Near-drowning of astronaut tied to wrong diagnosis, slow response

The near drowning of a space-station astronaut from water that had collected in his helmet during a spacewalk stemmed from acceptance of unusual conditions known to increase risks.

Willingness to accept as routine minor amounts of water in a space-walking astronaut's helmet and a misdiagnosis of a previous water leak helped set the stage for an incident last summer that could have cost an International Space Station crew member his life, according to an analysis of the event.

In a 122-page report released Wednesday, a mishap investigation board identified a range of causes for the near-tragedy, including organizational causes that carried echoes of accident reports that followed the loss of the shuttles Challenger and Columbia and their crews in 1986 and 2003.

About 44 minutes into a 6.5-hour spacewalk last July, Italian astronaut Luca Parmitano noted that water was building up inside his helmet the second consecutive spacewalk during which he reported the problem. Twenty-three minutes later, he and partner Chris Cassidy were ordered to end the spacewalk.

"The good news was that Luca was very close to the air lock when this happened," said Chris Hansen, space-station chiefengineer and head of the board,during a briefing Wednesday that outlined the findings. "When we terminated the EVA, Luca had a pretty close path to the air lock."

Still, as Parmitano worked his way back to the air lock, water covered his eyes, filled his ears, disrupted communications, and eventually began to enter his nose, making it difficult for him to breathe. Later, when crew mates removed his helmet, they found that it contained at least 1.5 quarts of water.

NASA officials immediately set up the five-member mishap investigation board to uncover the broader causes behind the incident, even as a team of engineers at the Johnson Space Center worked to find the precise mechanical cause for the buildup of water.

Engineers traced the leak to a fan-and-pump assembly that is part of a system that extracts moisture from the air inside the suit and returns it to the suit's water-based cooling system. Contaminants clogged holes that would have carried the water to the cooling system after it was extracted from the air. The water backed up and flowed into the suit's air-circulation system, which sent it into Parmitano's helmet. The specific cause of the contamination is still under investigation.

This image shows the Large Magellanic Cloud galaxy in infrared light as seen by the Herschel Space Observatory, a European Space Agency-led mission with important NASA contributions, and NASA's Spitzer Space Telescope. In the instruments' combined data, this nearby dwarf galaxy looks like a fiery, circular explosion. Rather than fire, however, those ribbons are actually giant ripples of dust spanning tens or hundreds of light-years. Significant fields of star formation are noticeable in the center, just left of center and at right. The brightest center-left region is called 30 Doradus, or the Tarantula Nebula, for its appearance in visible light.

This enhanced-color image shows sand dunes trapped in an impact crater in Noachis Terra, Mars. Dunes and sand ripples of various shapes and sizes display the natural beauty created by physical processes. The area covered in the image is about six-tenths of a mile (1 kilometer) across. Sand dunes are among the most widespread wind-formed features on Mars. Their distribution and shapes are affected by changes in wind direction and wind strength. Patterns of dune erosion and deposition provide insight into the sedimentary history of the surrounding terrain.

This image obtained by the framing camera on NASA's Dawn spacecraft shows the south pole of the giant asteroid Vesta. Scientists are discussing whether the circular structure that covers most of this image originated by a collision with another asteroid, or by internal processes early in the asteroid's history. Images in higher resolution from Dawn's lowered orbit might help answer that question. The image was recorded with the framing camera aboard NASA's Dawn spacecraft from a distance of about 1,700 miles (2,700 kilometers). The image resolution is about 260 meters per pixel.

This undated photo shows a classic type 1a supernova remnant. Researchers Saul Perlmutter and Adam Riess of the United States and US-Australian Brian Schmidt won the 2011 Nobel Physics Prize on October 4, 2011 for their research on supernovae.

A quartet of Saturn's moons, from tiny to huge, surround and are embedded within the planet's rings in this Cassini composition. Saturn's largest moon, Titan, is in the background of the image, and the moon's north polar hood is clearly visible. See PIA08137 to learn more about that feature on Titan (3,200 miles, or 5,150 kilometers across). Next, the wispy terrain on the trailing hemisphere of Dione (698 miles, or 1,123 kilometers across) can be seen on that moon which appears just above the rings at the center of the image. See PIA10560 and PIA06163 to learn more about Dione's wisps. Saturn's small moon Pandora (50 miles, or 81 kilometers across) orbits beyond the rings on the right of the image. Finally, Pan (17 miles, or 28 kilometers across) can be seen in the Encke Gap of the A ring on the left of the image. The image was taken in visible blue light with the Cassini spacecraft narrow-angle camera on Sept. 17, 2011. The view was obtained at a distance of approximately 1.3 million miles (2.1 million kilometers) from Dione and at a Sun-Dione-spacecraft, or phase, angle of 27 degrees. Image scale is 8 miles (13 kilometers) per pixel on Dione.

Combining almost opposite ends of the electromagnetic spectrum, this composite image of the Herschel in far-infrared and XMM-Newton's X-ray images obtained January 20, 2012, shows how the hot young stars detected by the X-ray observations are sculpting and interacting with the surrounding ultra-cool gas and dust, which, at only a few degrees above absolute zero, is the critical material for star formation itself. Infrared light that we can't see with our eyes has been color-coded, such that the shortest wavelengths are shown in blue and the longest in red. The middle wavelength range is green. Massive stars have blown bubbles, or cavities, in the dust and gas--a violent process that triggers both the death and birth of stars. The brightest, yellow-white regions are warm centers of star formation. Last year, European Space Agency (ESA) astronaut Luca Parmitano nearly drowned in space due to a leaky helmet, and yesterday NASA said this isnt the first time this suit model has misbehaved.

On July 16, 2013 two International Space Station crew members exited the US Airlock to begin a spacewalk. About 44 minutes into the spacewalk Parmitano reported water inside his helmet on the back of his head. Slowly the water began to fill up his helmet, and he was just moments away from actually drowning inside his own helmet while in space.

After this incident took place, NASA launched an investigation to help identify factors that may have contributed to the incident and it recommended changes that could be implemented to prevent it from happening again.

The investigation found that the same suit failure occurred a week earlier, and if this event was properly investigated it could have prevented Luca from being placed at risk the following week.

In summary, the causes for this mishap evolved from inorganic materials causing blockage of the drum holes in the EMU water separator resulting in water spilling into the vent loop; the NASA teams lack of knowledge regarding this particular failure mode; and misdiagnosis of this suit failure when it initially occurred on EVA 22, NASA said in its report.

The space agency said the inorganic materials blocking the water separator drum holes had not been seen during a spacewalk before and is still undergoing investigation. It said the investigation does not have the required data yet to determine the root causes of the contamination source.

Essentially, when the helmet first messed up the team misdiagnosed the problem, allowing Luca to be put in a position where he used the helmet. NASA said that had the issue been discussed in more detail at the end of the spacewalk, the team would have likely realized that the water experienced in the helmet needed to be investigated further before pressing ahead.

Last Shuttle Commander Virtually Flies Boeing CST-100 to Space Station

HOUSTON, Feb. 27, 2014-- Chris Ferguson, Boeing's director of Crew and Mission Operations and commander of the final Space Shuttle flight, virtually returned to space recently in the Boeing [NYSE: BA] Crew Space Transportation (CST)-100 simulator to satisfy a NASA testing requirement for the spacecraft.

Ferguson performed manual piloting activities including on-orbit attitude and translation maneuvers, docking and backing away from a virtual International Space Station and a manual re-entry to Earth in the simulator.

"It was great to be back in the pilots seat, even if I didn't leave the ground," Ferguson said. "It's important for the spacecraft to have manual controls because although it's designed to be largely autonomous, the pilot should always be able to back up that autonomy. Manual flight controls provide a sort of a belt-and-suspenders capability for piloting the spacecraft."

The testing for NASA officials satisfied a CST-100 development milestone known as "Pilot in the Loop." It is the final milestone before the spacecraft's critical design review.

Ferguson, a veteran of three shuttle missions and commander of STS-135, the final shuttle flight, has logged more than 40 days in space and 5,700 hours in high-performance aircraft. He now oversees the crew interface of the Boeing CST-100 spacecraft and plays a key role in development and testing of system concepts and technologies for the vehicle and integrated launch and ground systems.

"This was the one opportunity to really show off, from a user's perspective, just how real our vehicle is becoming," said Ferguson. "We demonstrated that the CST-100 is on track to return Americans to space in an American spacecraft."

The Boeing-developed simulator will be used for astronaut training as part of a full suite ot training devices for crew members and mission controllers.

Kelowna scientist joins NASA funded simulated mission to Mars on Hawaiian mountain

KELOWNA - A Kelowna scientist is joining the crew of a NASA-funded simulated mission to Mars after beating out hundreds of applicants from around the world.

Ross Lockwood and five others will spend four months inside a sealed environment high on the slopes of a Hawaiian mountain.

The main purpose of the mission, starting March 28, is to help the space agency develop psychological guidelines that will be used to select future astronauts capable of making a real trip to Mars.

``It's incredibly exciting to participate in a research project that will be used to help in space exploration,'' Lockwood said.

``I'm really looking forward to this, but I'm also a little bit nervous as well.''

Lockwood, 27, is doing his doctorate in condensed matter physics at the University of Alberta.

Along with his longtime interest in space-related research and astronomy, Lockwood has worked in educational programs at the University of Alberta Observatory.

His scientific background and experience in various university leadership roles helped him succeed in his application to participate in the simulated Mars mission, which is led by Cornell University and the University of Hawaii.

The researchers will enter the 11-metre diameter dome built in an old rock quarry at an elevation of 2,500 metres on the slopes of Mauna Koa.

Inside the habitat, they will spend several hours a day taking psychoogical tests and continuing with their own research endeavours as part of the venture dubbed HI-SEAS, for Hawaii Space Exploration and Analog Simulation.

Opinion: Three-Parent Babies Are an Ethical Choice

Perhaps the biggest fear raised by advances in genetic engineering like the ability to create a baby with three parents is that we are heading straight down a road that leads to eugenics the attempt to create perfect "designer" babies.

From "Star Trek" to "Star Wars" to many more films like "Gattaca" and "The Boys from Brazil," Hollywood has warned us time and again that genetic engineering of animals or people is nothing but bad news.

History sadly also shows the horror that eugenics programs have produced in Germany, Japan and many other nations when some people are deemed more desirable and others are seen as defective, burdensome or subhuman. Death camps, forced sterilization, restrictions on marriage, exile, concentration camps, prohibitions on immigration all have all been carried out in the name of eugenics.

But Hollywood is wrong. Not everything about genetic engineering is morally horrible. Nor is human genetic engineering always motivated by eugenic goals.

The FDA is considering approving an experiment to repair a genetic disease in humans by creating embryos with DNA from three parents. Genes would be transferred from a healthy human egg to one that has a disease and the "repaired" egg then fertilized in the hope that a healthy baby will result. The goal of the experiment in genetic engineering is not a perfect baby but a healthy baby.

One in 4,000 children inherit terrible diseases due to genetic mutations in their mitochondria. The mitochondria are the batteries of cells. They provide the energy that lets cells divide and grow. All mitochondria are inherited from the mother from her eggs. And, the mitochondria are separate, tiny units in the egg meaning you can pick them out and transplant them.

So if you know that a mitochondrial disease runs in your family, you might want to try getting pregnant using one of your eggs that has been genetically engineered to have mitochondria from a donor egg.

None of this should raise concerns about abortion. Only eggs, not embryos, would be involved. There are, however, safety concerns. We won't really know if mitochondrial transplants work until healthy young people exist who have been made using the technique. Studies in primates look promising but safety in humans is still a risk.

While giving his nod, Moily, however, sought to downplay the approval row, saying there was some "misunderstanding" over the issue and there was no "embargo" from Supreme Court on such trials. The minister's nod will pave the way for GM field trails for rice, wheat, maize and cotton.

Moily, however, made it clear that the trials are subject to approval by state governments and fulfilment of certain conditions.

The clearance will enable companies such as Bayer Bioscience, Mahyco and BASF and Monsanto India to carry out field trials for the crops. Bayer Bioscience, for example, has been given clearance to test GM rice across the country.

Earlier, Natarajan had blocked the approval obtained by GEAC in March of last year, reportedly because the issue was pending in the apex court.

But Moily said he had examined the case with officials and found that there was no embargo on the clearance by Supreme Court, Moily said.

"It is a report from a statutory committee. I don't think a ministry or a minister will have any business holding it back. We can't do that. We have to work with the rules... and the law of the land is same for everyone. That is why, as a law-abiding minister, I approved it," Moily told reporters here.

The minister also said that the Environment Ministry, Agriculture Ministry and the Department of Biotechnology had a "common position" on the issue, which would be placed before Supreme Court. Natarajan had, in fact, not "disagreed" with the decision of the GEAC, he further stated.

The next meeting of GEAC has been called on March 21 to discuss some more issues, he said.
