DNA Test Can Quickly Diagnose TB

ST. LOUIS, MISSOURI

Researchers have developed a new way of diagnosing tuberculosis. The new DNA-based test has advantages over 19th century diagnostic methods that are still in use.

Tuberculosis is an infectious disease that is especially common in low- and middle-income countries. The World Health Organization says it kills more than a million people a year.

The diagnosis of tuberculosis hasnt changed much since the 1880s. A sputum specimen coughed up by the patient is examined under a microscope, and suspected TB bacteria is grown in the laboratory. Its a time-consuming process, taking several weeks or more.

Now, Professor Mark Pallen from Britains University of Warwick describes a new process that can shorten the time dramatically by identifying strands of tuberculosis DNA in a sputum sample.

Were talking about three days to go from getting the sputum sample, getting the DNA out of it, and then getting the sequence sorted. Thats the kind of measure that were looking at, said Pallen.

And that speed is important because the sooner you have a diagnosis, the sooner you can start treatment.

The DNA-based diagnosis also can identify which particular strain of tuberculosis the patient has, including ones that have developed resistance to anti-TB drugs.

And knowing that you have drug resistance as early as possible helps you to treat the patient effectively. And the other thing is the treatment, once its started, fairly quickly if its appropriate treatment, will actually stop the patient being infectious, and so you can prevent forward transmission of the infection from that patient," he said.

The DNA sequencing required is still fairly expensive, but Pallen said costs rapidly are coming down.

Celgene's Otezla Label Expanded to Include Plaque Psoriasis – Analyst Blog

Celgene Corporation ( CELG ) announced a label expansion for its drug Otezla in the U.S. Otezla, already approved by the FDA for treating adults suffering from active psoriatic arthritis, can now be used in treatment-nave as well treatment experienced patients suffering from moderate-to-severe plaque psoriasis. The FDA approved the drug, an oral and selective inhibitor of phosphodiesterase 4 (PDE4), to treat those patients for whom phototherapy or systemic therapy is suitable.

Celgene said that following the FDA nod, Otezla becomes the first inhibitor in the above category to gain approval in the U.S. for the plaque psoriasis indication. Currently, psoriasis patients are mostly treated with injectable drugs like AbbVie's ( ABBV ) Humira and Amgen's ( AMGN ) Enbrel.

The FDA cleared Otezla for the new indication primarily on the basis of encouraging data from two multi-center, randomized, double-blind, placebo-controlled studies - ESTEEM 1 and ESTEEM 2. The U.S. regulatory body, however, recommended that patients taking Otezla should have their body weights monitored on a regular basis. Moreover, patients with a history of depression or suicidal behavior should inform the same to their doctors before commencing Otezla therapy.

We remind investors that Otezla gained FDA approval for the psoriatic arthritis indication in March this year (read more: Update on Celgene's Product Portfolio ). The drug contributed $5 million to Celgene's top line in the second quarter of 2014. Approval for the additional indication, which offers significant commercial potential, will boost Otezla's sales going forward. According to the National Psoriasis Foundation, the immune-mediated, non-contagious chronic inflammatory skin disorder affects 7.5 million Americans.

Celgene is seeking EU approval for Otezla in the combined psoriasis and psoriatic arthritis indication, with an opinion from the European Medicines Agency's Committee for Medicinal Products for Human Use expected by Dec 31, 2014. The candidate is further being evaluated for treating patients suffering from Behet's disease (phase III expected to commence by year end), ulcerative colitis (phase II b expected to commence by year end) and atopic dermatitis (phase II).

Celgene carries a Zacks Rank #3 (Hold). Better-ranked stocks in the health care space include Gilead Sciences ( GILD ) and Medivation ( MDVN ). Both the stocks sport a Zacks Rank #1 (Strong Buy).

Loss of p53 function compromises genetic homeostasis, which induces deregulated DNA replication, damages DNA, and subsequently results in increased resistance to anticancer agents. Pharmacological approaches using recombinant adenoviruses (Ad) have been developed to restore the p53 functions. Another approach for gene medicine is to modify Ad replication in a tumor-specific manner, which induces tumor cell death without damaging normal tissues in the vicinity. The Ad-derived gene medicines, Ad expressing the wild-type p53 gene and replication-competent Ad defective of the E1B-55kDa gene, have been tested for their clinical feasibility and became commercially available in China. These agents demonstrated their antitumor activities as a monotherapy and in combination with conventional chemotherapeutic agents. In this article, we summarize the outcomes of clinical trials in China, most of which have been published in domestic Chinese journals, and discuss potential directions of cancer gene therapy with these agents.

Cancer is currently one of the main causes of death in Western and Asian societies. Treatments for advanced-stage cancer are often difficult with a limited efficacy despite multimodal therapeutic strategies. Novel approaches are required to improve the prognosis. A molecular targeting with small-sized synthetic chemicals in a certain type of cancer is currently available and furthermore many candidate molecules are under investigation. Gene therapy is also a possible treatment modality and over the past decades has been investigated preclinically and clinically for their feasibility. According to data published in the Journal of Gene Medicine, there have been 1309 approved clinical trials in the world until July 2007 and 66.5% of the trials targeted cancer. Historically, a number of clinical trials have been conducted in the Western world but none of the gene medicines have yet been approved by governmental authorities for commercialization. In contrast, adenoviruses (Ad) expressing the human wild-type p53 gene and Ad defective of the E1B-55 kDa molecule, both of which were originally developed and clinically examined for the efficacy in USA, have been admitted in China. The Ad agents have become the first commercially available gene medicine in the world.
