Reiki Alliance, a professional reiki association, clearly states that reiki practitioners work as a complementnot a replacementto the medical care a patient receives. Reiki is also not a practice intended to instill doubt in other medical treatments and interventions. Clients are strongly discouraged from viewing reiki as a substitute for medical doctors, surgery, therapy or prescribed medications.
Clients should also be aware that while reikilike yoga or meditationmay have roots in spiritual practices from long ago, modern reiki is not a religion. Practitioners and clients come from all walks of life and belief systems. Receiving reiki is not intended to interfere with or change a clients spiritual or religious beliefs.
According to the National Center for Complementary and Integrative Health, reiki has been studied for a variety of conditions, including pain, anxiety, and depression, but most of the research has not been of high quality, and the results have been inconsistent.
Naturally, scientists who study reiki beg to differ with that assessment. They say institutions simply need to catch up with the rapidly evolving science. At this point, there are probably 60 or more published studies that I can point to that show reikis benefit, and yet theres still this persistent myth that no credible science backs reiki, says neuroscientist Natalie Dyer, Ph.D, president of the Center for Reiki Research, a nonprofit that aims to advance the scientific knowledge and study of reiki.
Dyer acknowledges, however, that some studies have lacked rigor and some have not found statistically significant benefits with reiki. She calls for more high-quality research to understand the practice, how it works and its limitations.
In the Yale study of reiki following a heart attack, researchers note that its unknown whether the beneficial effects of reiki treatment over music stem from the presence of another person, the presence of a person with healing intention, the light touch technique, or a combination of factors.
Like any intervention, reiki practitioners warn it isnt a cure-all and shouldnt be viewed as such. But its deemed safe and potentially effective enough that a considerable number15% and growingof U.S. hospitals offer reiki healing specifically. Leading institutes like Yale, Harvard, the University of Minnesota, the University of Arizona, UCLA and others offer reiki as a complement to medical interventions.
Costs for reiki sessions vary nationwide, but expect to pay about what youd pay for a massage. In a major metropolitan area, you might pay $80 to $150 for a one-hour reiki session.

Few movies lodged themselves in my childhood brain the way Fantasia did. A psychedelic 1940s Disney animation set to a thundering score played by a maniacal Philadelphia Orchestra, it broke artistic ground as quickly as it laid it.
The scene that branded itself on my memory, though, was that of the sorcerer's apprentice. Set to freshly composed symphonic torment, it follows Mickey Mouse playing understudy to a great magician (based on a 1797 ballad by Goethe, Der Zauberlehrling, which means the sorcerer's apprentice in German), and, with Mickey being too lazy or perhaps too enterprising to fetch the buckets of water that were part of his job description, he steals his master's wizardly hat and conjures a spell to have a mop do the work for him.
In a (mildly, now that I have rewatched it for research) terrifying escalation, however, Mickey hacks at the mop with an axe to halt its autonomous and uncontrolled toil against a seizure of blood-red backdrop, shadows billowing up like nightmares on the walls behind them, while the mop replicates like a clone army, trampling over our poor mouse as he succumbs to the purgatory of his decision.
The reason I relate this disturbing memory is to illustrate what tends to happen when a simple question is posed in emergency medicine. Today's cascade of questions arose during a discussion about the possibility of using intravenous dexamethasone to treat acute back pain. The question, Is dexamethasone effective in treating acute back pain? then splintered into a thousand further questions. Let us watch them reproduce, without the soaring soundtrack.
The totality of the current evidence says probably not. A few small-scale studies are desperately trying to prove the affirmative, but they are, as is so often the case, meager in number, negative on primary outcomes, single-centered, and nonreplicable.
It is at this point, however, that the questions begin to split off.
There has to be an inflammatory component, right? So surely there's some biological plausibility in here.

It gets a little fuzzy here. We were all taught that steroids need to be transported into the nucleus to exert their conformational change and effect, so we also know that steroids work in at least three ways: slow, quick, and hey, that's weird.
It is likely that the patients we see in the ED with acute (not chronic) back pain have a cornucopia of pathological mechanisms resulting in the pain we are assessing. The most common likely mechanism is the sudden wrenching of the facet joints, which have a rich neural network around them causing, among many things, severe muscle spasm. But they also have a relatively poor blood supply, delivering not much of anything to them.
There are other paths to pain, though. Herniated discs expose a hugely inflammatory core. Discs impinge on nerve roots, causing neuropathic pain. Hitting the inflammatory pathways will only modulate some of these, and without the benefit of advanced imaging in the ED (appropriately), we will not be sure how much each of these is contributory.
Horses need to be held here. Much of this is about the acute injury. We don't give steroids for acute ligamentous sprains. We (definitely) don't give steroids in acute brain injury. The risk-benefit profile must be way in the negative here. The side effects and risks of steroids are well known.
Man, this could be true. Sore throats, swollen throats, viral neuropathies, bronchospasm, cerebral edema, spinal cord compression, COVID requiring oxygen, arthritides, altitude sickness, all of them exquisitely responsive to dexamethasone. We give it when there's too much thyroxine; we give it when there's too little thyroxine. We give it for skin problems, eye problems, allergic problems, blood problems....
It is at this point that the questions begin marching over us, unyielding, heedless of the existential pain they might be inflicting.
You see where this is going: nowhere good. Like Mickey Mouse, who opened a Pandora's box, to thoroughly mix story and myth, a seemingly benign question has become something horrifying.
Let us return to the original question. Parenteral dexamethasone for acute back pain in ED: probably not justified. Perhaps the best treatment for back pain is avoidance, such as finding an alternative to back pain-inducing activities, like carting buckets of water.
