NIH researchers tackle thorny side of gene therapy

Pre-clinical studies in mice reveal ways to reduce cancer risk with modified treatment
National Institutes of Health researchers have uncovered a key factor in understanding the elevated cancer risk associated with gene therapy. They conducted research on mice with a rare disease similar to one in humans, hoping their findings may eventually help improve gene therapy for humans. Researchers at the National Human Genome Research Institute (NHGRI), part of NIH, published their research in the Jan. 20, 2015, online issue of the Journal of Clinical Investigation.
"Effective and safe gene therapies have the potential to dramatically reverse diseases that are life-threatening for affected children," said NHGRI Scientific Director Dan Kastner, M.D., Ph.D. "This study is an important step in developing gene therapies that can be safely used to benefit patients."
Toxic side effects actually are rarely observed by researchers who have designed gene therapies using an adeno-associated virus (AAV) as a vector to deliver the corrected gene to a specific point in the cell's DNA. AAVs are small viruses that infect humans but do not cause disease. A vector is a DNA molecule of AAV used as a vehicle to carry corrected genetic material into a cell. AAV viruses are uniquely suited for gene therapy applications.
But one prior study did find an association between AAV and the occurrence of liver cancer. The present research addresses this problem in gene therapy for an inherited disease in children called methylmalonic acidemia, or MMA.
For 10 years, NHGRI researchers have worked toward a gene therapy to treat MMA. The condition affects as many as 1 in 67,000 children born in the United States. Affected children are unable to properly metabolize certain amino acids consumed in their diet, which can damage a number of organs and lead to kidney failure. MMA patients also suffer from severe metabolic instability, failure to thrive, intellectual and physical disabilities, pancreatitis, anemia, seizures, vision loss and strokes. The most common therapy is a restrictive diet, but doctors must resort to dialysis or kidney or liver transplants when the disease progresses.
In prior MMA gene therapy studies, researchers showed that mice bred to develop the condition could be restored to health by AAV gene therapy injection shortly after birth. The mice in the study survived into adulthood and were free from the effects of MMA.
"The corrected gene delivered by AAV is the most effective therapy we have developed so far to treat MMA," said Charles Venditti, M.D., Ph.D., senior author and investigator in NHGRI's Genetic and Molecular Biology Branch. "However, we have identified an important safety parameter related to the AAV gene therapy in our mouse models that is critical to understand before we move to human patient trials."
Now, in a long-term follow-up of the treated mice -- after mice reached about two years of age -- the researchers documented a 50-70 percent higher occurrence of liver cancer in AAV-treated mice compared with a 10 percent liver cancer rate in untreated mice. Dr. Venditti's team determined that the AAV vector triggered the cancer.
The research team performed additional experiments to detect where in the mouse genome the AAV vector delivered the corrected gene and how that related to any cancer development. In many mice that developed liver cancer, the AAV vector targeted a region of the mouse genome called Rian, near a gene called Mir341 that codes for a microRNA molecule. MicroRNAs are small, non-coding RNA molecules involved in the regulation of gene expression. When the AAV was inserted near Mir341, the vector caused elevated expression of the gene, which the researchers believe contributed to the occurrence of liver cancer in the mice. The authors note that Mir341 is found in the mouse genome, however, it is not present in humans.

European Commission approves Novartis' psoriasis drug Cosentyx

The European Commission (EC) has approved Swiss drug-maker Novartis' Cosentyx (secukinumab, formerly known as AIN457) to treat moderate-to-severe plaque psoriasis in adults who are candidates for systemic therapy.
Cosentyx 300mg is the first and only interleukin-17A (IL-17A) inhibitor to be approved in Europe providing a new first-line biologic treatment option for these patients.
At present, all biologic treatments for psoriasis, including anti-tumor necrosis factor therapies (anti-TNFs) and Stelara (ustekinumab) are recommended for second-line systemic therapy in Europe.
Novartis Pharmaceuticals division head David Epstein said: "With this groundbreaking news from the European Commission, clear skin may now be a reality for patients living with psoriasis.
"Nearly half of psoriasis patients are not content with current therapies, including biologic treatments, showing a significant unmet need for patients.
"Cosentyx, with a first-line systemic indication for treatment of psoriasis will provide patients a better chance of achieving clear or almost clear skin."
In Phase III clinical trials, 70% or more patients treated with Cosentyx 300mg achieved clear skin (PASI 100) or almost clear skin (PASI 90), during the first 16 weeks of treatment.
The European approval follows the recent results of the Phase IIIb CLEAR trial, which showed that Cosentyx was superior to Stelara in clearing skin of patients living with moderate-to-severe plaque psoriasis.
The company said that Cosentyx also showed superiority to Enbrel (etanercept) in clearing skin in the FIXTURE study.

HANOVER, N.H. - Recent research has linked autism with a lack of "pruning" in developing brain connections, but a new Dartmouth study suggests instead it is the excessive growth of new connections that causes sensory overload in people with the disorder.
The results, which have broad implications for understanding the neurobiological basis of autism spectrum disorders, appear in The Journal of Neuroscience. A PDF of the study is available on request.
"We've been working on understanding how dysfunction of the gene Pten, which is known to cause some cases of autism, effects neuronal development, and I believe our findings represent the best understanding in science today for how an autism candidate gene changes the functional characteristics of developing neurons," says senior author Bryan Luikart, an assistant professor of Physiology and Neurobiology in the Geisel School of Medicine at Dartmouth.
Mutations in the gene Pten are among the most common single-gene mutations that cause autism and a group of interrelated syndromes. People with these diseases have increased chances of having autism, intellectual disability and epilepsy. Luikart's team is investigating the neurobiological basis of the complex symptoms of autism by modeling genetic changes associated with autism in humans in neurons of mice. For their new study, the researchers generated a model in which they injected retroviruses into the brains of developing mice to both knockout the Pten gene and to label the knockout neurons with a fluorescent marker. This allowed them to study how turning off the gene alters the structural and electrical development of the neurons. They found that knocking out the Pten gene caused overt overgrowth of the neurons, which resulted in an increase in the number of excitatory synapses, or the connections that transmit signals from a nerve cell to another cell. The ultimate result of this is that the neurons become hyperactive.
Recent media coverage has surrounded the idea that autism is associated with a lack of "pruning" or refinement of excitatory synapses later in development. But the Dartmouth study argues against this, saying it is not a failure of "pruning" that results in the ultimate increase in excitatory synapses, but an increase in new production of excitatory synapses. Further, they found a tight interrelationship between the structural and functional changes produced by the Pten gene knockout.
"The broader implication for this is that mutations in the gene Pten in humans likely result in an increased developmental proliferation of excitatory synaptic connections," Luikart says. "This may result in a given sensory experience stimulating neurons or even whole brain regions that would never be excited in a normal brain. Conceptually, this could be the neurobiological basis for the inappropriate responses to sensory stimulation that is often characteristic of patients with autism."

THE resignation of the chair person and some other members of the Central Board of Film Certification in India raises a controversy. In India, irrespective of whichever government is in office, adults are not allowed to have even the freedom of viewing scenes which may have political, moral, sexual and other kinds of physical content which officials may think corrupt their minds. The attitude betrays authoritarian and narrow- minded guarding of received notions and values. Kissing and intimacy are now being shown in Indian films, though in small doses. However , warnings against smoking and drinking are displayed whenever such acts are shown on screen, along with the pointless statement that the actors do not indulge in either vice. What a pity if that goes with a film like Devdas or I will cry tomorrow! Woody Allen did not have one of his recent films shown In India for that reason. Indian DVDs of international film classics are also sometimes subjected to cuts, for instance, of Fellinis La Dolce Vita. There was a joke that during the prime ministership of Morarji Desai, even Hamlet was banned because Claudius was shown drinking out of a goblet. Freedom of expression in cinema is sometimes bridled to protect obscurantism as was done in the case of Water for some time and when UP chief minister Mayavati wanted to clamp down on Samrakhshan on a mistaken notion about the content. Indian censorship should at least approximate to what is done in the advanced countries. Censorship should be done by an independent body. There should be no political or sectarian interference. But, of course, liberty should not degenerate into licence. Self-censorship should be ideal but based on intelligent understanding of creative imagination. It is hard to understand why some scenes of Titanic which won 11 academy awards were take out in India while a film like Dirty Picture could be shown without cuts.

Microsoft's Outlook email service in China was hacked by the country's censorship authority, an internet watchdog has claimed, as the government continues its crackdown on U.S. technology giants. Greatfire.org, which monitors censorship in China, said that Microsoft's servers were subject to a so-called man-in-the-middle (MITM) attack, enabling hackers to intercept private emails between users. A Microsoft spokesperson told CNBC they were aware of "a small number of customers impacted by malicious routing to a server impersonating Outlook.com." It is hard for a victim to detect this type of attack because the man-in-the-middle is often just eavesdropping and mining the data being sent. In this instance, hackers targeted the technology which communicates with Microsoft's servers and enables users to access emails on their smartphones. "We suspect that the Cyberspace Administration of China, which is directly in charge of censorship is directly responsible for the MITM attack against Outlook, and the recent related MITM attacks in China," Greatfire's report said. The attack follows a complete block of Google's Gmail in China and highlights the way in which Chinese authorities are allegedly tightly controlling communication in the world's second-largest economy.
