Genetic testing initiative for inherited retinal diseases shows great promise – ModernMedicine

Recently, I was watching a movie with my wife in which the plot revolved around the idea of futuristic medical technology and therapies including gene-targeted treatments. While most of it seemed far-fetched at best, there were some aspects that seemed not only feasible, but are actually being utilized in medicine today.
With this as the backdrop, I have been encouraged as a retinal specialist with all the excitement and promising new treatments in gene therapy. Given that there are several retinal diseases with known inherited patterns and confirmed genetic loci responsible for the clinical manifestations, our field is primed to benefit from the expansion of knowledge in this arena.
With the advent of genetic testing for inherited retinal diseases, this has provided another tool in our tool belt to help provide the most advanced care for our patients. The benefits of genetic testing include confirming clinical diagnosis to identify genetic disorders with systemic associations, provide information for family members, and potentially identifying patients that can benefit from gene targeted therapies.
A prime example of this is a new initiative called ID YOUR IRD, an inherited retinal disease (IRD) genetic testing program (Spark Therapeutics). According to the American Academy of Ophthalmology (AAO), genetic testing is appropriate for most patients with presumed genetically caused retinal degeneration and plays an important role in improving the accuracy of diagnosis and prognosis, providing patients and families with specific inheritance risks, and guiding treatment decisions.1
The ID YOUR IRD genetic testing panel includes 31 genes in which mutations have been found to cause certain early-onset, rod-mediated IRDs (see Figure 1). In particular, the three inherited retinal diseases that can be identified are retinitis pigmentosa (RP), choroideremia (CHM), and Leber congenital amaurosis (LCA). Adult and pediatric patients with signs and symptoms suggestive of certain early-onset, rod-mediated IRDs who are U.S. residents are eligible for the ID YOUR IRD initiative. An Experimental Gene Therapy Uses Viruses to Stop Age-Related Blindness – Futurism

In Brief Researchers from Johns Hopkins Medicine in Maryland have discovered a rather unusual way to treat a severe form of age-related blindness. They found a virus inserted into the retina can be used to halt or even reverse the disease. A Unique Treatment
They say you dont fight fire with fire. However, researchers from Johns Hopkins Medicine in Maryland have found that sometimes a virus may be the best weapon against a disease.Their studyhas been publishedin The Lancet
The researchers werelooking for ways to treat a particular type ofage-related macular degeneration (AMD)known as a wet AMD. Its a rare and more severe form of the disease,affecting just 10 percent of all AMD patients, and it causes new blood vessels to grow under the retina, which then leak blood and fluid into the eye, leading to vision problems.
The researchers knewthey could halt and even reverse the condition by suppressing an overactive protein called vascular endothelial growth factor (VEGF). Other researchers had been able to do it with monthly eye injections, but this team was hoping to do it with just one injection.
The best way they found to do this was by using a common cold-like virus called AAV2 as a carrier of gene that activates the production of a differentprotein,sFLT01, tocounter VEGF.
In a preliminary trial involving 19 men and women 50 years old and above, the researchers injected the patients with a form of AAV2that was genetically engineered to penetrate retinal cells and deposit the gene. After the virus deposited the gene, the cells began secreting sFLT01 which bound to VEGF and prevented it from stimulating leakage and growth of abnormal blood vessels, explained a Johns Hopkins press release.
The clinical trial showed promising results, with the condition of four of the patients improving dramatically after just one viral injection. Two others saw some reduction in the fluid build up, and the treatment didnt produce any side effects in any patients. Even at the highest dose, the treatment was quite safe. We found there were almost no adverse reactions in our patients, said researcher Peter Campochiaro.
Of the patients that didnt respond, the researchers discovered that five naturally produced antibodies that would attack the AAV2 virus, rendering it unable to complete its gene depositing mission. They think these antibodies could be prevalent throughout the population, making it difficult to determine how effecting the treatment would actually be.
Nevertheless, this research is a step in the right direction, especially with AMD expected to affect almost 5.44 million people in the U.S. by 2050. This preliminary study is a small but promising step towards a new approach that will not only reduce doctor visits and the anxiety and discomfort associated with repeated injections in the eye, but may improve long-term outcomes, Campochiaro said.

5 Authoritarian Regimes That Shape Facebook’s Censorship Policies – Breitbart News

Facebooks growth is slowing. It needs new markets and new audiences, which is why it is making a big push into foreign countries. However, some of these countries arent happy with the idea of letting their citizens have access to free-speech friendly platforms, and impose conditions on Facebooks operations within their borders.
So, does Zuckerbergs stated commitment to free speech trump the companys need to enter markets controlled by authoritarian, censorious governments? Readers can examine the following five examples, and judge for themselves.
1. China
Facebook was banned from China following riots in 2009 inrmqi and revelations that the Xinjiang independence activists behind the riots used the social network to organize. Facebook has been desperate to re-enter Chinas massive market ever since.
Mark Zuckerberghas met with Chinese president Xi Jinpingas well as Chinese propaganda chief Liu Yunshan. The Facebook CEO has even learned Mandarin and delivered speeches (albeit clumsy ones, according to Quartz) in the language during his multiple trips to China. According to reports, Zuckerberg even asked the Chinese president to name his baby during a meeting at the White House, although the president refused.
But Facebook has done more than cosy up to Chinese officials. According to reports, they are also building a censorship tool to block banned news sources in China from users timelines. Several Facebook employees have quit in protest at the development of the tool, which will reportedly give third parties like ISPs and governments the power to suppress posts.
Then again, Facebook is competing with domestic Chinese social networks, which pride themselves on blocking what they call fake news
2. Turkey
Turkey frequently censors its citizens on the internet. During the coup attempt against President Erdogan last year, all social media was blacked out across the country.Just last month, Turkey blocked access to Wikipedia.
Facebook has been working with Turkey to censor Kurdish militia in northern Syria. Although these groups are largely credited with rolling back the frontiers of the Islamic State, they are considered terrorists by Turkey, an extension of the Kurdistan Workers Party (PKK), that has staged attacks inside the country. Turkey is even accused of allowing ISIS fighters to cross its southern border to fight the Kurds.
A document leaked in 2012 revealed even more censorship on behalf of Turkey: according to guidelines on IP blocks and international compliance given to an external Facebook contractor, moderators were told to consider a wide range of Turkey-critical content to be an abuse standards violation. These included attacks on Kemal Ataturk, the founder of modern Turkey, maps of Kurdistan, images depicting the burning of the Turkish flag, and any content related to Abdullah Ocalan, the most influential leader of the Kurdish independence movement.
3. Pakistan
Pakistan, also known astheIslamic Republicof Pakistan, is currently undertaking a massive crackdown against what it describes as social media blasphemy. The state recently sent out a text message to millions of Pakistanis urging them to report their fellow citizens if they suspect them of blasphemous posting, effectively encouraging a citizen-led religious Stasi.
Much of the citizenry will be happy to oblige. Indeed, some Pakistaniswould like to go beyond simply reporting blasphemers:
Pakistan has asked Facebook for help identifying blasphemers on social media even those outside the country, so it can pursue their extradition.Facebook has not denied complying with the request, instead saying thatthe companyreviews all government requests carefully, with the goal of protecting the privacy and rights of our users.'
What is known is that Facebook has dispatched a delegation to Pakistanto address the governments concerns. Moreover,government officials have claimed that the company has helped them remove 85 of blasphemous material on Facebook.This would make Facebook complicit in Pakistans determination to quash religious dissent from its citizens, which includes a potential death penalty for the crime of blasphemy.
4. Russia
The media is determined to find evidence of collusion between President Trump and Russia, but there is considerably more evidence to be found of Facebook doing the bidding of the Russian government, which is frightening the social network by threatening to ban it from the country.
The pressure seems to have paid off in 2014, Facebook blocked a page supporting Alexei Navalny, described by theWashington Postas Putins biggest critic.
5. Germany and the European Union
Not all authoritarian countries are non-western. In response to the migrant crisis and the subsequent crime and terrorism wave sweeping Europe, Germany has taken a keen interest in scrubbing criticism of their catastrophic mass migration policies from social media.German police have even raided homes over alleged Facebook hate speech, and one couple was taken to court and sentenced for criticizing mass migration on the platform.
In September 2015, German chancellor Angela Merkel was overheardasking Mark Zuckerberg if he was working on clamping down against allegedly hateful content on the platform, to which Zuckerberg replied yeah. The German government has also threatened to fine Facebook if it does not clamp down on fake news, while the European Union has threatened non-legislative action if social networks like Facebook and YouTube do not tackle hate speech on their platforms.
Zuckerberg was true to his word. Following his overheard discussion with Merkel, Facebook has signed up to an E.U. pledge to suppress illegal hate speech and use their power to promote counter-narratives. Facebook also launched its own Initiative for Civil Courage Online, a Europe-wide campaign to clamp down on alleged hate speech during the migrant crisis. In just one month alone in September 2016, Facebook deleted over 100,000 posts in Germany for containing hate a figure that was attacked by the German government as too low.
Mark Zuckerberg is a strong supporter of Angela Merkels refugee policies, and has called on the U.S. to follow their lead.

‘Censorship is still happening at SABC’ – News24

The SABC continues to censor political views that oppose the ruling party. Thats according to ANC veteran Khulu Mbatha, author of the critical new book Unmasked: Why the ANC Failed to Govern.
Mbatha was scheduled to discuss his book on Motsweding FM on Thursday night in an interview set up by his publishers, KMM Review Publishing Company. After not receiving a call from the popular radio station, he contacted his publisher.
It was only on Friday morning that Mbatha learned why the interview was canned. He received a text from his publisher that had been sent from a producer at Motsweding FM.
The producer apologised for the lack of communication and explained what happened: I was advised earlier as I submitted my script to put the interview on hold because of the editorial policy of the SABC. I will be informed further on how to treat the interview moving forward because the content is against the ruling party.
Mbatha, once a special adviser to former president Kgalema Motlanthe, was angry when City Press contacted him on Friday morning.
I am very much disturbed. Censorship is still happening at the SABC. It is the worst form of censorship, he said, just before boarding a flight to the Franschhoek Literary Festival, where he will be talking about his book.
In Unmasked, Mbatha who is one of 101 ANC veterans who have called for a conference to deal with the ANCs crisis delivers a sharp critique of the party. The ANC, he writes, was never truly ready to rule in 1994 and has failed dismally to address the core issue of economic inequality in the country.
Kaizer Kganyago, SABC spokesperson, said the interview was postponed because the station wanted to have time to read the book.
An email was sent to the relevant party in this regard, and it is not true that the reason for postponing the interview was due to the contents of the book which are said against the ruling party. The reasons are clear and there is no mention of the ruling party.
On March 8, the council of the Independent Communications Authority of South Africa (Icasa), acting on the recommendation of the Complaints and Compliance Committee, declared the SABC boards 2016 amendment of its editorial policies to be invalid.
This was after the The SOS Coalition and Media Monitoring Africa (MMA), represented by the Legal Resources Centre, challenged amendments to the SABCs editorial policies over, among other things, the broadcasters refusal to show violent protests.
During former chief operating officer Hlaudi Motsoenengs reign at the SABC numerous adverts, commentators and programmes were reportedly canned for presenting views negative to President Jacob Zuma and the ANC.
Icasa found that, in amending its policies, the SABC had failed to consult the public, which is a breach of the SABCs licence conditions and of the Broadcasting Act.
The SABC was forced to revert to its original editorial policies of 2004.
Contacted on Friday, Icasa spokesperson Paseka Maleka said that Mbatha can lodge a formal complaint with Icasa and we shall engage the SABC on the matter.
MMAs William Bird was aghast when told about the SMS.
This is fundamentally outrageous, he said, with all the emphasis on trying to restore credibility at the public broadcaster. Its a flagrant violation of [Mbathas] right to freedom of speech. The board and parliament need to be investigating this as well.
Mbatha said he had contacted Communications Minister Ayanda Dlodlo about what happened with Motsweding FM.
She promised to get back to me, he said.

We Live in a Soft Society Filled With Censorship, Offense, Nanny-State and Weakness – The Whole Story

When you were younger, did you ever see a picture of a gory leg or a kid in the playground covered in blood after an unfortunate accident? If so, did you immediately request counselling and medication for your all new post-traumatic stress disorder?
Im in my late twenties now, but when I was a youngun if somebody from your school died naturally, was killed or even committed suicide; you went home and you let it all out this is what he meant to me or well I didnt know him but my friend Johnny did and he was really sick apparently and youd discuss your feelings with your parents and realise that for lack of a better term life goes on.
Adversity, hardship and misfortune breeds strength and attentiveness.
Nowadays, I see disasters and tragedies happen where the first thing people do is run to the counsellor and act as if what has happened is something the world and all of its living things has never faced before. Do we act as if little Billy has seen the death of a human because thats just the way of the world, or do we act like he has seen what nobody should ever see and that this will be the most traumatic sight in the world?
Think about WordswithMeaning!, now this is a website that, if there are tortures happening in the world, itll be the source for the visually shocking evidence. Today, the media tells you that a whole number of horrible things have happened and we just use our own imagination, when our imagination isnt even close to the terrible things that are going on behind the censorship. Watch one of those surgery programs and see how brutal the responsibility of removing or repairing a damaged organ is and then you might be that little bit stronger when somebody is impaled by a speeding truck that continues after leaving the poor victim to fend for his or herself.
Was there a road accident near the school? Call the counsellors. A flood you say? Call the counsellors. Actually, maybe this is why I am a counsellor.
Disasters, deaths and gore are all inevitable. You cant put a pixelated sheet over everything that your kids are exposed to. Yes it will shock them, it will cause distress but by covering their eyes from the cruel reality of life; all youre doing is shielding them and weakening their ability to understand that this world is full of pretty flowers and nasty things.
I have a very vivid memory of touching the cooker about five minutes after my mum said DONT TOUCH, ITS HOT! My mums response was for goodness sake, see, this is why I told you not to touch it. Go run it under the tap
On that day, I learned that heat = pain. Funny how Im almost thirty and I still remember the effect that had on me and how cautious I am about hot objects nowadays.
I am by no means saying you should go burn your children to teach them of lifes misfortune, but if they hurt themselves, let them know its only natural and that its a salutary reminder that they may not be so lucky the next time; cold running water will heal it now, but if you were to be careless again, you may require some serious medical attention.
Im constantly hearing the sad news that another life has been lost to the realms of cyber bullying or similar and yes, suicide is difficult to tackle, but if you put your 13 year old into the war that is life without some knowledge, experience and a sword then chances are theyll be injured within moments.
Sometimes we all have to learn the hard way in order to prepare ourselves for the worst.

The Real Science of Henrietta Lacks’s Immortal Cells | Inverse – Inverse

By the time Henrietta Lacks died in 1951 at the age of 31, she had already achieved a sort of immortality.
Without her knowledge, her doctor had harvested cells from a tumor on her cervix, where her cancer proliferated, and was attempting to keep them alive outside her body. Normally, human cells do not survive long once they have been severed from the organism they belong to: they will divide no more than about 50 times, then die through a process called apoptosis. Lackss doctor at Johns Hopkins Hospital, George Gey, figured he could cure cancer if only he had a line of cells that could reproduce indefinitely, but all of his cultures failed, until he met Henrietta. Her cells, later labeled HeLa, just kept dividing.
What was special about this woman, who was recently featured in an HBO movie, The Immortal Life of Henrietta Lacks?
The answer has to do with particular mutations in her cells caused by the human papillomavirus that had infected them. HPV inserts its own DNA into that of the host, resulting in a genetic hybrid. Not all HPV infections lead to cancer, and not all cancer has the potential to be an immortal cell line, but Lackss specific mutations had at least two characteristics that made her cervical cells special.
For one, HeLa cells are prolific dividers. Gey was surprised at just how quickly his cultures doubled in number. Even among cancers, these cells were reproductive superstars.
Secondly, they have an enzyme called telomerase that is activated during cell division. Normally, it is the gradual depletion of telomeres a repetitive strand of DNA on the ends of the chromosomes that stops cells from dividing indefinitely. But active telomerase rebuilds telomeres cut during division, allowing for indefinite proliferation.
HeLa cells are not the only immortal cell line from human cells, but they were the first. Today new immortal cell lines can either be discovered by chance, as Lackss were, or produced through genetic engineering.
The cells have been a boon to biomedical science, playing a role in the development of the polio vaccine and thousands of other patented discoveries. HeLa gives researchers a way to conduct repeatable experiments on human cells without testing directly on humans, although the cells are arguably no longer human at all.
Genetically, HeLa cells contain parts of Henrietta Lackss own DNA, mutations introduced by the strain or strains of HPV that infected her, as well as uncounted numbers of new mutations introduced organically through cellular division after the original cells were harvested from her body. A normal human cell has 46 chromosomes a HeLa cells tends to have between 70 and 90.
According to some scientists, the HeLa cell line should properly be considered its own species. It is a sort of single-cellular organism that reproduces asexually through division and evolves through mutations that compound over time. It is a domesticated species, dependent on humans for food and shelter the cow of microbial life, perhaps.
The story of the woman who non-consensually gifted HeLa to the world was largely unknown for decades after her death, even as the cells themselves contributed to significant advances in medicine. That changed thanks to The Immortal Life of Henrietta Lacks, a book that chronicles the efforts of author Rebecca Skloot to find Lackss family and tell her story. The HBO movie, starring Oprah Winfrey, is based on that account.

The ongoing debate about global warming and climate change includes arguments about whether our world actually faces such a problem, and whether it was caused by human activity.But theres another earthling-influenced phenomenon we dont hear too much about: how we affect what happens in space.
Space weather which can include changes in Earths magnetic environment are usually triggered by the suns activity, but recently declassified data on high-altitude nuclear explosion tests have provided a new look at the mechanisms that set off perturbations in that magnetic system.
Earth is surrounded by layers of radiation belts, and as the space agency says in the following video, the Cold War-era experiments of the 1950s and 60s produced extra radiation close to Earth.
While those military tests concluded long ago, the after-effects of such actions, now called human-induced space weather, are the focus of a new study published in Space Science Reviews, which offers research access to millions of scientific documents.
These explosions created artificial radiation belts near Earth that resulted in major damages to several satellites. Another unexpected impact of the high-altitude nuclear tests was the electromagnetic pulse that can have devastating effects over a large geographic area (such as the continental United States).
Other anthropogenic impacts on the space environment include chemical release experiments, high-frequency wave heating of the ionosphere and the interaction of VLF waves with the radiation belts.
Certain types of communications very low frequency, or VLF, radio communications have been found to interact with particles in space, affecting how and where they move. At times, these interactions can create a barrier around Earth against natural high-energy particle radiation in space.
The following video depicts this barrier or bubble around our planet, which, according to NASA, was created by those VLF waves.
A number of experiments and observations have figured out that, under the right conditions, radio communications signals in the VLF frequency range can, in fact, affect the properties of the high-energy radiation environment around the Earth, Phil Ericson, assistant director at the MIT Haystack Observatory in Massachusetts, said in a NASA statement.
The space agency added that while these VLF signals are used to communicate with submarines, they also extend out beyond our atmosphere, shrouding Earth in a VLF bubble that even spacecraft high above Earths surface can see.

Over the last fewmonths that the FCC has been under the leadership of Ajit Pai, efforts to remove net neutrality rules have been moving forward. It started with a controversial vote in Congress back in March, which cancelled previously approved FCC rules on internet privacy.
The FCC is open to comments regarding the NPRM until August 16, when it will then make its final decision. For those who wish to share their thoughts, the docket can be accessed here. Its still not clear, however, if Pai intends to put back the 2015 net neutrality rules that prohibit ISPs from throttling or blocking lawful internet content in favor of prioritized, paid access.
Debate over net neutrality centers on protecting internet freedom.Pai himself called his plan Restoring Internet Freedom, but does removing net neutrality, and changing the Title II classification rules, really protect internet freedom?
Pai thinks so, arguing that [t]he Internet was not broken in 2015 before net neutrality rules were put in place. Instead, the FCC chairman is optimistic. This is the beginning of the process, not the end, Pai said before the 2-1 vote.
Today, President Trumps FCC took the first step to dismantle net neutrality, said Rep. Frank Pallone (D-N.J.). This action will undermine the free and open Internet and hand its control over to a few powerful corporate interests.
In effect, what the FCC wants to do is to remove government regulation of ISPs and to give them total broadband access control. From that standpoint, taking out net neutrality is freedom but not for consumers. That being said, removing the ISPs common carrier or utilities classification would put them under the Federal Trade Commissions (FTC) authority.
The final set of rules will likely be presented this fall after the public comment period has ended.

This technology would take the form of an injectable neural lace composed of a mesh of electrodes that would augment the human brain, adding another layer to the cortex and limbic system that is capable of communicating with a computer (essentially creating cyborgs). This, hypothetically, creates an upgradable, updatable interface that could be applied in countless ways. Some of these include:
Brains and technology both operate using the same vectors: electricity and data. Musks Neural Lace would be a system that provides a way for them to communicate directly with each other. To borrow a simile from Phillip Alvelda, the Neural Engineering System Design (NESD) program manager (another nascent BCI), Todays best brain-computer interface systems are like two supercomputers trying to talk to each other using an old 300-baud modem [] Imagine what will become possible when we upgrade our tools. Applications could stretch from the remote operation of technology to the completely hands free and voiceless operation of computers. Researchers in Korea have already used a BCI to control turtles.
Elon Musks idea could both initiate brain activity and monitor it. The technology does not necessarily have to be a one-way communication stream, it is capable of both sending messages and creating information in the brain. The high-bandwidth interface could allow you to wirelessly transmit information to the cloud, to computers, or even directly to the brains of other people with a similar interface in their head. There is also the possibility of downloading content to augment your consciousness: think Neo learning kung-fu in the Matrix. While initial tests to improve intelligence havent been too successful, if brains and computers speak the same language, then computers can impart information to the brain. The technology is currently being used to allow paralyzed people to communicate, but its uses could extend far beyond that.
As part of this two-way communication stream, robotic arms could communicate fleshy messages by being connected to existing nerve structures. Rather than making the brain learn how to use a new part of the nervous system, robotic limbs could be quickly and easily integrated into the system. This has the potential to revolutionize prosthetic limbs for the disabled, but may also encourage people to rid themselves of their biological arms in favour of mechanical super limbs. Who knows!
As computers and brains would essentially be speaking the same language, emotions could be read as data using electrodes. This would shift technologys perception of humans from basic recognition to complex understanding. Robot helpers would be able to adapt to your emotional state rather than just responding to commands. Photos and videos could also be implanted with emotional metadata, meaning that one could feel what it would be like to be in any given scenario, rather than just trying to imagine it.
One issue with the lifespan of games is repetition; people become accustomed, know what to expect, or are limited by the programmed narrative. A BCI could improve this situation by having games respond to what your brain is feeling, remaining one step ahead and endlessly diverse. This would be most applicable to the horror genre, in which enemies could come at you when and where you least expect it, providing constant shocks, jumps, and thrills. The Black Mirror episode Playtest is an hour long exploration of just how terrifying this could be. Since AI has been shown tobe as creative as a human composer, this reality could be surprisingly close.

Its the time of year again for Google I/O, the companys annual developer conference. On Wednesday, the showcase started with a keynote from CEO Sundar Pichai who reiterated the companys latest approach for all that it does: artificial intelligence (AI). Google does a lot, from its original search engine function, to email, video services, and mobile software and the company wants to seeAI will be at the helm of it all.
Pichar highlighted this AI-first focus, something he mentioned at the I/O last year. Now, Google has given the rest of the world aglimpse of how machine learning will work behind every platform it has.
Of course, theres no amount of AI and machine learning-based tech that can work without a specialized processor to run them. Thats whyGoogle launched the second generation of its tensor processing unit chips, called Cloud TPU. These new TPUs will beaccessible to anyone to use for running and training artificial neural networksthrough Googles cloud computing platform.
Much of Googles research on AI is conducted by DeepMind, a company under its Alphabet group. But with all the advances being made in the discipline, Google still needs a platform to put resources together, including research, tools, and applied AI. Google.ai is precisely that, a way to democratize its AI research.
Image recognition is one of the first beneficiaries of machine learning development. There isnt a shortage of algorithms designed to perform visual recognition tasks. Now, Google wants to take this to awhole new level by bringing its search engine expertise to your camera.
Itll be available initially as part of Google Photos to scrape through your old photos for a bunch of data and Google Assistant, which will serve as your primary way to interact with Lens.
As the future of smart home devices begin to take shape thanks in large part to devices like the Amazon Echo Google doesnt want to get left behind. So, it launched its second iteration of Google Home no longer just a little smart speaker that can play music. Now, it also offers proactive assistance, hands-free calling, and visual responses, among other features.
Further specializing its search engine prowess, Google is bringing its power people in the U.S. lookingfor jobs that will suit them, and helping employersfind the employeesthey need. 46% of U.S. employers say they face talent shortages and have issues filling open job positions, Pichai explained.
