The human roulette of vaccination drives – DTNEXT

First came the good news. The European Union authorised a third vaccine. Then, the bad news. Regulators in country after country suggested restricting it to younger people until more testing was done. The decisions marked the start of a delicate new phase of vaccination drives, one in which a growing menu of coronavirus vaccines was accompanied by contentious debates about who should be given which shots. Those debates are a testament to the worlds good fortune in having several strong vaccines only a year into the pandemic. But every vaccine comes with its own idiosyncrasies, including gaps in clinical trial data. And that has thrown up agonising choices for countries already struggling to administer shots, forcing health officials to weigh their qualms about certain vaccines with the need to inoculate people before dangerous variants take hold.
After the EU authorised the AstraZeneca-Oxford vaccine last week, adding a third shot to the blocs arsenal, Germany, Italy, France, Belgium, Lithuania, Poland, Austria and Sweden all said they would restrict it to younger people or were considering doing so, citing a scarcity of data on the vaccines efficacy in older people.
Under those plans, older people would instead be scheduled to receive the Pfizer-BioNTech or Moderna vaccines, potentially leaving them unvaccinated for a period even as younger people are inoculated with the AstraZeneca shot. But those strategies, tangled as they may be, will at least make a new vaccine available to younger people, scientists said, a scenario far preferable to no one receiving AstraZenecas shot. Some scientists are now urging the United States to adopt the same approach, as the country remains without an alternative to the hard-to-store Pfizer and Moderna vaccines. Its regulators refuse to authorise AstraZenecas shot until another clinical trial generates more data, including on how it works in older people. In the absence of that data, targeting the vaccine to those in whom it is known to be effective was an urgently needed stopgap, scientists said all the more so now that the virus is rapidly acquiring new and dangerous mutations.
This is a pragmatic solution to a desperate situation, said Dr. Peter Hotez, a vaccine scientist at the National School of Tropical Medicine at Baylor College of Medicine in Texas. Everythings changed. The whole Biden plan made perfect sense up until about three to four weeks ago when we realised the pace of the variants emerging, and therefore you have to adjust. When the British scientists behind the AstraZeneca vaccine planned large-scale clinical trials in Britain and Brazil last year, they played it safe: They chose not to vaccinate older participants until they knew the vaccine was safe in younger ones, a decision that led to fewer older people being inoculated over the course of the trials. Britain, India and other countries authorised the vaccine for all adults anyway, relying on evidence that older people generate significant immune responses to the vaccine, an indicator that it will offer at least some protection. But European Union health officials have been more cautious, hewing closely to the clinical trial findings as they try to ensure that no one outside the most closely-studied groups is vaccinated. Yet, some scientists said that reserving the AstraZeneca shot for younger people would only delay injections for the people most in need of protection.
This complicates the problem, Walter Ricciardi, a professor of public health in Italy and an adviser to the Health Ministry, said of the plans.
What we need at the moment is to protect the most vulnerable people, which for sure is not the people younger than 55 years old. Confusion bubbled up in Italy after regulators there said AstraZenecas vaccine should, with some exceptions, preferably be used on adults under 55. Pfizer and Modernas shots, both mRNA vaccines that have shown roughly 95 percent efficacy in preventing Covid-19, should be given to the most vulnerable people, they said. AstraZenecas vaccine had 62 percent efficacy at two full doses in clinical trials, but it protected all participants against severe illness or death. In Italy, rumours quickly began to spread about who would receive which vaccine.
News reports suggested the AstraZeneca shot would be reserved for younger essential workers, like soldiers, teachers and janitors. But Italians spotted a hole in the plan: Some hundreds of thousands of public workers are too old to be allowed an AstraZeneca shot, but too young to qualify yet for an mRNA vaccine.
Age limits have also thrown vaccine plans into flux in Germany, where an immunisation committee authorised the AstraZeneca vaccine only for adults under 65. Given the limited supplies in Germany, those shots are likely to be reserved for younger medical workers and nursing home aides. Still, some people resisted taking AstraZenecas shot, rather than Pfizers a sign of the way people may grow choosier as more vaccines are authorised. And beyond the question of effectiveness was the matter of where the vaccines were made.
In England, the AstraZeneca shot has become known to many residents simply as the English one, making it all the more attractive. In Germany, the Pfizer shot, developed by scientists from the western city of Mainz, is spoken of with pride as the German one. Scientists have advised people to accept the first vaccine they are offered, given the widespread protections against severe disease and the societal need to tamp down the emergence of new variants. So far, countries have largely tailored vaccine offerings based on where the shots can be stored and transported. The mRNA vaccines must be kept at very cold temperatures, making it difficult to reach older people in rural areas.
The AstraZeneca vaccine, which can be stored in normal refrigerators, would be a boon to older, harder-to-reach residents. But limiting it to younger people would undo those advantages.
Britain, for its part, has achieved one of the worlds fastest vaccination programs in part by using both the AstraZeneca and Pfizer vaccines across all adults. But affections for the homegrown AstraZeneca vaccine remain strong, especially after the company published data on Tuesday suggesting that it could reduce transmission of the virus.

In early January, President Cyril Ramaphosa announced that South Africas land borders would be closed until at least mid-February due to the Covid-19 crisis. This includes the Beitbridge crossing between South Africa and Zimbabwe, a border that is exceptionally dangerous thanks to what some risk their lives trying to smuggle through it explosives that could ignite at any moment.
Mercy Rifundo, a mother of four from Zimbabwe, farmed beans and occasionally travelled to Johannesburg, where she would stay, to sell her produce.
On 17 August last year, she planned to travel into South Africa, this time apparently under the impression she was also transporting floor polish in her luggage on behalf of people who knew her landlord in Johannesburg.
When she got to the Beitbridge crossing a boy helped her carry her bags across the border. Rifundo then made her way to a taxi rank and got into a vehicle bound for Johannesburg.
Unknown to those around her, had her luggage been jostled a certain way or had the contents rubbed together in a particular manner, several people could have been instantly killed and others maimed.
This is because Rifundo had 496 blasting cartridge units in her luggage. Just one of these is enough to tear open a safe or kill someone.
The type of explosives discovered in her bags are often used in crimes, including ATM blasts, cash-in-transit heists and illegal mining, and could be from operations illicit or legal linked to mines in Zimbabwe.
Rifundo later claimed to be an unwitting explosives mule, but the severity of the crime she was part of was so extreme that the 42-year-old first-time offender is now serving a 15-year jail term in South Africa. This sentence was handed down earlier this month in the Musina Regional Court.
The case against her has emphasised the extreme risks that explosives smugglers who often carry dozens, even hundreds, of blasting cartridges at a time pose to those around them.
Christo Peltz, the safety and risk manager at AECI Mining Explosives, explained that, although there are different grades of explosives, a single blasting cartridge could contain 200 grams of explosive and easily kill someone if it was kept close to their body, and 50 could destroy a vehicle; 500 could wipe out the occupants of a taxi and have a lethal blast radius of 20m.
Limpopos acting Hawks head, Angie Matlabe, referring to the Rifundo case, said: We are determined to fight all types of organised crime in our country, including transnational crimes.
National police spokesperson Vish Naidoo said officers tried to monitor South Africas points of entry and exit. We are responsible for the control of the illegal movement of people and goods at all identified ports of entry (sea, land and air), simply meaning conducting classical policing in a border control environment, he said.
Police worked together with other units including the Hawks, and state agencies including the SA Revenue Service.
Earlier this week Daily Maverick published an Institute for Security Studies article, by Richard Chelin and Willem Els, that warned: The unsafe transportation, storage and use of explosives by criminals is a disaster waiting to happen.
It also said that the police, the lead regulator in terms of explosives in South Africa, were making use of an outdated Act and the police minister should sign and promote the new explosives regulations.
Although the Explosives Act was approved by the president in 2003, it is yet to come into operation. This presents a major problem for law enforcement.
Yet the police have been pushing ahead with arrests.
Every year, several explosives smugglers are intercepted at or near the Beitbridge crossing. During Rifundos trial it was heard that 11 explosives cases were reported in Musina in 2020, 12 in 2019 and 15 in 2018.
The state believed court sentences handed down to smugglers were acting as deterrents to potential illegal explosives transporters, hence the number of related cases had decreased. It is not clear, however, whether Covid-19 lockdown restrictions were taken into consideration when assessing these figures.
According to the state, in Rifundos case, she was arrested when police officers from South Africa stopped the vehicle she was travelling in from the Beitbridge border and searched its boot. This was when they discovered the explosives in her luggage.
In February 2019, Nqobizitha Ncube was also sentenced to 15 years in jail for trying to smuggle explosives from Zimbabwe to South Africa. The 30-year-old had been arrested on 26 September 2018 with two others, who went on to be acquitted.
Rifundo was charged under the Explosives Act as well as the Immigration Act she had no passport on her at the time of her arrest.
She initially pleaded guilty to the charges but then, during proceedings to see if the sentence against her should be reduced, she claimed she had not known she was transporting explosives, but thought she was simply carrying floor polish for people in Johannesburg.
In argument that Rifundo deserved a stiff prison sentence, Maria Makushu of the polices bomb disposal unit testified that the crime committed is serious in that the vehicle utilised to transport the explosives was not a vehicle designed for such and that explosives [are] sensitive to heat and friction and could with the mere bump in the road have exploded, according to a document on the court case provided by the Limpopo National Prosecuting Authority.
She testified that the kind of explosives [are] used in South Africa for illegal purpose[s], ATM bombings, cash-in-transit heists, illegal mining, and indicated that a single blasting cartridge can rip open a safe or kill a person.
The State further argued that, with Beitbridge being the second largest port of entry [which] multiple people frequent on a daily basis, also considering the amount of vehicle[s] and infrastructure, it was a serious offence. Taking into consideration that friction or heat can cause a blasting cartridge to ignite on its own, which would cause the rest of the 495 blasting cartridges to also ignite, this case must be considered as extremely serious.
It was effectively found that, in transporting the explosives, Rifundo had risked the property and livelihood of all people that frequented the border. The fact that she had four minor children was not enough to reduce her sentence. Rifundo was sentenced to 15 years direct imprisonment, and was additionally fined R20,000, or six months in jail, for being an illegal immigrant.
The prison sentence imposed on Rifundo, even as a first-time offender, is not unusual.
In February 2019, Nqobizitha Ncube was also sentenced to 15 years in jail for trying to smuggle explosives from Zimbabwe to South Africa. The 30-year-old had been arrested on 26 September 2018 with two others, who went on to be acquitted.
Ncube had pleaded guilty to the charges she faced, relating to explosives worth R200,000 found in her car.
In aggravation of sentence the state prosecutor, Patricia Dorothy Jacobs, emphasised that the offence was serious with 15 years imprisonment sentence for a first offender, said a statement issued by Mashudu Malabi-Dzhangi, the Limpopo National Prosecuting Authoritys spokesperson.
She also pointed [out] that the offence was prevalent in the border post of Limpopo She referred to the evidence of an explosives expert who earlier testified that the kind of explosives were used in commission of ATM bombings and cash-in-transit offences.
Other similar smuggling crimes date back several years. In November 2013, Elita Sibanda was sentenced to eight years in jail for trying to smuggle explosives into SA the previous month. She was 23 years old at the time.
While busy searching the suspects luggage, members discovered 26 plastic cartridges and 100 detonators, a Hawks statement on her conviction said. She indicated that she bought the explosives from a shop in Harare and that she was transporting [them] to South Africa with the intention to sell the devices to illegal miners.

A team of researchers led by Columbia University has developed a unique platform to program a layered crystal, producing imaging capabilities beyond common limits on demand.
The discovery is an important step toward control of nanolight, which is lightthat can access the smallest length scales imaginable. The work also provides insights for the field of optical quantum information processing, which aims to solve difficult problems in computing and communications.
We were able to use ultrafast nano-scale microscopy to discover a new way to control our crystals with light, turning elusive photonic properties on and off at will, said Aaron Sternbach, postdoctoral researcher at Columbia who is lead investigator on the study. The effects are short-lived, only lasting for trillionths of one second, yet we are now able to observe these phenomena clearly.
The research appears Feb. 4 in the journal Science.
Nature sets a limit on how tightly light can be focused. Even in microscopes, two different objects that are closer than this limit would appear to be one. But within a special class of layered crystalline materialsknown as van de Waals crystalsthese rules can, sometimes, be broken. In these special cases, light can be confined without any limit in these materials, making it possible to see even the smallest objects clearly.
In their experiments, the Columbia researchers studied the van der Waals crystal called tungsten diselenide, which is of high interest for its potential integration in electronic and photonic technologies because its unique structure and strong interactions with light.
When the scientists illuminated the crystal with a pulse of light, they were able to change the crystals electronic structure. The new structure, created by the optical-switching event, allowed something very uncommon to occur: Super-fine details, on the nanoscale, could be transported through the crystal and imaged on its surface.
The report demonstrates a new method to control the flow of light of nanolight. Optical manipulation on the nanoscale, or nanophotonics, has become a critical area of interest as researchers seek ways to meet the increasing demand for technologies that go well beyond what is possible with conventional photonics and electronics.
Dmitri Basov, Higgins professor of physics at Columbia University, and senior author on the paper, believes the teams findings will spark new areas of research in quantum matter.
Laser pulses allowed us to create a new electronic state in this prototypical semiconductor, if only for a few pico-seconds, he said. This discovery puts us on track toward optically programmable quantum phases in new materials.

Time is a notoriously hard concept to pin down. The first person Im aware of to take a serious crack at it is Aristotle, who offers up his definition of time in Book IV of his Physics (parts 10 and 11): time is number of movement in respect of the before and after.
This curiously circular definition citing the before and after in a definition of time somewhat evades the issue at hand is a relative one. For Aristotle (who seems to be more interested in playing with the concept of now than time itself anyway), time seems to be a sort of basis for change, although its exact nature is confusing, both for the philosopher himself and anyone unfortunate enough to be attempting to thoroughly digest his work.
Relative time turns out not to be that useful, and one of the great achievements of early modernity was capturing and taming time. The development of regular clocks allows for many things, not least more precise measurements of everything else. Its impossible to imagine, for instance, the grand edifice of Newtonian physics being built on water clocks and sundials.
Thanks to the magic of clocks, these days were used to time as a constant, the now ticking second by second into the future with implacable rhythm. This is very helpful both in being able to understand the immediate universe and to maintain a functioning society. But its also both physically wrong and hideously unnatural.
Einstein dealt with the non-static nature of time in 1905 with On the Electrodynamics of Moving Bodies (tl;dr: holding the speed of light constant means that time must flow differently for observers traveling at different speeds, a fact which has been proved experimentally) and although Im not drunk enough to read about quantum physics Im going to go ahead and assume that quantum theories of time are pretty gnarly too. Humanitys general concept of time only works on a limited, parochial scale.
And honestly, it doesnt work there either. We dont experience time in the way our machines do. It contracts, contorts, extending March 2020 into hideous decades, and turning what ought to be joyous hours into a stumble of drunken seconds. People dont experience time as a regimented flow. Sometimes we pretend to, and every now and then we force ourselves to sync up with it, but formal time is too abstract for us to stay with it for long.
Sports are great examples of our utter inability to mesh perceived time with real time. Since its Super Bowl week, lets take football. An NFL game nominally consists of four 15-minute quarters, but in practice lasts for hours. Why? Because the game clock twists and turns, freezing at some points but not others. The rules of football, with play clocks and timeouts and etc. act as a supplemental set of physics, but its not just the rules which determine how long a game goes.
On average the Super Bowl takes 3 hours, 44 minutes from start to finish. This is significantly longer than NFL average, despite no changes to the rules of the sport, and is entirely a product of capitalisms bizarre intersection with cultural events. Super Bowl ads cost a lot of money, the halftime show and therefore more time needs to be made (made?!) to accommodate both. In a certain sense, then, the societal environs of the game warp time within the game.
The Super Bowl, then operates on about three different layers of time, all distressed in barely-sensical ways. This is fine, because time makes no fucking sense and never will. Things happen, they appear to be irreversible because of ... entropy? ... and we all just hang around and deal with it. What is time? Honestly, I havent the foggiest idea. Scientists are finally figuring out how much dark matter the almost imperceptible material said to tug on everything, yet emit no light really weighs.
The new estimate helps pin down how heavy its particles could be with implications for what the mysterious stuff actually is.
The research sharply narrows the potential mass of dark matter particles, from between an estimated 10^minus 24 electronvolts (eV) and 10^19 Gigaelectron volts (GeV) , to between 10^minus 3 eV and 10^7eV a possible range of masses many trillions of trillions of times smaller than before.
The findings could help dark matter hunters focus their efforts on the indicated range of particle masses or they might reveal a previously unknown force is at work in the universe, said Xavier Calmet, a professor of physics and astronomy at the University of Sussex in the United Kingdom.

Calmet, along with doctoral student Folkert Kuipers, also of the University of Sussex, described their efforts in a new study to be published in the March issue of Physical Letters B.
By some estimates, dark matter makes up about 83% of all the matter in the universe. Its thought only to interact with light and ordinary matter through gravity, which means it can only be seen by the way it curves light rays.
Astronomers found the first hints of dark matter when gazing at a galactic cluster in the 1930s, and theories that galaxies are threaded with and fringed by vast halos of dark matter became mainstream after the 1970s, when astronomers realized galaxies were whirling faster than they otherwise should, given how much visible matter they contained.

Possible candidates for dark matter particles include ghostly, tiny particles known as neutrinos, theoretical dark, cold particles known as axions, and proposed weakly-interacting massive particles, or WIMPs. The new mass bounds could help eliminate some of these candidates, depending on the details of the specific dark matter model, Calmet said.
What scientists do know is that dark matter seems to interact with light and normal matter only through gravity, and not via any of the other fundamental forces; and so the researchers used gravitational theories to arrive at their estimated range for the masses of dark matter particles.
Importantly, they used concepts from theories of quantum gravity, which resulted in a much narrower range than the previous estimates, which used only Einstein's theory of general relativity.
"Our idea was a very simple one," Calmet told Live Science in an email. "It is amazing that people have not thought of this before."
Einstein's theory of general relativity is based on classical physics; it perfectly predicts how gravity works most of the time, but it breaks down in extreme circumstances where quantum mechanical effects become significant, such as at the center of a black hole.
Theories of quantum gravity, on the other hand, try to explain gravity through quantum mechanics, which can already describe the other three known fundamental forces electromagnetic force, the strong force that holds most matter together, and the weak force that causes radioactive decay. None of the quantum gravity theories, however, as yet have strong evidence to support them.
Calmet and Kuipers estimated the lower bound for the mass of a dark matter particle using values from general relativity, and estimated the upper bound from the lifetimes of dark matter particles predicted by quantum gravity theories. The nature of the values from general relativity also defined the nature of the upper bound, so they were able to derive a prediction that was independent of any particular model of quantum gravity, Calmet said.
The study found that while quantum gravitational effects were generally almost insignificant, they became important when a hypothetical dark matter particle took an extremely long time to decay and when the universe was about as old as it is now (roughly 13.8 billion years), he said.
Physicists previously estimated that dark matter particles had to be lighter than the "Planck mass" about 1.2 x 10^19 GeV, at least a 1,000 times heavier than the largest-known particles yet heavier than 10^minus 24 eV to fit with observations of the smallest galaxies known to contain dark matter, he said.
But until now, few studies had attempted to narrow the range, even though great progress had been made in understanding quantum gravity over the last 30 years, he said. "People simply did not look at the effects of quantum gravity on dark matter before."
Calmet said the new bounds for the masses of dark matter particles, could also be used to test whether gravity alone interacts with dark matter, which is widely assumed, or if dark matter is influenced by an unknown force of nature.
"If we found a dark matter particle with a mass outside the range discussed our paper, we would not only have discovered dark matter, but also very strong evidence that there is some new force beyond gravity acting on dark matter," he said.

Nature documentaries like the BBCs Planet Earth, Blue Planet, and most recently, A Perfect Planet, are amazing masterpieces of modern videography, displaying creation in detail and majesty. Every creature soaring through the sky, or streaking through the deep, or thundering over the savannah exhibits power, beauty, and unmistakable purpose. David Attenboroughs grandfatherly narration and Hans Zimmers moving musical scores only add to the childlike awe these films induce.
All of which makes it even more odd when Attenborough declares that all of this glory lacks purpose, or that it arose by chance and natural selection, and that none of it bears witness to any meaning or Mind beyond itself.
A recent article on atheism, also from across the pond, reminded me of this contradiction. In The Guardian, Harriet Sherwood described a new project from the University of Kent that seeks to discover whether disbelieving in God makes people less spiritual overall. According to the projects authors, atheism doesnt necessarily entail unbelief in other supernatural phenomena. Nor do unbelievers lack for a sense of purpose, despite lacking anything to ascribe ultimate meaning to [in] the universe.
In the article, Sherwood profiled several unbelievers, from an agnostic to a free thinker to Positivist pastor and Satanic priest (who makes it clear he doesnt believe in a literal Satan). All of them insist that life can be deeply meaningful and even moral without God.

Moments after men like Wells admit that good and evil are illusions, Lewis said, you will find them exhorting us to work for posterity, to educate, to revolutionise, liquidate, live and die for the good of the human race.
But how do unbelievers, naturalists as Lewis calls them, account for such ideas? Certainly, nature is no help. If thoughts of meaning and morality find their origin in arrangements of atoms in our brains, then they can no more be called true, Lewis observed, than can a vomit or a yawn.
Lewis concludes that when Wells and other unbelievers say we ought to make a better world, they have simply forgotten about their atheism. That is their glory, he concludes. Holding a philosophy which excludes humanity, they yet remain human. At the sight of injustice, they throw all their Naturalism to the winds and speak like men and like men of genius. They know far better than they think they know.
Id love to ask the people behind masterpieces like Planet Earth, or the unbelievers profiled in The Guardian, about this contradiction. Years ago, I had a similar conversation with a woman I was seated beside on an airplane. She had very strong moral opinions about all kinds of things, but scoffed at me, How can you believe in God! I gently asked her why she believed in right and wrong. It was a fun conversation, and it made me realize that it is possible to affirm the human gut-level intuition about beauty and wonder and morality, while questioning where all of those things come from.
And if you havent read Lewis masterful book Miracles, add it to the list. If its been a while, its worth revisiting. Fair warning: unbelievers should beware. As Lewis himself said, A young man who wishes to remain a sound atheist cannot be too careful of his reading.
