Newswise PHILADELPHIA A new MRI method to map creatine at higher resolutions in the heart may help clinicians and scientists find abnormalities and disorders earlier than traditional diagnostic methods, researchers at the Perelman School of Medicine at the University of Pennsylvania suggest in a new study published online today in Nature Medicine. The preclinical findings show an advantage over less sensitive tests and point to a safer and more cost-effective approach than those with radioactive or contrasting agents.
Creatine is a naturally occurring metabolite that helps supply energy to all cells through creatine kinase reaction, including those involved in contraction of the heart. When heart tissue becomes damaged from a loss of blood supply, even in the very early stages, creatine levels drop. Researchers exploited this process in a large animal model with a method known as CEST, or chemical exchange saturation transfer, which measures specific molecules in the body, to track the creatine on a regional basis.
The team, led by Ravinder Reddy, PhD, professor of Radiology and director of the Center for Magnetic Resonance and Optical Imaging at Penn Medicine, found that imaging creatine through CEST MRI provides higher resolution compared to standard magnetic resonance spectroscopy (MRS), a commonly used technique for measuring creatine. However, its poor resolution makes it difficult to determine exactly which areas of the heart have been compromised.
Measuring creatine with CEST is a promising technique that has the potential to improve clinical decision making while treating patients with heart disorders and even other diseases, as well as spotting problems sooner, said Reddy. Beyond the sensitivity benefits and its advantage over MRS, CEST doesnt require radioactive or contrast agents used in MRI, which can have adverse effects on patients, particularly those with kidney disease, and add to costs.
Today, magnetic resonance imaging (MRI)-based stress tests are also used to identify dead heart tissuewhich is the warning sign of future problems (coronary artery disease, for instance)but its reach is limited. MRI is often coupled with contrast agents to help light up problem areas, but it is often not sensitive enough to find ischemic (but not yet infarcted) regions with deranged metabolism, said Reddy.
After a heart attack, different regions of the heart are damaged at different rates. This new technique will allow us to very precisely study regional changes that occur in the heart after heart attacks, enabling us to identify and treat patients at risk for developing heart failure before symptoms develop, said study co-author Robert C. Gorman, MD, professor of Surgery, and director of Cardiac Surgical Research at Penn Medicine.
To demonstrate CESTs ability to detect heart disease, the researchers applied the creatine CEST method in an MRI scanner, in healthy and infarcted myocardium (muscle tissue in heart) in large animals. In the process, the nuclear magnetization of amine (NH2) creatine protons is saturated by a radiofrequency pulse from the MRI. After the exchange with water, the degree of saturation is observed as the water signal drops, and thus the concentration of creatine becomes apparent. (In the body, creatine is converted to creatinine, which can be measured through blood and urine tests and is an important tool for assessing renal function.)
The team showed that the creatine CEST method can map changes in creatine levels, and pinpoint infarcted areas in heart muscle tissue, just as MRS methods can. However, they found, CEST has two orders of magnitude higher sensitivity than MRS. That advantage could help spot smaller damaged areas in the heart missed by traditional methods, the authors say.

St. Petersburg Bans Doghunter Sites

Russian Internet censorship was broadened this week to include animal protection as two doghunter websites were banned in St. Petersburg.
Russia has a thriving underground community of vigilantes who kill off arguably dangerous stray dogs in areas where animal control measures are ineffective. An estimated 1 million strays roam the streets nationally and people have been wounded and even killed in attacks by hungry dogs.
The court ban was imposed at the request of city prosecutors, who said on their website that the sites violate Russian laws on information, the animal world and protection of minors.
The move may spell a change in Russias approach to dog-hunters, none of whom have so far been found guilty of animal abuse. The countrys animal rights legislation only criminalizes animal abuse committed out of hooliganism, for profit or in front of minors.
Meanwhile, doghunters claim to be doing the governments job in protecting the public. Some 16,600 people were attacked in Moscow alone by stray animals in 2008, the last year for which statistics are available, according to the city branch of the Federal Consumer Protection Service.
Russia was not known before for blacklisting websites accused of animal abuse. The country already censors online extremist content, pirated films and websites deemed to promote suicide, illegal drugs and child pornography.
The list of topics subjected to online censorship is increasing, with promotion of unauthorised rallies added in late December.

How the tea party undermines conservatism

One of the main problems with an unremittingly hostile view of government held by many associated with the tea party, libertarianism and constitutionalism is that it obscures and undermines the social contributions of a truly conservative vision of government.
Politics requires a guiding principle of public action. For popular liberalism, it is often the rule of good intentions: If it sounds good, do it. Social problems can be solved by compassionate, efficient regulation and bureaucratic management which is seldom efficient and invites unintended consequences in complex, unmanageable systems (say, the one-sixth of the U.S. economy devoted to health care). The signal light for government intervention is stuck on green.
For libertarians and their ideological relatives, the guiding principle is the maximization of individual liberty. It is a theory of government consisting mainly of limits and boundaries. The light is almost always red.
Conservatism offers a different principle of public action though a bit more difficult to explain than go or stop. In the traditional conservative view, individual liberty is ennobled and ordered within social institutions families, religious communities, neighborhoods, voluntary associations, local governments and nations. The success of individuals is tied to the health of these institutions, which prepare them for the responsible exercise of freedom and the duties of citizenship.
This is a limiting principle: Higher levels of government should show deference to private associations and local institutions. But this is also a guide to appropriate governmental action needed when local and private institutions are enervated or insufficient in scale to achieve the public good.
So conservatism is a governing vision that allows for a yellow light: careful, measured public interventions to encourage the health of civil society. There are no simple rules here. Some communities disproportionately affected by family breakdown, community chaos or damaging economic trends will need more active help. But government should, as the first resort, set the table for private action and private institutions creating a context in which civil society can flourish.
This goal has moral and cultural implications. Government has a necessary (if limited) role in reinforcing the social norms and expectations that make the work of civic institutions both possible and easier. Some forms of liberty say, the freedom to destroy oneself with hard drugs or to exploit men and women in the sex trade not only degrade human nature but damage and undermine families and communities and ultimately deprive the nation of competent, self-governing citizens.
But conservatives also need to take seriously the economic implications of this governing vision. Just as citizens must be prepared for the exercise of liberty, individuals must be given the skills and values human capital that will allow them to succeed in a free economy.
This is the essence of equal opportunity. But it is not a natural social condition. And many conservatives have failed to recognize the extent to which this defining American promise has been hollowed out.
Economic mobility is stalled for many poorer Americans, resulting in persistent, intergenerational inequality. This problem is more complex than an income gap. It involves wide disparities in parental time and investment, in community involvement and in academic accomplishment. These are traceable to a number of factors that defy easy ideological categorization, including the collapse of working-class families and the flight of decent blue-collar jobs.
